Rationale Treatment guidelines recommend a more conservative surgical approach than mastectomy for early stage breast cancer and a stronger emphasis on adjuvant therapy. Registry data at South Australian teaching hospitals have been used to monitor survivals and treatment in relation to these guidelines.Aims and objectives To use registry data to: (1) investigate trends in survival and treatment; and (2) compare treatment with guidelines.Methods Registry data from three teaching hospitals were used to analyse trends in primary courses of treatment of breast (...) cancers during 1977–2003 (n = 4671), using univariate analyses and multiple logistic regression. Disease-specific survivals were analysed using Kaplan–Meier product limit estimates and multivariable Cox proportional hazards regression.Results The 5-year survival was 79.9%, but with a secular increase, reaching 83.6% in 1997–2003. The relative risk of death (95% confidence limits) was 0.74 (0.62, 0.88) for 1997–2003, compared with previous diagnoses, after adjusting for tumour node metastasis stage, grade, age and place of residence. Treatment changes included an increase in conservative surgery (as opposed to mastectomy) from 51.7% in 1977–1990 to 76.8% in 1997–2003 for stage I (P < 0.001) and from 31.1% to 52.2% across these periods for stage II (P < 0.001). Adjuvant radiotherapy also became more common (P < 0.001), with 20.6% of patients receiving this treatment in 1977–1990 compared with 60.7% in 1997–2003. Radiotherapy generally was more prevalent when conservative surgery was provided, although also relatively common in mastectomy patients when tumour diameters exceeded 50 mm or when there were four or more involved nodes. The proportion of patients receiving chemotherapy increased (P < 0.001), from 19.6% in 1977–1990 to 36.9% in 1997–2003, and the proportion having hormone therapy also increased (P < 0.001), from 34.3% to 59.4% between these periods.Conclusions Survivals appear to be increasing and treatment trends are broadly consistent with guideline directions, and the earlier research on which these recommendations were based. The stability of the power grid is concernment due to the high demand and supply to smart cities, homes, factories, and so on. Different machine learning and deep learning models can be used to tackle the problem of stability prediction for the energy grid. This study elaborates on the necessity of IoT technology to make energy grid networks smart. Different prediction models, namely, logistic regression, naïve Bayes, decision tree, support vector machine, random forest, XGBoost, k-nearest neighbor, and optimized artificial neural (...) network, have been applied on openly available smart energy grid datasets to predict their stability. The present article uses metrics such as accuracy, precision, recall, f1-score, and ROC curve to compare different predictive models. Data augmentation and feature scaling have been applied to the dataset to get better results. The augmented dataset provides better results as compared with the normal dataset. This study concludes that the deep learning predictive model ANN optimized with Adam optimizer provides better results than other predictive models. The ANN model provides 97.27% accuracy, 96.79% precision, 95.67% recall, and 96.22% F1 score. (shrink)

The extensive community use of complementary medicine can no longer be overlooked in the practice of hospital medicine. Protocols need to be developed and implemented so that health professionals can deal with the issues surrounding the use of CM. Policy development has generally focussed on the supply of CM in hospital but another approach, which is based on consideration of the ethical and legal context, is presented here. Such an approach demands clarification of institutional policy for individuals who are competent (...) and those who are incapable of self-administration or non-competent. (shrink)

Existing research has shown that norm violations influence causal judgments, and a number of different models have been developed to explain these effects. One such model, the necessity/sufficiency model, predicts an interac- tion pattern in people’s judgments. Specifically, it predicts that when people are judging the degree to which a particular factor is a cause, there should be an interaction between (a) the degree to which that factor violates a norm and (b) the degree to which another factor in the (...) situation violates norms. A study of moral norms (N = 1000) and norms of proper functioning (N = 3000) revealed robust evidence for the predicted interaction effect. The implications of these patterns for existing theories of causal judgments is discussed. (shrink)

