Explanatory pluralism is the view that the best form and level of explanation depends on the kind of question one seeks to answer by the explanation, and that in order to answer all questions in the best way possible, we need more than one form and level of explanation. In the first part of this article, we argue that explanatory pluralism holds for the medical sciences, at least in theory. However, in the second part of the article we (...) show that medical research and practice is actually not fully and truly explanatory pluralist yet. Although the literature demonstrates a slowly growing interest in non-reductive explanations in medicine, the dominant approach in medicine is still methodologically reductionist. This implies that non-reductive explanations often do not get the attention they deserve. We argue that the field of medicine could benefit greatly by reconsidering its reductive tendencies and becoming fully and truly explanatory pluralist. Nonetheless, trying to achieve the right balance in the search for and application of reductive and non-reductive explanations will in any case be a difficult exercise. (shrink)

Yoga is not just a physical exercise but a holistic medical science; it is a philosophy of life, a spiritual knowledge. It is a profound philosophical thought process, but it is (...) also about having a simple, easy and balanced life-style. It is the path to gain eternal wisdom, ultimate truth and to unite the inner soul with the supreme soul through self-realization. It is a tradition in which sages have attained immeasurable bliss, indescribable happiness and inexplicable peace by entering the supreme consciousness which is present beyond mind. It is the science that inhibits the agitations of the mind and takes it to its highest level. Yoga is the spiritual journey from ignorance to knowledge, mortality to immortality, obvious to hidden and peace to ultimate tranquillity. It is the inner journey from thoughtfulness to emptiness, subjective to objective concentration, determinate to indeterminate samadhi and extrovertedness to introvertedness and being firm in judgement. This book is an humble attempt in this direction. We are determined to obtain more scientific evidence and build a science perspective not only for Yoga and Ayurveda but for the entire Vedic knowledge and wisdom. We shall lead the world in finding solutions for complex problems besetting humanity through the fusion of science and spiritualism. This book is the outcome of meticulous work put in by us in order to present various scientific standards used to test and validate this ancient system of Yoga, demographic study and personal experiences of several hundred people along with sufficient supporting proofs and documents. This is our first attempt to present Yoga backed by currently available scientific evidence, the goal is far more ambitious and the task of scientific documentation and research is an ongoing process. Although the advantages of flight at high altitude were early recognized, so also were the physiological problems standing in the way of its realization. The idea of surmounting such problems by means of a pressurized cabin was advocated as early as 1909, while the first attempt to translate the concept into actuality occurred in 1921. Neither it nor several successive attempts enjoyed any real success until a project launched by the U. S. Air Corps in 1935 produced a breakthrough aircraft (...) designated the XC-35. The major reason for the favorable termination of that venture was the thoroughness of the engineering involved. But it is equally notable that this was the first instance in the age of powered flight where there was an active collaboration between the scientists and engineers, a rather curious circumstance in view of the fact that the achievement of altitude record-setting balloon flights in the nineteenth century had owed a great deal to an interconnection of aeronauts and scientists' laboratories. This paper focuses on the physiological investigations which informed the XC-35 engineers while at the same time bringing into being a new aeromedical laboratory taking the first small step toward turning aeromedicine into space medicine. Scott Waterman's reflection on his experience with chronic pain and alternative treatments raises a fundamental question in medical epistemology: How can we know that an intervention will help people who are suffering?Waterman's details his trial of an alternative therapy with a dubious pathophysiological rationale. Despite the lack of research demonstrating its efficacy, and a lack of therapeutic benefit for him in particular, he acknowledges its benefit to others who were more attitudinally predisposed to it. RESUMEN La formación de profesionales competentes y comprometidos con el ideal de justicia social y solidaridad humana es un reto para las universidades médicas en Cuba. El protagonismo estudiantil en este contexto contribuye a formar jóvenes autodeterminados, críticos, reflexivos, que se hagan cargo de su desarrollo profesional y participen de forma creadora en la transformación de la sociedad. Se realizó una investigación cualitativa en la Universidad de Ciencias Médicas de Camagüey, con el objetivo de valorar la visión que tienen los (...) líderes estudiantiles sobre el protagonismo estudiantil, los factores que lo limitan y acciones para perfeccionarlo. Se utilizaron las técnicas entrevista estructurada, entrevista grupal y observación participante a una muestra de once dirigentes estudiantiles. Se logró identificar que los dirigentes estudiantiles tienen una visión acertada sobre el protagonismo estudiantil y señalan factores que limitan el protagonismo, asociados a los propios estudiantes, los profesores, las autoridades universitarias y la institución. ABSTARCT The formation of competent and committed professionals with the ideal of social justice and human solidarity is a challenge for the medical universities in