I review the recent case of Edna Folz, a 73 year-old woman who was suffering through the end stages of very advanced Alzheimer's dementia when her case was adjudicated by the Wisconsin Supreme Court. I consider this case as an example of how courts are increasingly misinterpreting the ethical and legal decision-making standards known as substituted judgment and best interests and thereby threatening individuals' treatment decision-making rights as developed by other courts over the past two decades and creating serious (...) roadblocks to health-care providers' ability to render appropriate patient care. The Wisconsin Supreme Court held that Edna's legal guardian could not authorize withdrawal of Edna's treatment, ruling that as a matter of law, if an incompetent person is not in a persistent vegetative state, it is not in his or her best interests for life-sustaining treatment to be withdrawn unless (s)he has executed an advance directive or other statement clearly indicating his or her desires. This article provides a discussion of the limits of both narrative and culture based on a close textual analysis of the short story, “People Like That Are the Only People Here: Canonical Babbling in Peed Onk,” by Lorrie Moore. In this story, a mother describes her experiences on a pediatric oncology ward when her infant son develops Wilms' tumor. The authors examine how the story satirically portrays the spurious claims of language, story, and culture to protect us from an unjust (...) universe and then exposes their false promises. The various personal, professional, and genre-specific narratives we use to create order and coherence from the terror of serious illness are ultimately ineffective. Similarly, the superficially comforting culture of the hospital ward cares more about creating the illusion of control than it does about the suffering of sick children. Language and culture cannot make sense of human anguish, the article concludes, yet they are all we have to hold back the chaos. Mystery and uncertainty, as part of the human condition, must become part of our stories and part of our culture. (shrink)

We provide a conceptual framework and a case study of how an organization links its mission and espoused values with its operating practices. Conceptually, we locate this mission integration theme within Simons’ management accounting and control framework, and then adapt Schatzki’s site ontology of social practice to develop general research expectations for case studies of espoused values/practice linkages. Empirically, we apply the conceptual framework to a case study of linkages among an actual company’s espoused values, human resource practices, and financial (...) management during its 40-plus year history. This chapter provides an overview of second-order logic and higher-order logic generally. It provides the basic formal languages, deductive systems, and model-theoretic semantics, including a brief account of George Boolos’s interpretation of second-order languages in terms of the plural construction. It then goes into some of the arguments in favor of second-order logic.

Graham Priest introduces an informal but presumably rigorous and sharp ‘provability predicate’. He argues that this predicate yields inconsistencies, along the lines of the paradox of the Knower. One long-standing claim of Priest’s is that a dialetheist can have a complete, decidable, and yet sufficiently rich mathematical theory. After all, the incompleteness theorem is, in effect, that for any recursive theory A, if A is consistent, then A is incomplete. If the antecedent fails, as it might for a dialetheist, then (...) the consequent may also fail to hold. One somewhat friendly purpose of my ‘Incompleteness and inconsistency’ was to improve the technical situation for the dialetheist, eschewing reliance on an informal provability predicate. Another, less friendly purpose was to bring out what I took to be some untoward consequences of the situation. It seems that Priest accepted at least some of the improvements that I attempted. In the second edition of In contradiction, he responded to the alleged untoward consequences. One purpose of this note is to revisit the technical and philosophical situation. There were some errors in my original presentation, brought out by discussion with Priest and by Hartry Field’s analysis of the second incompleteness theorem in such contexts. A second task here is to present a sort of Curry version of the Gödel incompleteness situation. I tentatively conclude that even for a dialetheist, an interesting and complete theory is not as easy to come by as it may look—at least not for theories of arithmetic that are plausible for a dialetheist. The Phenomenology of Spirit has been in rich and equal measures a source of both frustration and fascination to its readers. Coming to it from the more conventional texts of our tradition readers have been puzzled, first, by the structure of the Phenomenology. Despite his suggestions that he is following an actual historical development of some sort Hegel will pass from the Terror of 1793–94 to prehistoric religions of nature, or from Kantian universality in morality to the life of the (...) Greek polis. In addition the Phenomenology contains a vast number of allusions to particular texts and authors which seems disproportionate to its claim to have followed a necessary path to absolute knowledge. One may have the impression that the highway of despair has been so named because of its constant and confusing detours into wildernesses which have only the most peripheral connection with the promised land of spirit. All of this has prompted an amazing quantity of ingenious hermeneutical activity. The leading directions in such exegesis can be sorted out into the logical, the existential, and the poetic; each is governed by the intention of saving and preserving the integral value of the text. There is also, of course, a skeptical reading of the Phenomenology, often appealing to philological evidence, which attempts to suggest, that what we are dealing with is at best a patchwork of essays on various subjects and with different purposes, hurriedly put together to meet the demands of the printer. I regard the patchwork theory as a last resort and will pass over it in silence here, since I have not yet been reduced to its level of desperation or irony. The logical reading such as that given in recent years by Stanley Rosen claims that the Phenomenology presupposes the Logic rather than serving as an introduction to it; but Hegel repeatedly says that the Phenomenology is such an introduction or ladder to the standpoint of science. An existential approach to the text finds Hegel to have surrendered joyously to the drama and possibilities of the Lebenswelt, both contemporary and historical. The paper seeks to provide a context for debate on the impact of new work organisation on employees. By reviewing new work organisation literature and considering a practical case example the paper calls for further research which demonstrates a strategic and implementation approach that can yield benefits for business and employees.

