Racism as a Public Health Crisis Statements (RPHCs) acknowledge the reality that racism must be eradicated to ensure health justice: a fair and just opportunity for all individuals to be healthy. Scholars of critical race theory (CRT) have expressed doubt when it comes to the capacity of law-related institutions to catalyze or sustain anti-racist efforts. These strains of skepticism underscore the question of whether so many RPHCS were adopted precisely because, in many instances, they were merely symbolic acts. This commentary (...) argues that the trend in adopting RPHCs carries signs of interest convergence, and asserts that the alliance between government and the movement for health justice reflected in this phenomenon falls short of the substantive anti-racist action needed to realize health justice. The spate of RPHC adoption, in lieu of passing anti-racist policy or meaningfully empowering people of color, signifies that the movement for health justice must be strategic in determining whether to leverage, or be wary of, the power dynamics which shape political change. The health justice framework must expand its toolkit to include CRT. (shrink)

Two fundamentally different perspectives on knowledge diffusion dominate debates about academic disciplines. On the one hand, critics of disciplinary research and education have argued that disciplines are isolated silos, within which specialists pursue inward-looking and increasingly narrow research agendas. On the other hand, critics of the silo argument have demonstrated that researchers constantly import and export ideas across disciplinary boundaries. These perspectives have different implications for how knowledge diffuses, how intellectuals gain and lose status within their disciplines, and how intellectual (...) reputations evolve within and across disciplines. We argue that highly general claims about the nature of disciplinary boundaries are counterproductive, and that research on the nature of specific disciplinary boundaries is more useful. To that end, this paper uses a novel publication and citation network dataset and statistical models of citation networks to test hypotheses about the boundaries between philosophy of science and 11 disciplinary clusters. Specifically, we test hypotheses about whether engaging with and being cited by scientific communities outside philosophy of science has an impact on one’s position within philosophy of science. Our results suggest that philosophers of science produce interdisciplinary scholarship, but they tend not to cite work by other philosophers when it is published in journals outside of their discipline. Furthermore, net of other factors, receiving citations from other disciplines has no meaningful impact—positive or negative—on citations within philosophy of science. We conclude by considering this evidence for simultaneous interdisciplinarity and insularity in terms of scientific trading theory and other work on disciplinary boundaries and communication. In this book Gregory Browne rejects the views of David Hume and the Logical Positivists, and argues that there are necessary factual truths, which include a wide range of truths from many fields of knowledge. Browne argues for the necessity of Newton's Laws and truths about natural kinds, and for the factuality of definitional truths and truths of logic and mathematics. Browne synthesizes the work of Kripke, Putnam, Quine and others, but goes beyond the usual discussions of (...) the meanings and definitions of terms to discuss the references of various kinds of terms, and specifically to develop a theory of kinds, distinguishing 'Deep Kinds' and 'Shallow Kinds' . His theory of Deep Kinds does not accept all of the assumptions commonly associated wtih a theory of natural kinds. (shrink)

In this commentary on Karla Stroud and Kieran O’Doherty’s ‘Ethically Sustainable Governance in the Biobanking of Eggs and Embryos for Research’ (2015) I call into question the need for good governance to overcome the challenges facing biobanking of eggs and embryos. I argue that the principles of good governance for biobanking that Stroud and O’Doherty outline come up short in providing concrete normative guidance to resolve the challenges associated with a biobank for eggs and embryos.

Neoliberal ideology and exclusionary policies based on racialized identities characterize the current contexts in North America and Western Europe. Nursing knowledge cannot be abstracted from social, political and historical contexts; the task of examining the influence of race and racial ideologies on disciplinary knowledge and inquiry therefore remains an important task. Contemporary analyses of the role and responsibility of the discipline in addressing race‐based health and social inequities as a focus of nursing inquiry remain underdeveloped. In this article, we examine (...) nursing's engagement with ideas about race and racism and explore the ways in which nursing knowledge and inquiry have been influenced by race‐based ideological discourses. Drawing on Henry and Tator's framework ofdemocratic racism, we consider how strategic discursive responses—the discourses of individualism, multiculturalism, colour‐blindness, political correctness and denial—have been deployed within nursing knowledge and inquiry to reinforce the belief in an essentially fair and just society while avoiding the need to acknowledge the persistence of racist discourses and ideologies. Greater theoretical, conceptual and methodological clarity regarding race, racialization and related concepts in nursing inquiry is needed to address health and social inequities. (shrink)

Elective ventilation (EV) is ventilation applied, not in the interest of patients, but in order to secure transplantable organs. It carries with it a small risk that patients who would otherwise have died will survive in a persistent vegetative state. Is EV ever justifiable? We argue: (1) The only thing which can justify exposing patients to risk not taken for their benefit is their consent, and we cannot rely on implied consent or third party consent in the case of EV. (...) Thus, absent explicit consent of patients, EV is not justifiable. (2) It is not clear that explicit consent should be sought, or where it is offered honoured, given the potential EV has for deterring organ donors and causing stress to staff and families. (shrink)

