Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

Intuitively, one who counts a morally bad person as a friend has gone wrong somewhere. But it is far from obvious where exactly they have gone astray. Perhaps in cultivating a friendship with a bad person, one extends to them certain goods that they do not deserve. Or perhaps the failure lies elsewhere; one may be an abettor to moral transgressions. Yet another option is to identify the mistake as a species of imprudence—one may take on great personal risk in (...) counting a bad person as a friend. In this paper, I argue that none of these intuitive explanations are entirely convincing; for many such proposals run contrary to widely accepted features of friendship. However, they do point us in the direction of a more satisfying explanation—one which concerns a person’s moral priorities. An individual who counts a morally bad person as a friend is, I propose, one who betrays a distinct kind of defect in her values. (shrink)

This commentary focuses on the importance of attending to a variety of factors to improve ecological validity as well as considering the ultimate factors shaping behavior and the role of natural categories in the stability of stereotypes and their influence.

Background: Moral distress is recognized as a problem affecting healthcare professionals globally. Unaddressed moral distress may lead to withdrawal from the moral dimensions of patient care, burnout, or leaving the profession. Despite the importance, studies related to moral distress are scant in Thailand. Objective: This study aims to describe the experience of moral distress and related factors among Thai nurses. Design: A convergent parallel mixed-methods design was used. The quantitative and qualitative data were collected in parallel using the Measure of (...) Moral Distress for Healthcare Professionals and interview guide. The analysis was conducted separately and then integrated. Participants: Participants were Thai nurses from two large tertiary care institutions in a Southern province of Thailand. Ethical considerations: This study was approved by our organization's Institutional Review Board for Health Sciences Research, and by the Institutional Review Boards of the two local institutions in Thailand. Permission from the publisher was received to translate and utilize the Measure of Moral Distress (MMD-HP) under the license number: 4676990097151. Results: A total of 462 participants completed the survey questions. The top 7 causes of moral distress were related to system-level root causes and end-of-life care situations. Hierarchical multiple regression showed that work units, considering leaving position, and number of moral distress episodes in the past year were significant predictors of moral distress. Twenty interviews demonstrated three main themes of distressing causes: (1) powerlessness (at patients/family-, team-, and organizational-levels), (2) end-of-life issues, and (3) poor team function (poor communication and collaboration, incompetent healthcare providers, and inappropriate behavior of colleagues). The integration of data from both components indicated that the qualitative interviews enrich the quantitative findings, especially as related to the top 7 causes of moral distress. Discussion: Although the experience of moral distress among Thai nurses is similar to studies conducted elsewhere, the patient’s and family’s religious perspective that ties into the concept of moral distress needs to be explored. Conclusions: Although the root causes of moral distress are similar among different cultures, the experience of Thai nurses may vary according to culture and context. (shrink)

