Background: As actors with the key responsibility for the protection of human research participants, Research Ethics Committees (RECs) need to be competent and well-resourced in order to fulfil their roles. Despite recent programs designed to strengthen RECs in Africa, much more needs to be accomplished before these committees can function optimally.Objective: To assess training needs for biomedical research ethics evaluation among targeted countries.Methods: Members of RECs operating in three targeted African countries were surveyed between August and November 2007. Before implementing (...) the survey, ethical approvals were obtained from RECs in Switzerland, Cameroon, Mali and Tanzania. Data were collected using a semi-structured questionnaire in English and in French.Results: A total of 74 respondents participated in the study. The participation rate was 68%. Seventy one percent of respondents reported having received some training in research ethics evaluation. This training was given by national institutions (31%) and international institutions (69%). Researchers and REC members were ranked as the top target audiences to be trained. Of 32 topics, the top five training priorities were: basic ethical principles, coverage of applicable laws and regulations, how to conduct ethics review, evaluating informed consent processes and the role of the REC.Conclusion: Although the majority of REC members in the targeted African countries had received training in ethics, they expressed a need for additional training. The results of this survey have been used to design a training program in research ethics evaluation that meets this need. (shrink)

BackgroundInternational guidelines recommend ethical and scientific quality standards for managing and reporting adverse events occurring during clinical trials to competent research ethics committees and regulatory authorities. The purpose of this study was to determine whether clinical trial protocols in Cameroon are developed in line with national requirements and international guidelines as far as detecting, reporting and investigating of adverse events is concerned.MethodsIt was a documentary review of all approved clinical trial protocols that were submitted at the Cameroon National Ethics Committee (...) for evaluation from 1997 through 2012. Data were extracted using a preconceived and validated grid. Protocol review process targeted the title, abstract, objectives, methodology, resources, and the chapter on safety.ResultsIn total, 106 clinical trial protocols were identified from 2173 protocols seen in the archive and 104 included for review. Seventy six trials did not include the surveillance of adverse events as part of their objective. A total of 91 protocols did not budget for adverse event surveillance, 76 did not have a data safety management board, 11 included insurance for participants, 47 did not include a case definition for serious adverse events, 33 described procedures to detect adverse events, 33 described procedure for reporting and 22 described procedure for investigating adverse events.DiscussionsMost clinical trial protocols in Cameroon are developed to focus on benefits and pay little attention to harms. The development of national guidelines can improve the surveillance of adverse events in clinical trial research conducted in Cameroon. Adverse events surveillance tools and a budget are critical for an adequate planning for adverse event surveillance when developing trial protocols.ConclusionClinical trial protocols submitted in the Cameroon National Ethics Committee do not adequately plan to assess adverse events in clinical trial protocols. In order to improve on the safety of participants and marketed drug, there is a need to develop national guidelines for clinical trials by the government, and to improve evaluation procedures and monitoring of ongoing trials by the ethics committee. Everettian quantum mechanics tells us that the fundamental dynamics of the universe are deterministic. So what are the `probabilities' that the Born rule describes? One popular answer has been to treat these probabilities as rational credences. A recent alternative, Isaac Wilhelm's centered Everett Interpretation (CEI), takes the Born probabilities to be centered chances: the objective chances that some centered propositions are true. Thus, the CEI challenges the `orthodox assumption’ that fundamental physical laws concern only uncentered facts. I provide three arguments (...) against the centered Everett Interpretation. First, I argue that the CEI is in apparent tension with a significant motivation for adopting Everettian quantum mechanics: rejecting the attribution of special significance to observers or agents in fundamental physics. I suggest the CEI can avoid this tension, but only at the cost of sacrificing its central claim that there are objective chances in an Everettian multiverse. My second argument concerns the CEI’s claim that the centered Born rule is a fundamental physical law. I provide two plausible notions of fundamentality for physical laws, and I argue that the centered Born rule satisfies neither. My final argument is that the CEI’s branch-relative laws cannot explain or constrain an agent’s rational credences in the way that the CEI claims. (shrink)

