The genetically manipulated organism crisis demonstrated that technological development based solely on the law of the marketplace and State protection against serious risks to health and safety is no longer a warrant of ethical acceptability. In the first part of our paper, we critique the implicitly individualist social-acceptance model for State regulation of technology and recommend an interdisciplinary approach for comprehensive analysis of the impacts and ethical acceptability of technologies. In the second part, we present a framework for the analysis (...) of impacts and acceptability, devised—with the goal of supporting the development of specific nanotechnological applications—by a team of researchers from various disciplines. At the conceptual level, this analytic framework is intended to make explicit those various operations required in preparing a judgement about the acceptability of technologies that have been implicit in the classical analysis of toxicological risk. On a practical level, we present a reflective tool that makes it possible to take into account all the dimensions involved and understand the reasons invoked in determining impacts, assessing them, and arriving at a judgement about acceptability. I can well imagine all of those listed above, among many others, united in an imaginary painting similar to the School of Athens—in other words, a vision that reconciles their various views of the world in a harmony analogous to the unity of philosophers in Raphael’s great fresco. Raphael is a towering figure in the history of European culture. The year 2020 was the 500th anniversary of the death of the great painter in Rome at the age of thirty-seven. In the wake of recent exhibitions that commemorated the artist’s achievement, I propose to touch briefly and lightly on a few aspects arion 28.2 fall 2020 100 reflections on raphael of Raphael’s life and works. Please think of these remarks as a series of suggestions, not attempts at definitive interpretation. I think of this essay as a rather experimental piece. THE SCHOOL OF ATHENS i should like to begin with a few brief remarks on what must be Raphael’s most famous work of art. I mean his School of Athens (fig. 1), which is, like Leonardo’s Mona Lisa and Last Supper or like Michelangelo’s David and Creation of Adam, an image widely known and admired. What more is there to say about The School Of Athens, one of the central images in the Stanza della Segnatura? You might spend the rest of your life reading about this great work and never understand it fully. Nevertheless, I want to focus for a moment on a central detail of the work and, on the basis of my description, point to a significant aspect of the fresco that has been universally overlooked. Paul Barolsky 101 At the center of the School of Athens we behold the two great philosophers of classical antiquity: Plato and Aristotle. Plato holds a copy of his Timaeus, a dialogue that concerns the harmony of the heavens; Aristotle holds a copy of his Ethics, a work that pertains to human relations in this world. It was the goal of Renaissance thinkers to create a harmony or concord of the thought of these philosophers: a Concordia Platonis et Aristotelis. Raphael’s fresco, as is well known, presents the visual ideal of such harmony. Whereas Aristotle gestures downward, alluding to this world, in which his Ethics matter, Plato points heavenward, evoking the order of the cosmic realm described in Timaeus. Raphael makes visible the ideal of concordia when he movingly pictures the way in which the two philosophers gaze into each other’s eyes, as if to suggest this harmony. Borrowing from the language of Raphael ’s friend, Baldassare Castiglione, Raphael renders “the joining” of their souls. The monumental architecture of the scene, which echoes Bramante and has taproots in Piero della Francesca and Perugino, consists of three barrel-vaults beyond which we see patches of blue sky and puffs of cloud (fig. 2). The grandiose architecture of the image magnifies Raphael’s philosophers. Within this architectural framework, Plato’s gesture is deeply meaningful, since it prompts the viewer to look up and thus ascend toward the perfection of the celestial realm, beyond the imperfections of the physical world. Ce phénomène de précarisation, observable dans plusieurs contextes disciplinaires et nationaux, se manifeste notamment par l’augmentation marquée, depuis les années 1990, du nombre d’emplois postdoctoraux, proportionnellement au nombre de postes stabilisés. Parallèlement, les chercheur⋅e⋅s en début de carrière sont largement confronté⋅e⋅s à des discours institutionnels faisant la promotion des mobilités académiques et des collaborations internationales. L’abondance des discours autour de la mobilité des jeunes chercheur⋅e⋅s tend (...) cependant à masquer la pérennité des modèles de carrières nationaux, très variables d’un pays à l’autre du fait de l’histoire particulière des espaces concernés. À partir des données quantitatives et qualitatives sur les expériences de début de carrière académique en Suisse, notre article vise à pointer comment l’impératif de mobilité internationale – particulièrement fort dans le contexte helvétique contemporain – s’hybride avec le modèle de carrière en vigueur depuis le début du xxe siècle. Understanding the mechanisms and the time and spatial evolution of penumbra following an ischemic stroke is crucially important for developing therapeutics aimed at preventing this area from evolving towards infarction. To help in integrating the available data, we decided to build a formal model. We first collected and categorised the major available evidence from animal models and human observations and summarized this knowledge in a flow-chart with the potential key components of an evolving stroke. Components were grouped in ten sub-models (...) that could be modelled and tested independently: the sub-models of tissue reactions, ionic movements, oedema development, glutamate excitotoxicity, spreading depression, NO synthesis, inflammation, necrosis, apoptosis, and reperfusion. Then, we figured out markers, identified