Background: Managing HIV treatment is a complex multi-dimensional task because of a combination of factors such as stigma and discrimination of some populations who frequently get infected with HIV. In addition, patient-provider encounters have become increasingly multicultural, making effective communication and provision of ethically sound care a challenge. Purpose: This article explores ethical issues that health service providers in the United States and Botswana encountered in their interaction with patients in HIV care. Research design: A descriptive qualitative design was used (...) to collect data from health service providers and patients using focused group discussions. This article is based on responses from health service providers only. Participants and context: This article is based on 11 focused group discussions with a total sample of 71 service providers in seven US sites and one Botswana site. Ethical considerations: Ethical review boards at all the study sites reviewed the study protocol and approved it. Ethical review boards of the study’s coordinating centers, Rutgers University and the University of California at San Francisco, also approved it. The study participants provided a written informed consent to participate. Findings: HIV service providers encountered ethical challenges in all the four Beauchamp and Childress’ biomedical ethics of respect for patients’ autonomy, beneficence, justice, and nonmaleficence. Discussion: The finding that HIV service providers encounter ethical challenges in their interaction with patients is supported by prior studies. The ethical challenges are particularly prominent in multicultural care and resource-constrained care environments. Conclusion: Provision of HIV care is fraught with ethical challenges that tend to pose different issues depending on a given care environment. It is important that strong partnerships are developed among key stakeholders in HIV care. In addition, health service providers need to be provided with resources so they can provide quality and ethically sound care. (shrink)

Although research has found that long-term mindfulness meditation practice promotes executive functioning and the ability to sustain attention, the effects of brief mindfulness meditation training have not been fully explored. We examined whether brief meditation training affects cognition and mood when compared to an active control group. After four sessions of either meditation training or listening to a recorded book, participants with no prior meditation experience were assessed with measures of mood, verbal fluency, visual coding, and working memory. Both interventions (...) were effective at improving mood but only brief meditation training reduced fatigue, anxiety, and increased mindfulness. Moreover, brief mindfulness training significantly improved visuo-spatial processing, working memory, and executive functioning. Our findings suggest that 4 days of meditation training can enhance the ability to sustain attention; benefits that have previously been reported with long-term meditators. (shrink)

The focus on translational research in clinical trials has the potential to generate clinically relevant genetic data that could have importance to patients. This raises challenging questions about communicating relevant genetic research results to individual patients. An exploratory pharmacogenetic analysis was conducted in the international ovarian cancer phase III trial, AGO-OVAR 16, which found that patients with clinically important germ-line BRCA1/2 mutations had improved progression-free survival prognosis. Mechanisms to communicate BRCA results were evaluated, because these findings may be beneficial to (...) patients and their families. Communicating individual BRCA results was not anticipated during clinical trial design. Consequently, options were not available for patients to indicate their preference for receiving their individual results when they signed pharmacogenetic informed consent. Differences in local requirements, clinical practice, and opinion regarding the ethical aspects of how to convey genetic results to patients are all potential barriers to returning individual BRCA results to patients. Communicating the aggregate BRCA result from this study provided clinical investigators with a mechanism to disseminate the overall study finding to patients while taking individual circumstances, local guidelines and clinical practice into account. This study illustrates the importance of increasing the clarity and scope of informed consent and the need for patient engagement to ensure clinical trial participants can indicate their preference regarding receipt of potentially important individual pharmacogenetic results. A new translation and edition of Aristotle's Protrepticus (with critical comments on the fragments) -/- Welcome -/- The Protrepticus was an early work of Aristotle, written while he was still a member of Plato's Academy, but it soon became one of the most famous works in the whole history of philosophy. Unfortunately it was not directly copied in the middle ages and so did not survive in its own manuscript tradition. But substantial fragments of it have been preserved in several (...) works by Iamblichus of Chalcis, a third century A.D. neo-Pythagorean philosopher and educator. On the basis of a close study of Iamblichus' extensive use and excerption of Aristotle's Protrepticus, it is possible to reconstruct the backbone of the lost work, and then to flesh it out with the other surviving reports about the work from antiquity (for example in Alexander of Aphrodisias and other ancient commentators on Aristotle). It is also possible to identify several papyrus fragments of the work, and many references and literary allusions in later authors, especially Cicero, whose own lost dialogue Hortensius was a defense of philosophy modeleld on Aristotle's. Across languages, we find indefinites that overtly mark a speaker’s lack of knowledge: they signal that the speaker is unable to give any further information about who or what satisfies her existential claim (Becker, 1999; Haspelmath, 1997). From now on, we will refer to the marking of the speaker’s lack of knowledge as an epistemic effect and to the indefinites that induce epistemic effects as epistemic indefinites.