Les critiques actuelles des diagnostics psychiatriques, qu’elles viennent des antipsychiatres, des béhavioristes, des constructionnistes sociaux, des szasziens et des foucaldiens, rejettent généralement l’idée que le concept de trouble mental est légitime du point de vue médical, ne laissant donc aucun argument solide à partir duquel il soit possible de mener une critique constructive et d’établir un dialogue avec la psychiatrie. Ces positions ne réussissent également pas à expliquer les fortes intuitions populaires qui permettent aux gens de distinguer les troubles psychologiques (...) des autres conditions psychologiques négatives. Selon l’analyse de la dysfonction préjudiciable présentée dans cet article, un trouble mental est une condition : 1) préjudiciable, un concept évaluatif basé sur des valeurs sociales ; et 2) causée par une dysfonction psychologique, un concept factuel se rapportant à un mécanisme psychologique qui accomplit une fonction pour laquelle il a été biologiquement conçu . L’analyse DP est ainsi une position hybride qui requiert à la fois que des critères évaluatifs et factuels soient satisfaits pour garantir l’attribution d’un trouble. Nous argumentons que l’analyse DP réussit admirablement à expliquer les jugements intuitifs quant à ce qui est un trouble et à ce qui en n’est pas. La composante factuelle permet également de critiquer le critère diagnostique du DSM, ce que nous ferons à l’aide de plusieurs exemples . Ces critiques déterminent où le DSM a dévié de ses propres hypothèses et ainsi considéré comme des troubles ce qui n’en est pas. Elles expliquent également pourquoi les tentatives du DSM de remédier au problème en introduisant le critère du « cliniquement significatif » échouent.Current antipsychiatric, behaviorist, social-constructivist, Szaszian, and Foucauldian critiques of psychiatric diagnosis generally reject mental disorder as a medically legitimate concept, thus allowing no place to stand from which to mount a constructive critique and dialogue with psychiatry. These views also fail to explain the robust folk intuitions by which people distinguish psychological disorders from other negative psychological conditions. According to the harmful dysfunction analysis presented in this paper, a mental disorder is condition that is harmful, a value concept judged by social values ; and caused by a psychological dysfunction, a factual concept referring to failure of a psychological mechanism to perform a function for which it is biologically designed . The HD analysis is thus a hybrid account that requires both value and factual criteria to be satisfied before attribution of disorder is warranted. It is argued that the HD analysis has considerable success in explaining intuitive judgments about disorder versus non-disorder. The factual component also allows for a critique of DSM diagnostic criteria for specific categories, with several examples discussed. These critiques identify where the DSM has deviated from its own deepest assumptions and labeled non-disorders as disorders, and why the DSM’s own attempts to fix the problem, as with the «clinical significance» criterion, do not succeed. Background Post-September 11, 2001, the U.S. government has labeled thousands of Afghan war detainees "unlawful combatants". This label effectively deprives these detainees of the protection they would receive as "prisoners of war" under international humanitarian law. Reports have emerged that indicate that thousands of detainees being held in secret military facilities outside the United States are being subjected to questionable "stress and duress" interrogation tactics by U.S. authorities. If true, American military physicians could be inadvertently becoming complicit in detainee abuse. (...) Moreover, the American government's openly negative views towards such detainees could result in military physicians not wanting to provide reasonable care to detainees, despite it being their ethical duty to do so. Discussion This paper assesses the physician's obligations to treat war detainees in the light of relevant instruments of international humanitarian law and medical ethics. It briefly outlines how detainee abuse flourished in apartheid South Africa when state physicians became morally detached from the interests of their detainee patients. I caution U.S physicians not to let the same mindset befall them. I urge the U.S. medical community to advocate for detainee rights in the U.S, regardless of the political culture the detainee emerged from. I offer recommendations to U.S physicians facing dual loyalty conflicts of interest in the "war on terror". Summary If U.S. physicians are faced with a conflict of interest between following national policies or international principles of humanitarian law and medical ethics, they should opt to adhere to the latter when treating war detainees. It is important for the U.S. medical community to speak out against possible detainee abuse by the U.S. government. (shrink)

I argue that the issues of foodquality, in the most general sense includingpurity, safety, and ethics, can no longer beresolved through ``normal'' science andregulation. The reliance on reductionistscience as the basis for policy andimplementation has shown itself to beinadequate. I use several borderline examplesbetween drugs and foods, particularly coffeeand sucrose, to show that ``quality'' is now acomplex attribute. For in those cases thesubstance is either a pure drug, or a bad foodwith drug-like properties; both are marketed asif they were foods. (...) An example of theinadequacy of old ways of thinking is obesity,whose causes are as yet outside the purview ofmedicine, while its effects constitute anepidemic disease. The new drug/food syndromeneeds a new sort of science, what we call``post-normal.'' This is inquiry at the contestedinterfaces of science and policy; typically itdeals with issues where facts are uncertain,values in dispute, stakes high, and decisionsurgent. With the perspective of post-normalscience, we can better understand some keyissues. We see that ``safety'' is different from``risk,'' being pragmatic, moral, and recursive.Also, we understand that an appropriatefoundation for regulation and ethics is not somuch ``objectivity'' as ``awareness.'' In an agewhen ``consumers'' are becoming concerned``citizens,'' the relevant science must becomepost-normal. (shrink)