mediators and chose the level of complexity to model these sub-models. We first applied this integrative approach to build a model based on cytotoxic oedema development following a stroke. Although this model includes only three sub-models and would need to integrate more mechanisms in each of these sub-models, the characteristics and the time and spatial evolution of penumbra obtained by simulation are qualitatively and, to some extent, quantitatively consistent with those observed using medical imaging after a permanent occlusion or after an occlusion followed by a reperfusion. BackgroundThe spread of COVID-19 has disrupted the lifestyles of the world’s population. In the workplace, the pandemic has affected all sectors and has changed the way work is organized and carried out. The health sector has been severely impacted by the pandemic and has faced enormous challenges in maintaining healthcare services while providing care to those infected by the virus. At the heart of this battle, healthcare managers were key players in ensuring the orchestration of operations and the physical and (...) mental availability of employees during the crisis. Although few studies have been conducted to identify organizational practices or leadership skills to be adopted in a crisis context, the concrete behaviors of managers have not been documented yet. Therefore, this study aims at filling this gap by studying middle managers’ behaviors facing COVID-19 crisis in the healthcare sector.MethodsUsing a qualitative approach, eight focus groups were conducted online during the pandemic with 37 middle managers from the healthcare community of a Quebec health establishment from April to June 2020. Thematic analyses were conducted, and a mixed-methods approach was used to analyse the data based on Viitala’s hierarchical model of management skills.ResultsBased on the six managerial skills proposed in the model of Viitala, 21 specific management behaviors were identified as having been deployed by middle managers at the beginning of the pandemic. Considering that the health sector has been profoundly shaken by this health crisis, in addition to being an environment likely to experience other crises, managers need to develop practical skills in various crisis management situations. Thus, the results guide practitioners by highlighting the importance of team-oriented management behaviors, especially in a crisis context. (shrink)

Throughout the biological and biomedical sciences there is a growing need for, prescriptive ‘minimum information’ (MI) checklists specifying the key information to include when reporting experimental results are beginning to find favor with experimentalists, analysts, publishers and funders alike. Such checklists aim to ensure that methods, data, analyses and results are described to a level sufficient to support the unambiguous interpretation, sophisticated search, reanalysis and experimental corroboration and reuse of data sets, facilitating the extraction of maximum value from data sets (...) them. However, such ‘minimum information’ MI checklists are usually developed independently by groups working within representatives of particular biologically- or technologically-delineated domains. Consequently, an overview of the full range of checklists can be difficult to establish without intensive searching, and even tracking thetheir individual evolution of single checklists may be a non-trivial exercise. Checklists are also inevitably partially redundant when measured one against another, and where they overlap is far from straightforward. Furthermore, conflicts in scope and arbitrary decisions on wording and sub-structuring make integration difficult. This presents inhibit their use in combination. Overall, these issues present significant difficulties for the users of checklists, especially those in areas such as systems biology, who routinely combine information from multiple biological domains and technology platforms. To address all of the above, we present MIBBI (Minimum Information for Biological and Biomedical Investigations); a web-based communal resource for such checklists, designed to act as a ‘one-stop shop’ for those exploring the range of extant checklist projects, and to foster collaborative, integrative development and ultimately promote gradual integration of checklists. Dans le traité De la divination dans le sommeil, Aristote entend réfuter la thèse d’une origine divine des rêves qui semblent annoncer les événements futurs. Pour cela, il ne se contente pas de soutenir une thèse différente. Il dénonce également la faiblesse épistémologique de la pratique divinatoire, en proposant un modèle scientifique alternatif : une méthode de classification des rêves et de division par épuisement, qui exclut les rêves inspirés. Cette méthode conduit à souligner le caractère irréductible des faits de (...) hasard et à admettre le caractère inexplicable d’un grand nombre de rêves « véridiques ». La notion de coïncidence ( sumptôma ) joue sur ce point un rôle essentiel. Ce rôle est à la fois descriptif et critique : il permet de définir la classe des rêves dont la dimension véridique est absolument fortuite, et il dénonce l’illusion commune qui les relie à des événements réels. De ce point de vue, le chapitre 1 du De divinatione apporte une contribution, modeste mais spécifique, à la théorie des modalités physiques. (shrink)

