In this paper an Artificial Neural Network (ANN) model was used to help cars dealers recognize the many characteristics of cars, including manufacturers, their location and classification of cars according to several categories including: Buying, Maint, Doors, Persons, Lug_boot, Safety, and Overall. ANN was used in forecasting car acceptability. The results showed that ANN model was able to predict the car acceptability with 99.12 %. The factor of Safety has the most influence on car acceptability evaluation. This study contributes to the limited literature on VAT’s impact on student populations by assessing the financial strain on university students in Oman. The research provides novel insights into policymaking, suggesting VAT exemptions for essential educational goods and proposing financial literacy programs for mitigating the adverse effects of VAT which employs chi-square tests and regression analysis to quantify the financial effects of VAT on students' purchasing behavior, revealing that VAT negatively impacts purchasing power with a statistically significant p-value (< 0.05). The findings highlight substantial financial strain on students, particularly those in lower-income brackets, prompting policy recommendations for VAT exemptions on essential educational goods and the inclusion of financial literacy programs aimed at helping students mitigate the tax's adverse effects. This study evaluates the financial implications of the Value Added Tax (VAT) introduced in Oman in 2021, focusing on its impact on consumer behavior among university students at A’sharqiyah University. Utilizing a quantitative methodology, including chi-square tests and regression analysis, and a sample of 53 students, the research investigates the effects of VAT on purchasing power and consumption patterns. The findings demonstrate a significant disruption to students' financial capacity, with the majority reporting challenges in affording essential educational materials. This study delves into the use of impoliteness strategies within emails sent by undergraduate and postgraduate students to their professors, aiming to discern the variance in their implementation based on (Culpeper and Hardaker’s. 2017. Impoliteness. In: Culpeper, Jonathan, Haugh, Michael and Daniel Kadar (eds.), The Palgrave handbook of linguistic (im) politeness, 199–225. Basingstoke: Palgrave) model. Data, comprising emails from University of Jordan students and semi-structured interviews, underwent analysis to identify impoliteness strategies and themes. Findings indicate a higher prevalence of impolite (...) strategies among undergraduates, totaling 58 instances compared to 38 among postgraduates. Particularly, undergraduates exhibit a greater tendency toward positive impoliteness. Thematic analysis of interviews unveils three contributing factors: work experience, participation in writing skills courses, and familiarity with formality levels. Notably, undergraduates’ lack of work experience contrasts with postgraduates’, often impacting their email etiquette. Moreover, postgraduates’ extensive enrollment in writing courses during MA studies fosters a more formal approach in communication with professors. The study underscores implications for future research, emphasizing the need to explore these dynamics further. To enhance efforts against plagiarism, a deeper understanding of its various aspects is essential. This cross-sectional questionnaire study assessed the knowledge, attitudes, subjective norms, and self-reported plagiarism practices among health researchers in Egypt and Lebanon. A validated questionnaire was distributed to postgraduate students and faculty members across 15 universities through personal emails and health researcher-specific social media platforms. Out of 283 respondents, the mean knowledge score was 14.8 ± 3.7 out of a possible 27.0, indicating moderate knowledge of plagiarism. Attitudes (...) toward plagiarism were moderately permissive, with an average score of 62.2 ± 10.9, out of a maximum of 90.0. Subjective norms received a moderate score of 33.6 ± 6.7, where a favorable score should be low (10–23) suggesting that participants perceive society as disapproving of plagiarism and believe its prevalence is low. The mean self-reported Plagiarism Severity Score (PSS) was 4.1 ± 3.6, reflecting a low level of self-reported plagiarism, with a maximum possible score of 18. Multivariable analysis revealed a negative correlation between subjective norms and the PSS (β = −0.194, CI [−0.196: −0.010]). This finding is counterintuitive, as one would typically expect that a lower subjective norms score (indicating stronger disapproval of plagiarism), would correlate with fewer acts of plagiarism. One possible explanation for this result is the presence of negative perceptions of organizational justice—such as feelings of being unfairly treated—which might lead to higher rates of self-reported research misconduct. We recommend that universities in the Arab Middle East region develop research ethics training programs that include responsible conduct in research. However, it is essential for academic leaders to assess and address the research integrity climate within