There is little information regarding arterial elasticity in heart transplant recipients. This study aimed to characterize small and large artery elasticity (...) in heart transplant recipients in comparison with an asymptomatic population free of overt cardiovascular disease. A second goal was to identify demographic and clinical factors associated with arterial elasticity in this unique population.Methods: Arterial pulse waveform was registered noninvasively at the radial artery in 71 heart transplant recipients between 2008 and 2010. SAEs and LAEs were derived from diastolic pulse contour analysis. Comparisons were made to a healthy cohort of 1,808 participants selected from our prevention clinic database. Multiple regression analyses were performed to evaluate associations between risk factors and SAE and LAE within the heart transplant recipients.Results: LAE and SAE were significantly lower in heart transplant recipients than in the normal cohort. Female sex and history of ischemic cardiomyopathy were significantly associated with reduced LAE and SAE. Older age and the presence of moderate cardiac allograft vasculopathy were also significantly associated with reduced SAE. Transplant duration was associated with increased SAE.Conclusion: Heart transplants are associated with peripheral endothelial dysfunction and arterial stiffness, as demonstrated by a significant reduction in SAE and LAE when compared with a healthy population. Furthermore, cardiac allograft vasculopathy is associated with a significant reduction in SAE, suggesting a potential use in screening heart transplant recipients at risk for developing this complication.Keywords: heart transplant, endothelial function, arterial elasticity, cardiac allograft vasculopathy. Over the past seventy-five years, the UN has evolved significantly, often in response to geopolitical dynamics and new waves of thinking. In some respects, the UN has registered remarkable achievements, stimulating a wide range of multilateral treaties, promoting significant growth of human rights, and at times playing a central role in containing and preventing large-scale armed conflict. As part of the special issue on “The United Nations at Seventy-Five: Looking Back to Look Forward,” this essay argues that the organization has (...) been the most impactful in three areas: producing, shaping, and driving key ideas, particularly on development and rights; generating such effective operational agencies as UNICEF and the World Food Program; and, especially in the immediate post–Cold War period, addressing major conflict risks through the Security Council. Since then, however, the UN has struggled to meet emerging challenges on many fronts and been increasingly hampered by internal ossification and institutional sprawl as well as internecine dysfunction. The twenty-first century has confronted the UN with further challenges relating most notably to climate change; to risks arising from new technologies; and to the increasingly fraught relationships between China, Russia, and the United States. If the past seventy-five years can offer one lesson, it is that new thinking and new ideas will need to drive the organization to evolve still further and faster, or else risk irrelevance. (shrink)

The mechanisms for cultural transmission remain disputable and difficult to validate through observational field studies alone. If controlled experimental laboratory investigation reveals that a putative mechanism is demonstrable in the species under study, then inferences that the same mechanism is operating in the field observation are strengthened.

This paper presents methods to visualize the structure of trees that occur in genetic programming. These methods allow for the inspection of structure of entire trees even though several thousands of nodes may be involved. The methods also scale to allow for the inspection of structure for entire populations and for complete trials even though millions of nodes may be involved. Examples are given that demonstrate how this new way of “seeing” can afford a potentially rich way of understanding dynamics (...) that underpin genetic programming. The most important and common solution to the Pyrrhonian skeptic’s regress problem is foundationalism. Reason-giving must stop somewhere, argues the foundationalist, and the fact that it does stop does not threaten knowledge or justification. The foundationalist has a problem, though; while foundationalism might adequately answer skepticism, it does not allow for a satisfying reply to the skeptic. The feature that makes a belief foundationally justified is not the sort of thing that can be given to another as a reason. Thus, (...) if foundationalism is true, we can only fall silent in the face of a challenge to our epistemically basic beliefs. Call this the practical or existential problem of foundationalism. Thomas Reid offers a rather stunning solution to this problem. Humor, he thinks, can be used to defend basic beliefs which cannot be defended by argument. We develop and defend an account on which Reid is correct and emotions such as rueful amusement can be invoked to rationally persuade the skeptic to accept foundationally justified beliefs. Then, inspired by Kierkegaard, we extend the account to foundational moral and religious beliefs. (shrink)

