While the value of early detection of dementia is largely agreed upon, population-based screening as a means of early detection is controversial. This controversial status means that such screening is not recommended in most national dementia plans. Some current practices, however, resemble screening but are labelled “case-finding” or “detection of cognitive impairment”. Labelled as such, they may avoid the ethical scrutiny that population-based screening may be subject to. This article examines conceptualizations of screening and case-finding. It shows how the definitions (...) and delimitations of the concepts (the _what_ of screening) are drawn into the ethical, political, and practical dimensions that screening assessment criteria or principles are intended to clarify and control (the _how_ of screening, how it is and how it should be performed). As a result, different conceptualizations of screening provide the opportunity to rethink what ethical assessments should take place: the conceptualizations have different ethico-political implications. The article argues that population-based systematic screening, population-based opportunistic screening, and case-finding should be clearly distinguished. (shrink)

The phrase ‘person‐centred care’ (PCC) reminds us that the fundamental philosophical goal of caring for people is to uphold or promote their personhood. However, such an idea has translated into promoting individualist notions of autonomy, empowerment and personal responsibility in the context of consumerism and neoliberalism, which is problematic both conceptually and practically. From a conceptual standpoint, it ignores the fact that humans are social, historical and biographical beings, and instead assumes an essentialist or idealized concept of personhood in which (...) a person is viewed as an individual static object. From a practical standpoint, the application of such a concept of personhood can lead to neglect of a person's fundamental care needs and exacerbate the problems of social inequity, in particular for older people and people with dementia. Therefore, we argue that our understanding of PCC must instead be based on a dynamic concept of personhood that integrates the relevant social, relational, temporal and biographical dimensions. We propose that the correct concept of personhood in PCC is one in which persons are understood as socially embedded, relational and temporally extended subjects rather than merely individual, autonomous, asocial and atemporal objects. We then present a reconceptualization of the fundamental philosophical goal of PCC as promoting selfhood rather than personhood. Such a reconceptualization avoids the problems that beset the concept of personhood and its application in PCC, while also providing a philosophical foundation for the growing body of empirical literature that emphasizes the psychosocial, relational, subjective and biographical dimensions of PCC. In recent decades scholars have acknowledged that transactions in the informal economy have not vanished with modernization and industrialization as expected but rather remain an important contemporary aspect of overall production and consumption across the world, in both developing and developed countries. Yet little is known about the profile of the consumers in this realm or what drives them to purchase from the informal economy. A systematic review of the literature investigating consumption in the informal economy reveals a severely underdeveloped (...) area of consumer studies with significant gaps in terms of its theoretical approaches, methods and regional coverage. The findings of the existing literature is that multiple motives are used by consumers for justifying their purchases in the informal economy beyond the dominant simplistic view that they do simply for financial gain or for a lower price. The outcome is a recognition that responsibility to reducing this phenomenon with negative effects on governments, businesses, workers and consumers lies not just with public authorities but also practitioners who need to correct the failures in formal market provision. The significant gaps identified in the literature are then used to highlight a comprehensive future research agenda, which includes the need for the development of an institutionalist theoretical perspective when explaining consumers‘ participation in the informal economy and social marketing interventions. BackgroundEvolving medical technology, advancing biomedical and drug research, and changing laws and legislation impact patients’ healthcare options and influence healthcare practitioners’ (HCPs’) practices. Conscientious objection policy confusion and variability can arise as it may occasionally be unclear what underpins non-participation. Our objective was to identify, analyze, and synthesize the factors that influenced HCPs who did not participate in ethically complex, legally available healthcare.MethodsWe used Arksey and O’Malley’s framework while considering Levac et al.’s enhancements, and qualitatively synthesized the evidence. We searched (...) Medline, CINAHL, JSTOR, EMBASE, PsychINFO, Sociological Abstracts, and ProQuest Dissertations and Theses Global from January 1, 1998, to January 15, 2020, and