In our everyday moral deliberations, we attend to two central types of considerations – outcomes and moral rules. How these considerations interrelate is central to the long-standing debate between deontologists and utilitarians. Is the weight we attach to moral rules reducible to their conduciveness to good outcomes (as many utilitarians claim)? Or do we take moral rules to be absolute constraints on action that normatively trump outcomes (as many deontologists claim)? Arguments over these issues characteristically appeal to commonsense intuitions about (...) various cases. As a result, an important portion of the debate involves empirically tractable questions — questions that can be investigated by probing for people’s judgments in cases in which the two types of considerations come into conflict with one another. In theMetaphysics, Aristotle pointed out that some activities are engaged in for their own sake, while others are directed at some end. The test for distinguishing between them is to ask, ‘At any time during a period in which someone is Xing, is it also true that they have Xed?’ If both are true, the activity is being done for its own sake. If not, it is being done for the sake of some end other than itself. For example, if (...) I am thinking, it is true that I have thought. But if I’m making a blouse, it is not true that I have made a blouse, at least not this particular blouse. That’s not true until I have completed the project.There have been a number of attempts to deepen our understanding of this distinction. Anthony Kenny devoted a chapter of Action, Emotion, and Will to this issue, exploring the effect tense has on implication relations, and using that as a basis for dividing verbs of action into state-verbs, activity-verbs and performance-verbs. In more recent years the trend has been to generalize these categories so as to include occurrences other than actions, i.e., occurrences which do not involve intentions. While interest in this area tends to focus primarily on linguistic issues, such as the categorization of verbs, or on the logical analysis of sentences, there has been some interest in related metaphysical issues. In 1978 Alexander Mourelatos published ‘Events, Processes, and States,’ a paper which has turned out to be quite influential, in which he proposes an ontological trichotomy of occurrences. In his view, processes and events form distinct categories within the general category of occurrences. In this paper I will examine the reasoning underlying Mourelatos’s claim, arguing that the differences between processes and events cannot provide the basis for an ontological subcategorization of occurrences. This paper presents a model for the structure of universal frameworks in logic, mathematics, and physics that are closed to logical conclusion by the mechanism of paradox across a dualism of elements. The prohibition takes different forms defined by the framework of observation inherent to the structure. Forms include either prohibition to conclusion on the logical relationship of internal elements or prohibition to conclusion based on the existence of an element not included in the framework of a first element. The (...) model is applied to logical arguments in philosophy, mathematics, and physics and is initially a geometrical analysis of quantum theory and its application in experiment. This article questions the commonly accepted belief that God, like a ceramic potter, is able to control the outcome of events even as the potter controls the outcome of the clay-throwing process. However, those who are familiar with the artistic process of creating with clay know well that this is a symbiotic dynamic between the creator and the clay, as is that between God and human events.

This article addresses the precarious place of transgender and gender non-cis persons in relation to their discrimination-protections in recent legal, medical, and ethical policies in the United States. At present, there exists a contradiction such that trans persons are considered "pathological" enough that they are included in the latest iteration of the American Psychiatric Association's Diagnostic and Statistical Manual (DSM-V) as "gender dysphoric," but they are not included in the category of "disabled" under the Americans with Disabilities Act (ADA). As (...) such, trans persons in America are subject to the stigma of pathology (albeit with medical treatment) without the full protections of the ADA. By contrast, transgender and non-cis-gender Americans find their queer cohorts who are HIV-positive to be fully protected by the ADA. We ask whether transgender and non-cis-gender persons should embrace their (already pathologized) personhood as a disability. Sometimes "choosing disability" affords more rights than it deploys stigma. (shrink)

A primary focus is whether there are such analogous issues, and whether we can find in ancient philosophy a notion that is comparable to "person" as understood in modern philosophy. Essays on modern philosophy reappraise the validity of the notion of person, while essays on classical philosophy take up the related questions of what being "human" entails in ancient (...) ethics and psychology, and whether we should regard ourselves as, essentially, human or rational beings. Subcreative sets, introduced by Blum, are known to coincide with the effectively speedable sets. Subcreative sets are shown to be the complete sets with respect to S-reducibility, a special case of Turing reducibility. Thus a set is effectively speedable exactly when it contains the solution to the halting problem in an easily decodable form. Several characterizations of subcreative sets are given, including the solution of an open problem of Blum, and are used to locate the subcreative sets with respect to (...) the complete sets of other reducibilities. It is shown that q-cylindrification is an order-preserving map from the r.e. T-degrees to the r.e. S-degrees. Consequently, T-complete sets are precisely the r.e. sets whose q-cylindrifications are S-complete. (shrink)

It is often claimed that in the ancient world character was believed to be something fixed, given at birth and immutable during life. This belief is said to underlie the portrayal of individuals in ancient historiography and biography, particularly in the early Roman Empire; and tc constitute the chief point of difference in psychological assumptions between ancient and modern biography. In this article, I wish to examine the truth of these claims, with particular reference to Plutarch and Tacitus.