This paper examines masculinity in relation to the modern tracking shot in Daren Aronofsky's The Wrestler and Derek Cianfrance's The Place Beyond the Pines. These films make prominent use of a particular tracking shot in which the camera floats behind a male character, his head neatly centred in the frame. The camera in these ‘follow-shots’ seems unmistakably but loosely tethered to the character's body. This paper attempts to ascertain the phenomenological nature and critical significance of that relationship, which in recent (...) films expresses the intimate distance and dissonance between mobile cameras, masculinity and the male body in the twenty-first century. Objective The establishment of patient-centered measures capable of empirically determining meaningful cognitive change after surgery can significantly improve the medical care of epilepsy patients. Thus, this study aimed to develop reliable change indices (RCIs) and standardized regression-based (SRB) change norms for a comprehensive neuropsychological test battery in the German language. Methods Forty-seven consecutive patients with temporal lobe epilepsy underwent neuropsychological assessments, both before and 12 months after surgery. Practice-effect-adjusted RCIs and SRB change norms for each test score were computed. To (...) assess their usefulness, the presented methods were applied to a clinical sample, and binary logistic regression analyses were conducted to model the odds of achieving improvement in quality of life (QOL) after surgery. Results The determined RCIs at 90% confidence intervals and the SRB equations for each test score included in the test battery are provided. Cohen’s kappa analyses revealed a moderate mean agreement between the two measures, varying from slight to almost perfect agreement across test scores. Using these measures, a negative association between improvement in QOL and decline in verbal memory functions after surgery was detected (adjusted odds ratio = 0.09, p = 0.006). Significance To the best of our knowledge, this study is the first to develop RCIs and SRB change norms necessary for the objective determination of neuropsychological change in a comprehensive test battery in the German language, facilitating the individual monitoring of improvement and decline in each patients’ cognitive functioning and psychosocial situations after epilepsy surgery. The application of the described measures revealed a strong negative association between improvement in QOL and decline in verbal memory functions after surgery. (shrink)

Despite the interpersonal nature of family meetings and the frequency in which they occur, the clinical ethics literature is devoid of any rich descriptions of what clinical ethicists should actually be doing during family meetings. Here, we propose a framework for describing and understanding “transitioning” facilitation skills based on a retrospective review of our internal documentation of 100 consecutive cases wherein a clinical ethicist facilitated at least one family meeting. The internal documents were analyzed using qualitative methodologies, i.e., “codes”, to (...) identify emergent themes. We identified four different transitioning strategies clinical ethicists use to reach a meaningful resolution. These transitioning strategies serve as a jumping-off point for additional analyses, future research, evaluating clinical ethics consultation, and overall performance improvement of a consultation service. This essay collides with the aesthetic of wilderness cultivated by the North American retail chain Bass Pro Shops. Through elaborate displays and décor that render each store part rustic lodge, aquarium, amusement park, natural history museum, and hunting simulator, the stores represent the natural world and its inhabitants as abundant resources for human consumption. The stores’ aesthetic is primarily wrought through the arrangement of taxidermied animals. These animals include both traditional wildlife mounts posed in lifelike attitudes as well as animatronic (...) taxidermy that becomes “alive” in response to players’ achievements in a shooting range game. By exploring the stores’ traditional and animatronic taxidermy as well as its conflation of animal and machine, this essay explores the conception of environmental conservation and animal ontology upheld by Bass Pro Shops. ABSTRACT This article attempts to show a way in which social science research can contribute in a meaningful and equitable way to philosophical bioethics. It builds on the social science critique of bioethics present in the work of authors such as Renée Fox, Barry Hoffmaster and Charles Bosk, proposing the characteristics of a critical bioethics that would take social science seriously. The social science critique claims that traditional philosophical bioethics gives a dominant role to idealised, rational thought, and tends to (...) exclude social and cultural factors, relegating them to the status of irrelevancies. Another problem is the way in which bioethics assumes social reality divides down the same lines/categories as philosophical theories. Critical bioethics requires bioethicists to root their enquires in empirical research, to challenge theories using evidence, to be reflexive and to be sceptical about the claims of other bioethicists, scientists and clinicians. The aim is to produce a rigorous normative analysis of lived moral experience. (shrink)

Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between (...) studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice. (shrink)

The purpose of the article is to challenge widely accepted views of the relationship among rationality, morality, and prudence. It contends that we cannot understand either the rational or the moral enterprise without a correct philosophical view of the human self, and that such a view of the self is impossible without taking account of the rational and the moral enterprises themselves. The paper concludes that the moral point of view is anchored in the nature of selfhood so that one (...) can be neither rational and immoral nor prudent and immoral. (shrink)