their institutions, as organizational injustice could undermine the effectiveness of these educational efforts. Using Least Square Residual Minimization techniques, this paper develops an optimal reserve model, known as the OPREM model, which is essential in optimizing the costs of reserve holding. The paper also sets-out to test and compare the relative predictions of economic trends of the OPREM model as well as the predictions of alternative models in literature. Establishing the predictive accuracy of economic trends of these models are crucial for the gradual and cost-effective accumulation of reserves. The research concludes that, the (...) decision to optimize the cost of reserves under a stable currency environment is reliant on the gold impact factor and not on inflation or interest rates. We also found on further analysis of the OPREM that the OPREM model is better positioned to eliminate the procyclicality and perverse rush in reserve build-ups experienced in developing and emerging countries by effectively setting the reserve stock against economic trends. The research fixes the optimal reserves around a benchmark of 0.7–1.2 of previous year's optimal value. However, in the absence of past optimal values, a benchmark between 2 and 6 times of average inflows for short-term analysis or analysis with small data observations. However, for long-term analysis or analysis with large data frequency, the reserve stock should be fixed on a benchmark of 2–9 times of the average inflows. (shrink)

Consented autopsy is almost non-existent in the Middle-East where established social and cultural beliefs regarding the procedure might discourage family members from requesting a consented autopsy. Evidence suggests that new information is obtained from consented autopsies. It would not be in the best interest of medicine if social and cultural misconceptions succeed in erasing the existence of consented autopsies entirely.

Applying the resolution–scale relativity principle to develop a mechanics of non-differentiable dynamical paths, we find that, in one dimension, stationary motion corresponds to an Itô process driven by the solutions of a Riccati equation. We verify that the corresponding Fokker–Planck equation is solved for a probability density corresponding to the squared modulus of the solution of the Schrödinger equation for the same problem. Inspired by the treatment of the one-dimensional case, we identify a generalization to time dependent problems in any (...) number of dimensions. The Itô process is then driven by a function which is identified as establishing the link between non-differentiable dynamics and standard quantum mechanics. This is the basis for the scale relativistic interpretation of standard quantum mechanics and, in the case of applications to chaotic systems, it leads us to identify quantum-like states as characterizing the entire system rather than the motion of its individual constituents. BackgroundA different ethos with respect to the perception of medical ethics prevails in societies in transition such as those in the Arabian Peninsula, which makes it difficult to apply international principles of bioethics in medical practice. This study aimed to develop and psychometrically test an instrument that measures physicians’ awareness of bioethics and medical law and their attitudes towards the practice of medical ethics. Additionally, it examined physician correlates influencing the awareness of bioethics.MethodsFollowing a rigorous review of relevant literature by (...) a panel of experts, a 13-item instrument, the Omani physicians’ bioethics and medical law awareness (OBMLA) questionnaire was developed with the aim of assessing physicians’ awareness of bioethics and medical law. The study tool’s construct validity and internal consistency reliability were examined by exploratory factor analysis (EFA) and Cronbach’s alpha. In a cross-sectional study, the questionnaire was distributed among a random sample of 200 physicians at a tertiary hospital in Muscat, Oman. Participant characteristics that may influence awareness of bioethics and medical law were explored.ResultsThe EFA of the OBMLA questionnaire resulted in three well-loading factors: (1) Physicians’ bioethics practice subscale (2) incentive related bioethics subscale and (3) medical law awareness subscale. Internal consistency reliability ranged between Cronbach’s α: 0.73–0.8. Of the total 200 participants, 52% reported that teaching medical ethics during medical school was inadequate. The overall mean (standard deviation, SD) of the bioethics awareness score and Omani medical law awareness were 27.6 (3.5) and 10.1 (2.1) respectively. The majority of physicians (73%) reported that they frequently encountered ethical dilemmas in their practice and 24.5% endorsed the view that unethical decisions tended to occur in their practice.ConclusionThe study provides an insight into the practice of bioethics, and the awareness of bioethics and medical law among physicians in a teaching hospital in Oman. The OBMLA questionnaire appears to be a valid and reliable tool to assess a physician’s awareness of bioethics and medical law. In this preliminary study, it appears that participants have suboptimal scores on the indices which measure practice and awareness of bioethics and medical law. (shrink)

