A collection of personal narratives and essays, Living Professionalism is designed to help medical students and residents understand and internalize various aspects of professionalism. These essays are meant for personal reflection and above all, for thoughtful discussion with mentors, with peers, with others throughout the health care provider community who care about acting professionally.

Diabetes mellitus (DM) is a colossal burden in global health, its prevalence increases steadily throughout the world. This systematic review explores the pathophysiology and epidemiology of diabetes, which describes the interactions that implicate that is the result of the play between genetic, environmental, and lifestyle factors in the development and progression of the disease. The review also describes a plethora of pharmacological treatments available to manage diabetes, with the oral agent, insulin regimens, and new approaches to therapy. Integrating the wisdom (...) of the best clinical guidelines, those are from American diabetes association (ADA), European association for the study of diabetes (EASD), and International Diabetes Federation (IDF). Since these reviews are consistent with normative standards for evidence-based recommendations of optimal care of diabetes. The review goes further to cover technological advances in the management of diabetes. They include Continuous Glucose Monitoring, advanced insulin delivery systems, and Artificial Pancreas Systems. Such technologies hold more promising routes to better glucose control and patient quality of life. All this comes along with challenges that their deployment and maintenance that run through economic, adherence issues, and unequal access to healthcare. The future research and innovation in the battle against DM will include the application of personalized medicine and the development of advanced digital health solutions. This article will become a treasure for health professionals, researchers, and policymakers who have dedicated themselves to battling the adversities of diabetes mellitus. (shrink)

BackgroundThe ARRIVE (Animal Research: Reporting of In Vivo Experiments) guidelines are widely endorsed but compliance is limited. We sought to determine whether journal-requested completion of an ARRIVE checklist improves full compliance with the guidelines.MethodsIn a randomised controlled trial, manuscripts reporting in vivo animal research submitted to PLOS ONE (March–June 2015) were randomly allocated to either requested completion of an ARRIVE checklist or current standard practice. Authors, academic editors, and peer reviewers were blinded to group allocation. Trained reviewers performed outcome adjudication (...) in duplicate by assessing manuscripts against an operationalised version of the ARRIVE guidelines that consists 108 items. Our primary outcome was the between-group differences in the proportion of manuscripts meeting all ARRIVE guideline checklist subitems.ResultsWe randomised 1689 manuscripts (control: n = 844, intervention: n = 845), of which 1269 were sent for peer review and 762 (control: n = 340; intervention: n = 332) accepted for publication. No manuscript in either group achieved full compliance with the ARRIVE checklist. Details of animal husbandry (ARRIVE subitem 9b) was the only subitem to show improvements in reporting, with the proportion of compliant manuscripts rising from 52.1 to 74.1% (X2 = 34.0, df = 1, p = 2.1 × 10−7) in the control and intervention groups, respectively.ConclusionsThese results suggest that altering the editorial process to include requests for a completed ARRIVE checklist is not enough to improve compliance with the ARRIVE guidelines. Other approaches, such as more stringent editorial policies or a targeted approach on key quality items, may promote improvements in reporting. (shrink)

Responding to a major pandemic and planning for allocation of scarce resources under crisis standards of care requires coordination and cooperation across federal, state and local governments in tandem with the larger societal infrastructure. Maryland remains one of the few states with no state-endorsed ASR plan, despite having a plan published in 2017 that was informed by public forums across the state. In this article, we review strengths and weaknesses of Maryland’s response to COVID-19 and the role of the Maryland (...) Healthcare Ethics Committee Network in bridging gaps in the state’s response to prepare health care facilities for potential implementation of ASR plans. Prior research suggests that the presence of high-quality auditors constrains accrual-based earnings management, but it inadvertently leads to higher real activities manipulation. We investigate whether such trade-off exists between accrual-based and real earnings management activities in the presence of female or male auditors. We use a sample of UK firms for the period 2009 to 2016 and find that firms audited by female auditors do not resort to a higher-level real activities manipulation when their ability to engage in accruals management (...) is constrained. Overall, our results suggest that the benefits of hiring female auditors are overwhelmingly higher than the costs they might bring to the client firms. (shrink)

