ObjectiveHead and neck cancer treatments are known to significantly affect functionality and appearance, leading to an increased risk for body image disturbances. Yet, few longitudinal studies exist to examine body image in these patients. Based on a conceptual model, the current study aimed to determine, in patients newly diagnosed with HNC: the prevalence, level, and course of body image concerns; correlates of upon cancer diagnosis body image concerns; predictors of immediate post-treatment body image concerns; and association between body image concerns (...) and levels of anxiety, depression, suicidal ideation, support, and alcohol and drug misuse.MethodsTwo hundred and twenty-three, newly diagnosed with a primary HNC were assessed using structured clinical interviews and psychometric measures at three, and 6 months after diagnosis. Primary outcome was 3-month, as it was most salient to body image disturbance. Multiple linear regression analyses were conducted on the potential body image predictors, based on the model.ResultsSixty-eight percent of patients with HNC presented some level of body image concerns. Body image concerns at baseline and post-treatment were significantly related and significantly increased from pre- to post-treatment. Immediately post-treatment, 89% presented some level of body image concerns. Correlates of body image concerns in patients with HNC at baseline included: physical symptom burden, difficulties with communication and eating, coping with the cancer diagnosis using denial, suicidal ideation, and having had a past anxiety diagnosis. When controlling for sociodemographic and medical variables, body image concerns in patients with HNC in the immediate post-treatment were predicted by: baseline body image, physical symptom burden, and neuroticism.ConclusionThis longitudinal study helps identify patients more susceptible to experience body image disturbance following head and neck cancer. Clinicians ought to pay special attention to body image concerns upon cancer diagnosis, physical symptom burden, and neuroticism, and may want to target these factors in future preventive interventions. In resistance training, the use of predicting proximity to momentary task failure, and repetitions in reserve scales specifically, is a growing approach to monitoring and controlling effort. However, its validity is reliant upon accuracy in the ability to predict MF which may be affected by congruence of the perception of effort compared with the actual effort required. The present study examined participants with at least 1 year of resistance training experience predicting their proximity to MF in two different experiments using (...) a deception design. Within each experiment participants performed four trials of knee extensions with single sets to their self-determined repetition maximum and MF. For the first experiment participants used loads equal to 70% of a one repetition maximum performed in a separate baseline session. Aiming to minimize participants between day variability in repetition performances, in the second separate experiment they used loads equal to 70% of their daily isometric maximum voluntary contraction. Results suggested that participants typically under predicted the number of repetitions they could perform to MF with a meta-analytic estimate across experiments of 2.0 [95%CIs 0.0 to 4.0]. Participants with at least 1 year of resistance training experience are likely not adequately accurate at gauging effort in submaximal conditions. This suggests that perceptions of effort during resistance training task performance may not be congruent with the actual effort required. This has implications for controlling, programming, and manipulating the actual effort in resistance training and potentially on the magnitude of desired adaptations such as improvements in muscular hypertrophy and strength. Depression is a widespread concern in the United States. Neuromodulation treatments are becoming more common but there is emerging concern for racial disparities in neuromodulation treatment utilization. This study focuses on Transcranial Magnetic Stimulation (TMS), a treatment for depression, and the structural and attitudinal barriers that racialized individuals face in accessing it. In January 2023 participants from the Twin Cities, Minnesota engaged in focus groups, coupled with an educational video intervention. Individuals self identified as non-white who had no previous TMS (...) exposure but had tried at least one treatment for their depression. Results revealed that the intervention did not notably change knowledge or stigma about TMS, but attitudes surrounding traveling for treatment changed. Notably, barriers like affordability, frequency of treatment, and lack of knowledge persist. Participants expressed a desire for more information, personal connection, and a tailored educational approach. The study suggests a need for multifaceted strategies to reduce disparities, focusing on education, community-based resources, and policy interventions. Immediate actions that can be taken include the creation of a TMS education program focused on patient engagement and awareness about TMS. (shrink)

This essay seeks to ascertain the philosophical status of revelation in Kant's critical philosophy so as to come to a better understanding of the use of Scripture in his religious writings, especially Religion within the Boundaries of Reason Alone . In doing so it remains faithful to Kant's hermeneutic strictures according to which the bible must be expounded according to morality, in the sense of the categorical imperative, and its attendant pure practical postulates. Taking as clues Kant's repeated insistence in (...) several critical works that revelation must function symbolically, and the Religion 's most detailed discussions of the nature of biblical language, I argue for the account of the philosophical status of Scripture's narrative content being supplied in the 3 rd Critique's discussion of analogical schematism, and that Kant thus regards Scripture as possessing an aesthetic content. I follow Henry E. Allison's work in interpreting Kant's cryptic account of analogy in terms of his theory of aesthetic ideas, and show how such a connection amply illuminates Kant's use of Scripture. Finally, I ask how understanding revelation in terms of aesthetic ideas complies with Kant's demands for the philosophical irreducibility