Considering a growing nurse shortage and the need for qualified nurses to handle increasingly complex patient care situations, how ethical beliefs are influenced and the consequences that can occur when moral conflicts of right and wrong arise need to be explored. The aim of this study was to explore influencers identified by nurses as having the most impact on the development of their ethical beliefs and whether these influencers might impact levels of moral distress and the potential for conscientious objection. Nurses whose ethical beliefs were most influenced by their religious beliefs scored higher in levels of moral distress and demonstrated greater differences in areas of conscientious objection than did nurses who developed their ethical beliefs from influencers such as family values, life and work experience, political views or the professional code of ethics. Over the last decade, empirical research on compassion has burgeoned in the biomedical, clinical, translational, and foundational sciences. Increasingly sophisticated understandings and measures of compassion continue to emerge from the abundance of multi- and cross-disciplinary studies. Naturally, the diversity of research methods and theoretical frameworks employed presents a significant challenge to consensus and synthesis of this knowledge. To bring the empirical findings of separate and sometimes siloed disciplines into conversation with one another requires an examination of their disparate assumptions about (...) what compassion is and how it can be known. Here, we present an integrated theoretical review of methodologies used in the empirical study of compassion. Our goal is to highlight the distinguishing features of each of these ways of knowing compassion, as well as the strengths and limitations of applying them to specific research questions. We hope this will provide useful tools for selecting methods that are tailored to explicit objectives (methods matching), taking advantage of methodological complementarity across disciplines (methods-mixing), and incorporating the empirical study of compassion into fields in which it may be missing. Only 27 percent of Americans in a 1995 Harris poll said they had read or heard “quite a lot” about genetic tests. Nonetheless, 68 percent said they would be either “very likely” or “somewhat likely” to undergo genetic testing even for diseases “for which there is presently no cure or treatment.” Perhaps most astonishing, 56 percent found it either “very” or “somewhat acceptable” to develop a government computerized DNA bank with samples taken from all newborns, and their names attached to (...) the samples. This does not necessarily mean the public is unconcerned about genetic privacy. More likely it means that the public is still uninformed about the risks associated with genetic testing, and has not thought at all about the risks involved in storing identifiable DNA samples.A central question presented by genetic screening and testing is whether the genetic information so obtained is different in kind from other medical information, and, if so, whether this means that it should receive special legal protection. Advances in genomics have led to calls for developing population-based preventive genomic sequencing programs with the goal of identifying genetic health risks in adults without known risk factors. One critical issue for minimizing the harms and maximizing the benefits of PGS is determining the kind and degree of control individuals should have over the generation, use, and handling of their genomic information. In this article we examine whether PGS programs should offer individuals the opportunity to selectively opt out of the (...) sequencing or analysis of specific genomic conditions or whether PGS should be implemented using an all-or-nothing panel approach. We conclude that any responsible scale-up of PGS will require a menu approach that may seem impractical to some, but that draws its justification from a rich mix of normative, legal, and practical considerations. Farm to School (FTS) programs are increasingly popular as methods to teach students about food, nutrition, and agriculture by connecting students with the sources of the food that they eat. They may also provide opportunity for farmers seeking to diversify market channels. Food service buyers in FTS programs often choose to procure food for school meals directly from farmers. The distribution practices required for such direct procurement often bring significant transaction costs for both school food service professionals and farmers. Analysis (...) of data from a survey of Vermont farmers who sell directly to school food services explores farmers’ motivations and distribution practices in these partnerships. A two-step cluster analysis procedure characterizes farmers’ motivations along a continuum between market-based and socially embedded values. Further bivariate analysis shows that farmers who are motivated most by market-based values are significantly associated with distribution practices that facilitate sales to school food services. Implications for technical assistance to facilitate these sales are discussed. (shrink)

