Background Professional ethics in nursing exist to guide care and allow for decision-making to be patient-centered. In the current medicolegal landscape post-Roe and in light of bans on gender-affirming care, the decision-making processes of emergency nurses in the clinical environment of care as informed by both professional and personal ethics are an important area of inquiry. Aim The aim of this study was to examine the contribution of moral courage to decision-making by emergency nurses. Research Design A mixed-methods exploratory sequential (...) approach was used, using a standard demographics form and the Nurses Moral Courage Scale to collect quantitative data. These data were used to inform an interview guide for qualitative data collection. Situational analysis was used to analyze the interview data. Ethical Considerations Prior to recruitment, this study was reviewed and approved by the University of Massachusetts IRB (#00003909). Participants were provided with an informed consent document at the time of registration and at the time of interview; participants provided both signed consent and verbal assent. Participants were assigned study codes to maintain anonymity and data were maintained in a secure University cloud. Participants and Research Context US-based emergency nurses working in environments with care limitations. Results 70% of respondents reported that they would speak up if they were aware of a situation that was ethically challenging. Respondents reported that it was fairly easy or very easy to defend their values when addressing ancillary staff, coworkers, authorities outside the organization, patients, and patient families. Respondents reported challenges in defending their professional values to charge nurses, physicians, or administrators. In response to a serious ethical problem, 65.8% of respondents answered that they would bring up the problem for discussion, 21.1% would file an internal report, and 13.2% would report externally. Interview participants reported significant social and professional barriers to acting in response to an ethical violation. Willingness to act centered on personal values and not professional codes of ethics. Conclusions While emergency nurses in this study reported high perceived levels of moral courage, they also reported low willingness to act directly, citing burnout and significant social and professional barriers. No consensus yet exists on how to handle incidental fnd-ings in human subjects research. Yet empirical studies document IFs in a wide range of research studies, where IFs are fndings beyond the aims of the study that are of potential health or reproductive importance to the individual research participant. This paper reports recommendations of a two-year project group funded by NIH to study how to manage IFs in genetic and genomic research, as well as imaging research. We conclude that researchers (...) have an obligation to address the possibility of discovering IFs in their protocol and communications with the IRB, and in their consent forms and communications with research participants. Researchers should establish a pathway for handling IFs and communicate that to the IRB and research participants. We recommend a pathway and categorize IFs into those that must be disclosed to research participants, those that may be disclosed, and those that should not be disclosed. (shrink)

Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

What is it to base a belief on reasons? Existing attempts to give an account of the basing relation encounter a dilemma: either one appeals to some kind of neutral process that does not adequately reflect the way basing is a content‐sensitive first‐personal activity, or one appeals to linking or bridge principles that over‐intellectualize and threaten regress. We explain why this dilemma arises, and diagnose the commitments that are key obstacles to providing a satisfactory account. We explain why they should (...) be rejected anyway, and then offer a new, knowledge‐first virtue epistemological theory of the basing relation that shows how we can substantially theorize about mental processes without these commitments. The resulting view plausibly captures the way in which basing is an inherently mental and content‐sensitive process. (shrink)

Structural analogies connect Williamson’s epistemology and action theory: for example, action is the direction-of-fit mirror image of knowledge, and knowledge stands to belief as action stands to intention. These structural analogies, for Williamson, are meant to illuminate more generally how ‘mirrors’ reversing direction of fit should be understood as connecting the spectrum of our cognitive and practically oriented mental states. This paper has two central aims, one negative and the other positive. The negative aim is to highlight some intractable problems (...) with Williamson’s preferred analogical picture, which links the cognitive and the practical through the nexus of direction-of-fit mirroring. The positive aim of the paper is to propose a better alternative. In particular, we show that an achievement-theoretic proposal captures what is in common across the range of attitudes that exhibit the kind