The integration of ethical analysis in Health Technology Assessment (HTA) has proven difficult to implement even though it is explicitly recognized as an important component of such assessments in HTA literature. When compared to the standardized scientific method for systematic reviews in HTA, the diversity of ethical analysis has been characterized as a fundamental barrier to the integration of ethics. The present paper aims to identify the theoretical and practical differences between the approaches underpinning ethical analysis in HTA and clarify (...) the reasons for such diversity. Our systematic review of HTA literature pertaining to the barriers to the integration of ethics in HTA identified nine ethical approaches: Principlism, Casuistry, Coherence Analysis, Wide Reflective Equilibrium, Axiology, the Socratic approach, the Triangular model, Constructive Technology Assessment and Social Shaping of Technology. Citations pertaining to each approach were extracted and categorized according to three constitutive components of ethical argumentation established in a previous research evaluating nanotechnologies: i) the disciplinary foundation that grounds the validity of the ethical evaluation, ii) the characteristics of such evaluation, iii) the operational process involved in applying it to a particular case (i.e., its practical reasoning). This comparison shows that, 1) the difference between these approaches rests primarily on their disciplinary foundation (rooted in philosophy, philosophy/theology, or sociology), 2) their complexity can be observed in the distinct characteristics of ethical evaluation deriving from their differing disciplinary foundation, and 3) although four different types of operationalization procedure were identified, little information was available in regards to the practical reasoning associated with these approaches. L’intégration dans l’observation et la compréhension de toute la palette des motifs de déplacements et l’individualisation croissante des pratiques ont amené l’usage d’un terme plus générique, emprunté aux sciences sociales et notamment à ceux qui s’intéressent à la fluidité dans l’espace social, celui de mobilité. Ce glissement n’est pas purement sémantique : il (...) exprime le passage de l’empire de la nécessité aux capacités stratégiques et aux désirs des acteurs, de la représentation aréolaire d’un territoire rassemblant une « société locale » « contenant » les déplacements de ses membres à des représentations réticulaires , multi-territoriales . Sur tous ces points, les enquêtes initiées dans les années 1960 par les ingénieurs pour alimenter leurs modèles de trafic nous ont permis, grâce à l’élaboration d’indicateurs, de concepts, de représentations nouvelles, de comprendre en profondeur la dynamique et les rôles respectifs des rationalités individuelles et de la formation de normes sociales dans le champ spécifique de la mobilité quotidienne. C’est de cette construction dont nous rendons compte dans cet article, qui livre aussi à partir des quelques résultats saillants des recherches conduites, les convictions fortes en matière de dynamique de la mobilité qui s’en dégagent et les paradigmes qu’il conviendrait de mieux explorer.Spatial behaviour was studied in the past through group concepts, such as residential or regular migrations from home to work. The will to understand travel behaviour for the full range of trip purposes and the growing personalisation of behaviour leads to adopt the term of mobility from social sciences. This change is not purely semantic. It expresses the transition from a vision based on the needs to the consideration of the strategic capabilities of the persons, from areola-based representations of a local society to network and archipelago-based representations of individual behaviours. We have used, in conjunction with the international community of travel behaviour analysts, the travel surveys introduced in the sixties to feed traffic models, to build concepts, indicators and representations of travel behaviour, to understand in depth the transformations of mobility patterns and to analyze the roles of individual rationality and social norms. The paper relates this history, highlights some key results and strong beliefs, and questions the need to explore new paradigms. (shrink)

The present work is aimed at investigating the effects of myocardial infarction and ischemia on induction of ventricular fibrillation. Electrophysiologic effects of global and local ischemia (variation of the dispersion of refractory periods as well as conduction velocity) on initiation of reentry mechanisms was studied by means of computer simulations based on a cellular automata model of propagation of activation wave through a ventricular surface element. A local area of ischemia where effects of the dispersion of refractory periods are investigated (...) is then simulated. This is made using a Gaussian distribution characterized by its mean and standard deviation. These simulations show that ischemia is capable of initiating reentry phenomena which propagate through the whole ventricle; they are responsible for ventricular fibrillation which causes sudden cardiac death, even when ischemia only involves limited parts of the myocardium. Statistical study of the probability of reentries as a function of both of the size of ischemic zones and the rate of dispersion of refractory periods shows that the latter parameter is of primary importance in triggering cardiac reentries. The staying power of creationist objections to evolution needs explanation. It depends on the use of “blood” language. Both William Jennings Bryan and, a century later, Ken Ham connect evolution with the blood of predation and the blood of apes, and both also connect evolution with the blood of atonement. Drawing on Mary Douglas and Bettina Bildhauer, I suggest that blood becomes important to societies that image the social body on the human body. Blood reveals the body as porous and (...) vulnerable and therefore needing social work to be constructed as whole and bounded. Blood is the place where society conducts this work. I conclude that blood language is ineliminable from Christian discourse and indeed from discourses that model the social on the individual body. The solution, I suggest, is not to avoid the language of blood, but to continue to use it in ways that broaden its focus from human sin to human and animal suffering. (shrink)

