The practice and development of modern medicine requires large amounts of data, particularly in the domain of cancer. The future of personalized medicine lies neither with “genomic medicine” nor with “precision medicine”, but with “data medicine”. The establishment of this DM has required far-reaching changes, to establish four essential elements connecting patients and doctors: biobanks, databases, bioinformatic platforms and genomic platforms. The “transformation” of scientific research areas, such as genetics, bioinformatics and biostatistics, into clinical specialties has generated a new vision (...) of care. Molecular tumor boards are one response to these changes and are now providing better access to next-generation sequencing and new cancer treatments to patients with inoperable or metastatic cancers, and those for whom the usual treatment has failed. However, MTB face a crucial ethical challenge: maintaining and improving the trust of patients, clinicians, researchers and industry in academic medical centers supported by private or public funding rather than providing genetic data directly to private companies. We believe that, in this era of DM, appropriate modern digital communication networks will be required to maintain this trust and to improve the organization and effectiveness of the system. There is, therefore, a need to reconsider the form and content of informed consent documents at all academic medical centers and to introduce dynamic and electronic informed consent. (shrink)

We present here a new method for bioethics: systemic modelling. In this method, the complex phenomenon being studied (e.g. personalized medicine, genetic testing, gene therapy, genetically modified organisms) is modelled as a whole, to shed light on its organization and functioning, and major (bio)ethical issues and solutions for their resolution are then identified. This systemic modelling method is ideal for use in the identification of solutions, rather than their validation, with other methods then used to test the solutions found. We (...) provide a description and reproducible instructions for the application of systemic modelling in bioethics, together with a brief example of the application of this method to the study of the impact of personalized medicine on French society. The French Marxist philosopher and sociologist Henri Lefebvre meditates on the relationship between jouissance, space, and architecture. Commissioned as a part of a study on tourist new towns in Spain, the book identifies spaces devoted to pleasure, enjoyment, sensuality, and desire as sites where the possibilities for a society moving beyond Fordism are manifested. In order to study these possibilities, architecture needs to be redefined as a mode of imagination rather than being restricted to a specialized practice or a (...) collection of monuments. Taking the practices of habitation as the starting point of the inquiry, Toward an Architecture of Enjoyment is understood as a production of space on all scales, from the domestic interiors, through urban spaces, to landscapes. Extending the focus of the book from “architecture” to “space of jouissance” within a transdicisplinary perspective, Lefebvre opens the discussion towards questions of subversive spaces, rhythmanalysis of the body, and pedagogy of senses. He proposes a Marxist take of architecture different from and alternative to the voice of Manfredo Tafuri, which since then has dominated critical architectural theory and history. Toward an Architecture of Enjoyment not only fundamentally changes our view on French and international architecture culture after 1968 but it gives new impulses for today’s debates about architecture and the urban society. BackgroundSince 2006, the genetic testing company 23andMe has collected biological samples, self-reported information, and consent documents for biobanking and research from more than 1,000,000 individuals, through a direct-to-consumer online genetic-testing service providing a genetic ancestry report and a genetic health report. However, on November 22, 2013, the Food and Drug Administration halted the sale of genetic health testing, on the grounds that 23andMe was not acting in accordance with federal law, by selling tests of undemonstrated reliability as predictive tests for (...) medical risk factors. Consumers could still obtain the genetic ancestry report, but they no longer had access to the genetic health report in the United States. However, this did not prevent the company from continuing its health research, with previously obtained and future samples, provided that consent had been obtained from the consumers concerned, or with health reports for individuals from other countries. Furthermore, 23andMe was granted FDA authorization on February 19, 2015, first to provide reports about Bloom syndrome carrier status, and, more recently, to provide consumers with “carrier status” information for 35 genes known to cause disease.DiscussionIn this Debate, we highlight the likelihood that the primary objective of the company was probably two-fold: promoting itself within the market for predictive testing for human genetic diseases and ancestry at a low cost to consumers, and establishing a high-value database/biobank for research and personal information).SummaryBy dint of this marketing approach, a two-sided market has been established between the consumer and the research laboratories, involving the establishment of a database/DNA biobank for scientific and financial gain. It has been argued in the literature that the deflationists’ thesis about the dispensability of truth as an explanatory notion forces them to adopt a conservative theory of truth. I suggest that the deflationists’ claim that the notion of truth is akin to a logical notion should be taken more seriously. This claim casts some doubts on the adequacy of the conservativity requirement, while it also calls for further investigation to assess its philosophical plausibility.

