Background: Omalizumab, an anti-immunoglobulin E antibody, reduces exacerbations and symptoms in uncontrolled allergic asthma. The study objective was to estimate the costs and consequences of omalizumab compared to usual care from a US payer perspective. Methods: We estimated payer costs, quality-adjusted survival (QALYs), and the incremental cost-effectiveness ratio (ICER) of omalizumab compared to usual care using a state-transition simulation model that included sensitivity analyses. Every 2 weeks, patients could transition between chronic asthma and exacerbation health states. The best available evidence (...) informed the clinical and cost input estimates. Five years of omalizumab treatment followed by usual care was assumed to estimate a lifetime horizon. Omalizumab responders (60.5% of treated) were modeled as a separate scenario where nonresponders reverted back to usual care after 16 weeks of active treatment. Results: The mean lifetime discounted costs and QALYs were $83 400 and 13.87 for usual care and $174 500 and 14.19 for omalizumab plus usual care resulting in $287 200/QALY (95% interval: $219 300, $557 900). The ICER was $172 300/QALY when comparing omalizumab to usual care in the responder scenario. One-way sensitivity analyses indicated that the results were sensitive to the difference in treatment-specific utilities for the chronic state, exacerbation-associated mortality, omalizumab price, exacerbation rates, and response definition. Conclusions: The results suggest that adding omalizumab to usual care improves QALYs at an increase in direct medical costs. The cost-effectiveness of omalizumab is similar to other chronic disease biologics. The value increases when omalizumab response is used to guide long-term treatment. (shrink)

Complicity consists in one person contributing to someone else's wrongdoing. But there is a diverse cluster ways of being involved in another’s wrongdoing. For a ‘diagnosis by exclusion’, we first fix the meaning of complicity in contrast to that with which it is often wrongly conflated. Literally cooperating in wrongdoing with others, for instance, is more than complicity. Each and every cooperator is actually a co-principal in the wrong jointly committed; and each bears the full responsibility, shared with all co-principals, (...) for the outcomes of that wrongdoing. Other sorts of involvement with wrongs committed by others amount to less than complicity because the involvement there is causally inert and hence does not actually contribute to the other’s wrongdoing at all. Pardoning wrongs wholly in the past may be like that. So too may be some cases of wilfully overlooking wrongs that others commit under one's very nose. Complicity is a very precise way of being involved in wrongdoing. Complicity consists in a secondary contribution of a causal (or potentially causal) sort to a wrong committed by someone else. Bryan Norton's convergence hypothesis, which predicts that nonan‐thropocentric and human‐based philosophical positions will actually converge on long‐sighted, multi‐value environmental policy, has drawn a number of criticisms from within environmental philosophy. In particular, nonanthropocentric theorists like J. Baird Callicott and Laura Westra have rejected the accuracy of Norton's thesis, refusing to believe that his model's contextual appeals to a plurality of human and environmental values will be able adequately to provide for the protection of ecological integrity. These theoretical criticisms of convergence, (...) however, have made no real attempt to engage the empirical validity of the hypothesis, the dimension that Norton clearly takes to be the centerpiece of his project. Accordingly, the present paper attempts to provide an empirical analysis of the convergence argument, by means of a study of the Vermont public's environmental commitments and their attitudes toward national forest policy. Our findings support a generalized version of Norton's thesis, and lead us to suggest that environmental philosophers should try to be more inclusive and empirically minded in their discussions about public moral claims regarding nature. Complicity with wrongdoing comes in many forms and many degrees. We distinguish subcategories cooperation, collaboration and collusion from connivance and condoning, identifying their defining features and assessing their characteristic moral valences. We illustrate the use of these distinctions by reference to events in refugee camps in and around Rwanda after the 1994 genocide, and the extent to which international organizations and nongovernment organizations were wrongfully complicit with the misuse of refugees as human shields by the perpetrators of the genocide who (...) were allowed to run those camps. This book examines the Condorcet Jury Theorem and how its assumptions can be applicable to the real world. It will use the theorem to assess various familiar political practices and alternative institutional arrangements, revealing how best to take advantage of the truth-tracking potential of majoritarian democracy.

Given that university faculty members and supervisors practicing in the community have been involved in at least one research supervisor-graduate student relationship, it is surprising that so little attention has been paid to the ethical issues involved in such relationships. Indeed, as a student and her or his graduate research supervisor may be involved in a close working relationship for many years, it is understandable that several opportunities can arise that could be considered dual or multiple relationships. Examples of such (...) relationship issues discussed in this article include authorship matters, favoritism or inequitable treatment of students, and sexual relationships. Depending on the nature of the relationship, the impact on students can be quite severe; although, the effects on students have not been adequately studied. Existing ethical guidelines do not provide enough guidance in this area where students are in a position of diminished power with respect to the supervisor. Following the discussion of relationship issues, we suggest extensions of current guidelines to deal with these issues. (shrink)

Many large corporations now have written codes of ethics to guide the business/marketing activities of employees. The purpose of this study was to determine the frequency and types of topics which are covered in the ethics policy statements of large U.S. corporations. The results indicated that the topics covered most often (respectively) were: misuse of funds/improper accounting, conflicts of interest, political contributions, and confidential information. It is concluded that in addition to written ethics policy statements, top management should communicate ethical (...) values and demonstrate by example. (shrink)