Algerian universities rely on total quality management. TQM is one of the most successful strategic options for improving the quality of higher education. In addition, achieving academic accreditation and progress in international rankings. The study aims to address relevant contemporary issues by examining the impact of total quality management on the quality of higher education. The data were analyzed using a mixed-method approach; the study was done as a survey, with data collected via questionnaires issued to 610 students. The questionnaire (...) included Likert scale items that were quantitatively evaluated and modeled using structural equation modeling using Amos to accomplish the path analysis of the research model. Furthermore, qualitative data were acquired through interviews with 24 professors who are members of the Quality Cells, and qualitative data were evaluated using content analysis with NVivo. The study findings revealed that TQM has a direct and significant impact on the quality of graduates, scientific research, and community service in Algerian universities. In this work, a virtual submerged floating operational system based on parallel and serial robotic platforms is proposed. The primary aim behind its development lies in carrying out simulated underwater manipulation experiments in an easier and safer way. This VSFOS is consisted of a six-degree-of-freedom parallel platform, an ABB serial manipulator, an inertial sensor, and a real-time industrial computer. The 6-DOF platform is used to simulate the movement of an underwater vehicle, whose attitude is measured by the inertial sensor. The (...) ABB manipulator, controlled by the real-time industrial computer, works as an operational tool to perform underwater manipulation tasks. In the control system architecture, software is developed to receive the data collected by the inertial sensor, to communicate and send instructions. Furthermore, the real-time status of the manipulator is displayed in this software. To validate the proposed system, two experiments have been conducted to test its performance. In the first experiment, the test is carried out to check the communication function of VSFOS, while in the second one, the manipulator is intended to follow the movement of the parallel platform and perform simulated operational task in the space. The obtained results from these two experiments show clearly the effectiveness and the performance of the proposed VSFOS. (shrink)

Objective To develop an ethically, legally, and clinically appropriate ventilator allocation policy for AdventHealth Tampa and AdventHealth Carrollwood in Tampa, Florida, which could be enacted swiftly during the Covid-19 pandemic. Methods During Spring 2020, a subcommittee of the Medical Ethics Committee established consensus on the fundamental principles of the policy, then built on existing ethical, legal, and clinical guidance. Results The plan was finalized in May 2020. The plan triages patients based on exclusion criteria (imminent mortality), prognosis and expected benefit (...) of ventilation (using the Sequential Organ Failure Assessment), and change in prognosis over time. Decisions are made by committee in order to minimize moral distress among individual patient care providers. Conclusions Due to international concerns about healthcare resource shortages during the Covid-19 pandemic, hospitals need allocation policies informed by the crisis standard of care, the hospital’s ethical duty to plan for an emergency, and federal civil rights laws Policy Implications: This type of policy can serve as a model for other institutions to develop crisis standards of care resource allocation policies, which are a necessary component of disaster planning. The current study aimed at identifying the effect of strategic control on customer satisfaction with e-marketing in the Kingdom of Saudi Arabia. The study used the descriptive approach. A questionnaire was prepared and administered to a sample of (315) customers in the Kingdom of Saudi Arabia. The study found a statistically significant correlation at the significance level of (0.05) between all strategic control dimensions, customer satisfaction with e-marketing, and customer approval of the positivity towards the practices and advantages of strategic (...) control in e-marketing. There is also a statistically significant effect at the significance level of (0.05) for strategic control dimensions: (management information systems, strategic control methods, strategic evaluation) on customers' satisfaction in e-marketing. The current study recommended the need to pay attention to the strategic control function and benefit from its philosophy and the benefits that accrue to supporting customer satisfaction with e-marketing, and raising the efficiency and awareness of employees about strategic control in an organized manner in order to build their capabilities to support and implement control mechanisms and make the necessary improvements. This study investigates how essential retailers responded to the COVID-19 pandemic through stakeholder communications. Based on a comprehensive text analysis of the corporate websites of the 20 largest U.S. essential retailers during the first 19 months of the crisis, we categorize the public health measures communicated by these retailers and assess how these retailers adapted their messaging to address the concerns of different stakeholders over time. This analysis allowed us to create a framework for understanding the flow of retailer/stakeholder communication (...) during a health crisis, highlighting the important role businesses can play in alleviating stakeholder concerns when public health is on the line. We discuss the implications of our findings for retailers and public policy, aiming to enhance preparedness and response for future health emergencies. (shrink)

