The overarching issue with this case study is poor regulation and quality control over direct-to-consumer genetic testing, delivered in the absence of any medical oversight.

Genetic research into ageing, longevity and late-onset disease is becoming increasingly common. Yet, there is a paucity of knowledge related to clinical actionability and the return of pathogenic variants to otherwise healthy elderly individuals. Whether or not genetic research in the elderly should be managed differently from standard practices adapted for younger populations has not yet been defined. In this article, we provide an overview of ethical and practical challenges in preparing for a genetic study of over 14 000 healthy (...) Australians aged 70 years or older enrolled in the ASPirin in Reducing Events in the Elderly Healthy Ageing Biobank. At the time of consent, all participants in this study were free of life-threatening illness, cardiovascular disease or cognitive impairment. ASPREE is thus a cohort of healthy elderly individuals with seemingly minimal burden of genetic disease recruited without ascertainment bias. The cohort presents a unique opportunity to address the penetrance of known pathogenic variants in a population without disease symptoms; however, it also raises a number of ethical concerns regarding the interpretation and disclosure of variants with known clinical actionability. Some of the challenges include how to manage the interpretation, disclosure and actioning of pathogenic variants found in otherwise healthy elderly adults without disease symptoms, whether or not to disclose findings for the benefit of family members rather than elderly consented donors themselves, how to manage the return of genetic findings to the elderly individuals who are now in severe cognitive decline or terminal illness, how to ensure quality of information and clinical service upon disclosure of results to this demographic and how to prepare for the insurance implications of disclosing genetic information under Australian law. BackgroundThe ARRIVE (Animal Research: Reporting of In Vivo Experiments) guidelines are widely endorsed but compliance is limited. We sought to determine whether journal-requested completion of an ARRIVE checklist improves full compliance with the guidelines.MethodsIn a randomised controlled trial, manuscripts reporting in vivo animal research submitted to PLOS ONE (March–June 2015) were randomly allocated to either requested completion of an ARRIVE checklist or current standard practice. Authors, academic editors, and peer reviewers were blinded to group allocation. Trained reviewers performed outcome adjudication (...) in duplicate by assessing manuscripts against an operationalised version of the ARRIVE guidelines that consists 108 items. Our primary outcome was the between-group differences in the proportion of manuscripts meeting all ARRIVE guideline checklist subitems.ResultsWe randomised 1689 manuscripts (control: n = 844, intervention: n = 845), of which 1269 were sent for peer review and 762 (control: n = 340; intervention: n = 332) accepted for publication. No manuscript in either group achieved full compliance with the ARRIVE checklist. Details of animal husbandry (ARRIVE subitem 9b) was the only subitem to show improvements in reporting, with the proportion of compliant manuscripts rising from 52.1 to 74.1% (X2 = 34.0, df = 1, p = 2.1 × 10−7) in the control and intervention groups, respectively.ConclusionsThese results suggest that altering the editorial process to include requests for a completed ARRIVE checklist is not enough to improve compliance with the ARRIVE guidelines. Other approaches, such as more stringent editorial policies or a targeted approach on key quality items, may promote improvements in reporting. (shrink)

The lottery paradox has been discussed widely. The standard solution to the lottery paradox is that a ticket holder is justified in believing each ticket will lose but the ticket holder is also justified in believing not all of the tickets will lose. If the standard solution is true, then we get the paradoxical result that it is possible for a person to have a justified set of beliefs that she knows is inconsistent. In this paper, I argue that the (...) best solution to the paradox is that a ticket holder is not justified in believing any of the tickets are losers. My solution avoids the paradoxical result of the standard solution. The solution I defend has been hastily rejected by other philosophers because it appears to lead to skepticism. I defend my solution from the threat of skepticism and give two arguments in favor of my conclusion that the ticket holder in the original lottery case is not justified in believing that his ticket will lose. Our world is a world of change. Children are born and grow into adults. Material possessions rust and decay with age and ultimately perish. Yet scepticism about change is as old as philosophy itself. Heraclitus, for example, argued that nothing could survive the replacement of parts, so that it is impossible to step into the same river twice. Zeno argued that motion is paradoxical, so that nothing can alter its location. Parmenides and his followers went even further, arguing that the (...) very concept of qualitative change is inconsistent. Change in any respect is impossible, they argued, since change requires difference and nothing differs from itself.1 Few today would accept the Eleatic conclusion that change is impossible. But the topic of change continues to be a source of much debate, as it brings together various issues that are central to metaphysics, language and logic – including identity, persistence, time, tense, and temporal logic. As we consider various approaches to our topic, it will become clear that one’s perspective on change is often determined by one’s position in the broader philosophical landscape. (shrink)

