The present study aimed at investigating the relationship between environmental and individual factors and Stress of Conscience among nursing staff in psychiatric in-patient care. A questionnaire involving six different instruments measuring Stress of Conscience, the ward atmosphere, the psychosocial work environment, Perceived Stress, Moral Sensitivity, and Mastery was answered by 93 nursing staff at 12 psychiatric in-patient wards in Sweden. The findings showed that Sense of Moral Burden, Mastery, Control at Work and Angry and Aggressive Behavior were related to Stress (...) of Conscience. We conclude that Mastery and Control at Work seemed to work as protective factors, while Sense of Moral Burden and perceptions of Angry and Aggressive Behavior made the nursing staff more vulnerable to Stress of Conscience. Future research should investigate whether measures to increase the level of perceived control and being part of decision making will decrease the level of Stress of Conscience among the staff. The main thesis of this paper is that we sometimes are disposed to accept false and even jointly inconsistent claims by virtue of our semantic competence, and that this comes to light in the sorites and liar paradoxes. Among the subsidiary theses are that this is an important source of indeterminacy in truth conditions, that we must revise basic assumptions about semantic competence, and that classical logic and bivalence can be upheld in the face of the sorites paradox.

David Bloor’s thought experiment is taken into consideration to suggest that the rationality of the Other cannot be inferred by way of argument for the reason that it is unavoidably contained as a hidden supposition by any argument engaged in proving it. We are able to understand a different culture only as far as we recognize in it the same kind of rationality that works in our own culture. Another kind of rationality is either impossible, or indiscernible.

Let the moral question of personal identity be the following: what is the nature of the entities we should focus our prudential concerns and ascriptions of responsibility around? (If indeed we should structure these things around any entities at all.) Let the semantic question of personal identity be the question of what is the nature of the entities that ‘person’ is true of. A naive (in the sense of simple and intuitive) view would have it that the two questions are (...) so intimately connected that the entities we should focus our concerns and ascriptions around are, pretty trivially, the persons. In part, my aim here is to evaluate this naive view. However, I will not actually attempt to give a definite verdict on it. One central debate in recent literature on epistemic normativity concerns the epistemic norm for action. This paper argues that this debate is afflicted by a category mistake: strictly speaking, there is no such thing as an epistemic norm for action. To this effect, I introduce a distinction between epistemic norms and norms with epistemic content; I argue that while it is plausible that norms of the latter type will govern action in general, epistemic norms will only govern actions characteristically associated (...) with delivering epistemic goods. DNR decisions are frequently made in oncology and hematology care and physicians and nurses may face related ethical dilemmas. Ethics is considered a basic competence in health care and can be understood as a capacity to handle a task that involves an ethical dilemma in an adequate, ethically responsible manner. One model of ethical competence for healthcare staff includes three main aspects: being, doing and knowing, suggesting that ethical competence requires abilities of character, action and knowledge. Ethical competence can be (...) developed through experience, communication and education, and a supportive environment is necessary for maintaining a high ethical competence. The aim of the present study was to investigate how nurses and physicians in oncology and hematology care understand the concept of ethical competence in order to make, or be involved in, DNR decisions and how such skills can be learned and developed. A further aim was to investigate the role of guidelines in relation to the development of ethical competence in DNR decisions. Individual interviews were conducted with fifteen nurses and sixteen physicians. The interviews were analyzed using thematic content analysis. Physicians and nurses in the study reflected on their ethical competence in relation to DNR decisions, on what it should comprise and how it could be developed. The ethical competence described by the respondents related to the concepts being, doing and knowing. In order to make ethically sound DNR decisions in oncology and hematology care, physicians and nurses need to develop appropriate virtues, improve their knowledge of ethical theories and relevant clinical guidelines. Ethical competence also includes the ability to act upon ethical judgements. Continued ethical education and discussions for further development of a common ethical language and a good ethical working climate can improve ethical competence and help nurses and physicians cooperate better with regard to patients in relation to DNR decisions, in their efforts to act in the best interest of the patient. BackgroundIn cancer care, do not resuscitate (DNR) orders are common in the terminal phase of the illness, which implies that the responsible physician in advance decides that in case of a cardiac arrest neither basic nor advanced Coronary Pulmonary Rescue should be performed. Swedish regulations prescribe that DNR decisions should be made by the responsible physician, preferably in co-operation with members of the team. If possible, the patient should consent, and significant others should be informed of the decision. Previous studies (...) have shown that physicians and nurses can experience ethical dilemmas in relation to DNR decisions, but knowledge about what ethical reasoning they perform is lacking. Therefore, the aim was to describe and explore what ethical reasoning physicians and nurses apply in relation to DNR-decisions in oncology and hematology care.MethodsA qualitative, descriptive and explorative design was used, based on 287 free-text comments in a study-specific questionnaire, answered by 216 physicians and nurses working in 16 oncology and hematology wards in Sweden. Comments were given by 89 participants.ResultsThe participants applied a situation-based ethical reasoning in relation to DNR-decisions. The reasons given for this were both deontological and utilitarian in kind. Also, expressions of care ethics were found in the material. Universal rules or guidelines were seen as problematic. Concerning the importance of the subject, nurses to a higher extent underlined the importance of discussing DNR-situations, while physicians described DNR-decisions as over-investigated and not such a big issue in their daily work.ConclusionThe study revealed that DNR-decisions in oncology and hematology care gave rise to ethical considerations. Important ethical values described by the participants were to avoid doing harm and to secure a peaceful and “natural” death with dignity for their dying patients. A preference for the expression “allow for natural death” instead of the traditional term “do not resuscitate” was found in the material. (shrink)