Este artigo compara e contrapõe a abordagem naturalista pragmatista para a peculiaridade da linguagem, exemplificada, principalmente, mas, não exclusivamente, por John Dewey, com a extensa abordagem de Charles Taylor em seu O animal linguístico. Taylor, inspirado pelas obras de Hamann, Herder, e Humboldt, conta com recursos filosóficos e conceituais diferentes para o delineamento do que ele denomina de ‘a forma’ da capacidade linguística humana. Porém, Dewey e Taylor chegam a posições que se sobrepõem sem se identificar: a linguagem é a (...) característica definidora constitutiva dos seres humanos. Seres humanos são definidos pelo surgimento da ‘como’ consciência, uma ‘ruptura’ em nossa imersão imediata no mundo, e, como Peirce e Dewey mostraram de maneira tão lúcida, um reflexivo estar consciente do uso de signos e sistemas de signos de todos os tipos. Esses sistemas potencializam e transformam nosso acesso ao mundo e a nós mesmos. Eles não apenas rotulam um mudo já existente. Eles criam âmbitos de significados e valores que não surgiram sem eles. A distinção crucial de Taylor entre os modelos designativo e constitutivo da linguagem é apoiada plenamente pela consideração pragmatista da linguagem, a qual Taylor não declara. Essa distinção mostrará ser de importância especial para Dewey e Taylor na criação de paisagens existencialmente vitais de significado incorporados nas autodescrições e nas práticas delicadas das artes de auto-reflexão. Tanto Dewey quanto Taylor mostram que assim como as texturas abertas da experiência crescem por suas extremidades, assim a própria linguagem possui sua própria “extremidade” e nos aponta para os domínios “liminares” que sustentam o limiar do sentido para além do totalmente dizível. Esses domínios, que eles mostram de maneiras diferentes mas complementares, são acessados como realidades por formas não discursivas que abrangem as obras de arte, o que Taylor denomina de ‘representações,’ e rituais performativos e restaurativos, tanto pessoais, cívicos e religiosos que incorporam os significados. Revisioning macro-democratic processes in light of the processes and promise of micro-deliberation, Innovating Democracy provides an integrated perspective on democratic theory and practice after the deliberative turn.

In his seminal work, McTaggart :457–484, 1908; The nature of existence, Cambridge University Press, Cambridge, 1927) dismissed the possibility of understanding the B-Relations as irreducibly temporal relations, and with it dismissing the B-Theory of time, which assumes the reality of irreducible B-relations. Instead, he thought they were mere constructions from irreducible A-determinations and timeless ordering relations. However, since, philosophers have almost universally dismissed his dismissal of irreducible B-relations. This paper argues that McTaggart was correct to dismiss the possibility of B-relations, (...) and that would be B-theorists should be C-theorists and its concomitant commitment to the unreality of time. I do this by first elaborating C-Theory, noting that B-relations appear indiscernible from C-relations on close examination. This establishes an onus on B-theorists to distinguish B-relations from C-relations by elaborating the distinctively temporal character of the former. I then present a problem for the possibility of accommodating temporal character in B-relations. Following this, I question from whence derives our sense of the temporal character that purportedly resides in the irreducible B-relations. Finally, I extend the challenge against irreducible B-relations to a series of irreducible abstract temporal relations—so called Ersatz-B-Relations—modelled on them. (shrink)

Some of the most important contributions to animal ethics over the past decade or so have come from political, as opposed to moral, philosophers. As such, some have argued that there been a ‘political turn’ in the field. If there has been such a turn, it needs to be shown that there is something which unites these contributions, and which sets them apart from previous work. We find that some of the features which have been claimed to be shared commitments (...) of the turn are contested by key theorists working in the field. We also find that the originality of the turn can be exaggerated, with many of their ideas found in more traditional animal ethics. Nonetheless, we identify one unifying and distinctive feature of these contributions: the focus on justice; and specifically, the exploration of how political institutions, structures and processes might be transformed so as to secure justice for both human and nonhuman animals. This essay outlines an explanatory virtue of presentism: its unique ability amongst temporal metaphysics to deliver a partial explanation of the conservational character of natural laws. That explanation relies on presentism, uniquely amongst temporal metaphysics, being able to support an endurantist account of persistence. In particular, after reconsidering a former argument for endurantism entailing presentism by Merricks (Noûs 33:421-438, 1999), a new argument for this entailment, is expounded. Before delivering the explanation of the conservational character of natural laws, a brief (...) account of that explanandum is given, followed by an elaboration of the wider significance of an explanation for the conservational character for natural laws, particularly for the dispositional essentialist project of nomic reductionism, and explaining the non-simultaneity of causation. This paper elaborates the problem of temporal unity for dynamic presentism and diagnoses the source of that problem in the dynamic presentist’s discarding the traditional C-series in its avoidance of McTaggart’s (1908, 1927) A-series paradox. This C-series provided the fixed structure of time which the transitory aspects of time then followed, and thereby unify those transitory aspects. It then considers ersatzer presentism as an ostensible solution to the problem of temporal unity by providing a new abstract C-series (namely an ersatz-B-series) (...) for dynamic presentism. However, after a closer examination of the details of this proposal, it is found that the ersatz-B-series itself needs to be temporally variable to capture the transitory aspects of time that it is meant to track. Consequently, it cannot provide the fixed structure of time required to unify the transitory aspects of time into a temporal series. It is therefore suggested that dynamic presentists look for a more course-grained determiner of temporal progression to construct the presentist-friendly C-series required to unify their transitory aspects of time. The major advantage of STS is the kind of teaching it allows and demands. Twelve middle school teachers who were enthused with STS teaching selected two sections for a research study. One section was the experimental STS section; the other followed the course syllabus and textbook closely. The major findings indicate the advantages for STS as a teaching approach. Students at the STS approach learned as many science concepts as students who were taught such concepts directly. But the students in (...) the STS sections were superior in all other aspects, including understanding and use of 14 process skills, use of creative thinking skills, development of more positive attitudes, better ability to apply and connect ideas and skills to other situations, and developing a better understanding of the nature and history of science.