Recent studies have exposed the startling inadequacy of health care providers knowledge about and practice of effective pain management. For example, in one study, it was reported that 79 percent of a random sample of 454 medical-surgical inpatients experienced pain during hospitalization, and that 58 percent of patients with pain considered the pain horrible or excruciating. In another study, 67 percent of 2,415 randomly selected hospitalized patients had pain during the twenty-four hours prior to being interviewed, and 50 percent reported (...) pain at the time of the interview. In a study of seriously ill hospitalized patients reported in 1996, half of the patients complained of pain, and one-sixth reported that they experienced extremely severe pain at least half the time. According to one literature review, 75 percent of cancer patients have reported suffering pain, and one study estimates that 25 percent of cancer patients die with severe unrelieved pain. Chronic nonmalignant pain has been described as an extremely prevalent problem, and over two-thirds of nursing home residents experience serious pain. (shrink)

Moving beyond both realist and anti-realist accounts of mathematics, Shapiro articulates a "structuralist" approach, arguing that the subject matter of a mathematical theory is not a fixed domain of numbers that exist independent of each other, but rather is the natural structure, the pattern common to any system of objects that has an initial object and successor relation satisfying the induction principle.

Logical pluralism is the view that different logics are equally appropriate, or equally correct. Logical relativism is a pluralism according to which validity and logical consequence are relative to something. Stewart Shapiro explores various such views. He argues that the question of meaning shift is itself context-sensitive and interest-relative.

Recently, the U.S. House of Representatives voted, for the second time in two years, to ban all human-cloning research, whether the research involves reproduction or creating cells that might be used to understand and treat disease. As I explain in this article, the proposed legislation has important implications not only for human cloning research but also for research in general.

Resumo: Neste artigo, argumentamos a favor de uma versão sofisticada do realismo científico, tendo como eixo norteador a análise do desempenho do que consideramos ser o seu elemento de defesa estratégico: o “argumento do milagre”. O realismo científico é a perspectiva comprometida com as nossas melhores teorias científicas, isto é, com a existência de entidades, processos, relações etc., observáveis ou inobserváveis, indispensáveis para explicar o seu sucesso empírico, em particular, com aqueles componentes das teorias que são cruciais para se alcançar (...) novas previsões bem-sucedidas. O argumento do milagre é um tipo de abdução ou inferência da melhor explicação e se expressa na célebre formulação de Putnam : “O realismo científico é a única filosofia que não faz do sucesso da ciência um milagre.” Analisamos e rebatemos duas importantes modalidades de argumentação antirrealista: a indução pessimista e a circularidade viciosa da inferência da melhor explicação. Acreditamos estar justificados quanto à defesa da intuição básica do realismo científico, com o apoio de uma versão fortalecida e articulada do argumento do milagre, mediante as qualificações de novidade preditiva e fecundidade teórica: a ciência é bem-sucedida em explicar e prever fenômenos, inclusive novos, porque suas melhores teorias são verdadeiras e as entidades inobserváveis postuladas por essas teorias realmente existem. Concluímos que o argumento do milagre continua sendo basilar e estratégico na defesa do realismo científico.: In this article, we argue in favor of a sophisticated version of scientific realism, based on an analysis of what we consider to be the strategic element of its defense: the “miracle argument”. Scientific realism is the perspective applicable to our best scientific theories; that is, it applies to entities, processes, and relationships, etc., whether observable or unobservable, which are indispensable for explaining the empirical success of theories, and it applies particularly to those components of the theories that are crucial to achieving new and successful forecasts. The miracle argument is a type of abduction or inference to the best explanation, and is expressed in the famous formulation of Putnam : “[Scientific] realism is the only philosophy that doesn’t make the success of the science a miracle.” We analyze and refute two important modes of antirealist argument: pessimistic induction and vicious circularity of inference to the best explanation. We believe that we are justified in defending the basic intuition of scientific realism, supported by a strengthened version of the miracle argument, together with the attributes of predictive novelty and theoretical fecundity. From this perspective, science succeeds in explaining and predicting phenomena, including new phenomena, because its best theories are true, and the unobservable entities postulated by these theories actually exist. We conclude that the miracle argument continues to be basic and strategic in the defense of scientific realism. (shrink)