Body integrity identity disorder (BIID), previously called apotemnophilia, is an extremely rare condition where sufferers desire the amputation of a healthy limb because of distress associated with its presence. This paper reviews the medical and philosophical literature on BIID. It proposes an evidenced based and ethically informed approach to its management. Amputation of a healthy limb is an ethically defensible treatment option in BIID and should be offered in some circumstances, but only after clarification of the diagnosis and consideration of (...) other treatment options. The use of opaque, uninterpretable artificial intelligence systems in health care can be medically beneficial, but it is often viewed as potentially morally problematic on account of this opacity—because the systems are black boxes. Alex John London has recently argued that opacity is not generally problematic, given that many standard therapies are explanatorily opaque and that we can rely on statistical validation of the systems in deciding whether to implement them. But is statistical validation sufficient to justify implementation of these (...) AI systems in health care, or is it merely one of the necessary criteria? I argue that accountability, which holds an important role in preserving the patient‐physician trust that allows the institution of medicine to function, contributes further to an account of AI system justification. Hence, I endorse the vanishing accountability principle: accountability in medicine, in addition to statistical validation, must be preserved. AI systems that introduce problematic gaps in accountability should not be implemented. (shrink)

This paper offers a theoretical discussion on why the nursing profession has had a delayed response to the issue of climate change. We suggest this delay may have been influenced by the early days of nursing's professionalization. Specifically, we examine nursing's professional mandate, the generally accepted metaparadigm, and the grand theorists’ conceptualizations of both the environment and the nurse–environment relationship. We conclude that these works may have encouraged nurses to conceptualize the environment, as well as their relationship with it, mainly (...) in terms of the individual patient, and as such, nurses have not been encouraged to understand these concepts from a broader perspective. By not having the philosophical and theoretical foundations to understand the environment in relation to society, it is not surprising that nurses have had a delayed response to climate change and may not have viewed it as a professional concern. A planetary health perspective is suggested as a theoretical basis for nursing education, research and practice. Taking on a planetary health perspective could help nurses progress the profession and move healthcare systems towards supporting a climate‐resilient future. (shrink)

As scientists begin to study increasingly complex questions, many have turned to computer simulation to assist in their inquiry. This methodology has been challenged by both analytic modelers and experimentalists. A primary objection of analytic modelers is that simulations are simply too complicated to perform model verification. From the experimentalist perspective it is that there is no means to demonstrate the reality of simulation. Modern commercial society has been criticized for attenuating virtue and inhibiting the ethical self-realization of its participants. But Adam Smith, a founding father of liberal commercial modernity, anticipated precisely this critique and took specific measures to circumvent it. This article presents these measures via an analysis of his response to the critique of liberal commercial modernity set forth by Rousseau. It principally argues that Smith's distinctions of the love of praise from the love of praiseworthiness, and the love of glory (...) from the love of virtue, were elements of a normative moral education that sought to elevate civilized man's corrupted self-love, and thereby recover within modern commercial society a respect for ethical nobility. In response to cancer, patients may be thrown into precarious processes of remaking their purpose, identity, and connections to the world around them. Thoughtful and thorough responses to these issues can be supported by person-centered phenomenological approaches to caring for patients. The importance of perspectives on illness offered by theoretical phenomenology will become apparent through the example of the experience of nausea, or perhaps more accurately put—chemo sickness. The focus here is on how chemo sickness alters one's way of relating (...) to the world. I will examine the phenomenon of nausea through the lens of the phenomenological concept of mood—an existential feeling that engenders a diffused tone permeating through one's world and shapes the envisioned possibilities on one's horizon. Although ‘warrant’ has been used to mean something like ‘justified to the degree required for knowledge’, it has recently come to mean something else. Alvin Plantinga has recently used the word ‘warrant’ to mean “that, whatever precisely it is, which makes the difference between knowledge and mere true belief.” So, in Plantinga’s sense of the word, warrant is the justification condition plus some other condition designed to rule out Gettier examples. In almost all cases, reliabilists, foundationalists, and coherentists have not (...) been giving theories about ‘warrant’ in Plantinga’s sense. They have been busy giving theories about justification. In his paper, “Warrant Entails Truth,” Trenton Merricks argues that warrant, in Plantinga’s sense, entails truth. In this paper, I will show that Merricks has not succeeded in showing that warrant entails truth. (shrink)