Background: A limited number of studies have shown that patient advocacy can be influenced by both facilitators and barriers which can encourage and discourage nurses to act as patient advocates. Objective: This study’s aim was to describe Swedish nurses’ perceptions of influencers on patient advocacy. Research design and context: Interviews with 18 registered nurses from different Swedish clinical contexts were analysed using the phenomenographic method. Ethical considerations: Ethical revisions were made in accordance with national legislation and guidelines by committees for (...) research ethics at Karlstad University. Findings: Three levels of hierarchically related influencers on patient advocacy were found in the descriptive categories. The fundamental influencer, the nurse’s character traits, was described in the perceptions that advocacy is influenced by nurse’s having a moral compass, having control over the care situation, being protective and feeling secure as a nurse. The second most vital influencer, the nurse’s bond with the patient, was expressed in the perceptions of knowing the patient and feeling empathy for the patient. The third level of influencers, the organisational conditions, was described in the perceptions that the organisational structures and organisational culture influence patient advocacy. Discussion: The results correspond with findings from earlier research but add an understanding that influencers on patient advocacy exist at three hierarchically related levels. Conclusion: The nurse’s character traits are the fundamental influencer to patient advocacy, but in order to be comfortable and secure when advocating for patients, nurses also need to be familiar with both the patient and the situation. Veganic agriculture, often described as farming that is free of synthetic and animal-based inputs, represents an alternative to chemical-based industrial agriculture and the prevailing alternative, organic agriculture, respectively. Despite the promise of veganic methods in diverse realms such as food safety, environmental sustainability, and animal liberation, it has a small literature base. This article draws primarily on interviews conducted in 2018 with 25 veganic farmers from 19 farms in the United States to establish some baseline empirical research on this farming (...) community. Its qualitative perspectives illuminate farmer perceptions of and experiences with veganic growing, including definitions, knowledge acquisition, values, and challenges. Results highlight a lack of agreement about the meaning of veganic agriculture in terms of allowable inputs and scope. Participants have drawn on a wide array of veganic and non-veganic resources to ascend their veganic production learning curves, also relying on experimentation and trial-and-error. Their farming is motivated by a diversity of real and perceived benefits, most notably consistency with veganism, food safety advantages, and plant and soil health benefits. Veganic product sourcing and the dearth of veganic agriculture-specific resources present considerable challenges to farmers. The article briefly discusses possibilities for developing veganic agriculture in the United States, such as through a US-based certification system and farmers’ associations, based on considerations of the trajectory of the US organic farming movement and veganic developments in Europe. Finally, the article suggests the importance of expanded research into soil health and fertility in plant-based systems to support practicing and potential veganic farmers. (shrink)