Psychologists have traditionally approached phenomenology by describing perceptual states, typically in the context of vision. The control of actions has often been described as 'automatic', and therefore lacking any specific phenomenology worth studying. This article will begin by reviewing some historical attempts to investigate the phenomenology of action. This review leads to the conclusion that, while movement of the body itself need not produce a vivid conscious experience, the neural process of voluntary action as a whole has distinctive phenomenological consequences. (...) The remainder of the article tries to characterise this phenomenology. First, the planning of actions is often conscious, and can produce a characteristic executive mode of awareness. Second, our awareness of action often arises from the process of matching what we intended to do with what actually happened. Failures of this matching process lead to particularly vivid conscious experience, which we call 'error awareness'. These features of action phenomenology can be directly related to established models of motor control. This allows an important connection between phenomenology and neuroscience of action. Third, whereas perceptual phenomenology is normally seen as caused or driven by the sensory stimulus, a much more fluid model is required for phenomenology of action. Several experimental results suggest that phenomenology of action is partly a post hoc reconstruction, while others suggest that our awareness of action represents an integration of several processes at multiple levels of motor processing. Fourth, and finally, studies of the phenomenology of action, unlike those of perception, show a strong linkage between primary awareness and secondary awareness or self-consciousness: awareness of action is specifically and inextricably awareness of my action. We argue that the concepts of agency and of proprioaction are fundamental to this linkage. For these reasons, action represents a much more promising field than perception for attacking the problematic question of the relation between primary and secondary consciousness. For evolutionary biologists, the concept of chance has always played a significant role in the formation of evolutionary theory. As far back as Greek antiquity, chance and "luck" were key factors in understanding the natural world. Chance is not just an important concept; it is an entire way of thinking about nature. Charles Darwin's "Historical Sketch" has appeared as a preface to nearly every authorized edition of Darwin's Origin of Species since the second English edition was published in 1860. The "Historical Sketch" provides a brief history of opinion about the species question as a prelude to Darwin's own independent contribution to the subject, but its provenance is somewhat obscure. While some previous thinkers anticipated portions of Darwin's theory long before he did, none of them saw the complete picture as clearly as (...) Darwin. As such, he was able to claim originality and priority for the idea that has transformed our understanding of nature. His "Historical Sketch" was written as an attempt to address these issues. Some things are known about its production, such as when it first appeared and what changes were made to it between its first appearance in 1860 and its final form in 1866. Other questions remain unanswered. How did it evolve in Darwin's mind? Why did he write it at all? What did he think he was accomplishing by prefacing it to Origin of Species? Curtis Johnson approaches these questions, offering some clarity on the originality of Darwin's work. Darwin's "Historical Sketch" is the first comprehensive study of Darwin's "Preface" to Origin of Species. Johnson conveys the pressure Darwin felt from friends and other correspondents to showcase the originality of his theory, and he tackles questions of originality by carefully examining the 35 authors Darwin referenced in this monumental text. Pursuing Justice in Africa focuses on the many actors pursuing many visions of justice across the African continent--their aspirations, divergent practices, and articulations of international and vernacular idioms of justice. The essays selected by editors Jessica Johnson and George H. Karekwaivanane engage with topics at the cutting edge of contemporary scholarship across a wide range of disciplines. Neste artigo apresentamos uma perspectiva de compreensão da história da filosofia, na qual é possível acessar outros modos de como essa história pode ser interpretada. Nesta perspectiva ocorre a desconstrução epistêmica eurocentrada de legitimação da ideia de um “milagre” grego que permite o surgimento da filosofia nessa região em detrimento a outros lugares. O pressuposto indicado é o da pluriversalidade de Mogobe Ramose (2011) que apresenta a ideia de pensamento reflexivo e crítico como condição humana, independente de local. Em relação (...) ao continente africano trazemos a crítica de que a escravidão moderna promovida pela Europa desumanizou africanos e indígenas e, com isso, lhes nega a capacidade de pensamento. Após essa análise, apresentamos a filosofia afro-brasileira como um desdobramento da filosofia africana em seu deslocamento do continente africano para as Américas no período da escravidão e colonização. Apresentamos as bases epistêmicas dessa filosofia mantidas nas culturas afro-brasileiras. Destacamos, também, os seus subsídios corporais, salientando a sua dimensão ética e estética ao propor outros modos de se relacionar em que o coletivo é a alternativa ao individualismo, no qual se destaca uma ética comunitária e solidária. As narrativas constituídas no processo colonizador da modernidade são revistas em seus pressupostos de supremacia para dar lugar a uma filosofia que alcance o outro, que se estabeleça na alteridade. Tal proposta nos parece relevante para o ensino formal de filosofia, pois dialoga diretamente com a realidade de mundo existencial formativa do país, e aponta outras possibilidades de organização da vida em distintos aspectos, mas todos eles perpassados pela educação, neste caso, uma educação para ser e estar com o outro, o que solicita a revisão curricular na grade da filosofia, tanto na formação de professores quanto na disciplina destinada ao ensino médio. Climate change exposes ecological and socio-economic systems to risks. The identified disparities in knowledge about the social climate system are at the root of the difficulties in perceiving and understanding the diversity of risks related to climate change. The still huge gap between what science and technological innovation can contribute to mitigation and what is unmanageable by humans inevitably requires a continuous process of adaptation. This work is part of the research associated with the European project (under the ERA4CS) 'Citizen (...) Sensing - Urban climate resilience through participatory risk management systems', which proposes to develop a Participatory Climate Risk Management System, a two-way communication system where is part of a participatory risk management system, contributing to adaptive governance. These objectives will be achieved through the creation of a platform that will incorporate information gathered from four pilot studies: Norrköping (Sweden), Porto (Portugal), Rotterdam (The Netherlands) and Trondheim (Norway). Ultimately, the aim is to draw conclusions on how this application can contribute to improving citizens' literacy to cope with climate change adaptation and to raise awareness of this problem, as the involvement of citizens and stakeholders will only be improved if the perception of the risk of climate change is substantially increased. Awareness campaigns about the potential risks associated with extreme events are of great importance. Citizens tend to view climate change as a global phenomenon, ignoring and avoiding taking action until it is directly and seriously affected. (shrink)