Similar to parasites, cancer cells depend on their hosts for sustenance, proliferation and reproduction, exploiting the hosts for energy and resources, and thereby impairing their health and fitness. Because of this lifestyle similarity, it is predicted that cancer cells could, like numerous parasitic organisms, evolve the capacity to manipulate the phenotype of their hosts to increase their own fitness. We claim that the extent of this phenomenon and its therapeutic implications are, however, underappreciated. Here, we review and discuss what can (...) be regarded as cases of host manipulation in the context of cancer development and progression. We elaborate on how acknowledging the applicability of these principles can offer novel therapeutic and preventive strategies. We describe the informed consent procedures in a research clinic in Santiago, Chile, and a qualitative study that evaluated these procedures. The recruitment process involves information, counseling and screening of volunteers, and three or four visits to the clinic. The study explored the decision‐making process of women participating in contraceptive trials through 36 interviews. Women understood the research as experimentation or progress. The decision to participate was facilitated by the information provided; time to consider it and to discuss it with (...) partners or relatives; and perceived benefits such as quality of care, non‐cost provision of methods and medical care. For some women, participation was an opportunity to express altruism. The main obstacles for participation were perceived side effects or risks. The final risk‐benefit balance was strongly influenced by women’s needs. Women perceived that the consent form benefited the clinic, proving that participants had made a free decision, and benefited the volunteers by warranting their right to free medical care. The most important problem detected was occasional misunderstanding of the information given on the form. We concluded that a full decision‐making process enhances women’s ability to exercise their right to choose, and assures research institutions that trials are conducted without coercion and that the participants are committed to the study. As a result of the coronavirus (COVID‐19) pandemic, health professionals are faced with situations they have not previously encountered and are being forced to make difficult ethical decisions. As the first group to experience challenges of caring for patients with coronavirus, Chinese nurses endure heartbreak and face stressful moral dilemmas. In this opinion piece, we examine three related critical questions: Whether society has the right to require health professionals to risk their lives caring for patients; whether health professionals have the (...) right to refuse to care for patients during the coronavirus pandemic; and what obligations there are to protect health professionals? Value of care, community expectations, legal obligations, professional and codes of practice may compel health professionals to put themselves at risks in emergency situations. The bioethical principles of autonomy, justice, beneficence and non‐maleficence, as well as public health ethics, guide nurses to justify their decisions as to whether they are entitled to refuse to treat COVID‐19 patients during the pandemic. We hope that the open discussion would support the international society in addressing similar ethical challenges in their respective situations during this public health crisis. (shrink)

Informed consent is recognized as a primary ethical requirement to conduct research involving humans. In the investigations with the use of human biological material, informed consent (IC) assumes a differentiated condition on account of the many future possibilities. This work presents suitable alternatives for IC regarding the storage and use of human biological material in research, according to new Brazilian regulations. Both norms – Resolution 441/11 of the National Health Council, approved on 12 May 2011, and Ordinance 2.201 (NATIONAL GUIDELINES (...) FOR BIOREPOSITORIES AND BIOBANKS OF HUMAN BIOLOGICAL MATERIAL FOR RESEARCH PURPOSE) of the Brazil Ministry of Health, approved on 14 September 2011 – state that the consent of subjects for the collection, storage and use of samples stored in Biobanks is necessarily established by means of a Free and Informed Consent Form (ICF). In order to obtain individual and formal statements, this form should contain the following two mutually exclusive options: an explanation about the use of the stored material in each research study, and the need for new consent or the waiver thereof when the material is used for a new study. On the other hand, ICF suitable for Biorepositories must be exclusive and related to specific research. Although Brazilian and international regulations identify the main aspects to be included in the IC, efforts are still necessary to improve the consent process, so that the document will become a bond of trust between subject and researcher. Clinical ethics committees, with their typical threefold function of education, policy formation, and consultation, are present in nearly all U.S. hospitals today, and they are increasingly common in other healthcare settings such as long-term care and even home care. Ethics committees are at least as prevalent in Canadian hospitals as they are in U.S. hospitals, and their presence is growing in Europe, much of Asia, and Central and South America. Although ethics committees serve a variety of needs, their ultimate goal (...) ought to be to promote ethical practices or, in other words, to engender the integration of ethics into the life of the medical center. Of the three primary functions of ethics committees, ethics consultation has historically been the most controversial and problematic, and consult services in many healthcare institutions have struggled to thrive. Gideon Rosen (2014) has drawn our attention to cases of duress of a particularly interesting sort: the person's ‘mind is not flooded with pain or fear’, she knows exactly what she is doing, and she makes a clear-headed choice to act in, as Rosen says, ‘awful ways’. The explanation of why we excuse such actions cannot be that the action was not voluntary. In addition, although some duress cases could also be viewed as necessity cases and thus as justified, Rosen (...) wisely sets aside that complicating factor by zeroing in on those cases where the action clearly is not justified. So why do we excuse in these cases, where the action is not justified and the agent acted voluntarily?Rosen thinks the key lies in the ‘ill will’ condition, ‘the idea that an act is blameworthy only if it manifests insufficient concern or regard for those affected’. He says this is relatively uncontroversial; much of my paper is taken up with calling the ‘ill will’ condition into question. I also take issue with Rosen on just how justifications and excuses differ. I argue in favour of understanding justifications (in a context where we are asking how justifications and excuses differ) as not requiring truth, but only reasonable belief. (shrink)