Positivists or logical empkicists of the Vienna Circle have tried to solve it by postulating several types of so-called basic statements induced by immediate experience or observation. According to Neurath protocol sentences are distinguished from other synthetic sentences only m virtue of their syntactical form. Since consistency is a relation concerning sentences only, not a sentence and any immediate experience,he contends that there remains an unbridgeable gap (...) between observationstatements and observation itself and consequently he adopts a coherentist strategy of corroboration, which Schlick, as a thorough empiricist, wants to avoid. For that purpose he introduces the notion of "Konstatierung". Such ascertainments play a decisive role as endpoints in the corroboration of theories insofar as they serve to verify predictions which express expected ascertainments. In spite of the fact that they are synthetic wc recognize their truth when grasping their meaning and, therefore, error is precluded in their case, while protocol sentences remain mere hypotheses. Quine attempts to overcome the weaknesses of all prior positions by proposing a purely behavioristic definition of 'observation sentence' which avoids the pitfalls of former sense-data theories as well as the ones of cultural relativism or irrationalism. Considering science to be a bridge linking together our sensory stimuli he comes to the view that empirical support of a theory is a consequence of the logical relation between the theory formulation and the observation categoricals derivable from it. He considers the ideological and epistemological neutrality of his conception as one of its main merits. Its feasibility depends, however, on the question whether, by his behavioristic method, he effectively can explain how linguistic expressions acquire their meanings. (shrink)

ZusammenfassungAus der Sicht der offenen Transzendentalphilosophie kommt es weniger auf eine physikali‐stische Gesamttheorie der Welt an als auf eine Analyse der normativen Tätigkeiten, die wir in die‐ser ausüben. Es wird gezeigt, wie wir – vor allem auch in der Wissenschaft – selbst die Voraus‐setzungen schaffen, die es uns erlauben, aufgrund von Wertungen ausgewählte Ziele zu erreichen. Pragmatische Erwägungen bedingen in einer gegebenen Situation den Entschluss, einen bestimm‐ten Kontext abzugrenzen, indem wir die dazu erforderlichen Regeln annehmen. Dadurch erzeugen wir ein relatives (...) Apriori, das uns die Mittel liefert, um zwischen analytisch und synthetisch, zwischen Sprache und Theorie etc. zu unterscheiden.Die Methode führt zu einem pluralistischen Weltbild: Nach der Wahl der linguistischen und ontologischen Bestandteile kann die Wahrheitsfrage im Sinne Tarskis nur mehr intern geregelt werden, während sich rivalisierende Systeme als ganze allein aufgrund von pragmatischen Krite‐rien vergleichen lassen.Es werden die deskriptiven und normativen Aspekte der Begriffe des Wertes, der Norm und der Regel erörtert, um schliesslich auf die zentrale Rotle zu verweisen, die Konventionen als nicht der Willkür ausgesetzte, sondern rational begrundete Festlegungen in sämtlichen unseren Aktivitäten spielen.SummaryFollowing my conception of open transcendentalism philosophy is less concerned with a naturalistically conceived theory of the world than with the normative activities we display in this world. By our own cultural and intellectual presuppositions we create the means – scientific or others – required for the achievement of certain goals which have previously been accepted as valuable. Pragmatic considerations induce us to establish, in given situations, what 1 call contexts by adopting particular rules. We thus produce a pragmatically relativized apriori providing us with a clearcut distinction between the analytic and the synthetic, between language and theory etc.The method issues in a pluralistic world view: Once the matter concerning the choice of a linguistic system and an ontology is settled the question of truth in Tarski's sense can only be raised internally while theories as a whole are compared on purely pragmatic grounds.I discuss some descriptive and normative aspects regarding the notions of value, norm and rule in order to ascertain the primary role played by conventions in all our activities. I rid the word of the connotation of arbitrairiness often attached to it in Anglo‐Saxon philosophy by restricting its use to deliberate acts concerned with rationally justified decisions.ResumeSelon ma conception du transcendentalisme ouvert, la philosophic est moins concernée par la recherche?on;une théorie globale du monde, telle que la préconise le naturalisme, que par ľanalyse des activités normatives que nous exerçons dans celuici. De la spontanéité révolutionnaire, je dénnerai une prèmiere définition, extrêmement sommaire, qu'il faudra affiner. La spontanédité, c'est un mouvement et un événement qui ont des causes, qui ont des conditions, qui ont des raisons et des motivations, mais qui tendent a les deborder et à les dépasser. La spontan´ité constitue une expérience politique.

La relation de Descartes a saint Augustin a souvent ete etudiee. Tous les historiens de Descartes y font plus ou moins allusion, en particulier a l'occasion du cogito, quand ils pensent aux sources de sa philosophie: ceux de saint Augsuti aussi, quand ils envisagent l'influence de sa doctrine. Si le present livre reprend les diverses questions que cette relation peut poser, c'est evidemment avec la volonte de les voir sous un autre eclairage.