Are racial slurs always offensive and are racial stereotypes always negative? How, if at all, are racial slurs and stereotypes different and unequal for members of different races? Questions like these and others about slurs and stereotypes have been the focus of much research and hot debate lately, and in a recent article Embrick and Henricks (2013) aimed to address some of the aforementioned questions by investigating the use of racial slurs and stereotypes in the workplace. Embrick and Henricks (2013) (...) drew upon the empirical data they collected at a baked goods company in the southwestern United States to argue that racial slurs and stereotypes function as symbolic resources that exclude minorities but not whites from opportunities or resources and that racial slurs and stereotypes are necessarily considered as negative or derogatory irrespective of their particular context of use (pp. 197–202). They thus proposed an account of slurs and stereotypes that supports the context-insensitive position of Fitten (1993) and Hedger (2013) yet challenges the context-sensitive position of Kennedy (2002) and Croom (2011). In this article I explicate the account of racial slurs and stereotypes provided by Embrick and Henricks (2013), outline 8 of their main claims, and then critically evaluate these claims by drawing upon recent empirical evidence on racial slurs (both in-group and out-group uses) and stereotypes (for both whites and blacks) to point out both strengths and weaknesses of their analysis. Implications of the present analysis for future work on slurs and stereotypes will also be discussed. (shrink)

Progressive social movements correctly presume that justice demands treating people with humane regard: combining respect for human agency with concern for human vulnerability to suffering. Promoting humane regard is a critical means of acknowledging the moral claims of humanity. Some critics reject the underlying concept of a universal humanity, in virtue of which human beings form a distinct community of reciprocal moral obligation. Critics charge that the concept presumes indefensible dualisms (of mind and body, and humanity and nature); that it (...) wrongly assigns a privileged status to reason; and that it involves an unsupportable belief in human exceptionalism. I argue that we can assert the moral claims of humanity without privileging reason, repudiating nature, or denying that there are many valuable ways to be human. I also defend an account of human moral exceptionalism that does not imply human moral superiority, to show that we can meet morally weighty obligations to humans while affirming morally substantive connections to non-human communities and domains. But modern Western civilization seems to be especially prone to philosophical doubts about the moral enterprise because of widely shared assumptions and views in the modern age about the knowledge-yielding powers of the human mind. Affective variables have been shown to impact working memory and cognitive control. Theoretical arguments suggest that the functional impact of emotion on cognition might be mediated through shifting action dispositions related to changes in motivational orientation. The current study examined the effects of positive and negative affect on performance via direct manipulation of motivational state in tasks with high demands on cognitive control. Experiment 1 examined the effects of monetary reward on task-switching performance, while Experiment 2 examined the effects of (...) both rewards and punishments on working memory, using primary (liquid) reinforcers. In both experiments, dissociable trial-by-trial and contextual (block-related) enhancements of cognitive control during task performance were observed in relationship to motivational incentive value. Performance enhancements were equivalent in the reward and punishment conditions, but were differentially impacted by individual difference measures of trait reward and punishment sensitivity. Together, the results suggest both common and specific mechanisms by which approach and avoidance motivational states influence cognitive control, via activation of reward and punishment processing systems. Since its development in the middle of the 20th century, H.L.A. Hart’s oeuvre has become a beacon for the jurisprudential community. Hart turned out to be a giant on whose shoulders many have stood. Yet the significance in its own right of Hart’s work has been neglected in recent years because his work has too often been discussed in the context of the Hart-Dworkin-debate. Hopefully, Nicola Lacey’s recent biography of Hart,which is discussed in this review-article, will contribute to redress this (...) omission. BY OVEREMPHASIZING MATERIALISTIC VALUES, we have perverted the culture and set modern Western civilization on a self-destructive course. Some critics have said that the economy, science, and technology are the only healthy aspects of our society. We have what I have called a saber-toothed tiger civilization. In the evolutionary process, the saber-toothed tiger developed great tusks as effective weapons in combat, but perished because they obstructed its eating. We have developed a culture that is highly successful in advancing science and (...) technology and producing wealth and power, but in doing so we have undermined and subjectivized the sectors of the culture that underwrite our identity, the meaning of our existence, and the norms and values by which we live our lives, organize our society, and run our institutions, including even science and the economy. But unlike the saber-toothed tiger, we can discover our error and correct our course. (shrink)

This paper shows that moral progress is a substantive and plausible idea. Moral progress in belief involves deepening our grasp of existing moral concepts, while moral progress in practices involves realizing deepened moral understandings in behavior or social institutions. Moral insights could not be assimilated or widely disseminated if they involved devising and applying totally new moral concepts. Thus, it is argued, moral failures of past societies cannot be explained by appeal to ignorance of new moral ideas, but must be (...) understood as resulting from refusals to subject social practices to critical scrutiny. Moral philosophy is not the main vehicle for disseminating morally progressive insights, though it has an important role in processes that lead to moral progress. Yet we have grounds for cautious optimism, since progressive moral insights can be disseminated and can, sometimes, have constructive social effects. (shrink)