A justificativa baseou-se na necessidade de compreender a potencialidade das atividades cooperativas e a solução das necessidades informacionais. Concluiu-se que a folksonomia é uma ferramenta (...) de apoio em grupos colaborativos no Passei Direto, onde os usuários, no uso de tags em práticas de indexação, visam a enriquecer e ampliar as possibilidades de acesso às informações. No entanto, apesar de representar uma ferramenta de forte cunho colaborativo, a folksonomia também deixa lacunas no tocante à recuperação da informação, dada a falta de padronização e controle dessa ferramenta, embora essas características façam parte da sua essência. Research on authentic learning has been predominantly focussed on skills-based training: there is a paucity of research on models of authentic learning available for adaptation in the humanities undergraduate classroom. In this article, I will seek to address this gap by proposing that legal trials are ideal models for designing authentic learning scenarios in undergraduate teaching and learning contexts, with a specific focus on the humanities. First, I discuss why and how the structure of legal trials can produce authentic learning (...) environments. Second, I present an undergraduate classroom project that combined two disciplinary fields – Shakespearean drama and criminal law – in an effort to enhance student learning and engagement. I outline how the authentic learning scenario was implemented and evaluated and, finally, reflect on the barriers, challenges and potentially transformative effect of authentic learning environments on students and educators. Leading animal models are powerful tools for translational research, but they also present obstacles. Poorly conducted preclinical research in animals is a common cause of translational failure, but even when such research is well-designed and carefully executed, challenges remain. In particular, dominant models may bias research directions, elide essential aspects of human disease, omit important context, or subtly shift research targets. Recognizing these stumbling blocks can help us find ways to avoid them: employing a wider range of models, incorporating more (...) realistic environmental conditions, better aligning studies between animals and patients, and focusing on human biology and therapeutic goals. Such changes are costly; but insisting it would be impractical or unrealistic to change strategies offers no way out of the current impasse. Rather, we must acknowledge the obstacles as well as the advantages presented by core models, and direct some of our investments in translational research toward getting around them. The dominance of biomedical research by a handful of animal models introduces subtle but significant obstacles to translation. Recognizing these obstacles is a critical first step toward finding ways around them; this paper describes how dominant models can create roadblocks, and suggests strategies to improve the efficiency of translation from animal models to humans. This study sheds light on a key concept of Yogācāra intersubjectivity that played a significant role in medieval Chinese Yogācāra. Specifically, it analyzes how Kuiji 窺基 (632–682) reinterprets adhipati (activating and amplifying influence; Ch: zengshang 增上 or zengshang li 增上力 or zengshang yuan 增上緣) to account for intersubjective karmic interactions across different lifeworlds in the events of teaching and killing.As this line of investigation shows, Kuiji’s theory of adhipati attempts to sidestep the entanglement of the problems of intersubjectivity and incommensurable (...) worlds in Yogācāra. Extant scholarship on intersubjectivity often presumes one spatio-temporal world populated by sentient beings and insentient objects, which is fundamentally at odds with the Yogācāra tenet that each mental stream generates its distinct lifeworld. To address the pragmatic question of how different mental continuums of consciousnesses can impinge into each other’s lifeworlds, Kuiji employed adhipati in a specific way to account for intersubjective interplay across different, potentially incommensurable worlds. In this framing of intersubjectivity, the problem of one world or many worlds is explained non-dualistically: these lifeworlds are neither the same nor different, neither one nor many, but karmically interconnected.Expanding on recent discussions of the puzzle of intersubjectivity in Yogācāra, this paper shows that Kuiji’s deployment of adhipati provides an account of genuine intersubjectivity in which different mental continuums are recognized as subjects and engage in causal interactions among different spatio-temporal worlds. In Kuiji’s framing, this genuine intersubjectivity gives rise to the sense of one shared world. (shrink)

On April 5, 2012, the North American Nietzsche Society held a session, chaired by myself, at the American Philosophical Association Pacific Division Meeting in Seattle on reading Nietzsche as a figure within the history of philosophy. The white sclera is important in facilitating gaze perception in humans. Iris color may likewise influence gaze perception but no previous studies have directly assessed its effect. We therefore examined how the interaction between sclera and iris color influences human gaze perception. We recorded the eye movements of human participants as they performed a visual search task with human faces exhibiting directed or averted gaze. The faces either exhibited light or dark irises. In addition, the faces had sclera that were (...) depigmented or pigmented. We found that participants were quick and accurate in evaluating gaze regardless of iris color in faces with depigmented sclera. When the sclera were pigmented, participants were slower to evaluate the gaze of faces with both light and dark irises but these effects were most pronounced in the faces with dark irises. Furthermore, participants were generally less accurate in assessing faces with pigmented sclera when the irises were dark rather than light. Our results suggest that depigmented sclera are especially important for gaze perception in faces with dark irises. Because depigmented sclera likely evolved at a time when ancestral humans exhibited dark irises, the depigmented sclera may have been crucial for efficient and accurate gaze perception in ancestral humans. The adaptive reuse of industrial heritage, a critical component in addressing radical climate change-related urban transitions, is increasingly pertinent. This paper distinguishes ‘urban transitions’ from ‘urban transformation,’ emphasizing a more gradual, adaptive approach to urban development under the pressures of climate change. It explores the repurposing of industrial buildings and spaces, maintaining their cultural and historical value while meeting current urban needs. Through a mixed-methods approach, the paper analyses how adaptive reuse contributes to sustainable urban development, examines the scale and (...) impact of such projects from local quarters to city-wide implications, and discusses the potential negative consequences, including gentrif ication. The study spans various global regions, including Europe, the United States, Latin America, Canada, and Australia, using case studies to illustrate the effectiveness of adaptive reuse in promoting sustainability, revitalizing urban areas, and preserving cultural heritage. The paper questions the viability of traditional long-term sustainable urban development strategies in the face of rapid environmental and societal changes, suggesting a potential need for paradigm shifts in urban planning. In the management of common property resources, privatization is often advocated as the surest path to sustainability because of its reliance on human self-interest in natural resources decision making. This article demonstrates that the motive of self-interest, though powerful, does not necessarily lead to environmental outcomes that promote the common good. The key to avoiding the Tragedy of the Commons is not private ownership but controlling access.

