Background: Managing HIV treatment is a complex multi-dimensional task because of a combination of factors such as stigma and discrimination of some populations who frequently get infected with HIV. In addition, patient-provider encounters have become increasingly multicultural, making effective communication and provision of ethically sound care a challenge. Purpose: This article explores ethical issues that health service providers in the United States and Botswana encountered in their interaction with patients in HIV care. Research design: A descriptive qualitative design was used (...) to collect data from health service providers and patients using focused group discussions. This article is based on responses from health service providers only. Participants and context: This article is based on 11 focused group discussions with a total sample of 71 service providers in seven US sites and one Botswana site. Ethical considerations: Ethical review boards at all the study sites reviewed the study protocol and approved it. Ethical review boards of the study’s coordinating centers, Rutgers University and the University of California at San Francisco, also approved it. The study participants provided a written informed consent to participate. Findings: HIV service providers encountered ethical challenges in all the four Beauchamp and Childress’ biomedical ethics of respect for patients’ autonomy, beneficence, justice, and nonmaleficence. Discussion: The finding that HIV service providers encounter ethical challenges in their interaction with patients is supported by prior studies. The ethical challenges are particularly prominent in multicultural care and resource-constrained care environments. Conclusion: Provision of HIV care is fraught with ethical challenges that tend to pose different issues depending on a given care environment. It is important that strong partnerships are developed among key stakeholders in HIV care. In addition, health service providers need to be provided with resources so they can provide quality and ethically sound care. (shrink)

In Facing the Future, Belnap et al. reject bivalence and propose double time reference semantics to give a pragmatic response to the following assertion problem: how can we make sense of assertions about future events made at a time when the outcomes of those events are not yet determined? John MacFarlane employs the same semantics, now bolstered with a relative-truth predicate, to accommodate the following apparently conflicting intuitions regarding the truth-value of an uttered future contingent: at the moment of utterance, (...) if asked to evaluate the truth-value of the asserted future contingent one has the intuition that the assertion is neither true nor false, yet later, at the moment of the predicted event, one has the intuition that the assertion was, already, either true or false. Both MacFarlane and Belnap assume that assertions of future contingents have complete propositional content – the traditional propositional contents that, according to him ‘are the contents of assertions and beliefs’. We investigated the interface between biomedia and humans by inviting participants to interact with biometric devices that measured and visualized their body data. At first, they struggled with the alienating and disembodying nature of the devices and the constrained, reductionist representation of data. Through their bodily interactions with these devices, however, participants reframed the data and inserted their bodies into the process of data collection. Drawing on the ideas of Bergson, Grosz, Merleau-Ponty and Bachelard, we argue that by working with (...) their subjectivity in a mediated process of becoming, participants ‘filled in the intervals’ of the visual representations of their bodies to interpret them in ways that marked the duration and meaning of their selves. We conclude that even when presented with artificial representations, individuals convert the representation of the data into narratives inspired by their embodied experience, and the ‘virtual’ pasts of their own lives. We examine the concept of personhood in relation to people living with dementia and implications for the humanity of care, drawing on a body of ethnographic work. Much debate has searched for an adequate account of the person for these purposes. Broad contrasts can be made between accounts focusing on cognition and mental faculties, and accounts focusing on embodied and relational aspects of the person. Some have suggested the concept of the person is critical for good care; others suggest the (...) vexed debates mean that the concept should be abandoned. We argue instead that the competing accounts illuminate the very tensions in personhood which are manifest for all of us, but especially for people living with dementia, and argue that our account has explanatory power in shedding light on how precisely dehumanisation and constraints on agency may arise for people living with dementia, and for staff, within an institutional context. Background Although clinical ethicists are becoming more prevalent in healthcare settings, their required training and education have not been clearly delineated. Most agree that training and education are important, but their nature and delivery remain topics of debate. One option is through completion of a clinical ethics fellowship. Method In this paper, the first four fellows to complete a newly developed fellowship program discuss their experiences. They describe the goals, structure, participants and activities of the fellowship. They identify key elements (...) for succeeding as a clinical ethicist and sustaining a clinical ethics program. They critically reflect upon the challenges faced in the program. Results The one-year fellowship provided real-time clinical opportunities that helped them to develop the necessary knowledge and skills, gain insight into the role and scope of practice of clinical ethicists and hone valuable character traits. Conclusion The fellowship enabled each of the fellows to assume confidently and competently a position as a clinical ethicist upon completion. (shrink)