Positive Psychology has been instrumental in promoting wellbeing science in the modern era. However, there are still ways in which positive psychology interventions and positive education programmes can be improved to achieve more robust and sustained effects. One suggested method is to make wellbeing more salient and tangible through the use of objective tools that assess the relationship between psychological and physiological wellbeing, and enable wellbeing status and change to be seen. With the addition of an interdisciplinary team, as well (...) as technology-enabled and pedagogically sound learning tools and approaches, the potential for positive outcomes and impact increases exponentially. Monitoring wellbeing progress in this way can provide evidence, motivation and belief in positive psychology and wellbeing interventions. This can lead to engaged learning, sustained benefits and systemic impact. Positive psychology needs to strategically extend on the emerging work in this field to help everyone, including policy makers, notice and value wellbeing. (shrink)

On 3 August 2012, Shafilea Ahmed’s parents were convicted of her murder, nine years after the brutal ‘honour’ killing. The case offers important insights into how ‘honour’-based violence might be tackled without constructing non-Western cultures as inherently uncivilised. Critiquing the framing devices that structure British debates about ‘honour’-based violence demonstrates the prevalence of Orientalist tropes, revealing the need for new ways of thinking about culture that do not reify it or treat it as a singular entity that can only be (...) tackled in its entirety; instead, it is important to recognise that cultures consist of multiple, intersecting signifying practices that are continually ‘creolising’. Thus, rather than talking purely about culture, debates on ‘honour’-based violence should explore the intersection of culture with gender and other axes of differentiation and inequality. (shrink)

In this paper, we examine some of the ways in which Koizumi Junichiro took advantage of changes in television news to win the 2001 Liberal Democratic Party (LDP) presidential election and become prime minister of Japan. Koizumi adopted a strategy of political populism to increase his exposure in the media and develop a public reputation. Changes in the LDP selection procedure, in combination with long-term social and economic change and political reform, meant that the media mattered more to his campaign (...) than had previously been the case. We use data from the Japan Election Study II (JES II) to show that the effects of Koizumi' media-driven popularity and style of politics reversed the LDP' electoral fortunes in the Upper House Election in 2001. Shaftesbury believed that the height of virtue was impartial love for all of humanity. But Shaftesbury also harboured grave doubts about our ability to develop such an expansive love. In The Moralists, Shaftesbury addressed this problem. I show that while it may appear on the surface that The Moralists solves the difficulty, it in fact remains unresolved. Shaftesbury may not have been able to reconcile his view of the content of virtue with his view of our motivational psychology.

Erving Goffman’s “Asylums” is a key text in the development of contemporary, community-orientated mental health practice. It has survived as a trenchant critique of the asylum as total institution, and its publication in 1961 in book form marked a further stage in the discrediting of the asylum model of mental health care. In this paper, some responses from a range of disciplines to this text, 50 years on, are presented. A consultant psychiatrist with a special interest in cultural psychiatry and (...) mental health legislation, two collaborating psychotherapists in adult and forensic mental health, a philosopher, and a recent medical graduate, present their varying responses to the text. The editors present these with the hope of encouraging further dialogue and debate from service users, carers, clinicians, and academics and researchers across a range of disciplines. (shrink)

In 1998, the Labour government introduced legislation broadening British sentencing powers in relation to crimes aggravated by the offender's hostility towards the victim's actual or perceived race, religion, sexual orientation or disability. Gender is a notable omission from this list. Through a survey of eighty-eight stakeholders working in the violence against women (VAW) sector, this paper explores both the potential benefits and possible disadvantages of adding a gender-based category concerned with VAW to British hate crime legislation. The majority of participants (...) believed that a hate crime approach would offer significant benefits, especially in terms of the symbolic power of the law to send a message to society that VAW is unacceptable. However, most also recognised that the addition of a VAW category to current legislation would involve major practical and conceptual difficulties, not least those resulting from problematic assumptions about the nature of hate crimes versus VAW, and a general unwillingness on the part of policy-makers to address the socio-cultural inequalities that underpin VAW. Overall, the fact that the majority of participants favoured inclusion, on the basis that the possible symbolic benefits were likely to outweigh the potential practical disadvantages, is significant: it speaks to the power of hate crime legislation to challenge many forms of inequality and discrimination still endemic in British society. This article focuses on service provision for women who are involuntarily referred under the UK Mental Health Act (1983) into medium and high security care in England and Wales. We explore how physical and procedural security in such settings is prioritized over relational care (see also Fallon Report, Department of Health, 1999a and NHS Executive, 2000 – Tilt Report). We are not arguing against the importance of protecting the public from the acts of dangerous members of our society. However, we (...) are arguing that many of the women in our secure services are inappropriately placed and receive inappropriate forms of treatment and care. Rather than physical security, it is high relational care, which the women require. Further, we argue that current service provision often re-produces forms of violence and violation which have marked many of women's lives prior to their entry into the secure system. (shrink)