Background: Within oncology and hematology care, patients are sometimes considered to have such a poor prognosis that they can receive a do not resuscitate order from the physician responsible, stipulating that neither basic nor advanced coronary pulmonary rescue be performed in the event of a cardiac arrest. Studies on do not resuscitate decisions within oncology and hematology units, focusing on the specific role of the nurse in relation to these decisions, are scarce. Objective: The aim of this study was to (...) investigate hematology and oncology nurses’ experiences and perceptions of do not resuscitate orders, in order to achieve a deeper understanding of the nurses’ specific role in these decisions. Research design: A qualitative, descriptive methodology with individual semi-structured interviews was used. Participants and research context: A total of 15 nurses from eight hematology/oncology wards in four hospitals in Sweden were interviewed individually. Ethical considerations: In accordance with national regulations, an ethical review was not required for this study. The research followed international guidelines for empirical research, as outlined in the Helsinki Declaration. Findings: The nurses strived for good nursing care through balancing harms and goods and observing integrity and quality of life as important values. Experienced hindrances for good care were unclear and poorly documented decisions, uninformed patients and relatives, and disagreements among the caregivers and family. The nurses expressed a need for an ongoing discussion on do not resuscitate decisions, including all concerned parties. Conclusion: In order to provide good nursing care, nurses need clear and well-documented do not resuscitate orders, and patients and relatives need to be well informed and included in the decisions. To increase the understanding for each other’s opinions within the medical team, regular ethical discussions are required. (shrink)

Quality of Service (QoS) is becoming a crucial issue in service alteration because of the rapid growth in the number of cloud services. When evaluating cloud service functioning using several performance measures, the issue becomes more complex and non-trivial. It is therefore quite difficult and crucial for consumers to choose the best cloud service. This article explores the recent urban transformations of downtown Cairo, in particular around the area of Mohammed Mahmud Street and Tahrir Square, after a year and a half of violent confrontations between the protesters and the military junta. The article first looks at how these confrontations led to the segregation of the city through the use of buffer-concrete walls, army tanks, check-points and barbed-wire barricades that made life for its inhabitants impossible. The squeezing of Tahrir and its surroundings created mostly (...) a delineated and restricted war zone. This was undertaken after a series of killings and massacres took place. The article reflects upon the explosion of sardonic graffiti and epic murals that followed these events as a vibrant expression of dissenting street art, as well as the creation of a memorial space. Support in different modes, expressions and actions is at the core of the public welfare culture. In this paper, support is examined as an everyday interpersonal phenomenon with a variety of expressions in language and ways of relating, and its essential meaning is explored. The fulcrum for reflection is the lived experience shared by a young woman with mental health problems of her respective encounters with two professionals in mental health facilities. A phenomenological analysis of the contrasting accounts suggests that, (...) when the professional relationship includes openness and risk, a certain degree of freedom of action is possible for both parties involved in the inevitably asymmetrical relationship. Support as “given” eludes controllable and measurable objectives, but imposes itself on the lived experiences of both the giver and the receiver as subject to readiness for acceptance. By not making assumptions about what support is, we open ourselves to the possibility of reciprocally experiencing moments revealing the essential meaning of support as lived. (shrink)

Artificial intelligence (AI) national strategies provide countries with a framework for the development and implementation of AI technologies. Sixty countries worldwide published their AI national strategies. The majority of these countries with more than 70% are developed countries. The approach of AI national strategies differentiates between developed and developing countries in several aspects including scientific research, education, talent development, and ethics. This paper examined AI readiness assessment in a developing country (Palestine) to help develop and identify the main pillars of (...) the AI national strategy. AI readiness assessment was applied across education, entrepreneurship, government, and research and development sectors in Palestine (case of a developing country). In addition, it examined the legal framework and whether it is coping with trending technologies. The results revealed that Palestinians have low awareness of AI. Moreover, AI is barely used across several sectors and the legal framework is not coping with trending technologies. The results helped develop and identify the following five main pillars that Palestine’s AI national strategy should focus on: AI for Government, AI for Development, AI for Capacity Building in the private, public and technical and governmental sectors, AI and Legal Framework, and international Activities. When in the business of offering an account of the epistemic normativity of belief, one is faced with the following dilemma: strongly externalist norms fail to account for the intuition of justification in radical deception scenarios, while milder norms are incapable to explain what is epistemically wrong with false beliefs. This paper has two main aims; we first look at one way out of the dilemma, defended by Timothy Williamson and Clayton Littlejohn, and argue that it fails. Second, we identify (...) what we take to be the problematic assumption that underlies their account and offer an alternative way out. We put forth a knowledge-first friendly normative framework for belief which grants justification to radically deceived subjects while at the same time acknowledging that their false beliefs are not epistemically good beliefs. Assertion is the central vehicle for the sharing of knowledge. Whether knowledge is shared successfully often depends on the quality of assertions: good assertions lead to successful knowledge sharing, while bad ones don't. In Sharing Knowledge, Christoph Kelp and Mona Simion investigate the relation between knowledge sharing and assertion, and develop an account of what it is to assert well. More specifically, they argue that the function of assertion is to share knowledge with others. It is this function that (...) supports a central norm of assertion according to which a good assertion is one that has the disposition to generate knowledge in others. In his recent Analysis article (2012), Robert Williams considers two puzzles relating to indeterminacy. On the basis of these puzzles, he defends a seemingly radical view on the normative role of indeterminacy. He speaks of indeterminacy as ‘normatively silent’. There are two ways of understanding the view that Williams defends. On one understanding, the view ends up being indistinguishable from one of the more traditional views Williams rejects, the view that phenomena of different kinds fall under the umbrella level ‘indeterminacy’. According to anti‐reductionism in the epistemology of testimony, testimonial entitlement is easy to come by: all you need to do is listen to what you are being told. Say you like anti‐reductionism; one question that you will need to answer is how come testimonial entitlement comes so cheap; after all, people are free to lie.This paper has two aims: first, it looks at the main anti‐reductionist answers to this question and argues that they remain unsatisfactory. Second, it goes on a (...) rescue mission on behalf of anti‐reductionism. I put forth a novel, knowledge‐first anti‐reductionist account, which I dub ‘Testimonial Contractarianism’. According to the view defended here, in virtue of the social contract in play, compliance with the norms governing speech acts is the default position for speakers. Insofar as norm compliance is the default for speakers, I argue, all else equal, entitlement to believe is the default for hearers. (shrink)