The central contention of this book is that second-order logic has a central role to play in laying the foundations of mathematics. In order to develop the argument fully, the author presents a detailed description of higher-order logic, including a comprehensive discussion of its semantics. He goes on to demonstrate the prevalence of second-order concepts in mathematics and the extent to which mathematical ideas can be formulated in higher-order logic. He also shows how first-order languages are often insufficient to codify (...) many concepts in contemporary mathematics, and thus that both first- and higher-order logics are needed to fully reflect current work. Throughout, the emphasis is on discussing the associated philosophical and historical issues and the implications they have for foundational studies. For the most part, the author assumes little more than a familiarity with logic comparable to that provided in a beginning graduate course which includes the incompleteness of arithmetic and the Lowenheim-Skolem theorems. All those concerned with the foundations of mathematics will find this a thought-provoking discussion of some of the central issues in the field today. (shrink)

Background Antimicrobial resistance (AMR) is a global threat to human and livestock health. Although AMR is driven by use of antimicrobials, it is often attributed to “misuse” and “overuse”, particularly for antibiotics. To curb resistance, there has been a global call to embrace new forms of moral personhood that practice “proper” use, including prescription, dispensing and consumption of antimicrobials, especially antibiotics. This paper seeks to reflect on complex questions about how morality has become embedded /embodied in the AMR discourse as (...) presented in the data collected on antimicrobial prescription, dispensing and use in human and livestock health in Tanzania, primarily focusing on antibiotics. Methods This reflection is anchored on Jarrett Zigon’s morality framework that is comprised of three dimensions of discourse; the institutional, public, and embodied dispositions. The data we use within this framework are derived from a qualitative study targeting human and animal health care service providers and community members in northern Tanzania. Data were collected through 28 in-depth interviews and ten focus group discussions and analysed through content analysis after translation and transcription. In addition, a review of the Tanzania’s National Action Plans on antimicrobial resistance was conducted. Results Application of the framework demonstrates points of convergence and divergence in the institutional morality discourse articulated by the Tanzania National Action Plans, the public discourse and the embodied dispositions/ lived experiences of human and animal health care service providers and community members. We demonstrate that AMR is not just associated with “inappropriate” behaviour on the part of drug prescribers, dispensers, and users but also with shortcomings in health systems and service delivery. Conclusion Antibiotic dispensing and use practices that may be associated with the development of AMR should not be viewed in isolation from the broader health context within which they occur. Stewart Shapiro's aim in Vagueness in Context is to develop both a philosophical and a formal, model-theoretic account of the meaning, function, and logic of vague terms in an idealized version of a natural language like English. It is a commonplace that the extensions of vague terms vary with such contextual factors as the comparison class and paradigm cases. A person can be tall with respect to male accountants and not tall with respect to professional basketball players. The main (...) feature of Shapiro's account is that the extensions of vague terms also vary in the course of a conversation, even after the external contextual features, such as the comparison class, are fixed. A central thesis is that in some cases, a competent speaker of the language can go either way in the borderline area of a vague predicate without sinning against the meaning of the words and the non-linguistic facts. Shapiro calls this open texture, borrowing the term from Friedrich Waismann.The formal model theory has a similar structure to the supervaluationist approach, employing the notion of a sharpening of a base interpretation. In line with the philosophical account, however, the notion of super-truth does not play a central role in the development of validity. The ultimate goal of the technical aspects of the work is to delimit a plausible notion of logical consequence, and to explore what happens with the sorites paradox.Later chapters deal with what passes for higher-order vagueness - vagueness in the notions of 'determinacy' and 'borderline' - and with vague singular terms, or objects. In each case, the philosophical picture is developed by extending and modifying the original account. This is followed with modifications to the model theory and the central meta-theorems.As Shapiro sees it, vagueness is a linguistic phenomenon, due to the kinds of languages that humans speak. But vagueness is also due to the world we find ourselves in, as we try to communicate features of it to each other. Vagueness is also due to the kinds of beings we are. There is no need to blame the phenomenon on any one of those aspects. (shrink)