Normativity is assessed as we evaluate and compare the environmental reporting practices of a sample of French and Canadian companies through the lens of institutional legitimacy. More specifically, we examine how French and Canadian firms changed their reporting practices in reaction to the promulgation of laws and regulations in their respective countries, i.e., the NER and Grenelle II Acts in France, and National Instrument 51-102 and CSA Staff Notice NR 51-333, issued by the Canadian Securities Administrators. The firms’ voluntary disclosures (...) according to GRI guidelines are also investigated. Substantive legitimacy theory is used to explore the level of substantive disclosures provided by Canadian and French firms. The findings reveal that the French parliamentary regime is more successful than the Canadian stock exchange regulation in triggering environmental reporting, and that the GRI combined with local regimes prompts environmental disclosures. Notwithstanding the improvements in environmental reporting under all three regimes, a very low level of substantive disclosure is noted in both countries. (shrink)

Due to the huge amount of data being generating from different sources, the analyzing and extracting of useful information from these data becomes a very complex task. The difficulty of dealing with big data optimization problems comes from many factors such as the high number of features, and the existing of lost data. The feature selection process becomes an important step in many data mining and machine learning algorithms to reduce the dimensionality of the optimization problems and increase the performance (...) of the classification or clustering algorithms. In this paper, a set of hybrid and efficient genetic algorithms are proposed to solve feature selection problem, when the handled data has a large feature size. The proposed algorithms use a new gene-weighted mechanism that can adaptively classify the features into strong relative features, weak or redundant features, and unstable features during the evolution of the algorithm. Based on this classification, the proposed algorithm gives the strong features high priority and the weak features less priority when generating new candidate solutions. In the same time, the proposed algorithm tries to more concentrate on unstable features that sometimes appear and sometimes disappear from the best solutions of the population. The performance of proposed algorithms is investigated by using different datasets and feature selection algorithms. The results show that our proposed algorithms can outperform the other feature selection algorithms and effectively enhance the classification performance over the tested datasets. (shrink)

Juran has been called the father of quality, a quality guru, and the man who taught quality to the Japanese. One of his stellar contributions is the Juran trilogy of the three quality processes: quality control, quality planning, and quality improvement. This study aimed to identify the social responsibility of higher education institutions in supporting the entrepreneurship sector in the Kingdom of Saudi Arabia, To achieve this goal, the descriptive and analytical method was used, by designing a questionnaire and distributing it to the research sample, which consists of 200 students and workers at Imam Abdul Rahman bin Faisal University. The study found the importance of training and qualification in developing and stimulating entrepreneurship in the Kingdom, The lack of development of (...) educational curricula is considered an obstacle to development.In light of these results, the study recommended the necessity of investing in the education and training sector, qualifying workers and students, in addition to developing educational courses by making use of the experiences of others, especially in developed countries. The ethical climate in Turkey is beset by ethical problems. Bribery, environmental pollution, tax frauds, deceptive advertising, production of unsafe products, and the ethical violations that involved politicians and business professionals are just a few examples. The purpose of this study is to compare and contrast the ethical beliefs of American and Turkish consumers using the Ethical Position Questionnaire (EPQ) of Forsyth (1980), the Machiavellianism scale, and the Consumer Ethical Practices of Muncy and Vitell questionnaire (MVQ). A sample of 376 (...) subjects that consists of American consumers (n = 188) and Turkish consumers (n = 199) was used to compare the ethical beliefs and practices of the two samples. The MANOVA results for the two nationality groups found that five out of six criterion variables differed between the two groups. During the 1990s, biomedical scientists and Muslim religious scholars collaborated to construe Islamic responses for the ethical questions raised by the AIDS pandemic. This is the first of a two-part study examining this collective legal reasoning (ijtihād jamā‘ī). The main thesis is that the role of the biomedical scientists is not limited to presenting