The focus on translational research in clinical trials has the potential to generate clinically relevant genetic data that could have importance to patients. This raises challenging questions about communicating relevant genetic research results to individual patients. An exploratory pharmacogenetic analysis was conducted in the international ovarian cancer phase III trial, AGO-OVAR 16, which found that patients with clinically important germ-line BRCA1/2 mutations had improved progression-free survival prognosis. Mechanisms to communicate BRCA results were evaluated, because these findings may be beneficial to (...) patients and their families. Communicating individual BRCA results was not anticipated during clinical trial design. Consequently, options were not available for patients to indicate their preference for receiving their individual results when they signed pharmacogenetic informed consent. Differences in local requirements, clinical practice, and opinion regarding the ethical aspects of how to convey genetic results to patients are all potential barriers to returning individual BRCA results to patients. Communicating the aggregate BRCA result from this study provided clinical investigators with a mechanism to disseminate the overall study finding to patients while taking individual circumstances, local guidelines and clinical practice into account. This study illustrates the importance of increasing the clarity and scope of informed consent and the need for patient engagement to ensure clinical trial participants can indicate their preference regarding receipt of potentially important individual pharmacogenetic results. There is sometimes a sense in which one theory posits ‘less structure’ than another. Philosophers of science have recently appealed to this idea both in the debate about equivalence of theories and in discussions about structural parsimony. But there are a number of different proposals currently on the table for how to compare the ‘amount of structure’ that different theories posit. The aim of this paper is to compare these proposals against one another and evaluate them on their own merits.

Our world is complex—it is composed of many interacting parts—and this complexity poses a serious difficulty for theorists of social reform. On the one hand, we cannot merely work out ways of ameliorating immediate problems of injustice, because the solutions we generate may interact to set back the achievement of overall long-term justice. On the other, we cannot supplement such problem solving with theorizing about how to make progress towards a long-term goal of ideal justice, because the very interactions that (...) render problem solving unsatisfactory raise insurmountable epistemic difficulties for this latter approach. To accommodate complexity, we must therefore give up on ideal theory, and instead supplement problem solving with a new sort of theorizing that aims to work out how to make our institutional arrangements more progressive: more conducive to further improvements in general, though not necessarily to the achievement of any antecedently specified goal. More concretely, we must identify ways of improving our capacity to flexibly experiment with many promising solutions to problems as they arise, to select for those solutions that prove successful while eliminating those that do not, and to learn from both our successes and our inevitable failures. This paper presents a simple pair of first-order theories that are not definitionally (nor Morita) equivalent, yet are mutually conservatively translatable and mutually 'surjectively' translatable. We use these results to clarify the overall geography of standards of equivalence and to show that the structural commitments that theories make behave in a more subtle manner than has been recognized.

Transnational higher education, where students study on a ‘foreign’ degree programme whilst remaining in their home country, has seen exponential development over the last decade. In addition to the increase in students engaged in TNHE across the globe, the involvement of university teachers in TNHE has also risen in response to the demand for this form of international education. Although research into transnational education has doubled since 2006, there is a paucity of research focusing on transnational teacher education, especially outside (...) of North America. The global nature and scope of the expansion of TNHE remains underexplored, and the ways in which different countries are realising TNHE provision is little understood. This book explores the experiences and perceptions of teachers in transnational higher education, interrogating the ways in which university teachers negotiate cultural, linguistic, and disciplinary contexts in order to provide transformative learning experiences for their students. In exploring skilled performance in Contact Improvisation, we utilize an enactive ethnographic methodology combined with an interdisciplinary approach to examine the question of how skill develops in CI. We suggest this involves the development of subtleties of awareness of intra- and interkinaesthetic attunement, and a capacity for interkinaesthetic negative capability—an embodied interpersonal ‘not knowing yet’—including an ease with being off balance and waiting for the next shift or movement to arise, literally a ‘playing with’ balance, falling, nearly falling, momentum and (...) gravity. We draw on insights from an interdisciplinary approach, including from a developmental perspective concerning the experience of dyadic interpersonal embodied skill development in both infancy and CI. Building on Ravn and Høffding’s definition of expertise in improvisation as an “oscillatory process of assuming and relinquishing agency” we propose that a key aspect of expertise in CI involves oscillation between levels and processes of interkinaesthetic sense of agency. These interdisciplinary insights also elucidate limitations within current conceptualisations of sense of agency, including the relationship between sense of agency and sense of control. (shrink)