reviewed the references of the final articles. We included articles written in English that discussed the factors that influenced physicians and registered nurses (RNs) who did not participate in end-of-life (EOL), reproductive technology and health, genetic testing, and organ or tissue donation healthcare areas. Using Covidence, we conducted title and abstract screening, followed by full-text screening against our eligibility criteria. We extracted the article’s data into a spreadsheet, analyzed the articles, and completed a qualitative content analysis using NVivo12.ResultsWe identified 10,664 articles through the search, and after the screening, 16 articles were included. The articles sampled RNs (n = 5) and physicians (n = 11) and encompassed qualitative (n = 7), quantitative (n = 7), and mixed (n = 2) methodologies. The care areas included reproductive technology and health (n = 11), EOL (n = 3), organ procurement (n = 1), and genetic testing (n = 1). One article included two care areas; EOL and reproductive health. The themed factors that influenced HCPs who did not participate in healthcare were: (1) HCPs’ characteristics, (2) personal beliefs, (3) professional ethos, 4) emotional labour considerations, and (5) system and clinical practice considerations.ConclusionThe factors that influenced HCPs’ who did not participate in ethically complex, legally available care are diverse. There is a need to recognize conscientious objection to healthcare as a separate construct from non-participation in healthcare for reasons other than conscience. Understanding these separate constructs will support HCPs’ specific to the underlying factors influencing their practice participation. (shrink)

Increasing women’s participation in intrahousehold decision-making has been linked with increased agricultural productivity and economic development. Existing studies focus on identifying the decision-maker and exploring factors affecting women’s participation, yet the context in which households make decisions is generally ignored. This paper narrows this gap by investigating perceptions of women's participation and the roles of social norms in agricultural decision-making. It specifically applies a fine-scale quantitative responses tool and constructs a women’s participation index to measure men’s and women’s perceptions regarding (...) women’s participation in decisions about 21 agricultural activities. The study further examines the correlation between social norms in these perceptions as measured by the WPI for 439 couples in West Java, Indonesia. We find that first, men and women have different perceptions about women's decision-making in agricultural activities, but the same perceptions of the types of activities in which women have the most and the least participation. Second, joint decisions come in various combinations but overall, the women’s role is smaller. Third, social norms influence spouses' perceptions of decision-making participation, which explains most of the variation of the WPI. These results suggest that rigorous consideration of social norms is required to understand intrahousehold decision-making. (shrink)

Common symptoms of multiple sclerosis (MS) include motor impairments of the lower extremities, particularly gait disturbances. Loss of balance and muscle weakness, representing some peripheral effects, have been shown to influence these symptoms, however, the individual role of cortical and subcortical structures in the central nervous system is still to be understood. Assessing [18F]fluorodeoxyglucose (FDG) uptake in the CNS can assess brain activity and is directly associated with regional neuronal activity. One potential modality to increase cortical excitability and improve motor (...) function in patients with MS (PwMS) is transcranial direct current stimulation (tDCS). However, tDCS group outcomes may not mirror individual subject responses, which impedes our knowledge of the pathophysiology and management of diseases like MS. Three PwMS randomly received both 3 mA tDCS and SHAM targeting the motor cortex (M1) that controls the more-affected leg for 20 min on separate days before walking on a treadmill. The radiotracer, FDG, was injected at minute two of the 20 min walk and the subjects underwent a Positron emission tomography (PET) scan immediately after the task. Differences in relative regional metabolism of areas under the tDCS anode and the basal ganglia were calculated and investigated. The results indicated diverse and individualized responses in regions under the anode and consistent increases in some basal ganglia areas (e.g., caudate nucleus). Thus, anodal tDCS targeting the M1 that controls the more-affected leg of PwMS might be capable of affecting remote subcortical regions and modulating the activity (motor, cognitive, and behavioral functions) of the circuitry connected to these regions. As a form of functional diversity, spinal cord injury expressed by tetraplegia is one of the most serious events that can impact people, affecting their family and socioeconomic life. The type of care relationship established in these cases will be essential for preserving autonomy. The objective of this study was to understand how care relationships influence