Providing medical care and planning for a procedure such as amputation may have different cultural contexts, based on patients’ country, comfort, and contract with their physician. These contexts may create complexities for physicians as they interact with patients and caregiving relatives. Issues such as the personal choices of a caregiving relative may appear to unduly influence the decisions behind complex healthcare choices. We consider several possible scenarios in the background of the complex case presented in “Family Loyalty as a Cultural (...) Obstacle to Good Care: The Case of Mrs. Indira,” in this issue of JCE. An estimated 1.2 to 2.3 million Hindus live in the United States. End-of-life care choices for a subset of these patients may be driven by religious beliefs. In this article, we present Hindu beliefs that could strongly influence a devout person’s decisions about medical care, including end-of-life care. We provide four case examples (one sacred epic, one historical example, and two cases from current practice) that illustrate Hindu notions surrounding pain and suffering at the end of life. Chief among those (...) is the principle of karma, through which one reaps the benefits and penalties for past deeds. Deference to one’s spouse or family is another important Hindu value, especially among Hindu women, which can impact the decision-making process and challenge the Western emphasis on autonomy. In addition, the Hindu embrace of astrology can lead to a desire to control the exact time of death. Confounding any generalizations, a Hindu patient may reject or accept treatments based on the individual patient’s or family’s interpretation of any given tradition. Through an awareness of some of the fundamental practices in Hinduism and the role of individual interpretation within the tradition, clinicians will be better able to support their Hindu patients and families at the end of life. In a recent paper, Wigglesworth claims that syntactic criteria of theoretical equivalence are not appropriate for settling questions of equivalence between logical theories, since such criteria judge classical and intuitionistic logic to be equivalent; he concludes that logicians should use semantic criteria instead. However, this is an artefact of the particular syntactic criterion chosen, which is an implausible criterion of theoretical equivalence. Correspondingly, there is nothing to suggest that a more plausible syntactic criterion should not be used to settle questions (...) of equivalence between different logical theories; such a criterion is exhibited and shown to judge classical and intuitionistic logic to be inequivalent. This paper argues that much of the literature on interpreting scientific theories presupposes a certain picture of what interpretation involves: a picture according to which interpreting a theory is like translating from one language to another. In place of this “external” approach to interpretation, this paper proposes an “internal” approach, according to which interpretation is more concerned with delineating a theory’s internal semantic architecture.

This is an essay about general covariance, and what it says about spacetime structure. After outlining a version of the dynamical approach to spacetime theories, and how it struggles to deal with generally covariant theories, I argue that we should think about the symmetry structure of spacetime rather differently in generally-covariant theories compared to non-generally-covariant theories: namely, as a form of internal rather than external symmetry structure.

This article gives an explicit presentation of Newtonian gravitation on the backdrop of Maxwell space-time, giving a sense in which acceleration is relative in gravitational theory. However, caution is needed: assessing whether this is a robust or interesting sense of the relativity of acceleration depends on some subtle technical issues and on substantive philosophical questions over how to identify the space-time structure of a theory.

The Royal Liverpool Children’s Inquiry investigated the circumstances leading to the removal, retention, and disposal of human tissue, including children’s organs, at the Royal Liverpool Children’s NHS Trust . It recommended changes to procedures for obtaining consent for postmortems and retaining organs and tissues for research or education. However, the report contains five areas of confusion. Firstly, it allowed the cultural and historical traditions of horror over the use and misuse of body parts to suffuse the logical analysis of past (...) wrongs and future rights. Secondly, it makes an inappropriate conflation between seeking redress for past wrongs and shaping future policy. Thirdly, the report takes a muddled stance over the value of bodily integrity at burial. Fourthly, the report is inconsistent over the justification for future organ and tissue collections. Fifthly, the notion of “respect” is used with troublesome looseness. The extent to which subsequent policy work has furthered the search for greater ethical clarity over these difficult issues is discussed, together with reflection on three particular improvements that could be made to the process of such an inquiry. In the literature over the Ramsey-sentence approach to structural realism, there is often debate over whether structural realists can legitimately restrict the range of the second-order quantifiers, in order to avoid the Newman problem. In this paper, I argue that even if they are allowed to, it won’t help: even if the Ramsey sentence is interpreted using such restricted quantifiers, it is still an implausible candidate to capture a theory’s structural content. To do so, I use the following observation: if (...) a Ramsey sentence did encode a theory’s structural content, then two theories would be structurally equivalent just in case they have logically equivalent Ramsey sentences. I then argue that this criterion for structural equivalence is implausible, even where frame or Henkin semantics are used. (shrink)