of structure that knowledge-belief, action-desire/intention do, while at the same time avoiding the problems shown to face Williamson’s proposed picture. Moreover, we draw attention to several key theoretical benefits of embracing our proposed achievement-theoretic picture, including some of the key benefits of the knowledge-first programme that Williamson’s own analogies were designed to secure. Excellent patient care within the emergency department requires interdisciplinary training, teamwork, and communication to manage the chaos of the environment. Specifically, invasive procedures required to manage airway, breathing, and circulation via intubation, chest compressions, and establishing intravenous access can provide a direct benefit to save lives but also have the potential to harm both patients and health care clinicians alike; emergency health care clinicians can be exposed to significant amounts of blood and body fluids as well as other threats of (...) physical and psychological harm. The ethical components of care in this environment are often under‐recognized due to the need for rapid patient assessment and immediate action. Moreover, challenges to practice that can include lack of qualified personnel, equipment, and other resources to provide safe care to a large volume of patients can lead to moral distress in ED staff. Because the ED is a high‐uncertainty, high‐acuity environment, continuing interprofessional communication, collaboration, and planning is critical. Opportunities for multidisciplinary policy dialogue and the development of professional guidelines can make the ED a safer environment for both patients and providers. (shrink)

Fordham von ReynFatuma's* doctors were completely perplexed. It was 2003 and she had returned to the DARDAR clinic in her hometown of Dar es Salaam, Tanzania three times that week with vague complaints of various pains and aches. Her doctors were considering whether these symptoms were (...) due to the initiation of her recent treatment with antiretroviral medications for her HIV infection or related to the last administration of the investigational tuberculosis (TB) vaccine that the DARDAR Health Study was evaluating. Fatuma's doctor, Dr. Lillian Mtei, consulted with Dartmouth's chief of Infectious Disease, Dr. Ford von Reyn, who happened to be in Dar that week on a site visit. Neither physician could determine the basis for her symptoms. Dr. Mtei returned to the exam room once more and this time emerged with the answer: Fatuma had just learned that her three year old daughter was HIV-infected, likely contracted from Fatuma during or shortly after she gave birth to her daughter. Fatuma believed that her HIV diagnosis was a death sentence for her daughter since antiretroviral therapy was not widely available for children.Dr. von Reyn, Dartmouth's lead clinical researcher in Africa and a father, knew too well the harsh reality of inequities in a resource-limited country. He turned to Dr. Mtei and said decidedly, "we need to do something about this."Thus the DARDAR Pediatric Program was born.Pediatric HIV: A Neglected DiseaseThe need for HIV treatment among children in Tanzania, as in most of sub-Saharan Africa, is compelling. At the end of 2009, approximately 160,000 children aged zero to fourteen in Tanzania were living with HIV (Joint United Nations Programme on HIV/AIDS [UNAIDS], 2011). Even with successful roll out of prevention of mother to child transmission programs in Tanzania, there were nearly 29,000 perinatal infections in 2009. As of December 2010, only 18% of these were enrolled in HIV treatment programs (UNAIDS, 2011; World Health Organization [WHO], 2011). Without antiretroviral therapy (ART), one-half of HIV-infected children will die before their second birthday. Since clinical research to determine efficacy and safety of new medical treatments is conducted almost exclusively in adults, provision of new medical treatments to [End Page 75] children always lags behind advances for adults. Furthermore, children are often underrepresented in programs for antiretroviral drugs for the following reasons: 1) they are not prioritized for care; 2) facilities lack infant diagnostic capabilities; 3) there is limited pediatric HIV training and consequently, a shortage of pediatric providers; and 4) pediatric antiretroviral formulations are frequently unavailable. The unacceptable delays in pediatric HIV drug development and treatment access led the international organization Drugs for Neglected Diseases initiative to publish an editorial in which they referred to pediatric HIV as a "neglected disease" (Lallemant, Chang, Cohen, & Pecoul, 2011).The Tip of the Iceberg: More than Just MedicinesWith a grant from the New York-based Foundation for the Treatment of Children with AIDS, the DARDAR Pediatric Program, a joint collaboration between Geisel School of Medicine at Dartmouth and Muhimbili University of Health and Allied Sciences, began seeing patients in May 2006. Or as many of scholars and thinkers in this final ‘future-focused’ collective piece from the philosophy of education in a new key Series put it, what are the futures—plural and multiple—of the intersections of ‘philosophy’ and ‘education?’ What is ‘Philosophy’; and what is ‘Education’, and what role may ‘enquiry’ play? Is the future of education and philosophy embracing—or at least taking seriously—and thinking with Indigenous ethicoontoepistemologies? And, perhaps most importantly, what is that (...) ‘Future’? These debates have been located in the work of diverse scholars: from the West, from Global South, from indigenous thinkers. In this collective piece, we purposefully juxtapose diverse takes on the future of these intersections. We have given up the urge to organise, place together, separate with subheadings or connect the paragraphs that follow. Instead, we let these philosophers of education and thinkers who use philosophical texts and ideas to sit together in one long read as potentially ‘strange and unusual bedfellows’. This text urges us to understand how these scholars and thinkers perceive our educational philosophical futures, and how the work and thinking they have done on thinking about what the future of that new key in philosophy of education may look like is embedded in a much deeper and richer literature, and personal experience. This article explores Adam Smith's (1723-90) cosmopolitanism by examining his conception of the ideal global regime and his attitudes to classical cosmopolitanism, British imperialism, American independence, war, mercantilism, benevolence, global integration, specialization, patriotism and his own alleged nationalism. It is argued that Smith shares with the Stoics the ideal of a world community but his cosmopolitanism is based, not on the sympathetic workings of universal benevolence, but on mutual enablement and the desire for and satisfaction of exponential material enrichment. Such (...) a state precludes conflict and territorial rivalry. Patriotism will not disappear but will be moderated by a pacific desire for prosperity via free trade. Eventually fierce nationalistic sentiments and intense alliances between security allies will be subdued by a development-induced aversion to conflict and the habits of amicable strangership underlying commercial cosmopolitanism. The article outlines the efficient mechanisms by which this is to be achieved and, in so doing, shows why, for Smith, economic cosmopolitanism is at once desirable, necessary and natural. (shrink)

Adam Ferguson was a leading light of the Scottish Enlightenment who developed a systematic theory of historical progress in the context of a broader theory of spontaneous order. His exposition of social order outlines a vision of human affairs as harmonious, orderly, progressive and perfectibilist. History is conceived lineally and is presented in the form of a tri-stadial thesis in which progress is both natural and likely. It is also a Providentially inspired process and yet the second major theme of (...) Ferguson's history is that of decline. Ferguson apparently harboured grave fears about the third stage in our cultural and economic evolution, the ‘polished’ or commercial age, and he suggested that this stage contained the seeds of its own destruction.Ferguson fails to make explicit the precise relationship between the parallel themes of progress and decline within his corpus; indeed, his articulation of them within the superstructure of a Providential teleology casts them in direct conflict. The result is a strong sense of ambivalence which throws the whole idea of spontaneous order into doubt. How can Ferguson simultaneously subscribe to notions about progressive, spontaneous history and to others about the onset of decay in the presumably ‘natural stage’ of commercial or ‘polished’ society? This paper explores and seeks to resolve this conundrum textually via a discussion of Ferguson's response to the problem of theodicy and by using his commitment to spontaneous order as the key unifying construct. The University of Kentucky has required that all researchers and research-eligible individuals complete RCR training every 2 years to ensure there is at least a baseline of RCR training throughout the wider research community. The overall goal is to create a research climate that fosters RCR across the institution for approximately 14,400 researchers and research eligible faculty, staff, and trainees engaged in research or creative work. A systematic data strategy was developed and implemented to identify individuals required to complete the (...) RCR training and included real-time data tracking using data analytic tools and sophisticated data models. The RCR training consists of the completion of online asynchronous modules and an in-person RCR training event where case studies are discussed to apply the learning gained from the online modules and prepare participants to practice ethical decision-making in future real-world problem-solving. A train-the-trainer approach was implemented to conduct the in-person RCR training of all researchers and research eligible individuals. A library of interactive case scenarios was developed for the mandatory in-person RCR training applicable to various disciplines across the institution. A survey of participants on the RCR training provided empirical data from approximately 12% of participants. Over 72% of respondents indicated the training positively impacted their behavior. An initiative of this magnitude and reach is the broadest of its kind in the United States (US) and the article outlines the procedures and experiences in implementing the project. (shrink)