Cuba. The student-leading role in this context helps to form self-determined, critical, reflective young people, who take charge of his professional development and take part of creative form in the transformation of the society. A qualitative investigation was carried out in the University of Medical Sciences, with the objective to value the vision that the student leaders have on the student-leading role, the factors that limit it and actions to perfect it. There used the techniques structured interview, group interview and taking part observation to a sample of eleven student leaders. It was identified that the student leaders have an appropriate vision on the student-leading role and indicated factors that limit the leading role, associated with the proper students, the teachers, the university authorities and the institution. This essay analyzes the medical study of “monstrous birth” as a site of race-making in the nineteenth-century United States. It argues that the medical theorization of monstrosity was structured by multiple logics of race, which both shaped and emerged from medical authorities’ efforts to classify and interpret anomalous newborn bodies. Materialized at the intersection of these logics, the biological monster theorized a racial order that was hierarchical, temporalized, and vulnerable to the dangers of women’s reproduction. In this (...) context, monstrosity became a way to advance claims about the nature of racial hierarchy, articulate the threat and mechanism of racial degeneration, and negotiate the contradictions of shifting racial imaginaries across the nineteenth century. Exploring the medical engagement with monstrosity thus sheds light on entanglements of medical science and race-making in U.S. history, showing how practitioners produced and elaborated unstable concepts of scientific race, and revealing how race was linked to reproduction in the emergence of modern biomedical discourse. There is a fairly closed circle between culture, language, meaning, and truth such that the world of a given culture is a world understood in terms of the meanings produced in that culture. Medicine is, in fact, a subculture of a powerful type and has its own language and understanding of the range of illnesses that affect human beings. So how does medicine get at the truth of people and their ills in such a way as to escape its own (...) limited constructions? There is a way out of the closed circle implicit in the idea of a praxis and the engagement with reality that is central to it and the further possibility introduced by Jacques Lacan that signification is never comprehensive in relation to the subject's encounter with the real. I will explore both of these so as to develop a conception of truth that is apt for the knowledge that arises in the clinic. (shrink)

Se diseñó una estrategia educativa antitabáquica en la Residencia Estudiantil de Ciencias Médicas en Camagüey que fue valorada por criterio de especialistas y actualmente se encuentra en fase de implementación. En el artículo se evalúa su impacto. La misma se caracteriza por ser sistémica, dinámica, flexible, factible, participativa y dinamizadora de la esfera afectiva. Se ha empleado en la impartición de posgrado, capacitaciones a profesores guías e instructoras educativas; ha contribuido a incrementar los conocimientos en el orden teórico, metodológico, educativo (...) y salubrista en relación con la educación antitabáquica en estudiantes, profesores e instructoras educativas y ha favorecido el fomento de estilos de vida saludables. An anti-smoking educational strategy in Medical Sciences Dormitory in Camaguey was designed and assessed based on specialists' criteria and it is currently under the implementation phase. Its impact is assessed in the article. It is characterized by being systemic, dynamic, feasible, participative and emotional sphere stimulating. It has been used in the teaching of postgraduate courses and in guiding teachers and educational instructors' training; it has contributed to increasing students, professors and educational instructors' theoretical, methodological, and educational and health knowledge regarding anti-smoking education, therefore favoring healthy lifestyles promotion. Are there parallels between the "moment of insight" in science and the emergence of the "unknowable" in religious faith? Where does scientific insight come from? Award-winning biologist Robert Pollack argues that an alliance between religious faith and science is not necessarily an argument in favor of irrationality: the two can inform each other's visions of the world. Pollack begins by reflecting on the large questions of meaning and purpose--and the difficulty of finding either in the orderly world described by the (...) data of science. He considers the obligation to find meaning and purpose despite natural selection's claim to be a complete explanation of our presence as a species--a claim that calls upon neither natural intention, nor design, nor Designer. Next, the book focuses on matters of free will, from the choice of a scientist to accept evidence, to the choice of a religious person to accept a revelation, to a patient's loss of free will in medical treatment. Here Pollack addresses questions of ethics and offers a provocative comparison of two difficult texts whose contents remain incompletely understood: the DNA "text" of the human genome and the Hebrew record of Jewish written and oral law. In closing, Pollack considers the promise of genetic medicine in enabling us to glimpse our own future and offers a reconsideration of the possible utility of the so-called placebo effect in curing illness. Whether refuting a DNA-based biological model of Judaism or discussing the Darwinian concept of the species, Pollack, under the banner of free inquiry, presents a genuine, vital, and well-argued assay of the intersection of science and religion. Roughly 40% of the sizeable medical research and literature