A hipótese do artigo é que o dilema da constituição da representação política, que não tem por função refletir, mas interferir e operar uma unidade de “vontade política” a um povo ou uma nação, foi enfrentado pelos primeiros pensadores da representação moderna em diferentes contextos de grande transformação política e social: na Inglaterra absolutista no século XVII, por Thomas Hobbes, e na França pós-revolucionaria no século XVIII, pelo Abade Sieyès. O artigo procura identificar nos argumentos dos respectivos filósofos os primeiros (...) esforços teóricos sobre a representação moderna, através de uma comparação que evidencia dilemas e constrangimentos semelhantes em diferentes contextos teóricos. Um dos principais desafios da representação política, nos dois autores, permanece sendo a busca dos mecanismos capazes de transformar vontades dispersas e múltiplas de indivíduos numa só vontade unificada do corpo social. Embora os argumentos de legitimidade possam variar, para ambos os autores, a “vontade” do representante político é produto de uma elaboração conceitual e de uma produção racional que se dá com a constituição de um corpo, que no caso de Hobbes se dá com a criação do Estado, e em Sieyès com a representação nacional. A collection of classical and contemporary sources highlighting the radical potential of the individualist anarchist tradition.

The importance of history for religious ethics lies in the fact that, in religious communities existing over time, values are encountered in history, given forms dependent on the historical experience of the believing community, and recalled by the individual moral agent through memory in the context of participation in that community. This paper has to do with the nature of that memory and its implications for moral identity. Specifically, I utilize the concept of "significant history," derived from Gordon Kaufman's notion (...) of the "living past," arguing that moral identity is defined by memory of such history, present to the self in two aspects, individual and communal. The historical task of the religious ethicist, on this view, is to render faithfully both his own significant history and that of his community of faith. With the problem thus defined, I analyze several attempts to solve it, attempting to show how a religious ethical tradition ought to be treated as normative for contemporary analysis and decision-making. The similarity of documents in a large database of published Fractals articles was examined for redundancy. Three different text matching techniques were used on published Abstracts to identify redundancy candidates, and predictions were verified by reading full text versions of the redundancy candidate articles. A small fraction of the total articles in the database was judged to be redundant. This was viewed as a lower limit, because it excluded cases where the concepts remained the same, but the text was altered (...) substantially. Far more pervasive than redundant publications were publications that did not violate the letter of redundancy but rather violated the spirit of redundancy. There appeared to be widespread publication maximization strategies. Studies that resulted in one comprehensive paper decades ago now result in multiple papers that focus on one major problem, but are differentiated by parameter ranges, or other stratifying variables. This ‘paper inflation’ is due in large part to the increasing use of metrics (publications, patents, citations, etc) to evaluate research performance, and the researchers’ motivation to maximize the metrics. En la segunda sección, analizaré el principal problema del dialeteísmo. En la tercera sección, mostraré que los argumentos a favor del dialeteísmo no llegan a establecer la verdad de esta posición. Dialetheism is the position which claims that there are true contradictions. This paper deals with this position. In the first section, I will mention the main contributions that, in my perspective, dialetheism has made to philosophical logic. In the second section, I will analyze the main problem of dialetheism. In the third section, I will show that the arguments in favor of dialetheism are not conclusive. In a close reading of select graphic memoirs such as Jenell Johnson’s Present/perfect, Paula Knight’s The Facts of Life, Phoebe Potts’ Good Eggs, and Diane Noomin’s Baby Talk, this article examines how the authors use comics to foreground their predicament.
