The question whether PAT training programs should require aspiring therapists to undergo their own PAT—commonly referred to as “experiential training”—has received much attention within the field. In this article, we analyze the potential benefits of experiential training in PAT by applying the framework developed by Rolf Sandell et al. concerning the functions of any training therapy (the therapeutic, modeling, empathic, persuasive, and theoretical functions). We then explore six key (...) domains in which risks could arise through mandatory experiential training: physical and psychological risks; negative impact on therapeutic skill; justice, equity, diversity, and inclusion; dual relationships; privacy and confidentiality; and undue pressure. Ultimately, we argue that experiential training in PAT should not be mandatory. Because many PAT training programs already incorporate experiential training methods, our exploration of potential harms and benefits may be used to generate comprehensive risk-mitigation strategies. The combination of the issue of return of individual genetic results/incidental findings and paediatric biobanks is not much discussed in ethical literature. The traditional arguments pro and con return of such findings focus on principles such as respect for persons, autonomy and solidarity. Two dimensions have been distilled from the discussion on return of individual results in a genetic research context: the respect for a participant’s autonomy and the duty of the researcher. Concepts such as autonomy and solidarity do not (...) fit easily in the discussion when paediatric biobanks are concerned. Although parents may be allowed to enrol children in minimal risk genetic research on stored tissue samples, they should not be given the option to opt out of receiving important health information. Also, children have a right to an open future: parents do not have the right to access any genetic data that a biobank holds on their children. In this respect, the guidelines on genetic testing of minors are applicable. With regard to the duty of the researcher the question of whether researchers have a more stringent duty to return important health information when their research subjects are children is more difficult to answer. A researcher’s primary duty is to perform useful research, a policy to return individual results must not hamper this task. The fact that vulnerable children are concerned, is an additional factor that should be considered when a policy of returning results is laid down for a specific collection or research project. Successivement lieu d'accueil, maison d'enfermement, « machine à guérir », pivot de l'action sanitaire, il est aujourd'hui à la fois une administration et une entreprise de soins. Cependant, sous la pression de quelques évolutions sociales fondamentales, l'hôpital ne serait-il pas en train de s'ouvrir progressivement sur l'extérieur, de se transformer de (...) « citadelle » du service public de santé en « espace public » aux vertus culturelles et solidaires. Parmi ces évolutions, le développement d'initiatives solidaires en milieu hospitalier, telles que l'organisation de solidarités de proximité en partenariat avec le « dehors », paraît jouer un rôle essentiel.Hospitals are microcosms of society, symbolic places where basic human and social relationships are formed and places of memory and as such have witnessed great changes to their functions over the years. In turn they have been places of welcome, places of confinement, “healing machines”, the mainspring of health policies and nowadays seem to be both government-run institutions and healthcare businesses however contradictory that may sometimes appear. However, the pressure of a few basic social shifts seems to have led hospitals to gradually open up to the outside world, turning themselves from publich health system strongholds into “spaces open to the public” with culturally supportive values. Among these shifts in society, the development of support initiatives in the hospital environment such as local community-based solidarity projects run in partnership with “the outside world” seems to be playing an important role. Since the 1990s we witness a rise in public apologies. Are we living in the `Age of Apology¿? Interesting research questions can be raised about the opportunity, the form, the meaning, the effectiveness and the ethical implications of public apologies. Are they not merely a clever and easy device to escape real and tangible responsibility for mistakes or wrong done? Are they not at risk to become well-rehearsed rituals that claim to express regret but, in fact, avoid doing so? Research on an expert system regarding unemployment insurance law has pointed to the difficulties of explicitly representing temporal relations. The question has been addressed in the artificial intelligence literature with respect to planning systems and linguistic analysis. The approaches adopted do not appear to be directly transposable to legal discourse. The problem seems so far to have escaped notice amongst researchers attempting to develop legal expert systems. The paper explores in a preliminary way how lawyers use temporal concepts. It is (...) submitted that “legal time” only partly overlaps with real time. The focus of this study was on adolescent mental health. More specifically, the relationship between strength-based parenting (SBP) and subjective wellbeing (SWB) during adolescence, as assessed by a sample of adolescents, was examined at three time points over 14 months (N = 202, Mage = 12.97, SDage =.91, 48% female). SBP was positively related to life satisfaction and positive affect at each of the three time points, and was negatively related to negative affect. SBP and SWB both declined significantly over (...) time. When examining the causal relationships