This paper discusses Adam Smith's views of social justice. It first describes Smith's optimistic view of markets, for example with regard to the absence of negative externalities, which implies that he considered certain normative problems to be the exception rather than the rule. Then, Smith's views on redistribution are discussed: although he is sympathetic to progressive taxation, his main focus remains on free markets, which can partly be explained by his distrust of politicians. If one takes a closer look as (...) Smith's views of markets, however, it turns out that one of the reasons why he endorses them is their distributive features. He saw them as an antidote to the inequalities of the feudal age, offering all individuals an opportunity to work their way up to a decent standard of living and leading to more equality in the long run. Thus, Smith's account, while out-dated in certain ways, can serve as an inspiration for thinking about distributive justice not only as a question of redistributive taxation but also as a question of the institutional design of markets. In this article, we propose the Fair Priority Model for COVID-19 vaccine distribution, and emphasize three fundamental values we believe should be considered when distributing a COVID-19 vaccine among countries: Benefiting people and limiting harm, prioritizing the disadvantaged, and equal moral concern for all individuals. The Priority Model addresses these values by focusing on mitigating three types of harms caused by COVID-19: death and permanent organ damage, indirect health consequences, such as health care system strain and stress, as well as (...) economic destruction. It proposes proceeding in three phases: the first addresses premature death, the second long-term health issues and economic harms, and the third aims to contain viral transmission fully and restore pre-pandemic activity. -/- To those who may deem an ethical framework irrelevant because of the belief that many countries will pursue "vaccine nationalism," we argue such a framework still has broad relevance. Reasonable national partiality would permit countries to focus on vaccine distribution within their borders up until the rate of transmission is below 1, at which point there would not be sufficient vaccine-preventable harm to justify retaining a vaccine. When a government reaches the limit of national partiality, it should release vaccines for other countries. -/- We also argue against two other recent proposals. Distributing a vaccine proportional to a country's population mistakenly assumes that equality requires treating differently situated countries identically. Prioritizing countries according to the number of front-line health care workers, the proportion of the population over 65, and the number of people with comorbidities within each country may exacerbate disadvantage and end up giving the vaccine in large part to wealthy nations. He took his first portraits of the Duchess of York and her daughter, the infant Princess Elizabeth, in 1926, and he continued to photograph them, and other members of the Royal Family regularly, until his last royal sitting in 1956. Though sometimes perceived as a secondary figure, there have been some attempts to recover him as one of, if not (...) the first "sociologist." 1 Much has been written about Ferguson's contribution to modern social science; 2 this article focuses upon his highly prescient work in the area of conflict theory. It is important to note at the outset, however, that Ferguson's inspiration for these apparently novel insights is classical. His early social theory is curious because it "straddles traditions"; 3 on the one hand he owes a profound debt to classical civic humanism, [End Page 281] but on the other his work quite authentically signposts the emergence of social science proper. This bridging of traditions in Ferguson is a key theme of this paper.Ferguson relies heavily upon conflict in his explanation of historical processes and in the securing of social order. Conflict generates many positive unintended consequences; it leads to the formation of the state, formal defense institutions, and large scale communities; and it plays a pivotal role in the development of the moral personality and contributes to the maintenance of stability and social cohesion. Conflict preserves the balance of powers in government and prevents the encroachment of despotism. In this regard, then, conflict cannot be regarded as wholly unnecessary, destructive, or undesirable but as integral to progress and the march towards civilization. 4Gladys Bryson has noted that Ferguson's exposition of conflict was original with him and "set him off from the conventional position of his century." 5 Even so the benefits of conflict and dissension had been canvassed long before by Tacitus and Machiavelli, both of whom influenced Ferguson's analysis. 