BackgroundBurnout is a syndrome that results from stressors in the work environment that have not been successfully managed. The prevalence of burnout among schoolteachers was always controversial. COVID-19 pandemic added more stressors to teachers since they had to change their working styles in response to the pandemic lockdowns or curfews. In Saudi Arabia, the prevalence and determinants of burnout among school teachers were not measured by any other group during the COVID-19 pandemic stressors.MethodsA cross-sectional survey was conducted among 879 teachers (...) in the Jazan region, Saudi Arabia, using the Maslach Burnout Inventory, during April 2021. Multistage cluster random sampling was used.ResultsThe mean age of the participants was 41.4 years. Male teachers represented 52.6% of the participants while females represented 47.4%. Most teachers showed burnout symptoms. Consequences of burnout were observed, such as using psychotropic medications, absenteeism, lack of job satisfaction, and changing schools. Using the MBI scale, most teachers showed medium or high emotional exhaustion, low depersonalization, and low personal accomplishment.ConclusionMost teachers showed symptoms of burnout during the COVID-19 pandemic. Being an expert and ability to adapt to technology during the COVID-19 pandemic proved to effectively reduce burnout symptoms. Increasing incentives, early detection, and improving the work environment is recommended to diminish burnout consequences. (shrink)

The current study aimed at identifying the awareness level of artificial intelligence applications' risks among faculty members and its relation to the attitude towards digital culture at Imam Abdulrahman Bin Faisal University. The descriptive survey method was used. A questionnaire was designed to measure the awareness of the artificial intelligence applications' risks, and a scale for measuring the attitude towards digital culture. They were administered to a sample of [463] faculty members at Imam Abdulrahman Bin Faisal University. The study concluded (...) that there is an average awareness of the artificial intelligence applications' risks and the attitude towards digital culture among faculty members. A significant relationship was found out between awareness of the artificial intelligence applications' risks as a whole and the attitude towards digital culture. The results indicated that there are no differences in the awareness level of the artificial intelligence risks due to the variables of: gender, use duration of digital applications, and colleges. There are no differences in the variable of the attitude towards digital culture with its various dimensions: cognitive, psychomotor, and affective and its total score. The current study recommended designing a professional development plan for faculty members on the negative effects and risks of artificial intelligence, and launching an Arab platform based on artificial intelligence applications. The 3D Prandtl fluid flow through a bidirectional extending surface is analytically investigated. Cattaneo–Christov fluid model is employed to govern the heat and mass flux during fluid motion. The Prandtl fluid motion is mathematically modeled using the law of conservations of mass, momentum, and energy. The set of coupled nonlinear PDEs is converted to ODEs by employing appropriate similarity relations. The system of coupled ODEs is analytically solved using the well-established mathematical technique of HAM. The impacts of various physical parameters (...) over the fluid state variables are investigated by displaying their corresponding plots. The augmenting Prandtl parameter enhances the fluid velocity and reduces the temperature and concentration of the fluid. The momentum boundary layer boosts while the thermal boundary layer mitigates with the rising elastic parameter strength. Furthermore, the enhancing thermal relaxation parameter ) reduces the temperature distribution, whereas the augmenting concentration parameter drops the strength of the concentration profile. The increasing Prandtl parameter declines the fluid temperature while the augmenting Schmidt number drops the fluid concentration. The comparison of the HAM technique with the numerical solution shows an excellent agreement and hence ascertains the accuracy of the applied analytical technique. This work finds applications in numerous fields involving the flow of non-Newtonian fluids. (shrink)