How patients and nurses experience the acute care psychiatric environment The concept of the therapeutic milieu was developed when patients’ hospitalizations were long, medications were few, and one‐to‐one nurse–patient interactions were the norm. However, it is not clear how the notion of ‘therapeutic milieu’ is experienced in American acute psychiatric environments today. This phenomenological study explored the experience of patients and nurses in an acute care psychiatric unit in the USA, by asking them, ‘What stands out to you about this (...) psychiatric hospital environment?’ Three figural themes emerged, contextualized by time, which was a source of stress to both groups: for patients there was boredom, and for nurses, pressure and chaos. Although they shared some themes, nurses and patients experienced them differently. For instance, nurses felt caged‐in by the Plexiglas‐enclosed nursing station, and patients felt caged‐in by the locked doors of the unit. The findings from this US study do not support the existence of the therapeutic milieu as described in the literature. Furthermore, although the nurse–patient relationship was yearned for by nurses, it was nearly absent from patients’ descriptions. The caring experienced by patients was mainly derived from interactions with other patients. (shrink)

The metropolis of Cairo has witnessed unprecedented transformations since the January revolution of 2011. It witnessed evidently an escalation of war zones and confrontations between protesters and police forces; it also witnessed the militarization and policing of the urban sphere, the creation of segregating buffer walls that paralysed entire areas. However, the Tahrir effect remains evident in that it revolutionized the very notion of what a public space is about. It succeeded in imposing an entirely unprecedented novel choreography for the (...) city in which the ‘stage’ of Tahrir was the exemplary moment that triggered extended and replicated dramaturgical violent public confrontations, public performances and occupations in all the squares of Egypt. This article traces the transformations in relation to the debates pertaining to the ‘right to the city’ relating to the expanding visibility of the street vendors. Despite the military taking over after the rule of the Muslim Brotherhood, Cairo has witnessed for the past three years a mesmerizing flourishing art scene. (shrink)

My focus here will be Rudolf Carnap’s views on ontology, as these are presented in the seminal “Empiricism, Semantics and Ontology” (1950). I will first describe how I think Carnap’s distinction between external and internal questions is best understood. Then I will turn to broader issues regarding Carnap’s views on ontology. With certain reservations, I will ascribe to Carnap an ontological pluralist position roughly similar to the positions of Eli Hirsch and the later Hilary Putnam. Then I turn to some (...) interrelated arguments against the pluralist view. The arguments are not demonstrative. Some possible escape routes for the pluralist are outlined. But I think the arguments constitute a formidable challenge. There should be serious doubt as to whether the pluralist view, as it emerges after discussion of these arguments, will be worth defending. Moreover, there is an alternative ontological view which equally well subserves the motivations underlying ontological pluralism. (shrink)