Humeans hold that the nomological features of our world, including causal facts, are determined by the global distribution of fundamental properties. Since persistence presupposes causation, it follows that facts about personal identity are also globally determined. I argue that this is unacceptable for a number of reasons, and that the doctrine of Humean supervenience should therefore be rejected.

The Yijing/Binary System Episode involved Leibniz' discovery of a de facto representation of the binary number system in the sixty-four-hexagram Fu Xi "Yijing." The interesting similarities and differences are discussed between the thought of Leibniz and that of Shao Yong, both of whom, it is argued, understood and recognized the importance of the double geometric progression in the diagram.

She was 27 years old at the time and carried with her a manuscript that was later to become the peculiar biography of an eighteenth-century German-Jewish "pariah," Rahel Varnhagen (1771-1833). The Life of a fewish Woman, subtitle of the biography by Arendt, distills the largely unpublished (...) Varnhagen correspondence and marks the impact of that life upon German-Jewish history.1 Published more than "half a lifetime" later in 1957, it assays the politics of form in a highly unusual manner. By expunging all detail in order to analyze Varnhagen's introspection, Arendt implicitly questions selfhood as the form for identity and as the appropriate form for biography. She reveals that seeking to preserve the self determines the blank dumbness of an unnarratable life. Rahel Levin (Antonie Friederike Robert) Varnhagen, or simply "Rahel " as Arendt calls her, lived a life as complicated as her many names. The tortured relation she had to her identity, her insatiable preoccupation with self, reflects an effort to eradicate herJewishness by baptism and marriage and becomes a paradigm of a story about a woman lost to a sense of others and herself. Neither rich, cultivated, nor beautiful, Rahel lived her unhappiness fervently, with an ecstasy that made her appear to others as startlingly original. This helps to explain how she came to preside over the most important intellectual salon in Germany during the late eighteenth century. Princes, intellectuals, and artists flocked to her attic apartment, attracted by the impenetrable spectacle of her despair. Like Arendt, Varnhagen was unclassifiably exotic, an unattached, Jewish female seemingly without a history, aligned with no one and nothing. If"Jewishness" meant letting life "rain upon" her, Rahel Varnhagen let it strike her "like a storm without an umbrella." And like Arendt, she refused to hide the traits and talents of her personality under cover of morality and convention. Her letters are not self-congratulatory, for she never wittingly chose notoriety. Indeed, she spent much of her life trying to escape the stigma ofJewishness that marked her destiny. Notwithstanding the milieu of nineteenth-century German romanticism that drew the biographer toward her subject, various events surrounding the text make more compelling narratives than Varnhagen 's life: how the manuscript survived World War II, Arendt's emigration to the United States, and, especially, the public and private influence of Martin Heidegger.4 At the age of 18 (1925), Arendt had discovered Varnhagen during her brief love-affair with the 35-year-old philosophy professor at the University of Marburg. Since the Heidegger-Arendt correspondence remains closed, much about their youthful relationship and life-long intellectual affair remains a mystery open only to speculation. In 1933 Margaret Lasker, a biochemist who worked at the labs of Montefiore Hospital in New York, developed an accurate method for the differentiation between pentosuria and diabetes. Research into pentosuria, and mostly its genetic aspects, became Lasker’s lifelong passion. Since research was not part of her job description, she conducted the chief part of her study in her home kitchen. Lasker’s extensive and personal correspondence with her patients and their families may be the secret key for her success in (...) maintaining a prolonged research career against all odds. Laker’s last article was published in 1955 in Human Biology, presenting data on 72 cases of pentosuria, which occurs almost exclusively in Ashkenazi Jews. More than half a century later, and long after Lasker was gone, her well kept data and family records allowed the discovery of two mutations in the DCXR gene, by Mary-Claire King and her team. In this paper, we outline a new method for evaluating the human impact of machine-learning applications. In partnership with Underwriters Laboratories Inc., we have developed a framework to evaluate the impacts of a particular use of machine learning that is based on the goals and values of the domain in which that application