This bibliographical review of the modelling of the mitotic apparatus covers a period of one hundred and twenty years, from the discovery of the bipolar mitotic spindle up to the present day. Without attempting to be fully comprehensive, it will describe the evolution of the main ideas that have left their mark on a century of experimental and theoretical research. Fol and Bütschli's first writings date back to 1873, at a time when Schleiden and Schwann's cell theory was rapidly gaining (...) ground throughout Germany. Both mitosis and chromosomes were to be discovered within the space of thirty years, along with the two key events in the animal and plant reproductive cycle, namely fecondation and meiosis. The mitotic pole, a term still in use to this day, was employed to describe a morphological fact which was noted as early as 1876, namely that the lines and the dots of the karyokinetic figure, with its spindle and asters, looks remarkably like the lines of force around a bar magnet. This was to lead to models designed to explain the movements of chromosomes which take place when the cell nucleus appears to cease to exist as an organelle during mitosis. The nature of those mechanisms and the origin of the forces behind the chromosomes' ordered movements were central to the debate. Auguste Prenant, in a remarkable bibliographical synthesis published in 1910, summed up the opposing viewpoints of the vitalists, on the one hand, who favoured the theory of contractility or extensility in spindle fibres, and of those who believed in models based on physical phenomena, on the other. The latter subdivided into two groups: some, like Bütschli, Rhumbler or Leduc, referred to diffusion, osmosis and superficial tension, whilst the others, led by Gallardo and Hartog, focussed on the laws of electromagnetism. Lillie, Kuwada and Darlington followed up this line of research. The mid-20th century was a major turning point. Most of the modelling mentioned above was criticized and fell into disuse after disappearing from research publications and textbooks.This marked the onset of a new era, as electron microscopes made possible the materialization and detailed study of the macromolecular elements of the fibres, filaments and microtubules of the cytoskeleton. The successive phases of (a) de Harven and Bernhard's 1956 discovery of the centriole's ultrastructure, (b) its identification with the basal body of the cilia and flagella, confirming the theory set out by Henneguy and von Lenhossek (1898–99), (c) the universal presence of microtubules in animal, vegetal and eukaryotic protist cells, (d) the polymerization-depolymerization induced reversible transformations of the tubulin pool in mitosing cells (Inoue, 1960), (e) ultrastructural comparative studies of the mitotic apparatus of eukaryotes illustrating the Pickett-Heaps integrating concept of the MTOC (microtubule-organizing centre), (f) the possibility ofin vitro experiments on mtocs or on microtubules, brings us upon the present day, which has seen the focus placed on the concept of motor-proteins (kinesin, dynein) and on cell cycle models. The latter are based on a close coincidence between the observable modifications of the mitotic apparatus and the periodic variations in intracellular concentrations of calcium or of certain enzymes (cyclins, Cdc2) during the main transitions of the cell cycle. This article explores recent developments in the sociology of the arts, namely the new theoretical framework set up by the French sociologist Pierre-Michel Menger in order to approach the artistic activity. It aims to show how he has shaped new tools of understanding and modelling for exploring the arts, as a particular world of action. Laying down the foundation of a conception of action related to symbolic interactionism and drawing on the economic analysis of risk and uncertainty, Menger move (...) towards a model where uncertainty, not determinism, is the default category. Uncertainty is thus the principle unifying the analysis and understanding of the particularities of the arts, and the main argument in integrating sociological and economic studies on artistic activities. Correlatively, artistic creation is qualified as an act of work, the creative work being defined as a system of action in uncertain horizon. Within this model, the artists are seen less like “rational fools” and more like Bayesian actors, adopting strategies for managing risk and uncertainty. (shrink)

Although the essays in this volume acknowledge that the primary locus for the love and care of children is the family, they also engage other contexts including religious communities, courts, and legislative bodies.In a section dealing with social-psychological perspectives on this theme, Peter Benson and Eugene Roehlkepartain make the case that doing the best for children requires shifting from a deficit model that focuses on what children lack to a model that emphasizes their developmental assets. In his concluding essay in this section, Charles Reid links historical and contemporary concerns by focusing on the right to life and its implications for analyses of war and healthcare access.In a section on philosophical and theological perspectives, Cynthia Willett challenges liberal understandings of rights in view of the experiences of children and argues for an approach that recognizes a central paradox of human freedom: that relationships of interdependence do not weaken freedom but rather enhance it. This paves the way for developing a sense of collective responsibility for children based on shared commitments to justice and solidarity that reach beyond the privatized family. Richard Osmer tackles the concept of original sin in order to advance a vision of a missional church in which congregations work with families to encourage authoritative—rather than authoritarian—styles of parenting that model mutual forgiveness. Marcia Bunge’s contribution is the most practical, offering a list of practices for children, families, and congregations for nurturing children’s vocations.In a concluding section on legal perspectives, Michael Broyde notes that, in the context of custody cases in Jewish law, the primary question is not who loves the child the most but rather who can best meet the child’s needs in an atmosphere of respect. Margaret Brinig and Steven Nock argue that law can, in fact, foster the love of children and the potential children have for becoming loving themselves by promoting family structures that are stable and afforded status