Selon Boyd, l’évolution des théories et des pratiques morales est étroitement analogue à l’évolution de la connaissance scientifique, car dans les deux cas la connaissance est guidée par une réalité indépendante progressivement découverte.Dans cet article, nous rejetons cette analogie, en raison des multiples dissemblances entre connaissance morale et connaissance scientifique. Nous nous efforcerons de cerner les spécificités de la connaissance morale, (...) à partir de travaux empiriques dans le champ interdisciplinaire de la naturalisation de l’éthique (anthropologie cognitive, psychologie expérimentale, psychologie morale…). Toute théorie quantique de la gravitation (notamment la théorie quantique à boucles) semble ainsi ne pouvoir aboutir qu’en envisageant le temps comme une réalité seconde, dérivée et non plus originaire ou fondamentale. Mais à partir de quelle autre réalité plus fondamentale que lui le temps pourrait-il être engendré? Cette réalité pourrait-elle elle-même être en quoi que ce soit non changeante? N’a-t-on pas ainsi tort de confondre le temps et la variation (ou le changement)? L’enjeu de cet article est de distinguer soigneusement ces deux concepts en montrant, d’une part, que la variation est en vérité, sous certaines conditions théoriques à énoncer, un concept plus fondamental que celui de temps, et d’autre part, que ce dernier peut être engendré à partir d’une variation – ainsi pensée comme non empirique ou pure – en lui ajoutant un certain nombre de contraintes – qui la rendent alors empirique et dotée des principales propriétés constitutives du temps. Ces contraintes ou conditions restrictives nous semblent essentiellement de trois sortes : subsistance ou invariance, continuité et succession. Nous les développons successivement en montrant comment les relativités restreinte et générale et la physique quantique tendent déjà à violer partiellement ces contraintes. This is an extremely rich, wide ranging book, filled with insightful discussions of a large number of issues related to the general topic of the emotions. Its value lies not in its conclusions but in the details, which do not lend themselves easily to summarizing. So it must be said at the outset that this review will not do justice to the subtlety, complexity, and wit of this book.

Much of our understanding of human thinking is based on probabilistic models. This innovative book by Jerome R. Busemeyer and Peter D. Bruza argues that, actually, the underlying mathematical structures from quantum theory provide a much better account of human thinking than traditional models. They introduce the foundations for modelling probabilistic-dynamic systems using two aspects of quantum theory. The first, 'contextuality', is a way to understand interference effects found with inferences and decisions under conditions of uncertainty. The second, 'quantum entanglement', (...) allows cognitive phenomena to be modeled in non-reductionist ways. Employing these principles drawn from quantum theory allows us to view human cognition and decision in a totally new light. Introducing the basic principles in an easy-to-follow way, this book does not assume a physics background or a quantum brain and comes complete with a tutorial and fully worked-out applications in important areas of cognition and decision. Christopher Boorse’s biostatistical theory of medical disorder claims that biological part-dysfunction (i.e., failure of an internal mechanism to perform its biological function), a factual criterion, is both necessary and sufficient for disorder. Jerome Wakefield’s harmful dysfunction analysis of medical disorder agrees that part-dysfunction is necessary but rejects the sufficiency claim, maintaining that disorder also requires that the part-dysfunction causes harm to the individual, a value criterion. In this paper, I present two considerations against the sufficiency claim. First, I analyze Boorse’s (...) central argument for the sufficiency claim, the “pathologist argument,” which takes pathologists’ intuitions about pathology as determinative of medical disorder and conclude that it begs the question and fails to support the sufficiency claim. Second, I present four counterexamples from the medical literature in which salient part-dysfunctions are considered nondisorders, including healthy disease carriers, HIV-positive status, benign mutations, and situs inversus totalis, thus falsifying the sufficiency claim and supporting the harm criterion. Arguing that Spinoza's cognitivist theory of emotions is closer to the truth, it is shown that that provides the beginning of an understanding of how Freudian or, more generally, analytic therapies make philosophic sense. That is, we can begin to understand how people's emotional lives might be transformed by consideration and interpretation of their memories, beliefs, fantasies; in (...) other words, how knowledge might help to make one free. Wakefield’s harmful dysfunction analysis asserts that the concept of medical disorder includes a naturalistic component of dysfunction and a value component, both of which are required for disorder attributions. Muckler and Taylor, defending a purely naturalist, value-free understanding of disorder, argue that harm is not necessary for disorder. They provide three examples of dysfunctions that, they claim, are considered disorders but are entirely harmless: mild mononucleosis, cowpox that prevents smallpox, and minor perceptual deficits. They also reject the proposal that dysfunctions (...) need only be typically harmful to qualify as disorders. We argue that the proposed counterexamples are, in fact, considered harmful; thus, they fail to disconfirm the harm requirement: incapacity for exertion is inherently harmful, whether or not exertion occurs, cowpox is directly harmful irrespective of indirect benefits, and colorblindness and anosmia are considered harmful by those who consider them disorders. We also defend the typicality qualifier as viably addressing some apparently harmless disorders and argue that a dysfunction’s harmfulness is best understood in dispositional terms. (shrink)