O presente artigo traz reflexões que envolvem infância e experiência a partir da área da educação e, particularmente, das ponderações em torno das pesquisas com crianças pequenas e da educação infantil. Os apontamentos impõem limites relativos a este lugar de estudo e delimita estes como parte de uma crítica e autocrítica da educação e das possibilidades de consolidação de uma Pedagogia da Infância. Nesta direção, no artigo se toma para esta reflexão a análise do sentido da experiência para as principais (...) teorias educacionais ou projetos educativos para a educação na infância, a fim de levar a uma demarcação dos contornos da experiência educativa na/da infância institucionalizada. Para tal, apresenta alguns elementos relativos à experiência educativa como experiência social a partir de estudos empíricos que dão visibilidade às ações das crianças como parte de suas vidas nos coletivos infantis. Por fim, indica como desafios para a possível consolidação de uma Pedagogia da Infância uma aproximação da Pedagogia às manifestações infantis, a criança em situação e as possibilidades de os processos educativos se pautarem na escuta, na dialogicidade e nas relações, possibilitando incluir, nas práticas educativas, as significações produzidas pelas próprias crianças em seus contextos de vida e suas manifestações culturais e expressivas. Aim: Explore nurses’ values and perceptions regarding the practice of medical aid in dying. Background: Medical aid in dying is becoming increasing legal in the United States. The laws and American Nurses Association documents limit nursing involvement in this practice. Nurses’ values regarding this controversial topic are poorly understood. Methodology: Cross-sectional electronic survey design sent to nurse members of the American Nurses Association. Inductive thematic content analysis was applied to open-ended comments. Ethical Considerations: Approved by the institutional review board (#191046). (...) Participants: 1213 nurses provided 3639 open-ended comments. More than 80% of participants self-identified as white 58% held a graduate degree; and half were of Christian faith. Results: Values ranged on a continuum expressed through four themes: “Honoring Patient Autonomy without Judgment,” “Honoring with Limitations,” “Not until...,” and “Adamantly against.” Some felt it was a duty to honor the patients’ wishes, set aside own beliefs, and respect patients’ choices often with a spiritual connotation. Nurses held concerns about the process, policy, potential psychological harm, legal risk, and the need to learn more about MAID. Nurse who were adamantly against MAID associated the practice with murder/suicide and against religious beliefs. Disparate values were expressed about changing the MAID legislation to allow patient support with taking MAID medications and allowing MAID via advance directive. Conclusions: Nurses desire more education on MAID. There is not one universally held position on the nurse’s role during MAID. Healthcare policy/standards need to accommodate the wide variation in nurses’ values. Implications: Nurses desire education regarding their role in MAID. Nurses are encouraged to participate in policy discussions as the practice becomes increasingly legal. Managers need to expect that nurses, patients, and families will need psychological support to participate in MAID. Careful construction of policy/standards is needed to minimize conflict, moral distress, and psychological harm amongst nurses. Further research is needed. (shrink)

This article is an invitation to consider Paulo Freire’s Pedagogy of the Question as a text that leads the reader to consider the possibilities offered by the post-Covid 19 resumption of daily school life with children as a political act. Based on a conversation that Freire had with Antônio Faundez in 1985, this text encourages researchers and educators to consider ways of returning to school life in ways that are welcoming, responsive and loving in the wake of the experience of (...) “almost-exile” undergone during the covid-19 pandemic. It becomes necessary to think school life again from where the children truly are, restarting in full awareness of the adventures, the pains, the losses they have undergone, encouraging them to ask the questions that the pandemic experience has produced them, and to recognize the new forms of consciousness it has inaugurated. This paper explores the implications of the ideas found in the Freire-Faundez dialogue for this moment of reconstruction. (shrink)