This article considers the entanglements of neuroscience, economics and behaviourism in a two-year experiment (2017–18) with basic income in Finland. The participants in this mandatory, state-led experiment are unemployed individuals (25–58 years old) recruited by the National Social Insurance Institution. The experiment is a randomised controlled trial intended to provide useful information about the impacts of basic income on employment and well-being. Focusing on the epistemological foundations of the experiment, this analysis suggests that the Finnish trial with basic income should (...) be considered to be an example of the neuroliberal movement in policy-making as it uses behavioural economics and popularised neuroscience to optimise the cognitive abilities of the unemployed. The primary contribution of this paper is to raise concerns about how neuroliberalism reconfigures citizenship by obscuring the limits between freedom and control and how societies of control use neuroliberal models such as nudging to organise the disorganised and control the uncontrolled. As forms of employment and migration changed, the financial crisis which began in 2007 affected the human rights of individuals, particularly those in developing countries. How the media reported on these consequences is essential in understanding how and why public and political perceptions of the importance of human rights may have changed since the crisis began. Using quantitative and qualitative analysis of major newspapers in the UK and the USA, this paper seeks to understand the ways in which the media (...) presented the relationship between the negative changes in the economy and human rights. The analysis of the findings uses framing theory to understand how the text in news articles communicates the relevance of human rights to readers before concluding with some further avenues for research. In higher education programs that train students for professions with high standards of safety, such as aviation, shipping and healthcare, exercises in simulated environments provide opportunities for training in educational settings. This study explores the use of simulators in maritime education, taking an interest in how navigation training is achieved by using simulated environments. By conducting an interaction analysis of video data, the study examines how training students to coordinate with other vessels in traffic is topicalized in simulator exercises, focusing (...) on discourses of temporality in instructions. The results show how instruction during simulations is a continuous interactional achievement built on the ability to assess the fit between the assessment criteria at work in the specifics of the situation and the ongoing tasks as they unfold. During simulations temporality becomes a matter for instruction, both when assessing how to develop the students’ understanding and as a topic in its own right. The results highlight tightly coupled relationships among tasks, instruction and technology. The implications for simulator-based training call for refocusing on training tasks rather than specific skills, and emphasize the importance of professional guidance in order to guide the students toward the discourses of maritime work practice in simulator-based training. In this article I shall analyse and evaluate analytic theists’ views of what it takes to be a person of faith. I suggest that the subject can be approached by posing requirements a person must allegedly fulfil in order to count as a person of faith. These requirements can be referred to as aspects of faith. According to my analysis, four different aspects of faith can be distinguished: the cognitive, the evaluative-affective, the practical, and the interpersonal. This paper defends a novel view of hermeneutical epistemic injustice. To this effect, it starts by arguing that Miranda Fricker’s account is too restrictive: hermeneutical epistemic injustice is more ubiquitous than her account allows. That is because, contra Fricker, conceptual ignorance is not necessary for HEI: hermeneutical epistemic injustice essentially involves a failure in concept application rather than in concept possession. Further on, I unpack hermeneutical epistemic injustice as unjustly brought about basing failure. Last, I show that, if this view (...) right, HEI is a form of distributive injustice, and affords the corresponding traditional normative theorising. (shrink)

Neo-Fregeanism in the philosophy of mathematics consists of two main parts: the logicist thesis, that mathematics (or at least branches thereof, like arithmetic) all but reduce to logic, and the platonist thesis, that there are abstract, mathematical objects. I will here focus on the ontological thesis, platonism. Neo-Fregeanism has been widely discussed in recent years. Mostly the discussion has focused on issues specific to mathematics. I will here single out for special attention the view on ontology which underlies the neo-Fregeans’ (...) claims about mathematical objects, and discuss this view in a broader setting. (shrink)

This article reflects upon the monopoly and repertoires of violence in the city of Cairo perpetrated in counter-revolutionary moments by the successive military and Islamist regimes, which lack alternative visions and imaginaries. It counters the myth that the Egyptian revolution was non-violent. It also reflects upon some of the debates about the Arab revolutions, the question of militarization, and the return of ‘order’ with the re-emergence of the army in public life. It also reflects upon the multiplication of segregating walls, (...) first as buffer zones to isolate protesters, then as the walls of the gated communities and compounds of the rich, examining the extent to which the supposed revitalization of downtown Cairo actually represents the flight of wealth and capital to isolated areas far from the city center. Many theorists hold that there is, among value concepts, a fundamental distinction between thin ones and thick ones. Among thin ones are concepts like good and right. Among concepts that have been regarded as thick are discretion, caution, enterprise, industry, assiduity, frugality, economy, good sense, prudence, discernment, treachery, promise, brutality, courage, coward, lie, gratitude, lewd, perverted, rude, glorious, graceful, exploited, and, of course, many others. Roughly speaking, thick concepts are value concepts with significant descriptive content.