Invalid journals are recent challenges in the academic world and many researchers are unacquainted with the phenomenon. The number of victims appears to be accelerating. Researchers might be suspicious of predatory journals because they have unfamiliar names, but hijacked journals are imitations of well-known, reputable journals whose websites have been hijacked. Hijacked journals issue calls for papers via generally laudatory emails that delude researchers into paying exorbitant page charges for publication in a nonexistent journal. This paper presents a method for (...) detecting hijacked journals by using a classification algorithm. The number of published articles exposing hijacked journals is limited and most of them use simple techniques that are limited to specific journals. Hence we needed to amass Internet addresses and pertinent data for analyzing this type of attack. We inspected the websites of 104 scientific journals by using a classification algorithm that used criteria common to reputable journals. We then prepared a decision tree that we used to test five journals we knew were authentic and five we knew were hijacked. (shrink)

We have measured the change of lung mechanical parameters on isolated rabbit lungs exposed to chlorine gas (Cl{in2}). Experimental results show parallel increase in elastance and resistance of impaired lungs. We tried to determine whether this may be explained by a reduction of the ventilated areas in the lung, consecutive to closure of some airways. We have then tried to simulate these experimental results by studying the effects of various airways occlusions imposed on two concurrent models (symmetrical and dissymmetrical) of (...) the tracheo-bronchial tree. For each model, we successively evaluated the resistance of the normal lung, simulated a partial peripheral airways occlusion and estimated the induced changes in total resistance. Analytical expressions of the "occluded lung" elastance and resistance have been found for the symmetrical model but not for the dissymmetrical model (a graphical approach is proposed). With the symmetrical model, simulated results are comparable to experimental ones when the occlusion level is proximal. Whatever the dissymmetry level (δ) of the fractal tree model, we could not simulate the expected increase in resistance with the observed increase in elastance. We conclude that either the occlusion is non homogeneous or the lung impairment is not only a reduction in ventilated areas. (shrink)

One popular view in recent years takes the source of testimonial entitlement to reside in the intrinsically social character of testimonial exchanges. This paper looks at two extant incarnations of this view, what we dub ‘weak’ and ‘modest’ social anti-reductionism, and questions the rationales behind their central claims. Furthermore, we put forth an alternative, strong social anti-reductionist account, and show how it does better than the competition on both theoretical and empirical grounds.

According to anti‐reductionism in the epistemology of testimony, testimonial entitlement is easy to come by: all you need to do is listen to what you are being told. Say you like anti‐reductionism; one question that you will need to answer is how come testimonial entitlement comes so cheap; after all, people are free to lie.This paper has two aims: first, it looks at the main anti‐reductionist answers to this question and argues that they remain unsatisfactory. Second, it goes on a (...) rescue mission on behalf of anti‐reductionism. I put forth a novel, knowledge‐first anti‐reductionist account, which I dub ‘Testimonial Contractarianism’. According to the view defended here, in virtue of the social contract in play, compliance with the norms governing speech acts is the default position for speakers. Insofar as norm compliance is the default for speakers, I argue, all else equal, entitlement to believe is the default for hearers. (shrink)