of rational morality, and demonstrate how Kant remains strictly consistent on this score whilst also guaranteeing Scripture a properly aesthetic content. Fifty papers have been selected from the thirteen conferences and each of them is described in a short subsection individually written by one of the 24 authors. These subsections attempt to place the paper discussed in the context of the development of AI and Law, while often offering some personal reactions and reflections. This article revisits a principle of distributive justice accepted by most, if not all, scholars of climate justice. The principle at stake, the limit, protects those who are very badly off from bearing the costs of climate change mitigation. The persistent noncompliance of developed states with their obligations toward burden sharing, however, means that this principle is increasingly in tension with successful climate change mitigation, given it seems to require that those in poverty have continued access to emissions in cases (...) where alternative forms of energy are not provided. In the first half of the paper I outline this tension and show how the dominant expression of the limit in the literature, advanced by Henry Shue, must be abandoned. I argue that any attempt to articulate the limit in current circumstance, where the carbon budget is very scarce, must consider the climate harm associated with continued subsistence emissions. The second half of the article defends a principle, the exemption, which is best able to maintain the strong commitment to shielding those below the minimal threshold from the costs of mitigation, even in light of the potential harm that will result if they require emissions to fuel their energy needs. A century ago Henri Bergson was a world-wide celebrity. However, after the world wars his philosophy had already fallen into disfavor, disdain and oblivion. Prominent molecular biologists claimed to have hammered the final nail in the coffin of vitalism. Francis Crick himself, with prophetic hubris, called any future vitalist a crank. Things were not much different amongst analytic philosophers who, more concerned with clarity than precision, saw in Bergson’s works hardly more than poetry and mysticism. In fact, ‘vitalism’ became a (...) one-word argument against itself (just utter it and it would count as disproved). And yet, ironically, vitalism refused to die. Half a century ago, Gilles Deleuze wrote a seminal interpretation of Bergson’s philosophy. After providing a concrete articulation of Bergson’s method of intuition, Deleuze studied the progression of Bergson’s concepts of duration, memory, and the élan, and paired them with his own concepts of multiplicity, the virtual and differentiation. Now, in a lucid and crisp book, Craig Lundy unpacks (for the first time) Deleuze’s Bergsonism. Not only does the book afford a better grasp of Bergson’s genius, but it also allows us to trace the origin of some key notions in Deleuze’s philosophy. To listen is to be attuned to voices muffled by silence or camouflaged by a defensive rhetoric resonant with a voice inflected by festering wounds, existential and political. In exploring how one is to listen to these voices of silence, I draw upon Martin Buber’s concept of dialogical “inclusion” of others’ stories, to listen without interpretation to allow the voice behind his (...) – or her or their – story, be it merely etched viscerally in the language of silence, to dwell aside one’s own story in a dialogue unencumbered by perceptions of the Other forged by cultural, social, political constructs, and perhaps most insidiously one’s own defensive postures. Débats, addressed last summer to that sheet a series of letters descriptive of a rapid tour through the United States. He has just gathered these letters into a volume in which American readers will find a good deal of entertainment and a certain amount of instruction. M. de Molinari, in his capacity of French journalist, is of course lively and witty; but his vivacity is always in excellent taste. He is moreover extremely observant, and he often renders his impressions with (...) much felicity. He had apparently the advantage of coming to America without strong preconceptions in any direction; he was not pledged to find democratic institutions purely delightful, nor had he it on his conscience to lay in a stock of invidious comfort for oppressed Europeans. We have had in America too many observers of each of these categories. M. de Molinari’s conclusions seem disinterested and liberal, especially when we remember that they were addressed to a journal which is not remarkable (save when M. Laboulaye writes in it) for a deferential consideration of American affairs. They are, in the gross, very much the reflections with which sensible Americans themselves point the moral of their contemporary history. M. de Molinari’s weak point appears to have been that he had not time or inclination to look beneath the external surface of American manners, and that he was but scantily acquainted with the language of the people whom he had undertaken to examine. He usually writes his English words faultily – it is startling, for instance, to see a gentleman who has passed.. (shrink)

It is sometimes thought that the renowned essayist Isaiah Berlin was incapable of writing a big book. But in fact he developed some of his most important essays--including "Two Concepts of Liberty" and "Historical Inevitability"--from a book-length manuscript that he intended to publish but later set aside. Published here for the first time, Political Ideas in the Romantic Age is the only book in which Berlin lays out in one continuous account most of his key insights about the history of (...) ideas in the period that he made his own--the Romantic age. Distilling his formative early work in the history of ideas, the book also contains much that is not found elsewhere in his writings. The last of Berlin's posthumous books, it is of great interest both for his treatment of the subject and for what it reveals about his intellectual development. Written for a series of lectures at Bryn Mawr College in 1952, and heavily revised and expanded by Berlin afterward, the book argues that the political ideas of the Romantic age are still largely our own--down to the language and metaphors they are expressed in. Vividly expounding the central political ideas of leading European thinkers in the