Discipline based approach to knowledge and specialization goes back to the classical Graeco-Roman times. It was Isaac Newton’s Principia, which represented academic perfection of Renaissance inquiry, criticism, creativity and theorization. Principia set the epochal model of knowledge and led to the making of the Age of Enlightenment and the Age of Reason. August Comte heralded an era of Positivism in academic research and prepared the ground for the making of disciplines. As new knowledge accrued over centuries, specialization got diversified at (...) the expense of holistic understanding. Academics of disciplinary specialization sought to resolve the problem through multidisciplinary research. Inherently discipline centric, it got further distanced from social reality. Interdisciplinary approach began as a movement through convergence research aiming to restore holistic comprehension and draw knowledge closer to social reality. It is production of knowledge beyond disciplines and across their interfaces enabling research output to be regenerative, non-conventional and resolution oriented. (shrink)

BLWith Latin text and English translation The epic poem the Thebaid was composed by Statius about AD 80 to 92 in twelve books. The subject is the expedition of the Seven against Thebes in support of the attempt by Oedipus' son Polyneices to recover the throne from his brother Eteocles. Book IX is set in the midst of the fighting before the eventual death of the two brothers. In this new edition of Book IX Dr Dewar accompanies the Latin (...) text with apparatus criticus, a translation, and an extensive introduction and commentary. The introduction contains sections on Statius' life and works, a summary of the epic, its themes and characters, and poetry, the textual tradition, and Statius' influence on later European literature. The commentary, the first on the ninth book to be published in Britain this century, is written in the light of recent scholarship. It examines in close detail Statius' style and language, use of models, and literary intentions. It is Dr Dewar's hope that this edition will help to explain the poem's great popularity in the Middle Ages, and even restore something of its lost prestige. (shrink)

More specifically, my aim is to make precise the idea that the structure of the world is given by the relations inhering in the world, in such a way that the relations are ontologically prior to their relata. This Element explores what it means for two theories in physics to be equivalent, and what lessons can be drawn about their structure as a result. It does so through a twofold approach. On the one hand, it provides a synoptic overview of the logical tools that have been employed in recent philosophy of physics to explore these topics: definition, translation, Ramsey sentences, and category theory. On the other, it provides a detailed case study of how these ideas may be (...) applied to understand the dynamical and spatiotemporal structure of Newtonian mechanics - in particular, in light of the symmetries of Newtonian theory. This study articulates the relational constituents of good care beyond techno‐rational competence. Neoliberal healthcare means that notions of care are readily commodified and reduced to quantifiable assessments and checklists. This novel research investigated accounts of good care provided by nursing, medical, allied and auxiliary staff. The Heideggerian phenomenological study was undertaken in acute medical‐surgical wards, investigating the contextual, communicative nature of care. The study involved interviews with 17 participants: 3 previous patients, 3 family members and 11 staff. Data were analysed (...) iteratively, dwelling with stories and writing and rewriting to surface the phenomenality of good care. The data set highlighted the following essential constituents: authentic care: caring encompassing solicitude (fürsorge); impromptu care: caring beyond role category; sustained care: caring beyond specialist parameters; attuned care: caring encompassing family and culture; and insightful care: caring beyond assessment and diagnosis. The findings are clinically significant because they indicate the importance of nurse leaders and educators harnessing the potential capacity of all healthcare workers to participate in good care. Healthcare workers reported that participating in or witnessing good care was uplifting and added meaning to their work, contributing to a sense of shared humanity. (shrink)

This book seeks to provide an introductory outline of the history and theory of knowledge production, notwithstanding the vastness of the subject. It is a brief history of intellectual formation or history of ideas. One can see it as a textbook of historical epistemology, which in spatio-temporal terms historicises knowledge production and contextualises methodological development. It addresses the historical process of the social constitution of knowledge, that is, the social history of the making of knowledge.