The aim of this paper is to examine Putnam’s proof and show that it is not compelling. This paper analyzes the relationship between love and social justice activism, focusing in particular on ways in which activists rely on either the union account of love (to argue that when one person is oppressed everyone is oppressed), the sentimentalist account of love (to argue that overcoming injustice is fundamentally about how we feel about one another), or love as fate (to argue that it is in love’s nature to triumph over hatred and injustice). All three accounts, while understandable and (...) attractive, are seriously problematic, as they tend either to obscure important differences in the ways that various groups are socially situated or to enable inaction by trusting that justice is inevitable. This book provides original descriptive accounts of two schools of thought in the philosophy of beauty: the 20th-century "Anti-Aesethetic" movement and the 21st-century "Beauty Revival" movement. It also includes a positive defence of beauty as a lived experience extrapolated from Beauty-Revival position. Beauty was traditionally understood in the broadest sense as a notion that engages our sense perception and embraces everything evoked by that perception, including mental products and affective states. This book constructs and places in parallel with one another (...) the Anti-Aesthetic and Beauty-Revival movements. In the author's view, Anti-Aestheticism is devoted to a decisive negation of beauty-denying its importance as a philosophical notion and its significance as a lived experience. It suggests that beauty is a merely sensual experience, which can be used, at best, as a distraction from justice and, at worst, as an instrument of evil. Alternatively, the Beauty-Revival movement advances arguments for beauty as an experience that extends primarily to sensual experience, but which also calls forth mental products and cognitive and affective states evoked by that experience. After reconstructing these two positions, the author elaborates on the notion of beauty as a lived experience through three key moments which occur in the process of our experiencing beautiful objects. These moments are (a) the conditions that constitute an experience of beauty, (b) the attitudinal features most likely to lead to the experience of beauty, and (c) the results of the experience of beauty. This article explores some implications of the increasing reliance on clinical trials in contemporary health care, particularly health care payers’ efforts to use them in the so-called fourth hurdle decisions. How do these agencies manage medical uncertainty given the desire to produce clear guidelines for clinicians? Their solutions take account of trials in at least two ways, reflecting broader debates about the meaning of these medical experiments. Trials can be read as either ‘‘proofs of protocol’’—straightforward guides to action with individual (...) drugs in specific populations—or ‘‘proofs of principle’’— where extrapolation is made possible through an appeal to underlying biological mechanisms. These contrasting readings of trials are illustrated with reference to guidelines on heart disease prevention/cholesterol reduction using statins among the elderly in North America and the United Kingdom. Uncertainty in these cases does not lead to inertia but solutions use different fixed points to aid navigation, including both physiological principles and moral values. (shrink)

Eduardo Frei Montalva, co-founder of the Christian Democratic Party and President of Chile, represented for Jacques Maritain, French neo-Thomist philosopher, an example of prophetic leadership in contemporary times. According to Maritain, modem democracy could not survive without a profound spiritual revolution of political leadership--the "prophetic factor" of democracy--which he observed in Frei as a public official, senator, and ultimately the Presient of the Republic of Chile (1964-1970). Under his famed "Revolution in Liberty," Frei endeavored to meld socio-economic reforms with an (...) effort to build a more participatory democratic culture in his native land. Guided by Maritain's political philosophy, Frei's initiatives set into motion the possibility of a "third way" of politics in the Southern hemisphere. In the end, this revolution ended in political disappointment due to economic stagnation, social disruption, political infighting, and the impractical idealism of Christian democracy itself. Struck by the absence of love affairs, adventures, travels, and political engagement in Immanuel Kant's life, a noted commentator describes him as unformed, to a degree surpassing all other philosophers, by challenging life events. Declaring that Kant 'can be understood only through his work in which he immerses himself with unwavering discipline,' the writer evokes the image of a body of writing demanding to be understood through text-internal analytical methods alone. The theme of the enclosed Kantian text is virtually irresistible. (...) It dominates in teaching practice and in a large percentage of the expository literature, where Kant's ideas are paraphrased in more, or even less transparent prose. It is attributable to the fact that Kant is a difficult author, a fact that, despite his scorn for popular philosophy, he knew and to some extent regretted. The commentator too is apt to immerse him or herself in Kant's writings with unwavering discipline, leaving little time and energy for a study of Kant's surrounding context. Like Wordsworth's Isaac Newton, whose innate powers enable him to teach the truth to himself, Kant is seen as a walled-off genius whose innovations nevertheless reached to the whole world. But Kant's famous domesticity and addiction to routine did not preclude contact with an external world. His mind was formed--as was Newton's, as is that of any one of us-- by his encounters with books and essays, by his exchanges with correspondents and dinner guests, from whom he learned and by whom he was provoked and challenged. The name index of the Academy Edition of Kant's works and the range of authors in the catalogue of Kant's library books published by Arthur Warda in 1922 leave no doubt as to the breadth of his personal and literary acquaintances. Leibniz’s metaphysics has been cited as a source of the dynamic and organic worldview of romantic Naturphilosophie. This chapter evaluates that claim by examining two distinct lineages of Leibniz’s metaphysical conception of dynamic appetition. On one hand, by demonstrating the existence of a “vis viva” in inanimate objects and by ascribing two distinct powers—perception and appetition—to all plants and animals as well as to his incorporeal “monads,” Leibniz seemed to restore force to physics and experience and intentionality to animals. On (...) the other hand, on account of the publication history of his writings, Leibniz’s pre-established harmony and preformation theory were seen by most philosophers before Herder and Schelling as reducing humans as well as animals to machines without free-will and their development to a mechanically predetermined succession of states. While appreciation of Leibniz’s views on activity and creativity in nature did increase towards the end of the century, other philosophical antecedents for the “drive” motif in German philosophy should be considered. In this article, we examine how the demonstrative that may be used to notice an event in the world in such a way as to suggest it has highly unusual or transgressive properties and in so doing invite others to align with that implicit claim. Drawing on Freud’s notion of the uncanny, we examine instances of the transgressive that in circumstances in which participants at least entertain the possibility that they are experiencing anomalous or paranormal objects and entities. The analysis (...) outlines the basic features of the transgressive that and examines how it may be the vehicle for a range of delicate interpersonal activities. Throughout the article, we try to show the broader relevance of the analytic argument by reference to data from more mundane interactional settings and to other forms of discourse, including fiction, commentary and interview narrative.