period 1760-1830, including Helvetius, Condorcet, Rousseau, Saint-Simon, Hegel, Schelling, and Fichte, the book is written in Berlin's characteristically accessible style. Supererogation is the theory that some acts go beyond the call of duty. They are praiseworthy, but their omission is not blameworthy. Notice that supererogation has to do with praise and blame as well as with what is a duty or morally obligatory. Moral duty requires a moral system on the basis of which duty or obligation is assigned. Utilitarianism can provide a criterion of moral obligation, and it can also provide a criterion for moral praise and blame. Many diseases and disease symptoms still lack effective treatment. At the same time, certain controversial Schedule I drugs, such as heroin and cannabis, have been reputed to have considerable therapeutic potential for addressing significant medical problems. Yet, there is a paucity of U.S. clinical studies on the therapeutic uses of controlled drugs. For example, people living with HIV/aids experience a variety of disease- and medication-related symptoms. Their chronic pain is intense, frequent, and difficult to treat. Nevertheless, clinical trials of compassionate (...) management for their chronic symptoms, which should be a research priority, are stymied. Stakeholders’ Views on Barriers to Research on Controlled Substances Evelyn Rhodes, Michael Andreae, Tyler Bourgiose, Debbie Indyk, Rosamond Rhodes, and Henry Sacks We employed qualitative methods to develop an understanding of the barriers to research on potential therapeutic uses of Schedule I drugs so that they might be addressed. We elicited the perspectives of key stakeholder groups that would be involved in such studies: people living with HIV/aids, clinicians, and members of institutional review boards. As we identified obstacles to research, we found that all of the stakeholder groups arrived at the same conclusion, that clinical research on the therapeutic potential of these drugs is ethically required. He wanted his own philosophy constituted of flesh and blood, and (...) he believed in the centrality of narration in ethical writing. The personality of stories, the irrefutable nature of the individual's account had to form the essential basis in the teaching of ethics. It is no surprise, then, that in his posthumous Tales of Good and Evil, Help and Harm 1 Hallie leaves us with a series of powerful stories, biographical and autobiographical, as the final expression of his lifelong agonizing struggle to come to terms with his own ethical beliefs. There are no general proofs here. No abstract principles. No definitions of goodness and evil. Only stories that depict the being referred to by e. e. cummings as "manunkind" mysteriously achieving the "realizable ideal" of rescuer of strangers.Given Hallie's major work on the rescuers of Jews in the village of Le Chambon-sur-Lignon, Lest Innocent Blood Be Shed. Thus, the first biography he presents is that of Magda Trocmé, the wife of the pastor of the Protestant Church in Le Chambon-sur-Lignon, André Trocmé, who, with his fellow pastor Edouard Theis, is said to have inspired the rescue mission in the village. Hallie underscores the communal nonviolent effort by clergy and villagers alike in this "city of refuge." While he admires both André and Magda as practitioners of the "great virtues," he more readily identifies with Magda's secularism than with her husband's piety. He celebrates her generosity, her refusal to turn anyone away from her door, her "quiet habit of efficacious compassion," and her terse explanation as to why the rescuers in the village did what they had done: "What do you mean? It had to be done, that's all."As much as Hallie related to Magda's secularism, he was unable to identify fully with her because he felt tainted by the killing he had done as an artillery man in the United States Army during World War II. "I was a helpful killer who violated the negative demand against killing ["Thou shalt not kill"] in order to obey the positive one to spread life. But Magda and her villagers! Their ethical purity was beyond me. They obeyed the negative demands of ethics by hating and hurting no one even while they obeyed the positive demands of ethics by saving [thousands of refugees]." Compared to Magda and the villagers, Hallie clearly saw himself as someone operating in the grey zone.With the introduction of Joshua James and the community of Hull, Massachusetts, we ascend from Hallie's personal quagmire of moral ambiguity to the realm of the "ethical sublime." Here the author relates the story of James, whose mother and sister died at sea in 1837 when he was 11 years old, and who would go on to a career of life-saving activity on the Massachusetts coast. James founded the Life-Saving Rescue Service, an organization that evolved into the Coast Guard (or more specifically The Search and Rescue Service of the Coast Guard). He spent his entire life in Hull (1826-1902) and over the course of nearly sixty years saved hundreds of sailor's lives and tens of thousands of dollars worth of property.Many coastal villages around the world, Hallie explains, profited greatly from shipwrecks. Their cargoes and the stuff the ships were made of were highly valuable to the generally poor people who inhabited such villages. Novello, Henry The word 'eschatology' means doctrine about the eschata or 'last things.' In the neo-Scholastic manual style of theology that dominated Catholic theology before the twentieth century, eschatology was the doctrine of those things that awaited the individual person beyond death, as well as those things that awaited the whole of humanity at the end of time. The teaching on the last things appeared as an appendix at the end of dogmatic theology where it led a rather barren (...) existence as instruction on what comes 'after' the pilgrim life, with the result that it had little critical and transforming effect upon the state of the present world. Since the manual style of eschatology treated the last things as objects, Yves Congar described it as a 'physical' style of eschatology. Dermot Lane makes a further noteworthy observation when he criticizes the physical style of eschatology for appearing 'to claim too much' and seeming 'far too certain on matters of detail.' In no sense should eschatology be regarded as providing factual information about the details of our future existence. (shrink)