scientific information. They engaged in the human rights discourse pertinent to people living with HIV/AIDS, gave an account of the preventive strategy adopted by the World Health (...) Organization, and offered an (Islamic) virtue-based preventive model. Finally, these scientists tried to draft a number of Islamic legal rulings (aḥkām), usually seen in Islamic jurisprudence as the exclusive business of Muslim religious scholars. Background Triage in the emergency department (ED) is a crucial procedure that establishes the urgency of patient care based on symptoms at presentation. Although the efficacy of triage nurse training programs varies, their goal is to increase the precision and efficiency of their decision-making. Aim With a focus on research done between 2019 and 2024, the systematic review attempts to evaluate the effect of training programs on the accuracy and efficiency of triage nurses in emergency departments. Method We conducted a (...) thorough search of the PubMed, EMBASE, and Web of Science databases to find pertinent research. Studies assessing training treatments for triage nurses and reporting accuracy and efficiency-related results were included in the inclusion criteria. Both the quality assessment and data extraction were carried out separately by two reviewers. Result Ten studies that used a range of triage training techniques and covered a variety of geographic areas satisfied the inclusion criteria. Research findings consistently showed that, despite variations in study designs and outcome measures, training programs increased triage accuracy and efficiency. Interventions included clinical decision support system integration, training based on simulation, and conventional classroom-based instruction. Conclusion By increasing decision-making efficiency and accuracy in patient evaluation, training programs greatly improve the performance of triage nurses in emergency departments. To improve comparability and support evidence-based policies, future research must standardize training methodologies and outcome measurements. (shrink)

Background: Health systems globally face ncreasing challenges from health crises and epidemic threats, necessitating the pivotal role of epidemiological teams in emergency response. This study investigates the significance of collective competence within these teams, focusing on their ability to collect, analyze, and manage laboratory data effectively. Methods: A cross-sectional descriptive study design was used to survey epidemiological teams in Saudi Arabia. A structured questionnaire was developed based on existing literature and distributed electronically to participants. Data analysis was performed using the (...) Statistical Package for the Social Sciences (SPSS), using descriptive and inferential statistics to summarize participant characteristics and explore associations between variables. Results: A total of 208 epidemiological teams member completed the questionnaire, yielding insights into their The Role of Collective Competence in Epidemiological Teams. The findings reveal high levels of agreement regarding the effectiveness of management approaches, knowledge sharing, and team collaboration, with the average score exceeding 4.0 on a 5-point scale. Notably, team efficacy was found to significantly enhance the efficiency of laboratory data collection and analysis, accounting for approximately 25.9% of the variance in efficacy. Discussion: These findings underscore the need to invest in developing team competencies within epidemiological teams to improve emergency response capabilities. Hospital administrators play an essential role in the implementation of Antibiotic Stewardship Programs, which is instrumental in the fight against antimicrobial resistance. The foundation and sustenance of such programs has been made possible through leadership; this is through interdisciplinary collaboration that is done with policy formulation and stewardship inclusion into the institutional systems. Indeed, it is immense strength where ID physicians and clinical pharmacists with special training proactively involve themselves in the program. Beyond that, the accreditation standards that incorporate ASPs (...) really enhance compliance and institutional commitment. Obstacles to movement include an absence of resources, unwilling prescriber, and other competing health conditions for priority. To achieve such success, the hospital administrators must ensure adequate staffing, proper utilization of advanced technologies, and instillation of accountability through focused education and participation. Besides the reduction of resistance, ASPs contribute to such improvements in patient outcomes, efficient consumption of resources, and significant reductions in costs. As this narrative review indicates, leadership at the hospital level plays an important role in augmenting antibiotic stewardship. Addressing major barriers, along with strategic implementation, means that administrators may play an important role in further maximizing effectiveness while truly adding to the world-wide effort against antimicrobial resistance. The two significant zoonotic bacterial infections that have remained