the autonomy of people with tetraplegia and the dynamics that trigger practices of autonomy violation, maintenance and promotion. This research is inspired by problematization as (...) a methodology using the Arch of Maguerez as an analytical approach strategy that enables an engagement between empirical and theoretical data. Some models of care relationships identified in this study either promote or violate autonomy: the protectionist model, based on the biomedical interpretation model of disability and the bodily impediments caused by spinal cord injury; the participatory model, involving the patient's interest in adapting to their new condition of family and community life, seeking to understand the practical aspects of daily life; the sharing model, in which the complicity and understanding of new forms of bodily expression allow the caregiver to be almost an extension of the tetraplegic person's body; the delegate model, which occurs in relationships with professional caregivers; and the emancipatory model, which seeks to empower the patient, highlight potentialities, and encourage autonomy. The key components of these conceptual care models guided the elaboration of hypotheses for intervention with the objective of maintaining and promoting the autonomy of people with tetraplegia through supported care. Les résultats montrent que les ruptures de liens sont étroitement associées aux processus complexes d’affiliation et à l’empêchement du processus de séparation-individuation. O paradigma da intersubjetividade preconizado por Jürgen Habermas fundou uma forma de chegar a consensos racionalmente fundamentados. A sua obra aponta para a ideia de que o que mantém as pessoas numa atitude de ação comunicativa são as forças da fundamentação racional e da motivação racional. Ao analisarmos a dimensão intersubjetiva da prática da comunidade de investigação filosófica, demo-nos conta de que é possível verificar a passagem de uma autonomia da razão centrada no sujeito (um pensar por si mesmo), para (...) a prática de uma racionalidade comunicativa (centrada num pensamento em comunidade). Os pressupostos inevitáveis da comunicação fundam e mantêm as comunidades e permitem o clima de confiança e de cuidado que possibilita que os membros da comunidade pensem livremente e se deixem guiar pela força do we, do nós, do nos-otros. Não é somente em contexto de comunidades de investigação filosófica que se verificam as potencialidades do paradigma da intersubjetividade de Habermas. Também na discussão entre especialistas podemos encontrar essas potencialidades. Foi, em certa medida, este o rasto que conseguimos traçar ao investigarmos as diferentes abordagens de Ann Sharp, David Kennedy e Giuseppe Ferraro sobre a noção de comunidade de investigação filosófica. O diálogo estabelecido entre estes filósofos e o conceito de comunidade de investigação filosófica, os seus argumentos e as formas de concretização do diálogo filosófico em comunidade, dão conta da fecundidade da racionalidade comunicativa. Sabemos que o conceito de comunidade de investigação filosófica, tal como é pensado e vivido por Ann Sharp, é distinto do conceito de comunidade de investigação filosófica de David Kennedy. Percebemos que o círculo do pensar de Giuseppe Ferraro é uma outra abordagem ao pensamento filosófico em comunidade. No entanto, na nossa investigação estas abordagens entraram em diálogo e, em boa medida, devido ao que é permitido pelo próprio movimento de uma racionalidade comunicativa. Estas possibilidades de diálogo dão-se no pressuposto de uma relação intersubjetiva também entre conceitos, experiências e pensamento sobre os conceitos e as experiências. O caminho percorrido entre a comunidade de investigação filosófica entendida como espaço de intersubjetividade (Sharp, 1987), passando pela abertura da comunidade de investigação filosófica a outras dimensões não discursivas da comunicação (Kennedy, 1994) e avançando para o círculo do pensar que enche o espaço de tempo e o torna num lugar (Ferraro, 2018) revela como a comunidade de investigação filosófica é um conceito e uma experiência abertos e fecundos: um conceito em ação. Introduction: The potential therapeutic efficacy of real-time fMRI Neurofeedback has received increasing attention in a variety of psychological and neurological disorders and as a tool to probe cognition. Despite its growing popularity, the success rate varies significantly, and the underlying neural mechanisms are still a matter of debate. The question whether an individually tailored framework positively influences neurofeedback success remains largely unexplored.Methods: To address this question, participants were trained to modulate the activity of a target brain region, the visual motion (...) area hMT+/V5, based on the performance of three imagery tasks with increasing complexity: imagery of a static dot, imagery of a moving dot with two and with four opposite directions. Participants received auditory feedback in the form of vocalizations with either negative, neutral or positive valence. The modulation thresholds were defined for each participant according to the maximum BOLD signal change of their target region during the localizer run.Results: We found that 4 out of 10 participants