in the community. The collection concludes with an essay by Don Browning that emphasizes weaving together love’s affective elements with the very practical activities of caring for children and respecting their emerging selves.By inviting thoughtful reflection on how loving and being loved are central to the well-being of children in families and other institutions, Best Love makes an important contribution to the interdisciplinary field of childhood studies. Élie HALÉVY (1870-1937), philosophe et historien des idées, fut professeur à l'École libre des sciences politiques, l'ancêtre de l'actuel Sciences Po. Comme son autre grand ouvrage, l'Histoire du peuple anglais au XIXe siècle, paru en six tomes de 1913 à 1932, les trois tomes de La formation du radicalisme philosophique, parus en 1901 pour les deux premiers et en 1904 pour le troisième, reflètent pour partie ses enseignements de l'Ecole libre consacrés à l'histoire britannique. Le premier tome, La jeunesse de (...) Bentham 1776-1789, étudie la doctrine utilitariste non seulement chez celui qu'on regarde comme son fondateur principal, Jeremy Bentham, mais aussi chez les nombreux auteurs qui, en Grande-Bretagne et sur le continent, en dessinèrent avant lui les contours. Le deuxième tome, L’évolution de la doctrine utilitaire de 1789 à 1815, montre comment l'utilitarisme revêtit la forme non seulement d'une école de pensée, mais aussi d'un mouvement pour la réforme économique, sociale et politique. Le rôle coordonnateur nouveau de James Mill, ainsi qu'une convergence de vues avec les économistes, qui poussaient dans le sens des réformes, marquèrent notamment cette évolution. Le troisième tome, Le radicalisme philosophique, continue d'étudier la transformation de l'école en mouvement après la fin des guerres napoléoniennes, lorsque celle-ci commence à engranger ses premiers grands succès réformistes. Bentham, James Mill et les autres penseurs utilitaires sont alors réunis sous l'appellation de philosophic radicals. Le terme temporel de l'ouvrage est le Reform Act de 1932, première étape vers la modernisation du système électoral, que la propagande de ce groupe ne contribua pas peu à faire aboutir. Quoique l'ouvrage d'Halévy vaille en premier lieu par l'immense savoir qu'il déploie, et le nombre et l'excellence des citations qu'il propose, il comporte aussi des thèses historiques et philosophiques originales. On peut citer parmi les premières la thèse, qui relie les trois tomes, voulant que l'utilitarisme britannique trouve sa forme achevée dans l'intervention sur la société, lorsqu'il se mue en radicalisme philosophique, et parmi les secondes, la thèse, énoncée au début du premier tome, voulant qu'il existe trois modèles dominants de jonction des intérêts individuels (la fusion sympathique, l'identification naturelle et l'identification artificielle). Une autre grande thèse, à la fois historique et philosophique, affirme en substance que l'économie politique classique serait un département spécialisé de la pensée utilitaire. La question de savoir jusqu'à quel point Smith, Ricardo et Malthus ont pu adhérer au "principe d'utilité" de Bentham est toujours débattue. En même temps que les élucidations apportées à ce principe, elle contribue à expliquer l'intérêt que les historiens de la pensée économique continuent de porter à l'ouvrage. La réédition de La formation du radicalisme philosophique par les Presses Universitaires de France (P.U.F.) a impliqué un effort collectif. Monique Canto-Sperber en a conçu l'idée et assumé la direction, et l'ouvrage a paru en 1995 dans la collection Philosophie morale dont elle avait alors la responsabilité aux P.U.F. Chacun des trois tomes s'accompagne d'une postface, confiée au philosophe Jean-Pierre Dupuy pour le premier, au politologue Pierre Bouretz pour le second, et à l'économiste Philippe Mongin pour le troisième. Un travail de grande ampleur a été requis, d'une part, pour localiser les références de Halévy dans des sources plus récentes que celles dont il disposait, en particulier, s'agissant de Bentham, dans l'édition toujours en cours de ses œuvres, et d'autre part, pour traduire les très nombreux passages que Halévy citait en langue anglaise. Sophie Jallais et Mary Sokol se sont chargées de la première tâche et Jean-Pierre Cléro de la seconde. Prise après quelques années de circulation de l'ouvrage, la décision des P.U.F. d'en pilonner les exemplaires que ses responsables ne rachèteraient pas a semblé compromettre ce vaste travail. Good and Mary-Jo DelVecchio GoodIf, as Arthur Frank (2002) writes, "moral life, for better and worse, takes place in storytelling," this collection of narratives written by physicians working in field settings in global medicine gives us a glimpse of some aspects of moral experience, practice, and dilemmas in settings of poverty and low health care resources. These essays are written (...) from the midst of practice, in diverse settings in Africa, South Asia, Latin America, the Caribbean, and the United States. The majority of the writers are practitioners of contemporary 'global medicine'—physicians from North America or Europe who spend part of their lives flying into settings of poverty or special need to practice medicine and work with on-going collaborative clinical and public health systems—while a smaller number are working in the societies in which they were born. Although a few work for classic humanitarian organizations or have developed their own local NGO's, the great majority are engaged in the 'hybrid life' described by Robert Riviello and noted by Renée Fox, moving between academic medical centers and distant health care systems that are supported by local governments, major international organizations and funding agencies, and international NGO's like Partners in Health. These diverse forms of practice have become a common model for contemporary clinicians engaged in what has emerged as a new discipline of global health or global medicine, and these narratives give us a glimpse of some of the enduring, as well as newly emerging, forms of moral practice and ethical dilemmas characteristic of today's global