a concern due to the complex dynamics involved in transmission and global prevalence are anthrax and brucellosis. The present paper attempts to address some of the most important zoonotic pathogens, highlighting their epidemiology, clinical manifestations, diagnosis, treatment, and prevention strategies. Anthrax is largely transmitted through direct contact with the infected animals or their products resulting in cutaneous, inhalational, and gastrointestinal forms, all with specific clinical outcomes and approaches for treatment. Similarly, the (...) genus Brucella primarily transmits brucellosis to humans by occupational exposure and consumption of contaminated animal products. Its presentation may be acute, chronic, or in relapse. Diagnosis is made by a combination of serological, molecular, and culture-based techniques, while the treatment focuses on antimicrobial regimens specific to the pathogen. However, this increases the problem of antimicrobial resistance, which makes their effective management challenging, necessitating an integrated approach. Mitigating risks through biosecurity measures, vaccination, public health education, and a One Health approach that includes human, animal, and environmental health, all facilitate prevention and control of an outbreak. Advanced surveillance and diagnostic technologies further support the prompt interventions to contain outbreaks. It then goes on to highlight the importance of interdisciplinarity for decreasing the global burden of zoonotic bacterial diseases. (shrink)

Aim: The primary objective is to provide a comprehensive overview of healthcare literature, combining international and indigenous perspectives, to uncover commonalities and distinctions in approaches to patient care, treatment modalities, and research methodologies. (...) Method: A systematic and thorough review of 20 selected studies, encompassing international and Saudi Arabian research, was conducted. The studies, published between 2020 and 2024, were analyzed for their key themes, sub-themes, and trends, creating a detailed understanding of the current landscape of healthcare practices. Results: The synthesis revealed recurring themes, including the integration of digital technologies in diabetes care, advancements in medical nutrition therapy for obesity management, dynamic risk assessment in atrial fibrillation, and the potential of genetically modified food for nutritional security in Saudi Arabia. Sub-themes elucidated nuanced aspects of these overarching themes, offering a holistic perspective on contemporary healthcare strategies. Conclusion: International and indigenous research studies collectively underscore the importance of collaborative and context-specific approaches to healthcare. While global insights contribute to a broader understanding, indigenous studies emphasize the necessity of tailoring interventions to local needs. End-of-life organ donation is controversial in Islam. The controversy stems from: scientifically flawed medical criteria of death determination; invasive perimortem procedures for preserving transplantable organs; and incomplete disclosure of information to consenting donors and families. Data from a survey of Muslims residing in Western countries have shown that the interpretation of religious scriptures and advice of faith leaders were major barriers to willingness for organ donation. Transplant advocates have proposed corrective interventions: reinterpreting religious scriptures, reeducating faith leaders, and utilizing media (...) campaigns to overcome religious barriers in Muslim communities. This proposal disregards the intensifying scientific, legal, and ethical controversies in Western societies about the medical criteria of death determination in donors. It would also violate the dignity and inviolability of human life which are pertinent values incorporated in the Islamic moral code. Reinterpreting religious scriptures to serve the utilitarian objectives of a controversial end-of-life practice, perceived to be socially desirable, transgresses the Islamic moral code. Background: Context: Digital health interventions have become essential instruments in contemporary medicine, providing viable means of improving patient outcomes and healthcare provision. The goal of this study is to examine the impact, difficulties, and potential future directions of digital health interventions across a range of healthcare situations by synthesizing results from 12 carefully chosen studies. Aim: This study's objective is to thoroughly examine and summarize the body of research on digital health interventions, with an emphasis on acceptance and utilization as (...) well as implementation and efficacy. Method: Twelve pertinent papers published between 2016 and 2024 were found after a thorough search of electronic databases. Numerous digital health interventions were covered by these research, such as remote monitoring systems, telemedicine, and mobile applications. In order to find common themes, sub-themes, and patterns among the chosen research, thematic analysis was used in the data synthesis process. Results: A number of important themes emerged from the thematic analysis, including