Communication about diagnosis and medical treatment for children suffering from life-threatening illnesses is complex. It is a primary step in involving underage patients and families in care and lays the foundation for obtaining parental permission and patient assent for treatment. In practice child participation in care is often difficult to obtain due to patients' different and sometimes fluctuating preferences, but also parents' protective strategies. Physicians may be susceptible to parental wishes to limit information and feel uncomfortable discussing issues related to (...) uncertainty of cure with patients. A qualitative study in Romanian paediatric oncology units was conducted to explore children's involvement from the perspectives of parents and oncologists. Interviews with participants discussed 18 patient cases. Data were transcribed and thematic analysis was used to interpret and mine patients' involvement during treatment. Different facets of patient participation were identified: restricting, widening and enhancing involvement. A fourth category, unintentional involvement, occurred for all patients due to children's observations during long-term hospitalisations and access to Internet. Uncertainty overarched parental attitudes regarding the extent to which children should be included. Physicians usually complied with parental wishes to limit involvement, but together with parents involved patients at least in a practical way. Adults' protective attitude may backfire, as adolescents' online searches often expose patients to worse-case scenarios. Further research should acknowledge the hazards of restricted diagnosis disclosure and develop clinician tools to support families in communicating with patients. This should be paralleled by physician efforts to elicit patients' needs regarding participation. (shrink)

Background: The global COVID-19 pandemic has imposed challenges on healthcare systems and professionals worldwide and introduced a ´maelstrom´ of ethical dilemmas. How ethically demanding situations are handled affects employees’ moral stress and job satisfaction. Aim: Describe priority-setting dilemmas, moral distress and support experienced by nurses and physicians across medical specialties in the early phase of the COVID-19 pandemic in Western Norway. Research design: A cross-sectional hospital-based survey was conducted from 23 April to 11 May 2020. Ethical considerations: Ethical approval granted (...) by the Regional Research Ethics Committee in Western Norway (131421). Findings: Among the 1606 respondents, 67% had experienced priority-setting dilemmas the previous two weeks. Healthcare workers who were directly involved in COVID-19 care, were redeployed or worked in psychiatry/addiction medicine experienced it more often. Although 59% of the respondents had seen adverse consequences due to resource scarcity, severe consequences were rare. Moral distress levels were generally low (2.9 on a 0–10 scale), but higher in selected groups (redeployed, managers and working in psychiatry/addiction medicine). Backing from existing collegial and managerial structures and routines, such as discussions with colleagues and receiving updates and information from managers that listened and acted upon feedback, were found more helpful than external support mechanisms. Priority-setting guidelines were also helpful. Discussion: By including all medical specialties, nurses and physicians, and various institutions, the study provides information on how the COVID-19 mitigation also influenced those not directly involved in the COVID-19 treatment of patients. In the next stages of the pandemic response, support for healthcare professionals directly involved in outbreak-affected patients, those redeployed or those most impacted by mitigation strategies must be a priority. Conclusion: Empirical research of healthcare workers experiences under a pandemic are important to identify groups at risks and useful support mechanisms. (shrink)

Substantial research indicates that individuals with Major Depressive Disorder (MDD) remember more negative information compared to neutral and positive information. This phenomenon is commonly attributed to attentional biases toward negative over neutral and positive information. A recent attentional resources model suggests that in MDD, negative cues not only capture attention, but also lead to deeper processing of subsequent information, irrespective of its content. This study aimed to replicate findings supporting this attentional resources model and go beyond it by investigating the (...) effect of negative cues on encoding and retrieval processes. Forty-one participants with MDD and no comorbid diagnoses, and 42 healthy-controls completed the emotional recall task with negative or positive videos presented during encoding and retrieval stages of a neutral word-list memory test. During encoding, only the MDD group exhibited a difference between negative and positive videos, such that for negative videos memory recall was improved and for positive words it was reduced. Emotional videos had no effect when presented during retrieval. These results suggest that in MDD, encountering emotional cues not only biases retrieval processes toward recalling more negative content, but rather fundamentally alters the depth of information processing, while not leading to a broad-spectrum recruitment of cognitive resources. Ethical dilemmas are part of medicine, but the type of challenges, the frequency of their occurrence and the nuances in the difficulties have not been systematically studied in low-income settings. The objective of this paper was to map out the ethical dilemmas from the perspective of Ethiopian physicians working in public hospitals. A national survey of physicians from 49 public hospitals using stratified, multi-stage sampling was conducted in six of the 11 regions in Ethiopia. Descriptive statistics were used and the (...) responses to the open-ended question “If you have experienced any ethical dilemma, can you please describe a dilemma you have encountered in your own words?” were analyzed using a template analysis process. A total of 587 physicians responded, and 565 met the inclusion criteria. Twelve of 24 specified ethically challenging situations were reported to be experienced often or sometimes by more than 50% of the physicians. The most frequently reported challenge concerned resource distribution: 93% agreed that they often or sometimes had to make difficult choices due to resource limitation, and 83% often or sometimes encountered difficulties because patients were unable to pay for the preferred course of treatment. Other