A equipe da Enfermagem é privilegiada, pois, é o cuidador que permanece vinte e quatro horas na assistência direta do paciente, podendo ter uma visão ampliada da pessoa no processo do adoecimento, tendo a oportunidade de incorporar e desenvolver os princípios dos Cuidados Paliativos (CP). Na perspectiva holística da saúde a religião/religiosidade/espiritualidade é uma das possibilidades de cuidado e manejo, porém ainda muito negligenciada pelos profissionais da saúde, seja por desconhecimento ou preconceitos e tabus, mas que se torna essencial no (...) cuidado de pacientes em CP. Por meio deste relato de experiência, objetiva-se ressaltar a importância da religião/religiosidade/espiritualidade, no cuidado e manejo de pacientes em CP, a partir da experiência de uma profissional da enfermagem, sua prática em cuidados e docência, supervisionando alunos com pacientes em contexto dos CP. Anos acompanhando alunos na prática, demonstraram que os profissionais da enfermagem têm dificuldade de identificar o Sofrimento Espiritual (SE) dos pacientes, e quando identificavam, não sabiam o que fazer com esta demanda, delegando a outros profissionais da equipe, que também tinham dificuldades, causando um (des)cuidado na Dimensão Espiritual (DE) do paciente em CP, trazendo-o mais sofrimento e desconforto com sua finitude. Como resultado dessas reflexões, está sendo desenvolvida uma pesquisa de pós-doutorado, destinada a identificar a DE, construção de roteiro de educação permanente. Em síntese, esses profissionais tendo a capacitação para identificar o SE, explorar e cuidar da DE, poderão promover a proteção da saúde mental, tanto do paciente em CP quanto sua, e tornarem-se profissionais mais sensíveis na finitude. Este artigo apresenta reflexões sobre o letramento literário a partir da recepção da obra O olho de vidro do meu avô, de Bartolomeu Campos de Queirós, por meio da análise de depoimentos de leitores comuns – a literatura no cotidiano dos indivíduos - e de trechos de artigos científicos sobre a obra – a literatura no campo acadêmico. Apresentamos também uma sequência didática desenvolvida com alunos do 6º ano do Ensino Fundamental de uma escola pública de Itabirito, interior de Minas (...) Gerais, a partir da recepção do livro. Para a realização da prática pedagógica, usamos a proposta sugerida e experienciada por Rildo Cosson para o letramento literário, denominada pelo pesquisador como “Sequência Básica”, composta por quatro etapas: motivação, introdução, leitura e interpretação. Dessa forma, procuramos compreender a literatura, a leitura literária e o letramento literário na vida social e na escola, pois cabe a essa instituição proporcionar aos alunos experiências que desenvolvam essa competência para o letramento literário, a se perpetuar pela vida adulta. O principal objetivo desse processo no contexto escolar é formar um leitor que aprecie significações estéticas, capaz de se inserir em uma comunidade que faça suas próprias escolhas literárias, que saiba manipular seus instrumentos culturais e possa construir com eles um sentido para si e para o mundo em que vive. Dessa forma, é necessário reconhecer a importância de atividades que abarquem diferentes tipos de materiais escritos, mas que não sejam negligenciados os textos literários, multiplicando, dessa forma, os espaços de leitura e constituindo comunidades de leitores na escola e fora dela. A new Clinical Ethics Advisory Group was created to contribute to NHS Trust policies and guidelines in response to ethical issues arising from the COVID-19 pandemic. An ethical analysis framework used in medical education, the CoRE-Values Compass and Grid, was adapted to form a step-wise ‘ABC’ decision-making process. CEAG members found the framework simple to understand and use and the model facilitated time-efficient decisions that were explicitly justifiable on moral, ethical, professional and legal grounds. The incarnated action of drawing, as an aesthetic action of touching and being touched by the world when transposing the visible limits and entering into the intimacy of worldly invisibility, constitutes an experience that is (...) as recurrent and trivialized in school daily life as it is existentially complex due to its poetic power to enter the invisible and inaugurate worldviews. The historical disqualification of the image and imagination in Western thought, supported by the separation between subjectivity of the body and objectivity of the world, does not allow educational thought to consider the phenomenon of poetic imagination as an existential experience of language insertion in the world from the gesture of drawing. Gesture that finds its specificity in the instant the hand traces and inscribes lines on the surface of the supports as infinitely creative writing. The aesthetic gesture of drawing, temporalized by the rhythm of the body in the emergence of the fabulation that accompanies the repetition of the marks, implies a poetic experience of language that involves the fusion of two senses: that of the gesture in materiality and that of the mark configured in it, marked and cicatrized in the surface of the support by the body’s action that performed it. The approximation between education, arts and childhood allows us to highlight the philosophical and pedagogical tensions that involve the question of poetic imagination and to take another look at the action of drawing in the context of children's education. What emerges, from the dialogue between Gaston Bachelard, Maurice Merleau-Ponty and Jean-Luc Nancy, is the relevance of the educational intention of caring for the vital function of language as an aesthetic and poetic experience that is constituted in the processuality of the body to make something appear that produces and contains presence, that which promotes and expands the existential density of the real. This paper aims to explore the association between over-indebtedness and two facets of well-being – life satisfaction and emotional well-being. Although prior research has associated over-indebtedness with lower life satisfaction, this study contributes to the extant literature by revealing its effects on emotional