DNR decisions are frequently made in oncology and hematology care and physicians and nurses may face related ethical dilemmas. Ethics is considered a basic competence in health care and can be understood as a capacity to handle a task that involves an ethical dilemma in an adequate, ethically responsible manner. One model of ethical competence for healthcare staff includes three main aspects: being, doing and knowing, suggesting that ethical competence requires abilities of character, action and knowledge. Ethical competence can be (...) developed through experience, communication and education, and a supportive environment is necessary for maintaining a high ethical competence. The aim of the present study was to investigate how nurses and physicians in oncology and hematology care understand the concept of ethical competence in order to make, or be involved in, DNR decisions and how such skills can be learned and developed. A further aim was to investigate the role of guidelines in relation to the development of ethical competence in DNR decisions. Individual interviews were conducted with fifteen nurses and sixteen physicians. The interviews were analyzed using thematic content analysis. Physicians and nurses in the study reflected on their ethical competence in relation to DNR decisions, on what it should comprise and how it could be developed. The ethical competence described by the respondents related to the concepts being, doing and knowing. In order to make ethically sound DNR decisions in oncology and hematology care, physicians and nurses need to develop appropriate virtues, improve their knowledge of ethical theories and relevant clinical guidelines. Ethical competence also includes the ability to act upon ethical judgements. Continued ethical education and discussions for further development of a common ethical language and a good ethical working climate can improve ethical competence and help nurses and physicians cooperate better with regard to patients in relation to DNR decisions, in their efforts to act in the best interest of the patient. One very popular assumption in the epistemological literature is that belief and assertion are governed by one and the same epistemic norm. This paper challenges this claim. Extant arguments in defence of the view are scrutinized and found to rest on value-theoretic inaccuracies. First, the belief-assertion parallel is shown to lack the needed normative strength. Second, I argue that the claim that assertion inherits the norm of belief in virtue of being an expression thereof rests on a failed instance of (...) deontic transmission. Third, the inheritance argument from the norm for action is proven guilty of deontic equivocation. Last but not least, it is argued that, on a functionalist normative picture, assertion and belief are governed by different epistemic norms, in virtue of serving different epistemic functions. ABSTRACTSeveral philosophers have inquired into the metaphysical limits of conceptual engineering: ‘Can we engineer? And if so, to what extent?’. This paper is not concerned with answering these questions. It does concern itself, however, with the limits of conceptual engineering, albeit in a largely unexplored sense: it cares about the normative, rather than about the metaphysical limits thereof. I first defend an optimistic claim: I argue that the ameliorative project has, so far, been too modest; there is little value theoretic (...) reason to restrict the project to remedying deficient representational devices, rather than go on a more ambitious quest: conceptual improvement. That being said, I also identify a limitation to the optimistic claim: I show that the ‘should’ in ameliorative projects suffers from a ‘wrong-kind-of-reasons’ problem. Last but not least, I sketch a proposal of normative constraining meant to address both the above results. The proposal gives primacy to epistemic constraints: accordingly, a concept should be ameliorated only insofar as this does not translate into epistemic loss. (shrink)

This article conducts a comparative analysis of the seventeenth-century Iranian philosopher Mullā Ṣadrāʼs perspective on identity and change with that of the German philosopher, neuroscientist, and psychiatrist Georg Northoff. A key element of Mullā Ṣadrāʼs philosophy is the concept of substantial movement (al-ḥaraka aljawhariyya). By bridging neuroscientific considerations with Islamic philosophy, this study ventures into uncharted territory, presenting an innovative, interdisciplinary, and transcultural approach to the topic. The projectʼs uniqueness lies in its juxtaposition of Mullā Ṣadrāʼs and Northoffʼs perspectives, fostering (...) a framework for interdisciplinary discourse and cross-cultural dialogue. Bridging neuroscientific considerations with Mullā Ṣadrāʼs philosophy enhances our understanding of consciousness and allows for a deeper exploration of the relationship between mind and body. _The Promise of Happiness_ is a provocative cultural critique of the imperative to be happy. It asks what follows when we make our desires and even our own happiness conditional on the happiness of others: “I just want you to be happy”; “I’m happy if you’re happy.” Combining philosophy and feminist cultural studies, Sara Ahmed reveals the affective and moral work performed by the “happiness duty,” the expectation that we will be made happy by taking part in that which (...) is deemed good, and that by being happy ourselves, we will make others happy. Ahmed maintains that happiness is a promise that directs us toward certain life choices and away from others. Happiness is promised to those willing to live their lives in the right way. Ahmed draws on the intellectual history of happiness, from classical accounts of ethics as the good life, through seventeenth-century writings on affect and the passions, eighteenth-century debates on virtue and education, and nineteenth-century utilitarianism. She engages with feminist, antiracist, and queer critics who have shown how happiness is used to justify social oppression, and how challenging oppression causes unhappiness. Reading novels and films including_ Mrs. Dalloway_, _The Well of Loneliness_, _Bend It Like Beckham_, and _Children of Men_, Ahmed considers the plight of the figures who challenge and are challenged by the attribution of happiness to particular objects or social ideals: the feminist killjoy, the unhappy queer, the angry black woman, and the melancholic migrant. This article develops and defends a full account of the nature and normativity of resistance to evidence, according to which resistance to evidence is an instance of input-level epistemic malfunctioning. At the core of this epistemic normative picture lies the notion of knowledge indicators, as evidential probability increasing facts that one is in a position to know; resistance to evidence is construed as a failure to uptake knowledge indicators.

