The demand for organ transplants, both globally and in South Korea, substantially exceeds the supply, a situation that might have been aggravated by the enactment of the Life-Sustaining Treatment Decision Act (LSTDA) in February 2018. This legislation may influence emergency medical procedures and the availability of organs from brain-dead donors. This study aimed to assess LSTDA’s impact, introduced in February 2018, on organ donation status in out-of-hospital cardiac arrest (OHCA) patients in a metropolitan city and identified related factors. We conducted (...) a retrospective analysis of a regional cardiac arrest registry. This study included patients aged 16 or older with cardiac arrest and a cerebral performance category (CPC) score of 5 from January 2015 to December 2022. The exclusion criteria were CPC scores of 1–4, patients under 16 years, and patients declared dead or transferred from emergency departments. Logistic regression analysis was used to analyse factors affecting organ donation. Of the 751 patients included in this study, 47 were organ donors, with a median age of 47 years. Before the LSTDA, there were 30 organ donations, which declined to 17 after its implementation. In the organ donation group, the causes of cardiac arrest included medical (34%), hanging (46.8%), and trauma (19.2%). The adjusted odds ratio for organ donation before the LSTDA implementation was 6.12 (95% CI 3.09–12.12), with non-medical aetiology as associated factors. The enactment of the LSTDA in 2018 in South Korea may be linked to reduced organ donations among patients with OHCA, underscoring the need to re-evaluate the medical and legal aspects of organ donation, especially considering end-of-life care decisions. Acknowledging separable factors underlying negative symptoms may lead to better understanding and treatment of negative symptoms in individuals with schizophrenia. The current study aimed to test whether the negative symptoms factor (NSF) of the Positive and Negative Syndrome Scale (PANSS) would be better represented by expressive and experiential deficit factors, rather than by a single factor model, using confirmatory factor analysis (CFA). Two hundred and twenty individuals with schizophrenia spectrum disorders completed the PANSS; subsamples additionally completed the Brief Negative Symptom (...) Scale (BNSS) and the Motivation and Pleasure Scale—Self-Report (MAP-SR). CFA results indicated that the two-factor model fit the data better than the one-factor model; however, latent variables were closely correlated. The two-factor model’s fit was significantly improved by accounting for correlated residuals between N2 (emotional withdrawal) and N6 (lack of spontaneity and flow of conversation), and between N4 (passive social withdrawal) and G16 (active social avoidance), possibly reflecting common method variance. The two NSF factors exhibited differential patterns of correlation with subdomains of the BNSS and MAP-SR. These results suggest that the PANSS NSF would be better represented by a two-factor model than by a single-factor one, and support the two-factor model’s adequate criterion-related validity. Common method variance among several items may be a potential source of measurement error under a two-factor model of the PANSS NSF. (shrink)

BackgroundAlthough the Life-Sustaining Treatment (LST) Decision Act was enforced in 2018 in Korea, data on whether it is well established in actual clinical settings are limited. Hospital-acquired pneumonia (HAP) is a common nosocomial infection with high mortality. However, there are limited data on the end-of-life (EOL) decision of patients with HAP. Therefore, we aimed to examine clinical characteristics and outcomes according to the EOL decision for patients with HAP.MethodsThis multicenter study enrolled patients with HAP at 16 referral hospitals retrospectively from (...) January to December 2019. EOL decisions included do-not-resuscitate (DNR), withholding of LST, and withdrawal of LST. Descriptive and Kaplan–Meier curve analyses for survival were performed.ResultsOf 1,131 patients with HAP, 283 deceased patients with EOL decisions (105 cases of DNR, 108 cases of withholding of LST, and 70 cases of withdrawal of LST) were analyzed. The median age was 74 (IQR 63–81) years. The prevalence of solid malignant tumors was high (32.4% vs. 46.3% vs. 54.3%, P = 0.011), and the ICU admission rate was lower (42.9% vs. 35.2% vs. 24.3%, P = 0.042) in the withdrawal group. The prevalence of multidrug-resistant pathogens, impaired consciousness, and cough was significantly lower in the withdrawal group. Kaplan–Meier curve analysis revealed that 30-day and 60-day survival rates were higher in the withdrawal group than in the DNR and withholding groups (log-rank P = 0.021 and 0.018). The survival of the withdrawal group was markedly decreased after 40 days; thus, the withdrawal decision was made around this time. Among patients aged below 80 years, the rates of EOL decisions were not different (P = 0.430); however, mong patients aged over 80 years, the rate of withdrawal was significantly lower than that of DNR and withholding (P = 0.001).ConclusionsAfter the LST Decision Act was enforced in Korea, a DNR order was still common in EOL decisions. Baseline characteristics and outcomes were similar between the DNR and withholding groups; however, differences were observed in the withdrawal group. Withdrawal decisions seemed to be made at the late stage of dying. Therefore, advance care planning for patients with HAP is needed. (shrink)