were able to modulate brain activity in this region-of-interest during neurofeedback training. This rate of success is consistent with the neurofeedback literature. Whole-brain analysis revealed the recruitment of specific cortical regions involved in cognitive control, reward monitoring, and feedback processing during neurofeedback training. Individually tailored feedback thresholds did not correlate with the success level. We found region-dependent neuromodulation profiles associated with task complexity and feedback valence.Discussion: Findings support the strategic role of task complexity and feedback valence on the modulation of the network nodes involved in monitoring and feedback control, key variables in neurofeedback frameworks optimization. Considering the elaborate design, the small sample size here tested impairs external validity in comparison to our previous studies. Future work will address this limitation. Ultimately, our results contribute to the discussion of individually tailored solutions, and justify further investigation concerning volitional control over brain activity. (shrink)

Without ignoring the discrepancies between both of them, it tries to enlighten the reasons behind this interest and their shared concern for modern western culture, as well as for the subjective crisis that this culture seems to cause. Post-war Japan has seen profound and rapid social change and transformation. One of the most visible areas of change in Japan has been medicine, and particularly the ethical practices and policies that guide medical decision-making. The formal discipline of bioethics, Seimei Rinri in Japanese, has grown by leaps and bounds since the late 1970s, when it began to appear in the curriculum and professional activities of Japanese medical schools and philosophy departments. The introduction of bioethics to Japan was timely, as (...) innovation in medicine and technology was evolving in ways that revealed that the intersection of medicine, traditional Japanese values, and new cultural trends was an area of great moral complexity. In its infancy, bioethics in Japan was more or less an import from the United States, where the discipline took its roots. Quickly, however, it became clear that Japan's history and tradition would call for a different approach, and the engagement of slightly different ethical issues. Organ transplantation, for example, sparked much greater controversy in Japan than it ever did in the United States. Today, Japan has one of the most dynamic bioethics programs in the world, and it is one that reflects both traditional Japanese culture and the need for inter-cultural engagement in an increasingly global world. Through a series of original chapters written by bioethicists and covering a range of ethical issues, this anthology shows that, in contrast to previous assumptions, Japanese bioethics has, in fact, taken on an identity that is undoubtedly separate from its American origins. Rich philosophical questions raised by medicine, human subjects research, and psychiatric care are being posed by scholars in a way that reflect Japanese tradition and is no longer simply reflective of, or shaped by, American traditions and philosophical problems. The book highlights and showcases these trends through a series of chapters written by some of the leading scholars in contemporary Japanese bioethics, many of whom were pioneers of the field when it began and are now nearing retirement. Firms engage in corporate social responsibility (CSR) because they consider that some kind of competitive advantage accrues to them. We contend that resource-based perspectives (RBP) are useful to understand why firms engage in CSR activities and disclosure. From a resource-based perspective CSR is seen as providing internal or external benefits, or both. Investments in socially responsible activities may have internal benefits by helping a firm to develop new resources and capabilities which are related namely to know-how and corporate culture. In (...) effect, investing in social responsibility activities and disclosure has important consequences on the creation or depletion of fundamental intangible resources, namely those associated with employees. The external benefits of CSR are related to its effect on corporate reputation. Corporate reputation can be understood as a fundamental intangible resource which can be created or depleted as a consequence of the decisions to engage or not in social responsibility activities and disclosure. Firms with good social responsibility reputation may improve relations with external actors. They may also attract better employees or increase current employees’ motivation, morale, commitment and loyalty to the firm. This article contributes to the understanding of why CSR may be seen as having strategic value for firms and how RBP can be used in such endeavour. (shrink)