is deployed. By examining the use of artificial intelligence in particular domains, such as journalism, criminal justice, or law, we can develop more nuanced and practically relevant understandings (...) of key ethical guidelines for artificial intelligence. By decoupling the extraction of the facts of the matter from the evaluation of the impact of the resulting systems, we create a framework for the process of assessing impact that has two distinctly different phases. Origen of Alexandria can be credited as the founder of a Christian speculative angelology, in which Christ the Logos is both the creator and the interpreter of the angels. He introduces the angels as the first created rational beings who, in contemplating the divine Word (Logos), freely choose to direct their will as holy angels in service to or wicked demons in antagonism against the love of God. The first created rational beings are divided into three orders: the angels, the (...) demons, and the neutral spirits of human souls. The angels remain closest in contemplation of the Logos, yet, due to their negligence, descend to unfold in the angelic hierarchy. The angels and demons thereafter guide the movements of all spirits, substances, and signs in the created world. The neutral spirits of human souls can choose to follow either the guidance of guardian angels or demons. And yet after the Incarnation, the angels are distinguished from the demons by their choice to follow Christ. Origen’s angelology has often been regarded as an early Christian alternative to Middle Platonic daemonologies. And after Karl Barth, his angelology has come to be dispensed from Christian theology. However, as Jean Daniélou has observed, Origen had previously departed from Platonic daemonology in affirming that angelic mediacy must pass away like the light of the stars before the brilliance of the Logos of Christ. He had, in this way, already assimilated the mediacy of the angels to the absolute mediation of Christ. And, in assimilating angelic to christic mediacy, Origen also attributes the reason with which the world is moved to the divine reason of the divine Word (Logos) of Christ in God. Business leadership increasingly requires a master’s degree in business and graduate management admission test scores continue to be an important component of applications for admission to such programs. Given the ubiquitous use of GMAT scores as gatekeepers for business leadership, GMAT scores are likely to influence organizational ethical behavior through gender, cultural, and other biases in the GMAT. There is little prior literature in this area and we contribute by empirically documenting that GMAT scores are negatively related to the cultural (...) dimensions of masculinity and power distance and are positively related to math literacy, uncertainty avoidance, and individualism. We estimate that cultural factors may account for as much as an 80-point difference in cross-national mean GMAT scores which are also related negatively to local language literacy, national educational spending, wealth per capita, wealth inequality, and gender development. Most interestingly, we also find a significant negative association of GMAT scores with ethical orientation. These findings have important implications for business schools and corporate ethics and leadership. (shrink)

Our study focuses on the series of philosophical key concepts, sometimes difficult to define, which, in this seminal commentary, form the redactional framework, often misunderstood, of the Boethian Solution of an Aporia that claims to have demonstrated the impossibility (...) of the existence of the genera and species (among others by the argument of the regressus ad infinitum), as well as the redhibitory defects of their (consonant or not consonant) intellections. Many social trends are conspiring to drive the adoption of greater automation in society, and we will certainly see a greater offloading of human decisionmaking to robots in the future. Many of these decisions are morally salient, including decisions about how benefits and burdens are distributed. Roboticists and ethicists have begun to think carefully about the moral decision making apparatus for machines. Their concerns often center around the plausible claim that robots will lack many of the mental capacities that are (...) indispensable in human moral decision making, such as empathy. To the extent that robots may be robustly artificially intelligent, these concerns subside, but they give way to new worries about creating artificial agents to do our bidding, if those artificial agents have moral standing. We suggest that the question of AI consciousness poses a dilemma. Whether artificially intelligent agents will be conscious or not, we will face serious difficulties in programming them to reliably make moral decisions. (shrink)