6 In it he challenges the false dichotomy promulgated by contractarians of tranquil society on the one hand and violent, isolated chaos on the other; rather, he argues, we are social beings with belligerent tendencies. He also confronts the Aristotelian (and later Hobbesian) premise that the key purpose of society is stability and social harmony. Instead, Ferguson lays great stress on the positive benefits of competition and conflict in our social development and in the maintenance of community. Conflict is just as natural as repose and may even be equally desirable. In this Ferguson indicates his debt to Socrates' suggestion that "some circumstances in the lot of mankind" prompt us to be social, while others prompt us "to war and dissension." 7 As a highlander, Ferguson was well acquainted with the realities of conflict. Motivation crowding out can lead to a reduction of ‘higher’ virtues, such as altruism or public spirit, in market contexts. This article discusses the role of virtue in the moral and economic theory of Adam Smith. It argues that because Smith’s account of commercial society is based on ‘lower’ virtue, ‘higher’ virtue has a precarious place in it; this phenomenon is structurally similar to motivation crowding out. The article analyzes and systematizes the ways in which Smith builds on ‘contrivances of (...) nature’ in order to solve the problems of limited self-command and limited knowledge. As recent research has shown, a clear separation of different social spheres can help to reduce the risk of motivation crowding out and preserve a place for ‘higher virtue’ in commercial society. In this paper I respond to the view that Smith is actually sceptical about the value of material acquisition and explore whether he really believed that the pursuit of tranquillity and virtue—especially beneficence—offers a superior route to happiness than (...) the commercial world of materialist acquisition. I approach these issues partly by considering the roles of beneficence and sympathy in Smith's system and partly by analysing the story of ‘The Poor Man's Son’ related in Book IV of The Theory of Moral Sentiments. Despite the fact that the requirement to obtain informed consent for medical procedures is deeply enshrined in both U.S. moral and legal doctrine, empirical studies and anecdotal accounts show that women's rights to informed consent and refusal of treatment are routinely undermined and ignored during childbirth. For example, citing the most recent Listening to Mothers survey, Marianne Nieuwenhuijze and Lisa Kane Low state that "a significant number of women said they felt pressure from a caregiver to agree to having (...) an intervention that they did not want during birth". Adam Smith (1723–90) provided a novel and subtle account of the new social physics that emerged to accommodate the economic changes taking place in his time. This article explores Smith’s views on the effect of commercialization on friendship, and then questions one prominent interpretation of his approach, that of Allan Silver. Against the contested reading, we argue that the new ‘strangership’ described by Smith is not warm, but rather, cool-friendship enhancing. This special issue features China’s so-called rising presence as one of (...) the key nodes in these global intimacies. The essays by Mei-Hua Chen and Hong-zen Wang, Sealing Cheng, and Wei Wei contribute new approaches to migrant intimacies across borders through their ethnographically rich analyses. Chen and Wang explore cross-border marriages and sex work between Taiwanese men and Mainland Chinese and southeast Asian women; Cheng investigates refugee marriages and the difficulties men from various African nations face in seeking asylum in Hong Kong; and Wei analyzes Mainland Chinese queer reproduction that uses transnational Assisted Reproductive Technologies (ARTs) to enable queer parents to have a genetic link with their children and to be more accepted in Chinese society. In his essay, Petrus Liu pushes us to move beyond the idea that a theory of gender construction traveled to China from the West and was wholly adopted there, instead demonstrating the complexities of gender theories in China in the age of the Beijing Consensus. Christina Yuen Zi Chung and Sasha Su-Ling Welland explore a broad range of artworks that critically reflect on China’s efforts to create a China-centered global Global Intimacies: China and/in the Global South 252Preface trading network. Paul Amar offers a “deimperial queer analysis” of the all-time top-earning Chinese film, Wolf Warrior 2, illuminating how it both buttresses China’s extractive and militarized investments in African nations and manifests anti-imperialist and utopian impulses. Poems by Zhai Yongming and Xu Lizhi feature the gendered and sexualized precarity and violence of recent social transformations in China resulting from China’s intimate linkages with the global capitalist economy. Finally, Cai Yiping’s News and Views offers a nuanced engagement with the Chinese government’s formal proclamations on women’s rights.1 In the first essay, “Flexible Intimacies in the Global Intimate Economy : Evidence from Taiwan’s Cross-Border Marriages,” Mei-Hua Chen and Hong-zen Wang demonstrate the fallacy of treating cross-border marriages and cross-border