The German filmmaker Werner Herzog controversially associates “truth” and “reality” in film with Kant’s notion of the sublime by explicitly treating the sublime as a key element in developing his notion of ecstatic truth. I critically examine Herzog’s interpretation of Kant’s sublime and the relations he establishes between the sublime and his own key aesthetic notion of ecstatic truth. I examine how the sublime in Herzog’s films arises from encounters with the overwhelming force and power of nature experienced by his (...) characters in the feature films and participants in the documentaries. I question whether Kant’s conception of the sublime is the one that best aligns with Herzog’s aesthetic aims and I contrast Kant’s transcendental sublime with what I describe as the physiological sublime of Edmund Burke. I draw some conclusions about the moral dimension of the sublime in the context of the relationship between humans and nature within the frame of contemporary ecological concerns. (shrink)

The book discusses the central notion of logic, the concept of logical consequence, and its model-theoretic definition as truth-preservation in all models. Whether the model-theoretic definition captures the modal and epistemological features of our pre-theoretic notion depends on what models model. The book argues that, given a non-formal understanding of models, the universal quantifier used in the definition of consequence must be restricted: if literally all models had to be considered, no argument would ever be logically valid. A central challenge (...) of the philosophy of logic is therefore to supplement the model-theoretic definition by a criterion for admissible models. The problem of logical constants, so prominent in the current literature on logical consequence, constitutes but a special case of this much more general demarcation problem. The book explores the various dimensions of the problem of admissible interpretations and proposes that the standard views are unjustified or even unjustifiable. As a consequence, it develops a new vision of logic and suggests in particular that our notion of logical consequence is deeply imbued with metaphysics. (shrink)

This book examines the conceptual and temporal frames through which modern Western historiography has linked itself to classical antiquity. In doing so, it articulates a genealogical problematic of what history is and a more strictly focused reappraisal of Greek and Roman historical thought. Even with a significant increase in representation of minority identities in popular media – especially in stories of speculative fiction – the ways in which inclusivity is designed must be examined, with Lovecraft Country standing as a useful example for this scrutiny. Adapted from a novel of the same name, the show Lovecraft Country swapped the genders and sexualities of a few characters from the book to increase representation. The ways that these swaps reified tropes about diverse identities is the (...) central concern of this analysis. Using Fields and Fields's conceptualization of racecraft as a "mental terrain of pervasive beliefs," this piece seeks to identify how pervasive beliefs and rules about identity were implicated in the series. The characters of Montrose Freeman, Ruby Baptiste, and Christina Braithwhite all undergo gender and/or sexuality swaps in the adaptation from page to screen, but the ways in which stereotypical assumptions around gender and sexuality are preserved in the narrative work mostly to reproduce negative tropes around these identities. This argument details narratives of violence, avoidance, and sacrifice that are given to these gender- and sexuality-swapped characters, and illustrates how, ultimately, these narratives provide detrimental instead of productive representation of diverse identities. In this paper, I provide an account of the wrong that is done to women when everyday people fail to believe allegations of sexual assault made by women. I argue that an everyday person wrongs both the accuser and women causally distant from the accuser when they fail to believe the accuser’s allegation. First, I argue that there are responses that we, as everyday members of society, owe to victims of sexual assault. A condition enabling everyday people to respond in (...) the way owed to victims is that they have an outright belief that the accuser was assaulted. Actively suspending judgement about whether a crime occurred is in tension with the ability to respond to the victim in a way that supports and validates them. When an everyday person fails to have an outright belief in the truth of an allegation, they wrong the accuser because they risk failing to satisfy the conditions enabling them to fulfill their obligation to her. Second, I argue that everyday people wrong women who are causally distant from the accuser because in our social context, women are often treated in particular ways – especially in the sexual domain – because they are women. As a result, when women hear that an everyday person fails to believe a particular allegation, they easily project themselves into the accuser’s position and reasonably worry that if they were to be assaulted, they too would be met with doubt and disbelief by the people in their community. The paper develops an inductive extension of AGM-style belief base revision theory with the aim of formally implementing Freitag’s :254–267, 2015, Dialectica 70:185–200, 2016) solution to Goodman’s paradox. It shows that the paradox dissolves once belief revision takes place on inductively closed belief bases, rather than on belief sets.