Healthcare systems are grappling with critical challenges, including chronic diseases in aging populations, unprecedented health care staffing shortages and turnover, scarce resources, unprecedented demands and wait times, escalating healthcare expenditure, and declining health outcomes. As a result, policymakers and healthcare executives are investing in artificial intelligence (AI) solutions to increase operational efficiency, lower health care costs, and improve patient care. However, current level of investment in developing healthcare AI among members of the global digital health partnership does not seem to (...) yield a high return yet. This is mainly due to underinvestment in the supporting infrastructure necessary to enable the successful implementation of AI. If a healthcare-specific AI winter is to be avoided, it is paramount that this disparity in the level of investment in the development of AI itself and in the development of the necessary supporting system components is evened out. Recently, oysters have been identified by the US Food and Drug Administration (USFDA) as a risky food to eat because they may or may not contain the pathogenic bacteria Vibrio parahaemolyticus. The USFDA's attempts to manage the risk manifest themselves in a “Quantitative Risk Assessment”, a report that attempts to quantify and predict the number of oyster eaters that will fall ill from Vibrio. In seeking to produce knowledge and eliminate uncertainty, the USFDA, through the use of a discourse of (...) quantification, does the opposite. Instead, we argue, documents such as risk assessments are best understood as kinds of rhetorical practice. According to this perspective, these documents are epistemologically and ontologically reductive, produce uncertainty, politicize the act of eating, and serve an ironic function. (shrink)

Borders are essentially imaginary structures, but their effects are very real. This volume explores both geopolitical and conceptual borders through an interdisciplinary lens, bridging the disciplines of philosophy and literature. With contributions from scholars around the world, this collection closely examines the concepts of race, nationality, gender, and sexuality in order to reveal the paradoxical ambiguities inherent in these seemingly solid binary oppositions, while critiquing structures of power that produce and police these borders. The paper explores conceptual approaches to business ethics from the Japanese tradition and their potential to enhance our global approach to social and environmental sustainability, including discussion of a framework for understanding the embeddedness of the business in society. As globalization and economic and sociopolitical challenges proliferate, the nature of the connections between the USA and Asia is more important than ever. Following an expressed “pivot” or “rebalance” to Asia and the current nebulous alliances, we hope to raise the profile (...) of Japan’s potential to shape the conception and practice of business in society. We explore attempts to offer a universal business ethic, intended as guidance for businesses globally, and examine contributions of Japanese thought to these frameworks. Considering the traditional approaches of sanpoyoshi, or tri-directional welfare in business transactions, kyosei, which can mean “living and working together for the common good”, and mottainai, or “grateful and sustainable consumption,” the research explores the relationships between the private sector, government, and civil society. Further, we examine the related notion of moralogy, which has been described as a virtue-based stakeholder approach to business. We suggest that these concepts merit promoting the conception of the “homo socio-economicus” model to replace the prevailing “homo economicus” model that threatens what sound business should be. Through interviews with Japanese scholars and practitioners and exploration of Japanese cultural traditions, we present an overview of these approaches. With this perspective, we cite the case of the Tōhoku earthquake and Fukushima nuclear disaster as one illustrative example. We hope that this understanding of the embeddedness of business in society based on Japanese traditions and experience can contribute to a global conception of the role of business in society, relevant to the USA as well. Our goals are to contribute to existing discussions of Japanese business ethics and relevance to a global perspective, and to inspire ongoing exploration of applications of these ideas in teaching and scholarship. Despite unprecedented medical advancements and the near eradi­cation of many serious diseases, there are growing epidemics of preventable illness brought about in part by the overemphasis on individual autonomy and the neglect of obligations to others. Insofar as these diseases develop because of individual choice, this permissiveness hampers the moral analysis of growing epidemics like childhood obesity. While society has contributed to its rapid progression, childhood obesity finds its origins in lifestyle choices implemented at home. Consequently, parents have an unparalleled (...) duty to prevent and correct obesity and unhealthy lifestyles in their children. Failure to do so undoubtedly violates a parent’s duty and suggests medical neglect. However, our current understanding of medical neglect is too narrow to be applicable to chronic, preventable illnesses. Relevant principles of tort law may broaden our understanding of neglect to better reflect the nature of parental and societal liability in preventable illnesses. (shrink)