sex work as separate as well as grouping women dichotomously under one or the other set of practices. These two seemingly disparate categories are, in effect, produced by state immigration regulations and dominant intersectional discourses of gender, sexuality, and class. Rather than distinguishing between marriage migrants and economic migrants, or between real marriages and fake marriages, Chen and Wang demonstrate the overlapping means by which women from the Global South try to better their lives in what they call the global intimate economy—where the Global North appropriates the reproductive and intimate labor of women from the Global South. The authors develop the concept of “flexible intimacy” to highlight how migrant women treat marriage as an investment through which to improve their lives. They argue that many marriage migrants have economic motivations for cross-border marriage and that migrant sex workers, conversely, are often involved in intimate relationships. Chen and Wang focus their study on Taiwan, which they argue is economically part of the North but politically part of the South due to Mainland China’s “One China” policy, which has marginalized Taiwan in the international political arena. All rights reserved.We present new observations of the low-mass companion to HD 984 taken with the Gemini Planet Imager as a part of the GPI Exoplanet Survey campaign. Images of HD 984 B were obtained in the J and H bands. Combined with archival epochs from 2012 and 2014, we fit the first orbit to the companion to find an 18 au orbit with a 68% confidence interval between 14 and 28 au, an eccentricity (...) of 0.18 with a 68% confidence interval between 0.05 and 0.47, and an inclination of 119°with a 68% confidence interval between 114°and 125°. To address the considerable spectral covariance in both spectra, we present a method of splitting the spectra into low and high frequencies to analyze the spectral structure at different spatial frequencies with the proper spectral noise correlation. Using the split spectra, we compare them to known spectral types using field brown dwarf and low-mass star spectra and find a best-fit match of a field gravity M6.5 ±1.5 spectral type with a corresponding temperature of K. Photometry of the companion yields a luminosity of log=2.88 ± 0.07 dex with DUSTY models. Mass estimates, again from DUSTY models, find an age-dependent mass of 34 ±1 to 95 ±4 M Jup. These results are consistent with previous measurements of the object. (shrink)

This paper extends the sensibilities of the Gilligan-Kohlberg debate into classical political economy and makes links with modern psychotherapeutics and the psychological development of individuals. The model of moral maturity represented in contemporary psychological theories is posited as the direct descendant, not only of Immanuel Kant, as is generally argued, but also of the universal, homogenous agent of classical economics; the ‘rational economic man’ representedin the writings of Adam Smith and J. S. Mill. Both thinkers lent their support to the (...) creation of an order which produces (masculine) actors who are ’rational’ self-governing, separative, competitive, self-asserting, self-interested, acquisitive, pecuniary and driven to success by a desire for the social recognition afforded by consumerism; the same order which typically designates women moral failures and which renders them largely invisible. Liberalism generates conditions which enable it to sustain and reproduce itself at the psychic level. The assumption of the market as the paradigmatic social interaction and its status as the analytical and moral starting point in liberal capitalism infects the putatively private domain of ‘self-governance’ and contemporary psychotherapeutics; the development and management of the self in contemporary culture is a direct reflection of the imperatives and assumptions of market society. The separative self is the ‘healthy’ self, hence the parallel emphasis in modern therapy on ‘individuation’ as the highest level of psychological evolvement. Gilligan’s ‘care ethic’ is approached with some scepticism, nevertheless, her critique of Kohlberg guides us towards a more comprehensive critique of the ‘pneumatic’ and political assumptions of liberalism. Susan Wolf's paper "Meaning and Morality" draws our attention to the fact that Williams's objection to Kantian morality is primarily a concern about a possible conflict between morality and that which gives our lives meaning. I argue that the force of Williams's objection requires a more precise understanding of meaning as dependent on our intention to make our lives themselves worthwhile. It is not meaning simpliciter that makes Williams's objective persuasive but rather meaning as arising out of our positive (...) evaluation of the value of our lives as a whole. This type of meaning has a normative element: it involves a person's deep-seated commitment to make her actions consistent with ends that confer worth on her life itself. The more significant conflict with morality lies in the conflict between the normative force of moral requirements and the normative force of the need to have a life that is itself worthwhile.