Être transgenre implique des relations uniques à son propre corps, ainsi que la question du récit dans la formation du sujet – deux thèmes-clés chez Merleau-Ponty. Mais même si l’on accepte d’aller au-delà de l’histoire stéréotypée d’être « pris·e dans le mauvais corps », tant dans les communautés trans que dans le discours public plus large il existe une tendance fâcheuse à universaliser de tels récits – et ce, en dépit de l’hétérogénéité apparente que l’on retrouve dans les descriptions individuelles (...) à la première personne. La question émerge donc à savoir : comment rendre compte philosophiquement de cette différence à la première personne tout en visant une politique de solidarité? Cet essai cherche à utiliser la phénoménologie de Merleau-Ponty dans un sens pratique, en se concentrant sur sa description du « schéma corporel » pour atténuer cette tension. En effet, tandis que Merleau-Ponty nous fournit un ensemble général d’instruments pour comprendre la relation entre incarnation et comportement, ses exemples phénoménologiques sont souvent fortement singuliers. De la sorte, dans le contexte des subjectivités transgenres, le but est de creuser davantage cet espace discursif, comme prologue pour une praxis qui soit plus réceptive à l’expérience vécue des personnes trans. 장일순(1928~1994)은 강원도 원주에서 태어나 일생의 대부분을 원주에서 생활한 지역 사상가이지만, 그가 제시한 생명사상은 지역성을 넘어선 보편적 사유로서 중요한 의미를 갖는다. 생태문제와 기후문제가 점점 심각해지는 오늘날의 관점에서 볼 때, 장일순의 생명 사상은 선구적일 뿐만 아니라 윤리적으로도 실천적인 대안을 제시하고 있다는 점에서 현 재성을 가진다. 장일순은 생명의 전일성에 대한 깨달음을 강조하고, 이를 통한 인식변화와 행동의 변화를 강조한다는 점에서 실천적 신비주의자이자 윤리사상가라고 할 수 있다. 그 는 동학의 신비주의적 생명사상과 ‘시천주(侍天主)’ 와 ‘양천주(養天主)’의 실천사상을 적 극적으로 수용하는 한편, 이것을 특정 종교의 가르침이 아닌 보편적인 (...) 생명사상으로 발전 시켰다. 그는 강력한 도덕 명령에 의한 의무보다는 생명을 바라보는 인식의 변화를 통한 자발적인 태도변화를 강조하였으며, 이러한 윤리적 실천은 서양의 ‘심층생태학’과도 많은 접점을 가진다. 장일순의 생명사상은 세계를 상호관계성 속에서 파악함으로써 생명 간의 소통가능성과 ‘마음’의 변화가능성을 주장할 뿐만 아니라 행동의 적극적인 변화를 촉구한 다. 그는 또한 생명과 세계의 전일성을 이해함으로써 자아와 세계의 이분법적 분열을 극복 할 수 있다고 주장한다. 그의 생명사상은 정체성보다는 다양성과 평등을 특징으로 하는 연 대에 기반하며, ‘한살림’운동은 이러한 특징을 구체적으로 실현하는 생명네트워크로서 시 작되었다. ‘한살림’은 우주․생태․공동체적 각성을 기본정신으로 삼고 있으며, 보편성을 지닌 실천 문화활동으로서 장일순 생명사상의 윤리적 실천가능성을 보여주고 있다. 성리학자들에게 있어 理와 氣의 개념 정리는 자기가 처해있는 현실세계를 이해하고, 인간의 도덕적 윤리관을 자신의 철학적 관점으로 이해하는데 중요한 근간이 된다. 따라서 氣라는 것은 성리학에 있어서 인간과 자연, 존재와 당위, 사실과 가치의 전 영역을 포괄하는 개념으로 사용되어 왔다. 즉 氣를 자연으로서의 氣와 인간중심과 윤리적 차원의 氣, 그리고 자연과 인간의 심성 차원의 氣로 세분화할 수 있다. 이러한 연구에 있어서 대표적인 조선성리학자들은 花潭 · 退溪 · 栗谷이 손꼽힌다.BR 退溪는 ‘理貴氣賤’이라 하여 氣를 천하게 여겼고, 花潭과 栗谷은 ‘氣’를 중시하는 이론체계를 전개하였다. 그러나 花潭은 ‘一氣長存’이라 말하며 (...) 氣의 항존성을 주장한 반면, 栗谷은 ‘湛一淸虛之氣’라 하여 수양론적 도구로서 氣를 인식하는 등 그 차별성을 보이고 있다.BR 성리학 체계 내에서 조선 초기의 이론을 정립한 花潭은 氣의 측면에서 ‘우주와 자연’의 존재론적 차원의 철학체계를 전개 하였다면, 뒤이어 조선 사화기에 등장한 退溪는 이상세계를 구현하기 위해 理를 강조하며 ‘觸處皆理’라는 신념을 가지고 理의 당위성을 중요시하였다. 그리고 四端七情論에 의해 이론체계가 구체화 되면서 氣에 대한 인식이 심성론으로 그 경향성이 바뀌고 있다. 그러나 栗谷은 현실세계의 문제를 인식하고 氣를 湛一淸虛한 것으로 설정하며 수양론적 입장으로서 그 전개 양상이 바뀌고 있다. 이는 花潭의 관점과는 큰 차이를 보이고 있다. Temporal experience and its radical alteration in schizophrenia have been one of the central objects of investigation in phenomenological psychopathology. Various phenomenologically oriented researchers have argued that the change in the mode of temporal experience present in schizophrenia can foreground its psychotic symptoms of delusion. This paper aims to further the development of such a phenomenological investigation by highlighting a much-neglected aspect of schizophrenic temporal experience, i.e., its non-emotional affective characteristic. In this paper, it denotes the type of an experience (...) wherein an afflicted individual experiences a pervasive pull or attraction coming from the past, present, and future. By employing Husserl’s account of affection, I argue that such an affectively prominent temporal experience is not yet another abnormality that happens to be present in schizophrenia. Instead, it is indicative of the core disturbance that underpins the schizophrenic temporal mode of experience. I identify such a disturbance as ‘affective modification dysfunction’ and employ it as a core concept with which I synthesize and organise heterogeneous components of schizophrenic temporal experience in their conceptual unity. For the sake of clear description, I organise those components into the following categories: 1.) Time Stop 2.) Ante-festum 3.) Déjà vu/vécu and 4.) Time Fragmentation. I conclude by demonstrating how approaching schizophrenic temporal experience from its affective dimension can further help us better understand its pre-psychotic phase known to precipitate schizophrenic primary delusion, i.e., delusional mood. In light of the