This paper develops a novel account of the nature of disinformation that challenges several widely spread theoretical assumptions, such as that disinformation is a species of information, a species of misinformation, essentially false or misleading, essentially intended/aimed/having the function of generating false beliefs in/misleading hearers. The paper defends a view of disinformation as ignorance generating content: on this account, X is disinformation in a context C iff X is a content unit communicated at C that has a disposition to generate (...) ignorance at C in normal conditions. I also offer a taxonomy of disinformation, and a view of what it is for a signal to constitute disinformation for a particular agent in a particular context. The account, if correct, carries high stakes upshots, both theoretically and practically: disinformation tracking will need to go well beyond mere fact checking. (shrink)

As forms of employment and migration changed, the financial crisis which began in 2007 affected the human rights of individuals, particularly those in developing countries. How the media reported on these consequences is essential in understanding how and why public and political perceptions of the importance of human rights may have changed since the crisis began. Using quantitative and qualitative analysis of major newspapers in the UK and the USA, this paper seeks to understand the ways in which the media (...) presented the relationship between the negative changes in the economy and human rights. The analysis of the findings uses framing theory to understand how the text in news articles communicates the relevance of human rights to readers before concluding with some further avenues for research. The article analyses programmes against gender-based violence (GBV) in Cambodia in order to understand what notions of power, agency and resistance reside within these programmes. The text relies on in-depth interviews with four different organisations in Cambodia. The interviews display a number of hands-on practices of resistance against GBV, which require a broad discussion of identity in order to be fully understood. In particular, the organisations emphasize the importance of approaching men—in men's groups, as trainers and role models—in the resistance (...) against GBV. In their approach to Cambodian men, the trainers mixed representations of a more ‘particular’ character with representations of a more ‘universal’ appearance. Both in the establishment of new subject positions and new discourses, the Cambodian trainers leaned upon and alternated between universal and particular notions. In addition, men's ‘particular’ subject positions became the very lens through which they considered ‘universal’ notions of violent masculinities. New aspects of the resistance against GBV thus become visible as the concepts of universalism and particularism are put in use. It is in the nexus between ‘universal’ and ‘particular’ representations that a non-violent masculinity is fostered. (shrink)

When in the business of offering an account of the epistemic normativity of belief, one is faced with the following dilemma: strongly externalist norms fail to account for the intuition of justification in radical deception scenarios, while milder norms are incapable to explain what is epistemically wrong with false beliefs. This paper has two main aims; we first look at one way out of the dilemma, defended by Timothy Williamson and Clayton Littlejohn, and argue that it fails. Second, we identify (...) what we take to be the problematic assumption that underlies their account and offer an alternative way out. We put forth a knowledge-first friendly normative framework for belief which grants justification to radically deceived subjects while at the same time acknowledging that their false beliefs are not epistemically good beliefs. According to anti-reductionism in the epistemology of testimony, testimonial entitlement is easy to come by: all you need to do is listen to what you are being told. Say you like anti-reductionism; one question that you will need to answer is how come testimonial entitlement comes so cheap; after all, people are free to lie. This paper has two aims: first, it looks at the main anti-reductionist answers to this question and argues that they fail. Second, it goes on a (...) rescue mission on behalf of anti-reductionism. I put forth a novel anti-reductionist account, which I dub ‘Testimonial Contractarianism’. According to the view defended here, in virtue of the social contract in play, compliance with the norms governing speech acts is the default position for speakers. Insofar as norm compliance is the default for speakers, I argue, all else equal, entitlement to believe is the default for hearers. (shrink)

This paper has two aims. The first is critical: I identify a set of normative desiderata for accounts of justified belief and I argue that prominent knowledge first views have difficulties meeting them. Second, I argue that my preferred account, knowledge first functionalism, is preferable to its extant competitors on normative grounds. This account takes epistemically justified belief to be belief generated by properly functioning cognitive processes that have generating knowledge as their epistemic function.