on recently approved drugs is “ghost managed” by the pharmaceutical industry and its agents. Research is performed and articles are written by companies and their agents, though apparently independent academics serve as authors on the publications. Similarly, the industry hires academic scientists, termed key opinion leaders, to serve as its speakers and to deliver its continuing medical education courses. In the ghost management of knowledge, and its dissemination through key (...) opinion leaders, we see the pharmaceutical industry attempting to hide or disguise the interests behind its research and education. Background and Aim: Today universities admit International Students as well as national students. Tehran University of Medical Sciences has been also started admitting International Students in regards of its Internationalization aims. Student’s satisfaction is of high importance in order to gain the given goals. The purpose of this study was to evaluate the satisfaction of International students of TUMS. -/- Materials and Methods: This was a descriptive study. The target group was international students of TUMS, the participants were (...) selected through availability sampling. The research instrument was researcher-made questionnaire which the reliability calculated as 0.97 by Chronbach’s alpha. The data was analysied by Microsoft Excell version 2010. -/- Results: The overall satisfaction of the students was 70% (satisfied and completely satisfied) while the overall dissatisfaction was 10% (dissatisfied and completely dissatisfied). -/- Conclusion: Given satisfaction of more than half of the International students, TUMS has performed reasonably. Based on the results obtained in this research the university may pay attention to the fields reported as dissatisfied. (shrink)

Se realizó una revisión bibliográfica con el objetivo de sistematizar los principales postulados existentes respecto a la Historia Regional como disciplina y su relación con la asignatura Historia de Cuba. La información aportada se procesó según los métodos científicos de análisis y síntesis e histórico lógico. Ello incluyó la interpretación de los criterios vertidos por especialistas que permiten considerar a Camagüey como una región histórica, en estrecho vínculo con las condiciones en las que se imparte la Historia de Cuba en (...) la Universidad de Ciencias Médicas de Camagüey, en la que la escasez de bibliografía sobre este tópico en particular influye sobre el estudio de la asignatura. Ello recalca la necesidad de desarrollar investigaciones de este tipo, con el propósito de que dichos estudios puedan ser empleados como materiales de apoyo a la docencia. A literature review was made in order to systematize the existing main postulates regarding Regional History as a discipline and its relationship with the subject History of Cuba. The information provided was processed according to scientific methods of analysis. synthesis and historico-logical. This included the interpretation of the criteria stated by specialists that allow to consider Camag|ey as a historical region, in close liaison with the conditions in which the history of Cuba is taught at the University of Medical Sciences in Camaguey, where the lack of literature on this topic particularly influences on the study of the subject. This underscores the need to develop research of this type, so these studies can be used as support for teaching materials. (shrink)

The question what scientific progress means for a particular domain such as medicine seems importantly different from the question what scientific progress is in general. While the latter question received ample treatment in the philosophical literature, the former question is hardly discussed. I argue that it is nonetheless important to think about this question in view of the methodological choices we make. I raise specific questions that should be tackled regarding scientific progress in the medical sciences and demonstrate (...) their importance by means of an analysis of what evidence-based medicine (EBM) has, and has not, to offer in terms of progress. I show how critically thinking about EBM from the point of view of progress can help us in putting EBM and its favoured methodologies in the right perspective. My conclusion will be that blindly favouring certain methods because of their immediately tangible short-term benefits implies that we parry the important question of how best to advance progress in the long run. This leads us to losing sight of our general goals in doing research in the medical sciences. (shrink)

This paper takes a phenomenological hermeneutic orientation to explicate and explore the notion of the grey zone of health and illness and seeks to develop the concept through an examination of the case of alcohol consumption. The grey zone is an interpretive area referring to the irremediable zone of ambiguity that haunts even the most apparently resolute discourse. This idea points to an ontological indeterminacy, in the face of which decisions have to be made with regard to the health of (...) a person (e.g., an alcoholic), a system (e.g., the health system), or a society. The fundamental character of this notion will be developed in relation to the discourse on health and the limitations of different disciplinary practices. The case of alcohol consumption will be used to tease out the grey zone embedded in the different kinds of knowledge made available through the disciplinary traditions of medical science, with its emphasis on somatic well-being, and anthropology, with its focus on communal well-being. This tension or grey zone embedded in different knowledge outcomes will be shown to have a discursive parallel with the dialogue between the Athenian, the Spartan, and the Cretan in Plato’s Laws. Making use of the dialogical approach as described by Gadamer, the Athenian’s particular resolution of the tension will be explored as a case study to demonstrate