ongoing validity crisis in psychiatric classification, phenomenologically oriented psychiatric study has gained traction. This paper assesses two modes of investigation proposed by phenomenologists in studying mental disorders: the ideal type approach and the essential type approach. Despite the recent suggestion that they are antithetical approaches, I argue that they should constantly constrain and inform each other. In short, I advance a mutual complementarity thesis. Having established this thesis, I conclude by demonstrating how this proposal can function (...) as an heuristic strategy for effectively facilitating the recently proposed psychiatric research initiative, i.e. the ontological project of phenomenological psychopathology. This article examines how growing fears, insecurities and uncertainties during the COVID-19 pandemic have prompted an emotional distance from others. The aim is to explore how global solidarity and nationalism are challenged and constructed as collective emotional processes concerning ‘others’. Drawing on social theories of emotions during crises and emotions towards others, this study looks at policy decisions around vaccines and health services and their associated emotions in the context of Korea, which has a relatively small migrant population and a (...) short history of supporting people in lower-income countries. The study finds that the COVID-19 pandemic has strengthened nationalism, both ethnic nationalism and cosmopolitan nationalism. This points to the need to highlight global norms such as human rights and justice and cultivate foundational emotions such as empathy and compassion. This article suggests paying attention to the role of emotions in generating othering practices and developing global solidarity. (shrink)

It is questions that children need to do higher-order thinking in a community of inquiry. There, more fundamental questions should be asked with some efforts to understand clearly and analyze the given texts. The initial questions should be elaborated into more fundamental ones through dialogues and discussions, and the process may be changed by the given conditions and contexts. In this paper, I show by some real cases how teachers can help children make more fundamental questions in a community of (...) inquiry. (shrink)

Business ethics has been described as a prime academic growth industry. This paper reports the findings of a survey aimed at establishing the status of ethics in the curricula of Canadian Schools of Management and Administrative Studies. It was found that twenty-three of the forty-two responding schools offer courses in business ethics and that they offer a total of twenty-five ethics courses, twenty of which are offered as electives. Forty-two percent of the schools not offering a course in business ethics (...) plan to offer such a course by 1989. This means that by 1989 seventy-four percent of the responding schools should have a business ethics component in their curricula. (shrink)

This paper compares the findings of content analyses of the corporate codes of ethics of Canada's largest corporations in 1992 and 2003. For both years, a modified version of a technique used in several other studies was used to determine and categorize the contents of the codes. It was found, inter alia, that, in 2003, as in 1992, more of the codes were concerned with conduct against the firm than with conduct on behalf of the firm. Among the changes from (...) 1992 to 2003 were a significant increase in the frequency of mention of environmental affairs, legal responsibility as the basis of codes and enforcement/compliance procedures. Silence has been a relatively neglected phenomenon despite its significance in psychiatric research. Acknowledging this oversight, there has been a recent move towards systematically describing the first-personal experience of silence in mental disorders within the field of philosophy of psychiatry. This paper contributes to this research effort by highlighting the underexplored interpersonal aspect of silence crucial for both psychopathological and therapeutic research. More specifically, I develop the interpersonal aspect of distressing silence associated with depression, recently coined as ‘empty silence’. Complementing (...) its original analysis, I argue that this distressing silence not only involves the loss of linguistic agency but also the loss of social agency. Having established this claim, I situate it within the wider research context and open an ethical dimension to the current analysis. I explore this by identifying a therapeutic silence others can establish for an individual undergoing a severe depressive episode. I provisionally term it ‘undemanding silence’ and argue that this form of silence can help restore the loss of social agency by providing a distinct form of social affordance that depressed individuals can easily realise at their own pace. This study uses a specific method to analyze the contents of the codes of ethics of the largest corporations