medicine.Many of these stories are filled with the tension and pathos that makes clinical narratives powerful to read. Physicians are forced to make unfathomable decisions about whether to resuscitate and keep a baby alive, using all means available, or to let the infant die, whether to carry out a procedure that will save an infant in birth but would put the mother at risk, about how to deal with a cancer found in a very advanced state. However, virtually all of the stories transcend these classical stories of the decisions physicians are forced to make by being placed in settings of great scarcity of resources, which stand in particular contrast to the academic medical settings in North America or Europe where many of these global medicine specialists have trained and many continue to practice. The unspeakable inequality of resources emerges throughout these narratives as a kind of master theme, as the underlying moral, as opposed to plot, which gives meaning and sense to the stories. For those who practice medicine in such settings, global inequalities and scarcity of medical resources are deeply felt. They produce the rage that Jeffrey Deal describes. ("I learned to despise American churches during that time, a time when my world encompassed a dying newborn in Sudan, as well as the air conditioned padded seats [End Page 121] of our affluent suburban church.") They produce the distinctive experiences of helplessness and pain felt by clinicians who cannot separate how they would be able to treat such a patient at home from how they have to practice in these settings. For those clinicians working in their own societies, scarcity of resources often produces deep anger at the corrupt governments that fail to support basic human services for the great majority of their population. ("We were temporary actors performing in the cynical play of scarcity on permanent display in Port-au-Prince," writes Haitian physician Paul Pierre about his experience prior to joining Partners in Health.)But for those who regularly travel back and forth across the divide between rich and poor, whether across national boundaries or within their own society, the challenge to live a moral life is unendingly complicated. While becoming a part of a struggle for global justice with colleagues and advocates from both sides of the divide, few of the narrators can avoid personal feelings of ambivalence and guilt at being able to easily move from one side of that divide to the other back home. The medical model of childbearing assumes that a pregnancy always has the potential to turn into a risky procedure. In order to advocate humanized birth in high risk pregnancy, an important step involves the enlightenment of the professional’s preconceptions on humanized birth in such a situation. The goal of this paper is to identify the professionals’ perception of the potential obstacles and facilitating factors for the implementation of humanized care in high risk pregnancies. Twenty-one midwives, obstetricians, and health administrator professionals (...) from the clinical and academic fields were interviewed in nine different sites in Japan from June through August 2008. The interviews were audio taped, and transcribed with the participants’ consent. Data was subsequently analyzed using content analysis qualitative methods. Professionals concurred with the concept that humanized birth is a changing and promising process, and can often bring normality to the midst of a high obstetric risk situation. No practice guidelines can be theoretically defined for humanized birth in a high risk pregnancy, as there is no conflict between humanized birth and medical intervention in such a situation. Barriers encountered in providing humanized birth in a high risk pregnancy include factors such as: the pressure of being responsible for the safety of the mother and the fetus, lack of the women’s active involvement in the decision making process and the heavy burden of responsibility on the physician’s shoulders, potential legal issues, and finally, the lack of midwifery authority in providing care at high risk pregnancy. The factors that facilitate humanized birth in a high risk include: the sharing of decision making and other various responsibilities between the physicians and the women; being caring; stress management, and the fact that the evolution of a better relationship and communication between the health professional and the patient will lead to a stress-free environment for both. Humanized birth in a high risk pregnancy is something that goes beyond just curing women of their illnesses. It can be considered as a token of caring, and continued support, which positively consolidates the doctor-patient relationship. As yet, it has not been described as a practiced guideline, due to its ever-changing complexities. (shrink)

Bien connues, les dix-sept lithographies conçues par Eugène Delacroix d’après le Faust de Goethe, insérées dans un livre imposant paru en 1828 sous la double enseigne du libraire Auguste Sautelet et de l’imprimeur-lithographe Charles Motte, constituent une série souvent analysée par les historiens de l’art dans la perspective de l’œuvre du peintre. En modifiant cette approche, l’article la considère sous l’angle de son support, le livre dans lequel elle a été insérée, et en relation avec ce qui l’entoure, (...) le texte de la tragédie et les propos de Delacroix, en valorisant un faire de l’imagination, d’inspiration multiple et de facture inter-iconique. Grâce au collationnement d’une quinzaine d’exemplaires du livre, conservés dans les dépôts publics de plusieurs pays, on découvre en quoi la quinzième lithographie, l’apparition de Marguerite au sabbat, est exceptionnelle. L’interprétation proposée se fonde sur le rôle de l’ imagination dans ce Faust français, la pratique graphique de Delacroix, le passage afférent, et Le Voyage en Italie de Goethe. En s'inscrivant dans une problématique posée par Pierre Schaeffer à propos d'une utilisation des moyens de communication au profit de groupes sociaux d'intérêts communs, cet article présente les conditions d'émergence et de fonctionnement