Cyberweapons are software and software, at least intuitively, is nonphysical. Several authors have noted that this potentially renders problematic the application of normative frameworks like UN Charter Article 2(4) to cyberweapons. If Article 2(4) only proscribes the use of physical force, and if cyberweapons are nonphysical, then cyberweapons fall outside the purview of Article 2(4). This article explores the physicality of software, examining Stuxnet in particular. First, I show that with a few relatively uncontroversial metaphysical claims we can secure the (...) conclusion that Stuxnet is physical. In particular, there exist instances of Stuxnet that are both located in space and causally efficacious, and this is very strong evidence for their being physical. Second, I argue that the question of physicality is actually irrelevant for the moral evaluation of an attack like Stuxnet because of its undeniably physical effects. Finally, I argue that some features of Stuxnet should make us optimistic about the prospects for discrimination and proportionality in cyberwarfare. (shrink)

Steroid hormones are signaling molecules important for normal growth, development and differentiation of multicellular organisms. Brassinosteroids (BRs) are a class of polyhydroxylated steroids that are necessary for plant development. Molecular genetic studies in Arabidopsis thaliana have led to the cloning and characterization of the BR receptor, BRI1, which is a transmembrane receptor serine/threonine kinase. The extracellular domain of BRI1, which is composed mainly of leucine‐rich repeats, can confer BR responsivity to heterologous cells and is required for BR binding. Previous research on the conversation analytic phenomenon of ‘repair’ has focused on its design and function in spoken interaction. Conversely, research on written text or writing rarely focuses on interaction. In this article, we examine repair in written discourse; specifically in online settings. The data corpus comprises one-to-one quasi-synchronous Facebook ‘chat’. First, we show that, as in spoken interaction, repair happens. This basic observation supports conversation analytic arguments that features of talk, like repair and laughter, do not ‘leak randomly’ into (...) interaction but are precision-timed and designed to accomplish action. Second, we report on two types of repair: visible repair which can be seen and oriented to by both participants in the interaction, and message construction repair, which is available only to the message’s writer. While the practice of message construction repair is made possible through the affordances of the online medium, it nevertheless shows how participants in written interaction are oriented to the same basic contingencies as they are in spoken talk: building sequentially organized courses of action and maintaining intersubjectivity. We suggest that assumptions about differences between spoken and online interaction are premature. Rather, we argue that online interaction should be treated as an adaptation of an oral speech-exchange system. (shrink)

Pascal’s Pensées is often read as a work of religious apologetics. Yet the Pensées in fact deserves to be read philosophically, on the grounds that in the text Pascal develops a sophisticated, original, and philosophically-productive method. This method is grounded in the three discrete types of binaries in the Pensées. This essay examines Pascal’s uses of these three forms of binaries to make two points: first, the exegetical point that for all its seeming haphazardness, the Pensées is in fact organized (...) around and grounded in a very specific methodology; and second, that this methodology, itself grounded in the use of binaries, is philosophically productive in specific ways that serve to advance Pascal’s project. In this paper, I leverage the pragmatist tradition in philosophy, the collective wisdom of scholarship in clinical ethics consultation, and earlier attempts to apply pragmatism in clinical ethics to develop a new vision of clinical ethics practice called New Clinical Pragmatism. It argues that clinical ethics methodology, from the New Clinical Pragmatist’s perspective, amounts to the recommendation that consultants should customize a methodological approach, drawing on the various available methods, depending on the demands of the specific case, and should avoid (...) attempts to identify a ‘true’ methodology but to the incoherence and inevitable failure of those attempts. I argue that pragmatism’s emphasis on practical wisdom and experimentation allow the New Clinical Pragmatist to do this while avoiding irrationality in choosing methods. I discuss how the New Clinical Pragmatist gives a unique, constructive perspective on key aspects of clinical ethics consultation such as the choice of common morality vs. internal morality of medicine approaches, process standards, bioethics mediation, and narrative ethics, and suggest how New Clinical Pragmatism’s relaxed approach to choice of methodology encourages consultants to balance attention to the particulars of the case with knowledge of what the many insightful scholars of clinical ethics methodology have found useful in the past. I also argue that New Clinical Pragmatism is consistent with efforts to professionalize clinical ethics consultation. (shrink)