in Australia, Canada and Sweden and compares the findings of similar content analyses in 2002 and 2006. It tracks changes in code contents across the three nations over the 2002–2006 period. There were statistically significant changes in the codes of the three countries from 2002 to 2006: the Australian and Canadian codes becoming more prescriptive, intensifying the differences between these and the (...) Swedish codes. The contents of these codes and the nature of the changes they have undergone over time are culturally driven: Australia's and Canada's reflecting their similar cultural profiles and Sweden's reflecting its differences from these countries on organizationally relevant cultural dimensions. The study reveals that corporate codes of ethics are living documents as reflected by the significant longitudinal changes in the frequencies of mention of several of the 60 items underpinning the content analysis of the codes of ethics. Consequently, and in light of their growing prevalence and importance as instruments of corporate governance, they should not be treated as static but as dynamic documents that are subject to various environmental factors. The clear implication of the findings of this study is that for corporate codes of ethics ‘one size does not fit all’ and that these instruments must be carefully monitored and revised to reflect changing conditions. Delusional mood is a well-recognized psychological state, often present in the prodromal stage of schizophrenia. Various phenomenological psychopathologists have proposed that delusional mood may not only precede but also contribute to the later formation of schizophrenic delusion. Hence, understanding experiential abnormalities involved with the delusional mood have been considered central for the understanding of schizophrenic delusion. Ranging from traditional and contemporary phenomenological and neurobiological accounts, it has been often mentioned that the peculiar affective saliency of the world experience may underpin (...) the emergence of delusional mood. In this paper, I employ Edmund Husserl’s account of affection and affective syntheses to clarify the nature of such an experience and illuminate how this experiential abnormality contributes to the emergence of the delusional mood. I conclude by relating the phenomenological account I advance here with a neurobiological account of aberrant salience hypothesis and chart out a possible way toward mutual enlightenment for both approaches. ABSTRACT This article aims to elucidate a philosophical foundation of a post-labor paradigm through the transindividual technical-psychic-collective culture based on Gilbert Simondon and Bernard Stiegler. Simondon predicts that the problem of the alienation of labor due to mechanical industrialization can be overcome through the spread of post-industrial technical culture based on both technical mentality and information technology. In contrast, Stiegler claims that, along with information networks, hyper-industrialization rather than post-industrialization has arrived and that, in order to recover human values in (...) a machine empire devoid of caring, the strengthening of the ability for non-automation based on automaticity is necessary. However, Simondon’s technical culture beyond labor implies a posthumanistic vision in that it assumes the capacity of technology to mediate between the preindividual and the transindividual beyond technical instrumentalism, which is anthropocentric, and opens up transductive relationships among humans and non-humans. I will argue that Stiegler’s urgent proposal that seeks to save human life from the control of a techno-capital system, such as the reinvention of work transcending employment, must be concretized within the Simondonian posthumanistic project. RESUMO O objetivo deste artigo é elucidar a base filosófica do paradigma pós-laboral por meio da cultura técnica-psiquica-coletiva transindividual de Gilbert Simondon e Bernard Stiegler. Simondon prediz que o problema da alienação laboral surgido da industrialização mecânica pode ser vencido mediante a expansão da cultura técnica pós-industrial baseada tanto na mentalidade técnica quanto na tecnologia da informação. Em contraste, Stiegler afirma que, além das redes de informação, a hiper-industrialização se estabeleceu para além da pós-industrialização e que, para recuperar valores humanos em um império das máquinas sem carinho, é necessário o fortalecimento da capacidade de não automação baseada em automatismo. Entretanto, a cultura técnica além do trabalho de Simondon implica uma visão pós-humanista em que se presume a capacidade da tecnologia em mediar entre o pré-individual e o transindividual, que é antropocêntrico, e inaugura relações transdutivas entre humanos e não humanos. Defendo que a proposta urgente de Stiegler busca salvar a vida humana do controle de um sistema de tecno-capital, como a reinvenção do trabalho transcendendo o emprego deve ser concretizada dentro do projeto pós-humanista simondoniano.