It is frequently claimed that the human mind is organized in a modular fashion, a hypothesis linked historically, though not inevitably, to the claim that many aspects of the human mind are innately specified. A specific instance of this line of thought is the proposal of an innately specified geometric module for human reorientation. From a massive modularity position, the reorientation module would be one of a large number that organized the mind. From the core knowledge position, the reorientation module (...) is one of five innate and encapsulated modules that can later be supplemented by use of human language. In this paper, we marshall five lines of evidence that cast doubt on the geometric module hypothesis, unfolded in a series of reasons: (1) Language does not play a necessary role in the integration of feature and geometric cues, although it can be helpful. (2) A model of reorientation requires flexibility to explain variable phenomena. (3) Experience matters over short and long periods. (4) Features are used for true reorientation. (5) The nature of geometric information is not as yet clearly specified. In the final section, we review recent theoretical approaches to the known reorientation phenomena. (shrink)

Empirical studies indicate that business compliance with the UK Modern Slavery Act is disappointing, but they struggle to make sense of this phenomenon. This article offers a novel framework to understand how business organizations construct the meaning of compliance with the UK Modern Slavery Act. Our analysis builds on the endogeneity of law theory developed by Edelman. Empirically, our study is based on the analysis of the modern slavery statements of 10 FTSE 100 (Financial Times Stock Exchange 100 Index) companies (...) in the food and tobacco sector, backed by interviews with business, civil society, and public officers. We offer a dynamic model that draws attention to the role of compliance professionals in framing ambiguous rules and devising a variety of organizational responses to modern slavery law. Contrary to extant research that tends to praise organizations for going “beyond compliance”, our study underlines the risks of managerialization of modern slavery law, whereby merely symbolic structures come to be associated with legal compliance, even when they are ineffective at tackling modern slavery. O salto é irrepresentável, não é passível de ser modulado por nenhum cálculo ou representação do ente, uma vez que saltar, segundo as meditações de Heidegger, indica lançar-se em uma via totalmente outra do percurso ôntico seguido (...) pela metafísica em toda a história ocidental. O salto é o deslocamento do ser-aí da zona da entidade rumo ao acolhimento da recusa do ser. Nesse sentido, o ser-aí recepciona o fundamento na sua mais singela e profunda vigência, isto é, como o Nada do ente e como fosso abissal. (shrink)

The three papers in this dissertation attempt to explore and defend a kind of middle ground with respect to the question of moral objectivity. In the first paper I use the case of disgust to show how not to go about raising skepticism about moral judgment; in doing so, I argue that disgust can be vindicated with an account on which it tracks social contagion as well as physical contamination. Therefore, the question of whether disgust is an appropriate reaction to (...) moral wrongness can sometimes be answered in the affirmative. In the second paper, I use empirical data from anthropology and psychology to argue that moral disagreement makes trouble for the claim that morality is objective, but I don’t reject objectivity entirely— in the third paper I go on to argue that moral relativism best makes sense of a morality that appears to be objective with respect to some questions but not others. The paper provides a new argument against the classical invariance criterion for logical terms: if all terms with a permutation invariant extension qualify as logical, then for any arbitrary true contingent sentence K of the meta-language, there would be a logically true object-language sentence 'φ' such that K follows from the sentence 'φ is true'. Thus, many logically true sentences would be a posteriori. To prevent this fatal consequence, we propose to alter the invariance criterion: not only the term's extension, (...) but also its semantic clause must satisfy certain invariance conditions. The paper ends with the observation that the new criterion makes explicit the dependency of the classification of terms into logical and non-logical ones at the different levels of the Tarskian hierarchy of languages.