the necessarily particular analysis involved in a grey zone resolution. The Concussion in Sport Group guidelines have successfully brought the attention of brain injuries to the global medical and sport research communities, and has significantly impacted brain injury-related practices and rules of international sport. Despite being the global repository of state-of-the-art science, diagnostic tools and guides to clinical practice, the ensuing consensus statements remain the object of ethical and sociocultural criticism. The purpose of this paper is to bring to bear a broad range of multidisciplinary challenges to the processes (...) and products of sport-related concussion movement. We identify lacunae in scientific research and clinical guidance in relation to age, disability, gender and race. We also identify, through multidisciplinary and interdisciplinary analysis, a range of ethical problems resulting from conflicts of interest, processes of attributing expertise in sport-related concussion, unjustifiably narrow methodological control and insufficient athlete engagement in research and policy development. We argue that the sport and exercise medicine community need to augment the existing research and practice foci to understand these problems more holistically and, in turn, provide guidance and recommendations that help sport clinicians better care for brain-injured athletes. Background and Aim: Global trends and national policies have made internationalization and paying attention to the international markets of higher education inevitable on the one hand and becoming a legal requirement of Iranian medical sciences universities on the other hand. Therefore, the main goal of this article was to show, by examining the experience of international marketing of higher education in Tehran University of Medical Sciences, what are the futures of international marketing of higher education in (...) medical sciences? Materials and methods: in this documented data and the views, knowledge and lived experiences of the beneficiaries of the University of Medical Sciences were the basis of the study, and in the following, the methods of documentary studies, trend analysis, brainstorming, expert panel, theme analysis and scenario planning were used. Results: The most important achievement of this article is that among the challenges facing the international marketing of higher education in the University of Medical Sciences, two driving forces have the highest importance and uncertainty in shaping the future of this issue, including: sustainable security and cultural development. Conclusion: The normalization of political and economic relations with the world along with appropriate cultural development inside and outside the university and acceptance of cultural, racial, religious, intellectual, ethnic and gender differences strengthens the marketing process of higher education. In such a situation, the risk of investing in providing international higher education will be reduced. Therefore, the most expensive and the most important uncertainty facing the country's universities in international marketing is the attitude and approach of science and technology governance to the relations and structure governing the international system. This paper examines the evolution of ideas about narcotic addiction. In the eighteenth and early nineteenth centuries, addiction was not viewed as a medical condition, but as a ‘bad habit’. The contemporary reaction to De Quincey's Confessions demonstrates the general lack of medical involvement. The question of opium eating and longevity, first generated by the Mar case, brought increased medical interest and an embryo connection with the anti-opium crusade. In the second half of the century, addiction was (...) more fully ‘medicalised’ through the profession's involvement in the question of hypodermic morphine and the allied development of a disease theory of addiction. The ‘problem’ of hypodermic morphine owed much to medical perceptions; and the disease theory, too, was not scientifically autonomous. Medical thinking developed in close alliance with moral biases, temperance and anti-opium beliefs. Treatment structures and methods were elaborated in line with increased medical interest. In the early twentieth century there was a distinct move on the part of the government, and the Home Office in particular, to defeat the ‘disease’ view, and to substitute a penal policy. The 1926 Rolleston Report marked not simply a reassertion of the earlier moral-cum-medical emphasis, but a recognition of partnership between doctors and the state in which the limited nature of narcotic addiction at first ensured a liberal reaction. RESUMEN La educación médica superior tiene la responsabilidad social de formar educandos autónomos, independientes y autorregulados. Por ello las estrategias de aprendizaje permiten la adquisición de información y el establecimiento de relaciones a partir del conocimiento previo; son procesos de toma de decisiones, consciente e intencional. Objetivo: Exponer las necesidades del desarrollo de estrategias de aprendizaje en los estudiantes de primer año de Estomatología. Método: Se realizó una investigación descriptiva transversal. El universo estuvo constituido por los estudiantes de 1er año (...) de la carrera de Estomatología, del que fueron seleccionados 120 educandos que conformaron la muestra y a los que se les aplicó la Escala de Estrategias de Aprendizaje ACRA-Abreviada para alumnos universitarios. Los datos fueron procesados utilizando la estadística descriptiva. Resultados: Las estrategias que más utilizan son las que los llevan a un aprendizaje memorístico y reproductivo, las estrategias de apoyo al aprendizaje son utilizadas por el 47.79% de los educandos y existen inadecuados hábitos de estudios. Discusión: El estudio permitió corroborar que las insuficiencias que presentan los estudiantes en cuanto al uso de estrategias de aprendizaje