well-being, which is a crucial component of well-being that has received less attention. Besides subjective well-being, reported health, and sleep quality were also assessed. The findings suggest that over-indebted consumers have lower life satisfaction and emotional well-being, as (...) well as poorer health and sleep quality. Furthermore, over-indebtedness impacts life satisfaction and emotional well-being through different mechanisms. Consumers decreased perceived control accounts for the impact of over-indebtedness on both facets of well-being. Financial well-being, partly mediates the impact of indebtedness status on overall life satisfaction. The current study contributes to research focusing on the relationship between indebtedness, well-being, health, and sleep quality, and provides relevant theoretical and practical implications. ABSTRACT In this study, we present an analysis of the utterance Aonde tu vai rapaz [Where’d Ya Go Man], focusing on linguistic-enunciative resources from a dialogic perspective. The methodology consists of a dialogic analysis interconnected to the concepts of chronotope, ideology, utterance, dialogism, and social axiologies. Results reveal that institutionalized ideologies tend to nullify the one in everyday life. However, those results are confronted in some way with the counter word that is manifested by the ladrão de marabaixo chants. Intonation (...) refracts pain, lamentation, indignation, anger, and slander, in addition to expressing value judgments of disapproval and opposition. Re-enunciation of the everyday-life word allows us to take a glimpse at segregation, denunciation, powerlessness, and the silencing of culture and identity. However, at the same time, it reverberates consolidated values such as African descent, identity, resistance, resilience, and re-existence. (shrink)

This book is a philosophical reflection on the experience of time from within exile. Its focus on temporality is unique, as most literature on exile focuses on the experience of space, as exile involves dislocation, and moods of nostalgia and utopia. Marcia Sá Cavalcante Schuback proposes that in exile, time is experienced neither as longing back to the lost past nor as wanting a future to come but rather as a present without anchors or supports. She articulates this present (...) as a "gerundive" mode, in which the one who is in exile discovers herself simply being, exposed to the uncanny experience of having lost the past and not having a future. To investigate this, the book establishes a conversation among three authors whose work has exemplified this sense of gerundive time: the German philosopher Martin Heidegger, the French writer and essayist Maurice Blanchot, and the Brazilian writer Clarice Lispector. The book does not aim to discuss how these authors understand the relation between time and exile but enters in conversation with them in relation to this question. Attempting to think and express this difficult sense of time from within exile, the book engages with the relation between thought and language, and between philosophy and literature. Departing from concrete existential questions, it opens new philosophical and theoretical modes through which to understand what it means to be present in times of exile. (shrink)

Probabilistic support is not transitive. There are cases in which x probabilistically supports y , i.e., Pr( y | x ) > Pr( y ), y , in turn, probabilistically supports z , and yet it is not the case that x probabilistically supports z . Tomoji Shogenji, though, establishes a condition for transitivity in probabilistic support, that is, a condition such that, for any x , y , and z , if Pr( y | x ) > Pr( y (...) ), Pr( z | y ) > Pr( z ), and the condition in question is satisfied, then Pr( z | x ) > Pr( z ). I argue for a second and weaker condition for transitivity in probabilistic support. This condition, or the principle involving it, makes it easier (than does the condition Shogenji provides) to establish claims of probabilistic support, and has the potential to play an important role in at least some areas of philosophy. All extant purely probabilistic measures of explanatory power satisfy the following technical condition: if Pr(E | H1) > Pr(E | H2) and Pr(E | ∼H1) < Pr(E | ∼H2), then H1’s explanatory power with respect to E is greater than H2’s explanatory power with respect to E. We argue that any measure satisfying this condition faces three serious problems—the Problem of Temporal Shallowness, the Problem of Negative Causal Interactions, and the Problem of Nonexplanations. Baron develops and assesses the criticism, which she sees as comprising two objections: that duty plays too large a role, leaving no room for the supererogatory, and that Kant places too much value on acting from duty. Clearly written and cogently argued, Kantian Ethics Almost without Apology takes on the most philosophically intriguing objections to Kant's ethics and subjects them to a rigorous yet (...) sympathetic assessment. In the world of philosophy of science, the dominant theory of confirmation is Bayesian. In the wider philosophical world, the idea of inference to the best explanation exerts a considerable influence. Here we place the two worlds in collision, using Bayesian confirmation theory to argue that explanatoriness is evidentially irrelevant.

Andrew Cling presents a new version of the epistemic regress problem, and argues that intuitionist foundationalism, social contextualism, holistic coherentism, and infinitism fail to solve it. Cling’s discussion is quite instructive, and deserving of careful consideration. But, I argue, Cling’s discussion is not in all respects decisive. I argue that Cling’s dilemma argument against holistic coherentism fails.