Can we engineer conceptual change? While a positive answer to this question would be exciting news for philosophy, there has been a growing number of pessimistic voices in the literature. This paper resists this trend. Its central aim is to argue not only that conceptual engineering is possible but also that it is not even distinctively hard. In order to achieve this, we will develop a novel approach to conceptual engineering that has two key components. First, it proposes a reorientation (...) of the conceptual engineering project away from fixing conceptual defects and towards bringing about conceptual innovation. Second, it offers a new account of when conceptual engineering is successful in terms of etiological functions. We then turn to the reasons that have motivated various forms of pessimism about conceptual engineering and show that, on our novel approach, none of them stands up to scrutiny. (shrink)

BackgroundIn cancer care, do not resuscitate (DNR) orders are common in the terminal phase of the illness, which implies that the responsible physician in advance decides that in case of a cardiac arrest neither basic nor advanced Coronary Pulmonary Rescue should be performed. Swedish regulations prescribe that DNR decisions should be made by the responsible physician, preferably in co-operation with members of the team. If possible, the patient should consent, and significant others should be informed of the decision. Previous studies (...) have shown that physicians and nurses can experience ethical dilemmas in relation to DNR decisions, but knowledge about what ethical reasoning they perform is lacking. Therefore, the aim was to describe and explore what ethical reasoning physicians and nurses apply in relation to DNR-decisions in oncology and hematology care.MethodsA qualitative, descriptive and explorative design was used, based on 287 free-text comments in a study-specific questionnaire, answered by 216 physicians and nurses working in 16 oncology and hematology wards in Sweden. Comments were given by 89 participants.ResultsThe participants applied a situation-based ethical reasoning in relation to DNR-decisions. The reasons given for this were both deontological and utilitarian in kind. Also, expressions of care ethics were found in the material. Universal rules or guidelines were seen as problematic. Concerning the importance of the subject, nurses to a higher extent underlined the importance of discussing DNR-situations, while physicians described DNR-decisions as over-investigated and not such a big issue in their daily work.ConclusionThe study revealed that DNR-decisions in oncology and hematology care gave rise to ethical considerations. Important ethical values described by the participants were to avoid doing harm and to secure a peaceful and “natural” death with dignity for their dying patients. A preference for the expression “allow for natural death” instead of the traditional term “do not resuscitate” was found in the material. (shrink)

This study investigates the association of a broad set of variables with the ethical decision making of management accountants in Libya. Adopting a cross-sectional methodology, a questionnaire including four different ethical scenarios was used to gather data from 229 participants. For each scenario, ethical decision making was examined in terms of the recognition, judgment and intention stages of Rest’s model. A significant relationship was found between ethical recognition and ethical judgment and also between ethical judgment and ethical intention, but ethical (...) recognition did not significantly predict ethical intention—thus providing support for Rest’s model. Organizational variables, age and educational level yielded few significant results. The lack of significance for codes of ethics might reflect their relative lack of development in Libya, in which case Libyan companies should pay attention to their content and how they are supported, especially in the light of the under-development of the accounting profession in Libya. Few significant results were also found for gender, but where they were found, males showed more ethical characteristics than females. This unusual result reinforces the dangers of gender stereotyping in business. Personal moral philosophy and moral intensity dimensions were generally found to be significant predictors of the three stages of ethical decision making studied. One implication of this is to give more attention to ethics in accounting education, making the connections between accounting practice and Islam. Overall, this study not only adds to the available empirical evidence on factors affecting ethical decision making, notably examining three stages of Rest’s model, but also offers rare insights into the ethical views of practising management accountants and provides a benchmark for future studies of ethical decision making in Muslim majority countries and other parts of the developing world.