son similares a las descritas por los autores consultados. ABSTRACT The high medical education has the social responsibility of forming autonomous, independent and auto-regulated pupils. For it the learning strategies allow the acquisition of information and the relations establishment from the previous knowledge; there are processes of decision making, conscious and intentional. Objective: To exhibit the needs for the development of strategies of learning in the students of the first year of Stomatology. Method: A transverse descriptive investigation was carried out. The universe was constituted by the students of 1st year of the career of Dentistry, of which there were selected 120 pupils who shaped the sample and to whom there was applied the Scale of Strategies of Learning ACRA-Abbreviated for university pupils. The information was processed using the descriptive statistics. Results: The strategies that they use more are those which take them to memory and reproductive learning, the support strategies to learning are used by 47.79 % of the pupils and inadequate studies habits exist. Discussion: The study allowed to corroborate that the insufficiencies that the students present as for the use of strategies of learning are similar to the described ones by the consulted authors. Background: Systematized Nomenclature of Medicine—Clinical Terms (SNOMED CT, hereafter abbreviated SCT) is acomprehensive medical terminology used for standardizing the storage, retrieval, and exchange of electronic healthdata. Some efforts have been made to capture the contents of SCT as Web Ontology Language (OWL), but theseefforts have been hampered by the size and complexity of SCT. Method: Our proposal here is to develop an upper-level ontology and to use it as the basis for defining the termsin SCT in a way that (...) will support quality assurance of SCT, for example, by allowing consistency checks ofdefinitions and the identification and elimination of redundancies in the SCT vocabulary. Our proposed upper-levelSCT ontology (SCTO) is based on the Ontology for General Medical Science (OGMS). Results: The SCTO is implemented in OWL 2, to support automatic inference and consistency checking. Theapproach will allow integration of SCT data with data annotated using Open Biomedical Ontologies (OBO) Foundryontologies, since the use of OGMS will ensure consistency with the Basic Formal Ontology, which is the top-levelontology of the OBO Foundry. Currently, the SCTO contains 304 classes, 28 properties, 2400 axioms, and 1555annotations. It is publicly available through the bioportal athttp://bioportal.bioontology.org/ontologies/SCTO/. Conclusion: The resulting ontology can enhance the semantics of clinical decision support systems and semanticinteroperability among distributed electronic health records. In addition, the populated ontology can be used forthe automation of mobile health applications. (shrink)

Throughout the 20th century, philosophers have criticized the scientific understanding of the human body. Instead of presenting the body as a meaningful unity or Gestalt, it is regarded as a complex mechanism and described in quasi-mechanistic terms. In a phenomenological approach, a more intimate experience of the body is presented. This approach, however, is questioned by Jacques Lacan. According to Lacan, three basic possibilities of experiencing the body are to be distinguished: the symbolical (or scientific) body, the imaginary (or ideal) (...) body and the real body. Whereas the symbolical body is increasingly objectified (and even digitalized) by medical science, the phenomenological perception amounts to an idealization of the body. The real body cannot be perceived immediately. Rather, it emerges in the folds and margins of our efforts to symbolize or idealize the body, which are bound to remain incomplete and fragile. In the first part of the article (1-3), Lacan's conceptual distinction between the symbolical, the imaginary and the real body will be explained. In the second part (4-5), this distinction will be further clarified by relying on crucial chapters in the history of anatomy (notably Mundinus, Vesalius, Da Vinci and Descartes). (shrink)

The story reveals other stories underneath and over the top of the others. Stories counter stories, revealing who is in the know and who does the telling. To “tell the old, old story,” as the song goes, is not as simple as it may seem. Religion and medical science are caught in (...) the politics and cultural wars about abortion. (shrink)

Imagination and idealism are particularly important creative epistemic virtues for the medical sciences if we hope to improve the health of the world’s ageing population. To date, imagination and idealism within the medical sciences have been dominated by a paradigm of disease control, a paradigm which has realised significant, but also limited, success. Disease control proved particularly successful in mitigating the early-life mortality risks from infectious diseases, but it has proved less successful when applied to the (...) chronic diseases of late life (like cancer). The time is ripe for the emergence and prominence of a supplementary medical research paradigm, the paradigm of ‘healthy ageing’ which prioritises the goal of rate (of ageing) control rather than disease control. This is the difference between extending the human healthspan versus extending survival by managing (or trying to eliminate) the multi-morbidities, frailty and disability currently prevalent in late life. The idealism of the disease control paradigm is myopic because it ignores the health constraints imposed by the inborn ageing process itself, a biological reality which is already inflicting significant economic and disease burdens on the world’s ageing populations. Unless the medical sciences retard the rate of biological ageing, these problems will continue to be amplified as larger numbers of persons survive into late life. (shrink)