frequently reported difficulties were doubts about doing good or harming the patient, relating to conflicting views, concern for family welfare, disclosure issues and caring for patients not able to consent. Few reported dilemmas related to end-of-life issues. The 200 responses to the open-ended question mirrored the quantitative results. Ethiopian physicians report ethical challenges related more to bedside rationing and fairness concerns than futility discussions and conflicts about autonomy as described in studies from high-income countries. In addition to the high report of experienced challenges, gravity of the dilemmas that are present in their narratives are striking. Recognition of the everyday experiences of physicians in low-income settings should prompt the development of ethics teaching and support mechanisms, discussion of ethical guidelines as well as increase our focus on how to improve the grave resource scarcity they describe. According to Brehm's intensity of emotion theory, if an emotion has motivational properties, its intensity should be non-monotonically affected by factors similar to those determining the intensity of motivational states. These factors are called deterrents. In the case of emotion, one category of deterrents consists of factors that can potentially interfere with feeling the emotion, such as reasons for not feeling the emotion. Two experiments were carried out to examine whether happiness is a motivational state and, thus, if its intensity (...) is non-monotonically determined by the importance of reasons for not feeling happy. We expected happiness to be reduced by a low importance reason for not feeling happy, to remain high in the presence of a moderately important reason, and to be reduced or eliminated by a very important reason. Both experiments supported the cubic function that results from these expectations, and when the results of the two studies were combined, each of the individual legs of the cubic function was found to be reliable along with the cubic function itself. This article describes the progress and impact of the King’s College London Centre for Military Ethics since its collaboration with the Colombian military forces’ educational institutions. More specifically, the article focusses on expanding the military ethics course across different educational facilities of the Colombian Army and the Colombian Navy and Air Force. The impact of the education delivered using an online course designed to be completed without a tutor is analysed and presented. The Faust motif provides an opportunity to explore the spectrum of attitudes among Christians toward science and technology by placing them into a historic context. Depending on one's understanding of the relationship of God and the world, the accomplishments of a Leonardo, a Paracelsus, a Faust, an Oppenheimer, or some future scientist credited with the “production” of the first successfully cloned human being can be interpreted as divine or diabolic in origin. I use the example of Faust to demonstrate that (...) the Christian assessment of the scientific enterprise is closely correlated to the level of doctrinaire dualism informing the particular version of Christianity that inspires the assessment. I show that, contrary to what seems obvious, Faust's damnation originated not in medieval times but in early modern northern Europe, reflecting a dualistic obsession with human sinfulness more characteristic of Reformation Germany than of Renaissance Italy. Encouraged by hellfire‐and‐brimstone preachers, the common folk saw demons, devils, and witches in every dark corner, while humanist scholars sought to recapture the brilliant past of the Greeks and the Romans. Goethe's interpretation represents a return to earlier versions of the story, while some continue to accuse contemporary Faustians of Satanic connections for seeking forbidden knowledge and daring to play God by manipulating the stuff of life. The question „what is reality?“ that opens Realontologie (1923), the establishing book in Hedwig Conrad-Martius’s (CM) oeuvre, establishes her realistic metaphysics. In her opinion, the firmly established „blinding insight“ in modern philosophy regarding the unfathomable contrast between the ideal and the real blocks any access to the question of reality. Her realontological philosophy seeks the „gate of reality“, meaning the datum-point where things „elevate“ themselves from non-existence or mere ideal existence but do not yet arrive at „operative Being“ or realistic (...) fulfillment. CM identifies three characteristics of the real by which it meets the „task of Being“ imposed upon it to personally describe and fulfill the essence inherent in it: formationality (Leibhaftigkeit), self-standing and primordiality. The suggested interpretation of each of these three seeks to unravel the seeming contrast between the real and the ideal in favor of what she regards as the only genuine and primordial opposition that separates the real from nonexistence. Human embryos produced in labs since the 1970s have generated layers of uncertainty for law and policy: ontological, moral, and administrative. Ontologically, these lab-made entities fall into a gray zone between life and not-yet-life. Should in vitro embryos be treated as inanimate matter, like abandoned postsurgical tissue, or as private property? Morally, should they exist largely outside of state control in the zone of free reproductive choice or should they be regarded as autonomous human lives and thus entitled to constitutional (...) protection like full-fledged citizens? Administratively, if they deserve protection, what institutional and policy mechanisms are best suited to carrying out the necessary oversight? Using a method termed comparative problematization, this article traces divergent answers to these questions produced in three countries—the United States, United Kingdom, and Germany—across the last twenty-five years. Comparison reveals distinct bioconstitutional foundations that give rise to systematically different understandings of each state’s responsibilities toward human life and hence its particular treatment of claims on behalf of embryonic lives. The relativistic optics of the nondispersive media endowed with the metric gij(x) [Eq. (1.6)] and with a nonlinear connection [Eq. (1.2)] is studied. The d-connection [Eqs. (3.3)– (3.4)] relates the conformal and projective properties of the space- time. A post-Newtonian estimation for the metric gij(x) is also given. It is shown that the solar system tests impose a constraint [Eq. (4.20)] on a combination of the post- Newtonian parameters describing the model.