interoperability problems, patient engagement, healthcare provider acceptability, regulatory concerns, and usability issues. The sub-themes that fell under these categories emphasized the variables that affect the uptake and efficacy of digital health interventions. Conclusion: The synthesis of data emphasizes how digital health initiatives can increase clinical results, improve patient care, and streamline the delivery of healthcare. But in order to fully reap the benefits, major obstacles like interoperability problems, regulatory difficulties, and usability impediments need to be solved. The recommendations put forth aim to promote innovation and evidence-based practice in digital health by stressing user-centered design, improving digital literacy among medical practitioners, and encouraging interdisciplinary collaboration. This study was designed to examine the determinants of and differences between the ethical beliefs of two groups of Japanese students in religious and secular universities. Multiple regression analysis revealed that students of the Japanese religious university perceived that young, male, relativistic, and opportunistic students tended to behave less ethically than did older, female, and idealistic students. Students of the Japanese secular university perceived that male, achievement-oriented, and opportunistic students tended to behave less ethically than did female and experience-oriented students. (...) Opportunism was found to be one of the most important determinants in explaining misconduct. Multivariate analysis of variance (MANOVA) and multiple discriminant analysis (MDA) revealed that students of the Japanese secular university tended to score higher on achievement and humanism, and lower on theism and positivism than did students of the Japanese religious university. The purpose of this study is to explore the linguistic aspects of police FIR using Michael Halliday’s system of transitivity and three-dimensional model of CDA, encompassing the macro-sociological analysis of social practice. FIR is a genre of discourse, referring to a report prepared by the police, which reaches the police station first after the commission of a cognizable offence. FIR needs thorough and careful reading, and very difficult to understand by the people of a particular community. In this regard, the (...) Transitivity system is a medley of six different processes realizing ideational meaning which is further strengthened by CDA, which also helps the people of a community to comprehend complex discourses. In accordance with this study, the researchers used the qualitative approach as a paradigm. They used close reading technique as a strategy and relying on purposive sampling in the selection of data. Further textual analysis is used as a method of analysis. The researcher took five FIRs which were analyzed with respect to the transitivity system. This study reveals that the FIRs conform to five processes out six processes of the transitivity system and the three-dimensional model of CDA. Moreover, this study can be extended to other police reports and common people may easily expand their awareness to understand the language and context of FIRs. (shrink)

In recent years, more and more mines have been left idle, and the formed mine wasteland has become a visual pollution of the urban landscape, and the study of landscape design of mine parks has become a focal topic. To address this issue, the sample of this study was selected from Xuzhou Anran Mountain Quarry Abandoned Quarry, to enhance the ecological environment and landscape value of the site through the sample's naturalistic style design and performance. The goal of the study (...) is to explore the innovative design and performance of naturalistic style in the landscape of Xuzhou abandoned quarry park. Methods of this study: observation forms, questionnaires and interview forms, relevant cases and other research methods were used to analyse the overview of mine transformation in China, the landscape characteristics, ecological status and problems of Xuzhou mine wasteland. The design strategy of naturalistic style in the landscape of Xuzhou mine park is proposed.The results of the study show that (1) mine rehabilitation. China's big cities have more mine rehabilitation, and the proportion of cases in small cities is relatively small. (2) Characteristic aspects. Abandoned mines in Xuzhou are characterised by serious ecological damage and chaotic and dilapidated site landscapes. (3) Design strategy. It should be carried out from the ecological restoration and management of the site environment, the aesthetic expression of naturalistic landscape, and the use of natural elements and natural materials. In addition, through the in-depth analysis of the sample landscape design, the originally desolate abandoned mine was transformed into a naturalistic style landscape space, adopting ecological and cultural strategies. The results of the study contribute to the ecological and economic development of the region, and also provide theoretical and practical guidance for the rehabilitation of other mines in China. Hepatocellular carcinoma (HCC) presents a significant problem that