In "Saving One Another: Philodemus and Paul on Moral Formation in Community" Justin Reid Allison compares how the Epicurean philosopher Philodemus and the Christian apostle Paul envisioned the members of their communities helping one another to grow into moral maturity. Allison establishes that Philodemus and Paul are more similar than previously noticed in their conception and practice of moral formation in community, and that these similarities offer a critical opportunity to consider important differences between the two as well. By (...) deepening the comparison to include differences alongside similarities, and to include theological and socio-economic facets of communal moral formation, Allison shows that Philodemus and Paul uniquely shed fresh light on one another's texts when understood in comparative perspective. The construction of difference is central to racism, sexism and other forms of oppression. This paper examines the similar and dissimilar ways in which race and gender have been constructed in the United States and analyzes the consequences of these differences in construction for the development of social policy and the growth and nature of movements for social change.

The aim of this paper is to analyze, with approximate order, two ethical positions. To achieve this goal, the text will look in their introductory considerations, the context and objectives of Kierkegaard´s Works of Love, since there is clearly expressed the ethics second the Danish author. In its first part, the focus of the article will consist in evaluating the rationality of ethics of duty especially through an investigation of goodwill and freedom, which is developed in the third section of (...) the Groundwork of the Metaphysics of Morals. In the second part, the paper will analyze the speech Kierkegaardian Thou shalt love, which is part of the Works of Love. Finally, in conclusion, be carried out approaches and pointed out the significant differences between the two thinkers. Here will be used more intensively, also the reflections of commentators from both authors. However, the main focus of the research will take place around the Kierkegaardian reflections, which will be evaluated in a more exhaustive. Reflection on Kant's moral philosophy does not go just the same, but rather seek to understand it in dialogue with the philosophy of Kierkegaard. (shrink)

This article compares Hume's metaphysical views with those of his contemporary, the American theologian and philosopher, Jonathan Edwards. It shows how, although the two men developed their theories in isolation from one another, their minds were nevertheless following almost identical paths on several of the most central issues in metaphysics (including the natures of body and mind, personal identity, causation, and free will). Their final conclusions were, however, radically different. In short, wherever Hume came to rest in a skeptical position, (...) Edwards would initially approach the very same position, but would then pull back at the last minute and bring in God to fill the gaps, yielding a Christian system of philosophy with an idiosyncratically Humean flavour. Although Lakoff and Johnson’s Conceptual Metaphor Theory has been influential across many disciplines, little research has tested the psychological reality of conceptual metaphors using established experimental memory paradigms. Here we employ an episodic memory task based on the Deese-Roediger-McDermott false memory paradigm to explore this possibility. We find that after reading lists of sentences based on underlying conceptual metaphors that participants are more likely to falsely remember the nonpresented conceptual metaphors themselves as well as new sentences consistent with the CM (...) mapping than control items that do not share this mapping. This finding provides experimental support for conceptual metaphors and highlights the utility of using episodic tasks to explore the assumptions of this theory. (shrink)