BackgroundConventionally, transcranial direct current stimulation (tDCS) aims to focalize the current reaching the target region-of-interest (ROI). The focality can be quantified by the dose-target-determination-index (DTDI). Despite having a uniform tDCS setup, some individuals receive focal stimulation (high DTDI) while others show reduced focality (“non-focal”). The volume of cerebrospinal fluid (CSF), gray matter (GM), and white matter (WM) underlying each ROI govern the tDCS current distribution inside the brain, thereby regulating focality.AimTo determine the regional volume parameters that differentiate the focal and (...) non-focal groups.MethodsT1-weighted images of the brain from 300 age-sex matched adults were divided into three equal groups- (a) Young (20 ≤ × < 40 years), (b) Middle (40 ≤ × < 60 years), and (c) Older (60 ≤ × < 80 years). For each group, inter and intra-hemispheric montages with electrodes at (1) F3 and right supraorbital region (F3-RSO), and (2) CP5 and Cz (CP5-Cz) were simulated, targeting the left- Dorsolateral Prefrontal Cortex (DLPFC) and -Inferior Parietal Lobule (IPL), respectively. Both montages were simulated for two current doses (1 and 2 mA). For each individual head simulated for a tDCS configuration (montage and dose), the current density at each region-of-interest (ROI) and their DTDI were calculated. The individuals were categorized into two groups- (1) Focal (DTDI ≥ 0.75), and (2) Non-focal (DTDI < 0.75). The regional volume of CSF, GM, and WM of all the ROIs was determined. For each tDCS configuration and ROI, three 3-way analysis of variance was performed considering- (i) GM, (ii) WM, and (iii) CSF as the dependent variable (DV). The age group, sex, and focality group were the between-subject factors. For a given ROI, if any of the 3 DV’s showed a significant main effect or interaction involving the focality group, then that ROI was classified as a “focal ROI.”ResultsRegional CSF was the principal determinant of focality. For interhemispheric F3-RSO montage, interaction effect (p < 0.05) of age and focality was observed at Left Caudate Nucleus, with the focal group exhibiting higher CSF volume. The CSF volume of focal ROI correlated positively (r ∼ 0.16, p < 0.05) with the current density at the target ROI (DLPFC). For intrahemispheric CP5-Cz montage, a significant (p < 0.05) main effect was observed at the left pre- and post-central gyrus, with the focal group showing lower CSF volume. The CSF volume correlated negatively (r ∼ –0.16, p < 0.05) with current density at left IPL. The results were consistent for both current doses.ConclusionThe CSF channels the flow of tDCS current between electrodes with focal ROIs acting like reservoirs of current. The position of focal ROI in the channel determines the stimulation intensity at the target ROI. For focal stimulation in interhemispheric F3-RSO, the proximity of focal ROI reserves the current density at the target ROI (DLPFC). In contrast, for intrahemispheric montage (CP5-Cz), the far-end location of focal ROI reduces the current density at the target (IPL). (shrink)

Due to the complexity and versatility of network security alarm data, a cloud-based network security data extraction method is proposed to address the inability to effectively understand the network security situation. The information properties of the situation are generated by creating a set of spatial characteristics classification of network security knowledge, which is then used to analyze and optimize the processing of hybrid network security situation information using cloud computing technology and co-filtering technology. Knowledge and information about the security situation (...) of a hybrid network has been analyzed using cloud computing strategy. The simulation results show that a cyber security crash occurs in window 20, after which the protection index drops to window 500. The increase in the security index of 500 windows is consistent with the effectiveness of the concept of this document method, indicating that this document method can sense changes in the network security situation. Starting from the first attacked window, the defense index began to decrease. In order to simulate the added network defense, the network security events in the 295th time window were reduced in the original data, and the defense index increased significantly in the corresponding time period, which is consistent with the method perception results, which further verifies the effectiveness and reliability of this method on the network security event perception. This method provides high-precision knowledge of network security situations and improves the security and stability of cloud-based networks. (shrink)

Extensive empirical work has examined ethical perceptions of different occupational groups in marketing. Additionally, researchers have explored ethical apperceptions of industrial customers and retail consumers. Minimal effort, though, has been directed at investigating differences in ethical perceptions between buyers and sellers, notwithstanding considerable theoretical arguments for doing so. This paper reports the results of a study that focused on differences between retail customers’ and retail salespeople’s perceptions of questionable buying and selling behaviors. Findings indicate that the two groups differ in (...) some respect depending on which group is engaging in the questionable conduct. Managerial and future research implications are provided. This paper critically assesses whether quantum entanglement can be made compatible with Humean supervenience. After reviewing the prima facie tension between entanglement and Humeanism, I outline a recently-proposed Humean response, and argue that it is subject to two problems: one concerning the determinacy of quantities, and one concerning its relationship to scientific practice.