between SBP and SWB, two different statistical models were applied: latent growth-curve models (LGM) and random-intercept cross-lagged panel models (RI-CLPM). The LGM revealed a strong positive relationship between changes in SBP and SWB. Specifically, this model showed that SBP at one time point predicted adolescent SWB at future time points. However, when the more stringent statistical test was completed through RI-CLPMs, no cross-lagged paths reached significance. Thus, while parenting is a significant predictor of wellbeing for pre-teens and teens in real time, it is not predictive of wellbeing at future time points. Parents, thus, cannot assume that their current levels of SBP are ‘banked’ by their children to support future wellbeing. Instead, SBP needs to be an ongoing, contemporary parenting practice. Furthermore, the fact that perceptions of SBP decline in this age bracket suggest that SBP interventions may be helpful in supporting adolescent mental health. (shrink)

Academic achievement is an important issue for families and Haitian adolescents from socio-economically disadvantaged backgrounds. It represents a necessary step to escape difficult living conditions and achieve some social mobility. Any failure of these adolescents academically risk of them know a lot more difficult than that of their parents. It is therefore important to consider the factors that explain the academic success of some young people playing in a precarious environment. This article aims to analyze, in a clinic psychosocial perspective, (...) the results of a qualitative study of eight (8) Haitian school adolescents in some poor neighborhoods of Port-au-Prince. The focus is on the construction of the narcissistic rage of the young as mobilization of psychic energy in the service of a wounded self, resulting in the case of these subjects, certain educational practices of their parents and a plug gradual realization of the precarious socio-economic conditions that characterize their lives and different symbolic insults, humiliation and contempt associated with these precarious conditions. Thus, the analysis shows how this insatiable thirst for revenge and self-repair, related to narcissistic wounds which are the subject of these teenagers, are a key element of their educational investment. The genetically manipulated organism crisis demonstrated that technological development based solely on the law of the marketplace and State protection against serious risks to health and safety is no longer a warrant of ethical acceptability. In the first part of our paper, we critique the implicitly individualist social-acceptance model for State regulation of technology and recommend an interdisciplinary approach for comprehensive analysis of the impacts and ethical acceptability of technologies. In the second part, we present a framework for the analysis (...) of impacts and acceptability, devised—with the goal of supporting the development of specific nanotechnological applications—by a team of researchers from various disciplines. At the conceptual level, this analytic framework is intended to make explicit those various operations required in preparing a judgement about the acceptability of technologies that have been implicit in the classical analysis of toxicological risk. On a practical level, we present a reflective tool that makes it possible to take into account all the dimensions involved and understand the reasons invoked in determining impacts, assessing them, and arriving at a judgement about acceptability. This article addresses the issue of the interface between public health and clinical health within the context of the search for networking approaches geared to a more integrated delivery of health services. The articulation of an operative interface is complicated by the fact that the definition of networking modalities involves complex intra- and interdisciplinary and intra- and interorganizational systems across which a new transversal dynamics of intervention practices and exchanges between service structures must be established. A better understanding of the (...) situation is reached by shedding light on the rationale underlying the organizational methods that form the bases of the interface between these two sectors of activity. The Quebec experience demonstrates that neither the structural-functionalist approach, which emphasizes remodelling establishment structures and functions as determinants of integration, nor the structural-constructivist approach, which prioritizes distinct fields of practice in public health and clinical health, adequately serves the purpose of networking and integration. Consequently, a theoretical reframing is imperative. In this regard, structuration theory, which fosters the simultaneous study of methods of inter-structure coordination and inter-actor cooperation, paves the way for a better understanding of the situation and, in turn, to the emergence of new integration possibilities. Buscamos, com o presente artigo, investigar o impacto da práxis de um Projeto de Extensão Universitária, que consiste em oficinas de música para crianças e adolescentes com o que designamos autismo ou outras neurodivergências, sobre a formação discente, levando em consideração as dimensões clínica e ético-política da ação extensionista. Dividimos a metodologia em duas partes: a primeira corresponde à revisão bibliográfica tipo narrativa, por meio da qual abordamos textos da fenomenologia da vida e das éticas da alteridade, comunitária e da (...) libertação; a segunda parte diz respeito aos relatos de experiência feitos pelas/os discentes participantes do Projeto e coautoras/es do presente artigo. Os/as discentes responderam a questões relacionadas a dois eixos temáticos construídos a priori: o impacto da Oficina de Música sobre sua formação e a percepção sobre