Systems of universal health coverage may aspire to provide care based on need and not ability to pay; the complexities of this aspiration call for normative analysis. This special issue arises in the wake of a judicial inquiry into preferential access in the Canadian province of Alberta, the Vertes Commission. I describe this inquiry and set out a taxonomy of forms of differential and preferential access. The inception of the current wave of the movement was in the American Board of Internal Medicine's Project Professionalism. En sus obras se dan cita de igual forma tanto la fotografía, el video y la instalación como la escritura autobiográfica, de bitácora y de correspondencia. A partir de la discusión teórica y conceptual sobre el giro textual y giro visual en la producción de pensamiento y de la dimensión autobiográfica en el arte, en este artículo me propongo explorar los modos en (...) que se reúnen y cruzan visualidad y escritura en la obra de Calle y cómo estas estrategias configuran un lugar inédito e híbrido —fronterizo e imperfecto— para el relato autobiográfico, repercutiendo en las formas con que el arte aborda uno de sus temas centrales, esto es, la presentación y la representación, y las nociones de verdad y ficción contenidas en ellas. Este trabajo aborda las diferentes reflexiones acerca de la libertad que se suscitaron en el contexto de la discusión que autores como Wellmer, Bohrer, Hamacher y Menke establecieron con la concepción habermasiana que entiende a la libertad como el resultado de un proceso de discusión y deliberación racional. A pesar de las diferencias que subyacen al pensamiento de cada uno de estos autores, en todos ellos se manifiesta la idea de que, la libertad exclusivamente concebida en relación a la decisión (...) racional, es una noción restringida. Estos autores coinciden en señalar la necesidad de incorporar a la noción de la libertad un momento espontáneo, no racionalmente controlable, asociado a la experiencia estética. Proponemos que ese carácter espontáneo, que estos autores asocian a lo estético, puede rastrearse en las filosofías de Adorno y Horkheimer como subyacente a otros ámbitos de la cotidianeidad. Theoretical paradigms, the frameworks within which thinking occurs, never capture the complexity of reality and are necessarily selective. Factors most important to understanding the phenomenon in question will be included and explained in a coherent, meaningful manner. But facts and ideas inconsistent with underlying assumptions may then appear less plausible, and, indeed, may be systematically overlooked or ignored. Health-related paradigms have practical importance because they influence what counts as a fact, what theories appear plausible and important, what research questions should (...) be pursued, and, therefore, what health-care interventions make sense. Apparaît dans cette phrase, comme dans l’histoire de la phénoménologie, l’espace d’une déhiscence au sein du corps vécu. Être son corps n’impliquerait pas que le corps soit entièrement soi – et le corps vécu, dès lors, ne serait plus un corps propre. D’où la question que nous tâcherons ici de déployer : l’aliénation au sein du (...) corps vécu n’est-elle pas la condition même de sa manifestation comme corps matériel? Nous tâcherons, dans un premier temps, de remonter à la source du concept de « corps propre » (Leib) chez les postkantiens, afin de montrer que le corps n’apparaît comme « propre », dans l’histoire de la philosophie, qu’en disparaissant comme « corps ». Ce qui nous conduira, dans un second temps, à interroger la (ré-) apparition du corps dans Le deuxième sexe, et plus précisément dans la description que Beauvoir fait du cycle menstruel. (shrink)

Guided by concepts from life history (LH) theory, a large human research literature has tested the hypothesis that exposures to extrinsic mortality (EM) promote the development of faster LH strategies (e.g., earlier/faster reproduction, higher offspring number). A competing model proposes that, because EM in the past was intimately linked to energetic constraints, such exposures specifically led to the development of slower LH strategies. We empirically address this debate by examining (1) LH variation among small-scale societies under different environmental conditions; (2) (...) country-, regional- and community-level correlations between ecological conditions, mortality, maturational timing, and fertility; (3) individual-level correlations between this same set of factors; and (4) natural experiments leveraging the impact of externally-caused changes in mortality on LH traits. Partially supporting each model, we found that harsh conditions encompassing energetic stress and ambient cues to EM (external cues received through sensory systems) have countervailing effects on the development of LH strategies, both delaying pubertal maturation and promoting an accelerated pace of reproduction and higher offspring number. We conclude that, although energetics are fundamental to many developmental processes, providing a first tier of environmental influence, this first tier alone cannot explain these countervailing effects. An important second tier of environmental influence is afforded by ambient cues to EM. We advance a 2-tiered model that delineates this second tier and its central role in regulating development of LH strategies. Consideration of the first and second tier together is necessary to account for the observed countervailing shifts toward both slower and faster LH traits. (shrink)

Abstract:Accepting the controversial thesis that Aristotle is an ethical egoist, I argue that Aristotle's brand of ethical egoism is immune to four important objections to such a position. I analyse the causes of the four objections showing (a) that Aristotle does not have a conception of happiness which would give rise to the first three, and (b) that his account of practical reasoning, of which I provide an original interpretation, can deal with the fourth.

According to the 2011 Australian Census, Catholics made up just over a quarter of the Australian population: there were 5,439,268 Catholics in a total Australian population of 21,507,719. In the five years between the 2006 and 2011 Censuses, the number of Catholics increased by over 312,000, or 6.1 per cent. During the same period, the total Australian population increased by 8.3 per cent.