des réseaux spécifiques de communication sociale à partir de l'exemple concret de la radio Antélim, et il s'interroge sur une éventuelle préfiguration d'Internet. Dans un premier temps, un rappel des théorisations, recherches et expériences, menées au cours des années 1960-1970, dégagera les conditions d'une communication participative, interactive (...) et multipolaire, modèle alternatif à celui, dominant, des mass média. Puis, l'étude de l'expérimentation d'Antélim menée en France entre 1978 et 1985 avec le milieu de la marine marchande mettra en lumière l'origine du projet, sa mise en oeuvre et présentera une évaluation de l'expérience. Enfin, la question de la préfiguration d'Internet par ces réseaux spécifiques de communication sociale via la radio sera posée pour tenter de définir des parallèles possibles entre ces deux dispositifs.By entering in a problem posed by Pierre Schaeffer about a use of means of communication for the benefit of groups of common interests, this article presents the conditions for the emergence and operation of specific networks of social communication from the example of the radio Antélim, and he wondered about a possible forerunner of the Internet. At first, a reminder of the theories, research and experiments, conducted in the years 1960-1970, will identify the conditions for participatory communication, interactive and multipolar alternative model to the dominant, mass media. Then, the study of Antélim experimentation conducted in France between 1978 and 1985 with the middle of the Merchant will highlight the origin of the project, its implementation and present an evaluation of the experience. Finally, the question of the forerunner of the Internet by these specific networks of social communication via the radio will be asked to try to identify possible parallels between these two devices. McKennaThe Self Between: From Freud to the New Social Psychology of France, by Eugene Webb; ix & 268 pp. Seattle: University of Washington Press, 1993, $35.00.That psychology and sociology are one science is the fundamental premise guiding Eugene Webb’s The Self Between, which he defines early on as “a self constituted dynamically and continuously by (...) the relationships it finds itself involved i n.” The “new social psychology of France” designated by his subtitle is mostly the one inspired by René Girard’s mimetic model of human behavior, to which four of Webb’s seven chapters are specifically devoted.Girard’s idea, which he learned from our literary masterpieces, is that human desire is mimetic, that it imitates other desires in its choice of objects and in pursuit of their possession. As desires converge on a single object, the locus of conflict will be between subjects rather within their individual psyches, a consequence that motivates Girard’s ongoing critique of Freudian psychoanalysis.But Freud’s theories are everywhere disputed in this book, which reports reliably on the devastating critiques advanced by François Roustang, Marie Balmary, and Mikkel Borch-Jacobsen. Roustang has convincingly (and entertainingly) uncovered Freud’s blindness to the pathologies of transference and discipleship investing the foundation of psychoanalysis. A pattern of rivalry for recognition and preeminence guides Freud’s career in a way that is reenacted around Lacan. Balmary, somewhat a Lacanian her self, has turned psychoanalytic theory against Freud to explore his own family secrets, while severely questioning the master’s bid for a universal theory pursued via self-analysis amidst a period of emotional upheaval. And Borch-Jacobsen has shown how th e dynamics of hypnosis that Freud rejected early on were brought back as transference, while arguing for his own part that mimetic identification with the other, rather than any primordial drives, is what brings the subject into being. These informat ive chapters help bring us up to date on significant debates that have developed since Lacan drew the French intellectual world, and so many Americans after it, into the orbit of psychoanalysis.But by far the bulk of Webb’s attention goes to Girard’s work and to that of [End Page 191] scholars and practitioners who have sought to extend it further in the social sciences. Principle among them is Jean-Michel Oughourlian, whose Puppet of Desire (translated by Webb in 1991) demonstrates how the mimetic model plays out in psychotherapy, notably in the workings of hypnotism and somnambulism, in cases of demonic possession, and in obsessional neurosis. In each case, the subject is unwitt ingly captive of another’s desire in a way that produces behavioral anomalies and disorders that are nonetheless available to lucid, jargon-free diagnosis. What is appealing here is the deployment of a rational explanation for irrational behavior, once we grasp the paradoxical rationale of desire; equally appealing prospects of actual healing are outlined here and at the end of the book, where possibilities of freedom and responsibility are resurrected from a technical vocabulary that smothers them.Girard’s idea of the self as relation, as interdividuality, rather than as a discreet unit, has social and political dimensions to which we only find tantalizing allusions in his works. They have been pursued with acumen by such writers as André Orléan, Paul Dumouchel and especially Jean-Pierre Dupuy, who figures importantly among those aligning systems theory with theories of social and economic organization. BackgroundIn the context of translational research, researchers have increasingly been using biological samples and data in fundamental research phases. To explore informed consent practices, we conducted a retrospective study on informed consent documents that were used for CARPEM’s translational research programs. This review focused on detailing their form, their informational content, and the adequacy of these documents with the international ethical principles and participants’ rights.MethodsInformed consent forms (ICFs) were collected from CARPEM investigators. A content analysis focused on information related to (...) biological samples and data treatment (context of sampling and collect, aims, reuse, consent renewal), including the type of consent. An automatic assessment of the readability of the ICFs were performed with the IT program “Flesch Score”.Results29 ICFs from 25 of 49 studies were analyzed after selection criteria were applied. Three types of consent were identified: 11 broad consents, six specific consents, and two opt-out consents. The Flesch Scores showed that most of the documents were too complex to be fully understood by most of the potential research participants. Most of the biological samples were collected during the healthcare routine, but the information content about secondary use of biological samples varied between ICFs. All documents mentioned personal data treatment but information about their reuse was not standardized in the ICFs.ConclusionsOur review of current IC procedures of CARPEM showed that practices could be improved considering new translational research methods. “Old fashion written ICFs” should be adapted to the translational research approach, to better respect individual rights and international research ethics principles. In this context, theoretically, a digital tool allowing dynamic information and consent of participants, through an electronic interactive platform may be a good way to promote more active participation in research. Nevertheless, its feasibility in the complex environment of biological samples and data research remains to prove. The way of a combination of a broad consent followed by dynamic information may be alternatively tested. (shrink)

From an analysis of the famous principle ‘‘Nature does nothing in vain’’ in Aristotle’s Locomotion of animals, this paper addresses the question of the meaning and boundaries of finalism in zoology, as well as the related idea of conformity with nature. It is argued that, although ‘‘Nature does nothing in vain’’ seems to allude, at first glance, to cosmic or global teleology, the treatise On the Locomotion of Animals gives a different meaning to the formula : the teleological account, in (...) this context, depends on a local and relative finalism, which is defined in accordance with the specific and generic properties of animals, and not by reference to any alleged cosmic axiology or global causal chain. However, this principle should not be reduced to organic teleology strictly speaking, in so far as it involves also external and necessary conditions, which are common rules governing the locomotion of animals. Drug exceptional access programs (DEAPs) exist across Canada to address gaps in access to pharmaceuticals. These programs circumvent standard procedures, raising epistemic, economic, social and political issues. This commentary provides insights into these issues by revealing the context and procedures on which these programs depend.

In the last lines of Politics VII, 3, Aristotle states that the happy city acts nobly. This implies that the city has a practical life, and that this life has its end in itself. This claim seems to contradict the famous distinction, which has been made elsewhere by Aristotle, between the practical and theoretical lives. It is argued in this paper that there is, here, neither contradiction nor inconsistency in Aristotle’s conception of human action. Some readings, according to which this (...) passage deals with the role of the philosophers in the happy city, are also ruled out. The proposed solution consists in showing that this text refers to a broader conception of action, one that is beyond the sphere of human practical activities: any genuine praxis has its goal in itself, even when it produces external effects. Devant le constat du déclin toujours plus rapide de la diversité biologique et les limites des res- sources disponibles pour l’enrayer, il est nécessaire de déterminer quels moyens devraient être engagés dans sa protection. Pour ce faire, une méthode efficace serait d’évaluer les bénéfices tirés de la biodiversité afin d’estimer rationnellement les coûts légitimes de sa protection. L’évaluation économique, qui se présente d’emblée sur un mode quantitatif, serait alors un outil précieux. Dans ce texte, nous présentons différentes méthodes d’évaluation économique (...) de la biodiversité ainsi que certaines de leurs limites méthodologiques. Par la suite, nous montrons qu’en dépit de ses avantages pratiques, l’évaluation économique échoue à représenter l’ensemble des valeurs en jeu dans la protection de la biodiversité. Nous décrivons alors trois types de valeurs incommen- surables avec des bénéfices économiques : la valeur de legs, qui renvoie aux obligations de trans- mission du patrimoine naturel dont nous avons hérité ; la valeur d’existence, qui renvoie à la considération morale d’intérêts autres que ceux des êtres humains ; la valeur de transformation, qui renvoie à la capacité d’examiner et de critiquer nos préférences et inclinaisons afin d’inclure celles-ci dans une vision du monde rationnelle et cohérente. Pour conclure, nous plaidons en faveur de l’élaboration de méthodes de concertation et d’évaluation participatives et pluralistes permettant de rendre compte de la diversité et de l’hétérogénéité des valeurs de la biodiversité. The texts of Diogenes of Oinoanda (2nd century AD) who invited his readers to an Epicurean life is the largest ancient inscription ever discovered. Over 70 new finds have increased the number of known wall blocks and fragments to nearly 300, offering new insights into Diogenes' distinctive presentation of philosophy. This collection of essays discusses the philosophical significance of these discoveries and is the first of this kind (...) entirely devoted to Diogenes of Oinoanda. Particular attention is paid to his philosophical aims and polemical strategies. Diogenes was apparently well aware of still ongoing philosophical debates, engaging in polemics against Presocratic philosophers, Platonics, and especially Stoics. His views about important issues like happiness, fear, old age, and the afterlife are explained on the bases of Epicurean physics and theology, ethics, politics, theory of knowledge, and psychology.