Disability through the Lens of Justice offers a contextual framework for considering the limitations that disability places on individuals. Specifically, those that prevent individuals from having control in certain domains of their life, by restricting the availability of acceptable options or the ability to choose between them. Begon argues that our theory of justice should be concerned with the lives individuals can lead, and not with whether their bodies and minds function typically. The problem that disability raises is not the (...) mere fact of difference, but the ways in which that difference is accommodated (or not) and the limitations it may cause. In Disability Through the Lens of Justice, Begon offers a new framework to the disability and justice model. She argues that achieving justice does not require 'normalisation', or the elimination of difference, but through implementating a model which enables all individuals to control their lives as they choose. Background: Occupational healthcare is nowadays more and more regulated by economic demands of gain and efficiency. Aim and research questions: The aim of this study is to reach a new understanding of ethics in occupational healthcare by uncovering this from occupational healthcare nurses’ own views. The research questions are as follows: (1) What is the core of an ethical value base in occupational healthcare? (2) What does it mean for occupational healthcare nurses to fulfill their ethical value base in occupational (...) healthcare? Research design, participants, and research context: The study uses a hermeneutical approach. The context of the study is occupational healthcare. The material was collected through interviews with four occupational healthcare nurses in Finland. The texts were interpreted through hermeneutical reading. Ethical considerations: The data storage, study participation, and data handling for research purposes were approved by the participants when they provided their informed consent. Findings: The study revealed that the core of ethical values in occupational healthcare is something universal that concerns justice, honesty, and faithfulness. To fulfill the ethical value base means to continuously strive toward the inner core of love and bravely choose to listen to the this core in the name of the client and the universal good even if it involves a struggle or a sacrifice. Discussion: The ethical value base is not primarily tied to profession but is something that is general and universal. Nor do ethics and the implementation of ethical values stand in opposition to, for example, external values such as demands for efficiency and economic gain. However, ethics may promote efficiency. Conclusion: The fulfillment of nurses’ ethical value base in occupational healthcare requires that ethics first and foremost is allowed to enter the leadership and is prioritized on a leadership level. Creating caring cultures that encourage openness and support ethical discussions is fundamental. (shrink)

Recently, a range of educational theorists have explored and extended upon popular currents in political theory through articulating “open” and “unknowing” pedagogies. Such contributions represent a radical turn away from the presumed “universals” found in proclamations of justice and emancipation and, ultimately, the centering of class analysis. At the same time, inspired by and building upon Bourdieuian theory, another cluster of educational research has developed a nuanced understanding of the social, cultural, and educational mechanisms involved in class reproduction. In this (...) essay, Jessica Gerrard offers a critical — though sympathetic — response to these dual trends. Bringing together theories of reproduction in conversation with theories of pedagogical possibility, Gerrard argues for a renewed understanding of working-class relations to education that incorporates an understanding of working-class action and struggle. An ICU and Palliative Care specialist featured in the Netflix documentary Extremis offers a framework for a better way to exit life that will change our medical culture at the deepest level In medical school, no one teaches you how to let a patient die. Jessica Zitter became a doctor because she wanted to be a hero. She elected to specialize in critical care--to become an ICU physician--and imagined herself swooping in to rescue patients from the brink of death. (...) But then during her first code she found herself cracking the ribs of a patient so old and frail it was unimaginable he would ever come back to life. She began to question her choice. Extreme Measures charts Zitter's journey from wanting to be one kind of hero to becoming another--a doctor who prioritizes the patient's values and preferences in an environment where the default choice is the extreme use of technology. In our current medical culture, the old and the ill are put on what she terms the End-of-Life Conveyor belt. They are intubated, catheterized, and even shelved away in care facilities to suffer their final days alone, confused, and often in pain. In her work Zitter has learned what patients fear more than death itself : the prospect of dying badly. She builds bridges between patients and caregivers, formulates plans to allay patients' pain and anxiety, and enlists the support of loved ones so that life can end well, even beautifully. Filled with rich patient stories that make a compelling medical narrative, Extreme Measures enlarges the national conversation as it thoughtfully and compassionately examines an experience that defines being human.