Periurban bypasses are enclaves that appear to be left behind of conventional spatial and technological processes. With the focus on cities and their development, the hinterland serves as a resource that barely makes its appearance in mainstream policy debates. Hidden even further in the periurban are areas whose inhabitants are marginalised in many ways. Developing an ethical framework for assessing periurban bypasses is rendered difficult by the complexity of attribution of harm to particular agents. Nevertheless, by using multiple modes of (...) interpretation and assessment of periurban bypasses, it is possible to create ethical profiles that identify social agents and elite networks for generating these harms. The core of the work is a lengthy hermeneutically-oriented discussion of political judgment, which projects the notion of political competence as a language mediated capacity of human subjects to recognize the common good by way of discourse. This discursive conception of the political which is mediated on the one hand by a relationship to the moral and on the other to the conception which can be contrasted with the modern paradigm of politics as the episteme of power relations. The earlier (...) chapters build up to this contrast of the classical and the contemporary paradigms and stake out a claim for the primacy of the political in the classical mould. The work is a return to the classical idea of the political by way of a long detour via the theory of judgment, the Kuhnian theory of paradigms, Ricoeur's theory of textual hermeneutics and rhetorical theory. Ranging from discussions of sociological and political theory to philosophy of language and philosophical logic, the work seeks to connect the Aristotelian-Kautilyan conception of the political to contemporary debates in the metatheory of the social sciences. Suppose that one thinks that certain symmetries of a theory reveal “surplus structure”. What would a formalism without that surplus structure look like? The conventional answer is that it would be a reduced theory: a theory which traffics only in structures invariant under the relevant symmetry. In this paper, I argue that there is a neglected alternative: one can work with a sophisticated version of the theory, in which the symmetries act as isomorphisms.

Background: Compassionate care is an international priority of healthcare professionals. There is little understanding about how workplace issues impact provision of compassionate care in nursing practice. Therefore, it is important to address the workplace issues and organizational factors which may hinder compassionate care delivery within nursing practice. Objective: The aim of this study was to explore workplace and organizational barriers to compassionate care from the nurses’ perspective. Research design: The study used a qualitative exploratory design, and data were analyzed by (...) conventional content analysis. Participants and research context: A total of 15 nurses working in different fields of nursing were recruited from four hospitals at northwest of Iran. Participants were selected by purposive sampling. Semistructured interviews were conducted for data collection. Ethical consideration: Ethical approval of this study was gained from the Ethical Review Board of Tabriz University of Medical Sciences. Findings: The main theme which emerged from data analysis was “unsupportive organizational culture.” This theme had two main categories including “excessive workload alongside inadequate staffing” and “the lack of value on compassionate care.” Discussion: Organizational barriers to development of compassionate in clinical practice were identified in this study. A closer examination of these barriers is required to move compassionate practice from an individual responsibility to a collective responsibility that is owned and shared by organizations. Conclusion: For compassionate care to flourish, policy makers, managers, and healthcare providers must foster an organizational atmosphere conducive to compassionate care. (shrink)

The rapid advancement of algorithmic trading has demonstrated the success of AI automation, as well as gaps in our understanding of the implications of this technology proliferation. We explore ethical issues in the context of autonomous trading agents, both to address problems in this domain and as a case study for regulating autonomous agents more generally. We argue that increasingly competent trading agents will be capable of initiative at wider levels, necessitating clarification of ethical and legal boundaries, and corresponding development (...) of norms and enforcement capability. Bortolotti’s Delusions and Other Irrational Beliefs defends the view that delusions are beliefs on a continuum with other beliefs. A different view is that delusions are more like illusions, that is, they arise from faulty perception. This view, which is not targeted by the book, makes it easier to explain why delusions are so alien and disabling but needs to appeal to forensic aspects of functioning.