The pivotal insight that paved Conrad-Martius’ (1880–1966) (CM) way in elucidating the ontological exclusivity of the I, denoted as “I-adhering being” (Ichhaftes Sein), is that despite its peculiarity and incomparability to any other mode of being, only the ontological foundations of the real being in general might enable a faithful comprehension of the I. The phenomenological interpretation suggested in this article presents CM’s ontological understanding of the I vis-a-vis her philosophy of Being, in particular in regard to three of its (...) general characteristics – existence, intelligibility, and self-adherence (Sichheit/Selbsthaftigkeit) – which provide the critical approach to the ontological study of the I. Finally, the understanding of the ontological exclusivity of the I-adhering being is achieved by means of an explication of the joining together of its typical affinities and discrepancies in regard to Being in general. One of the most important issues today is the conflict between identity groups. Can the concept of toleration provide resources for thinking about this? The standard definition of toleration – rejection or disapproval of a practice or belief followed by a constraint of oneself from repressing it –has limits. If we seek to make political and social conditions of toleration among diverse people a stable reality, we need to flesh out more deeply and widely what that depends upon. The essence (...) to which it has been reduced was not toleration’s original impulse. In the sixteenth century, the objective was to create conditions of peaceful collective life among diverse groups of believers. I examine one strand of change in moral valuation underpinning political toleration: ideas about the body, time and the self as explored by Michel de Montaigne. We can extract from this analysis a way to think about grounding toleration today: a recognition of the value of particular, embodied selves. (shrink)

This essay examines the early reception of Scheler’s anthropological thought in Spain. The article traces the two main ways of reception of Scheler’s work in the School of Madrid and the School of Barcelona. I argue that, on the one hand, the Spanish translations of the works of the early phenomenologists – Scheler was a central figure among them – contributed to the diffusion of Scheler’s thought; and that, on the other hand, the Spanish authors subjected Scheler’s thought to a (...) critical assimilation and further developments. Scheler influenced the Spanish philosophy of the human affectivity, in which the capacity to feel, the values and love as a kernel of the human being occupy an exceptional position. This reception shows the originality of Spanish authors that incorporate Scheler’s ideas to their anthropological interest and develop these ideas in the direction of their own phenomenological anthropology. Hedwig Conrad-Martius, the woman pioneer of the realistic phenomenological school, describes the reality to which her philosophizing is addressed as “totally non-material corporeality”. With this contradictory expression she seeks to affirm two foundational aspects regarding reality: the spatial that achieved material realization in real existents and the concealed non-spatial that is at the cradle of the establishing of reality and remains present behind its phenomenal and material appearing. This article focuses on three ontological elements in HCM’s idea of reality—“essence”, “abyss”, (...) and “self”—whose meaning both implies and raises the issue of the non-spatiality of Being in a complex manner. Moreover, the three seek the same objective of coming to terms with the force in real beings that will never ever be able to shine in its entirety. By means of philosophical explication of the mentioned elements and the illumination of the dialectic of each of them with the corresponding spatial aspects, this article demonstrates the evolution of HCM's understanding of the issue of spatiality that mirrors her metaphysics as a whole. Señala el olvido de la perspectiva de género, mainstreaming, en el diseño de las políticas públicas familiares de los estados de bienestar europeos. Apunta a que tras ese olvido se esconde la división sexual del trabajo: hombres espacio productivo, mujeres tareas domésticas y la no consideración del trabajo doméstico y de los cuidados como trabajo.