as dimensões clínica e ética do Projeto. Avaliamos as narrativas por meio da análise de conteúdo, a partir da qual construímos as seguintes categorias de análise: impacto transformador da práxis da Oficina; e modelo biomédico e pensamento crítico, referentes ao eixo 1; arte, acolhimento e potencial terapêutico; pessoas que não cabem em um diagnóstico; e uma clínica ético-política, referentes ao eixo 2. Concluímos que a ação extensionista tem tido um impacto positivo sobre a formação discente, contribuindo para uma clínica estética, ética e política não normalizadora, que possibilita a liberação das forças da vida, a afirmação da vida em sua multiplicidade, sobretudo daquelas pessoas que tiveram e continuam tendo suas vidas negadas e invisibilizadas pelo sistema de produção (econômico e discursivo) dominante. Episodic counterfactual thoughts (CFT) and autobiographical memories (AM) involve the reactivation and recombination of episodic memory components into mental simulations. Upon reactivation, memories become labile and prone to modification. Thus, reactivating AM in the context of mentally generating CFT may provide an opportunity for editing processes to modify the content of the original memory. To examine this idea, this paper reports the results of two studies that investigated the effect of reactivating negative and positive AM in the context of either (...) imagining a better (i.e. upward CFT) or a worse (i.e. downward CFT) alternative to an experienced event, as opposed to attentively retrieving the memory without mental modification (i.e. remembering) or no reactivation. Our results suggest that attentive remembering was the best strategy to both reduce the negative affect associated with negative AM, and to prevent the decay of positive affect associated with positive AM. In addition, reactivating positive, but not negative, AM with or without CFT modification reduces the perceived arousal of the original memory over time. Finally, reactivating negative AM in a downward CFT or an attentive remembering condition increases the perceived detail of the original memory over time. (shrink)

As the spell of Jacques Derrida grows stronger, with more translations and analyses appearing every season, it is possible--and necessary--to determine what in his work is truly new and what continues philosophical and literary traditions. Although Martin Heidegger ahs been mentioned before as a precursor of deconstruction, Herman Rapaport is the first to develop the connections between the writings of the German philosopher and Derrida. Heidegger and Derrida discusses the French philosopher's adoption of certain Heideggerean themes and his (...) extension or overturning of them. But Rapaport does more than show how deconstruction builds on the philosophical foundations laid by Heidegger (and also by Hegel, Nietzsche, and Freud). In the most comprehensive study of Derrida's works to date, he tackles the problem of writing an intellectual history about a figure who has put into question the possibility of such a construction and acknowledges Derrida's concerns with Jewish history in relation to Western thought. A variety of evidence demonstrates that memory is a reconstructive process prone to errors and distortions. However, the complex relationship between memory encoding, strength of memory reactivation, and the likelihood of reporting true or false memories has yet to be ascertained. We address this issue in a setting that mimics a real-life experience: We asked participants to take a virtual museum tour in which they freely explored artworks included in the exhibit, while we measured the participants’ spontaneous viewing time of (...) each explored artwork. In a following memory reactivation phase, participants were presented again with explored artworks, followed by novel artworks not belonging to the same exhibit. For each of these objects, participants provided a reliving rating that indexed the strength of memory reactivation. In the final memory recognition phase, participants underwent an old/new memory task, involving reactivated vs. baseline targets, and activated and baseline lures. The results showed that those targets that were spontaneously viewed for a longer amount of time were more frequently correctly recognized. This pattern was particularly true for reactivated targets associated with greater memory strength. Paradoxically, however, lures that were presented after targets associated with higher reliving ratings in the reactivation phase were more often erroneously recognized as artworks encountered during the tour. This latter finding indicates that memory intrusions, irrespective of the viewing time, are more likely to take place and be incorporated into true memories when the strength of target memory is higher. In this article, we present findings from a participatory action research program in France on the exercise of human rights and supported and substitute decision-making, inspired by the United Nations Convention on the Rights of Persons with Disabilities (“CRPD”). Bringing together persons with the lived experience of disability, academics, and health and social care and support professionals, the project used the method of “experience-based construction of public problem” to transform experience into collective expertise. This enabled the exploration of support that (...) persons in vulnerable situations, whose capacity to exercise their human rights has weakened, need to make decisions in their lives and participate meaningfully in public debate. The relationship between the awareness of rights and exercise of rights is discussed. We argue for the need to balance out the positions of different contributors in participatory action research, in a reasoned manner, by recognizing the scientific and citizen-based participation of all partners.