necessitates a greater understanding of its underlying molecular complexity in order to improve diagnostics and therapies. Recent studies have shed light on the critical role that mitochondrial dysfunction plays in the development and course of HCC. Once thought to be primarily involved in the synthesis of cellular energy, mitochondria are now known to be key participants in the regulation of a variety of cellular functions that go beyond bioenergetics. The purpose of this (...) review is to elucidate the diagnostic and therapeutic implications of mitochondrial dysfunction in HCC by synthesising ideas from conventional and contemporary scientific literature. Within diagnostics, the discovery of mitochondrial biomarkers such as mutations in mitochondrial DNA and abnormalities in respiratory chain complex activities provides a state-of-the-art viewpoint for the timely identification and accurate description of HCC. In addition, non-invasive imaging methods that focus on mitochondrial function offer prospective means of improving clinical diagnostic accuracy. Moving on to treatment, a growing area of study investigates how mitochondria-targeted therapies might be used to treat HCC. Novel substances and repurposed medications with the potential to induce selective apoptosis in cancer cells, reduce oxidative stress, and restore mitochondrial homeostasis are highly promising. In the fight against hepatocellular carcinoma, an understanding of the complex interactions between mitochondrial dysfunction and HCC opens new diagnostic opportunities and paves the way for individualised treatment plans. This paper presents some thoughts focusing on the problems of ethnico-cultural communalism, its meaning in the current global socio-political context and its implications with regard to principles of democracy, citizenship and the nation-state. It gives particular attention to the issues of interculturality and intercultural communication (IC) as central markers in the contemporary socio-political landscape. This is why ethnicocultural groups scattered around the world generally have effective communal media that reinforce their social, cultural and political cohesion at local, national and global levels. Because, as well as expressing their positions and views on the issues facing a nation's society, this formal discourse provides group members with a legitimate and coherent framework for action and argument. When Muslims thought of establishing milk banks, religious reservations were raised. These reservations were based on the concept that women's milk creates ‘milk kinship’ believed to impede marriage in Islamic Law. This type of kinship is, however, a distinctive phenomenon of Arab tradition and relatively unknown in Western cultures. This article is a pioneer study which fathoms out the contemporary discussions of Muslim scholars on this issue. The main focus here is a religious guideline (fatwa) issued in 1983, referred to (...) in this article as ‘one text’, by the Egyptian scholar Yūsuf al-Qaradāwī who saw no religious problem in establishing or using these banks. After a number of introductory remarks on the ‘Western’ phenomenon of milk banks and the ‘Islamic’ phenomenon of ‘milk kinship’, this article analyses the fatwa of al-Qaradāwī‘one text’ and investigates the ‘two contexts’ in which this fatwa was discussed, namely, the context of the Muslim world and that of Muslim minorities living in the West. The first context led to rejecting the fatwa and refusing to introduce the milk banking system in the Muslim world. The second context led to accepting this system and thus allowing Muslims living in the West to donate and receive milk from these banks. Besides its relevance to specialists in the fields of Islamic studies, anthropology and medical ethics, this article will also be helpful to physicians and nurses who deal with patients of Islamic background. Background: Pembrolizumab, an inhibitory anti-PD-1 antibody, has led to significantly meaningful improvements in advanced melanoma. This meta-analysis discusses and presents the efficacy and safety of pembrolizumab versus control treatments in randomized controlled trials. Methods: Electronic databases were searched systematically for RCTs using pembrolizumab in advanced melanomas. Pooled estimates were calculated under random-effect models for overall response rates, PFS, OS, and TRAEs. Results: Nine RCTs including 5132 patients were analyzed. Pembrolizumab significantly realized better results in ORR, PFS and OS HR 0.62, (...) 95% CI 0.54, 0.71, P < 0.001 and HR 0.70, 95% CI 0.63, 0.78, P < 0.001 for control. PFS and OS were consistently better in first, second, and further lines and in groups stratified for PD-L1 status. Grade 3–5 TRAEs were lower with pembrolizumab than with chemotherapy 13.3% vs. 25.9% and ipilimumab 16.9% vs. 27.3%. Conclusions: Pembrolizumab monotherapy meaningfully improves ORRs, delays progression, and prolongs survival in advanced melanoma compared to current standard regimens, with the latter given its favorable toxicity profile. As such, pembrolizumab should be considered the new standard of care for treatment-naïve and previously treated subjects. (shrink)