Tools and technologies expand our capacities, including our cognitive capacities. Microscopes extend our perceptual capacities. Notebooks extend the natural limits of memory. These facts are important, for all that they are obvious. The extended cognition hypothesis wants more. Some external devices and processes are literal parts of cognitive processes themselves. When there is fast and reliable access to external data or processes , then the cognitive processes that occur uncontroversially inside the brain literally and controversially extend out into the world (...) to incorporate external structures or processes. Retrieval of an address from memory and retrieval of the same information from a notebook can both be fully cognitive processes involving fully cognitive representations. So say the proponents of extended cognition.It is an attractive metaphor. But is there any good reason to take it as literal truth? According to Maria Markus, the development of a particularly open and interested moral-psychological disposition towards the other is critical to the endeavour of subjects to realize the decent society. Drawing on the work of George Herbert Mead, it will be argued that such a sense of decency involves not just a normative commitment to reciprocity but a reflexive appreciation of the significance of the other to the formation of the self. Meads sketches of intersubjective freedoms are shown to provide a (...) critical perspective on unjust social relations from the standpoint of the utopian horizon of a decent society. Meads conception converges, as well as contrasts, with Markuss perspective on civil society and the regulative ideal of the decent society. Significantly, Markuss account of the problem of the potential divergence between the decency of institutions and the decency of human relationships highlights the difficulties of interconnecting democratic justice and intersubjective freedoms. In any healthcare system in which demand exceeds supply—which means any typical public healthcare system—patients cannot always get the care they want or need when they want or need it. It is also unrealistic to suppose that it will ever be otherwise. There have been such advances in medicine and growth in the population that even if we forgot about all other goods such as education, roads, social services, and so forth and put the entire budget into healthcare, there would (...) still be a gap between supply and demand. Moreover, even if we could by that expedient make them match and had eyes only for health, we still should not. For it is now understood that healthcare is the least important determinant of health, lining up well behind poverty and social status. But if suboptimal care is to be our destiny, we must plan how it is to be delivered. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:Bookmarks Raman Selden's A Reader's Guide to Contemporary Literary Theory is now published in the United States by the University Press of Kentucky ($17.00 cloth, $7.00 paper). It is a discerning introduction for students (and anyone else) to the current state of "theory"—a word which in this context seems for the present to have lost its neutral sense. Given the tendentious climate of literary studies, Selden's book is all (...) the more remarkable for its sober, equable attitude. Whether he is describing Russian formalism, Lacan, the Frankfurt School, reader-response theory, or Gerald Prince's "narratee," he is always a fair guide, generous without descending to the breathlessness "theory" so often incites. While I miss a detailed or persuasive account ofArnoldian, New Critical, or Leavisite criticism to serve as a foil for his discussion of poststructuralism, Seiden does nevertheless usually give some idea of what or who the enemy is for each of his theorists. Selden's quotations are well chosen, and he is adroit in elucidating the internal problems of theories, as, for example, when he says that Cixous's approach runs the risk of "driving women into an obscure unconscious retreat where silence reigns interrupted only by uterine 'babble.' " This is a danger, according to Seiden, well understood by Kristeva, who views women writers as "caught." He makes the point by citing Kristeva. "On the one hand," he says, "as writers they inevitably collude with [what Kristeva describes as] 'phallic dominance, associated with the privileged father-daughter relationship, which gives rise to the tendency towards mastery, science, philosophy, professorships, etc....' " On the other hand, Kristeva continues, "we flee everything considered 'phallic' to find refuge in the valorisation of the silent underwater body, thus abdicating any entry into history." Raman Seiden does not attempt to dazzle, but there is substance for thought on almost every one of the 146 pages in his book. Far weightier are two literary theory anthologies which have come our way. Hazard Adams and Leroy Searle have edited Critical Theory Since 1965 (Florida State University Press, $29.95 paper). It resembles a telephone book and widi two-column pages it packs a lot in. The book begins with Stanley Cavell's 325 326Philosophy and Literature splendid essay "Aesthetic Problems of Modern Philosophy," and continues with Chomsky, John Searle, Kermode, Derrida and his American followers, Ingarden, Foucault,Jauss, Frye (a figure Seiden has chosen to ignore), Girard, Cixous and various feminists, Culler, Abrams—and on it goes. It was a pleasant surprise to find Clifford Geertz's "Blurred Genres" here; curiously missing is any contribution from Richard Rorty. And though the tide says the material starts in '65, Adams and Searle actually take us back farther. The main text ends with Said on page 622, but we are dien given a 233-page appendix—an anthology in itself—which begins with Frege and Peirce and goes through selections from various linguists and philosophers from Saussure, Bakhtin, and Whorf, to Heidegger on Hölderlin, Wittgenstein, Gadamer, and Lacan. Adams and Searle have contributed good introductions for each author, as well as a wide-ranging general introduction (Adams) and an afterword (Searle). In the last paragraph of his opening remarks, Adams says, "The theme of poststructuralism in its deconstructive phase is that poems resist interpretation. Demonstration of how this is so has been important (though not as new as some have thought), but repeated demonstration of this has already eventuated in its own form of reduction and thematization. Every text submitted to deconstruction considered as a critical method will yield the same theme, which may be true enough but remains only that story: the allegory of uninterpretability. It quickly becomes a repetition of the older formalist discovery that every text is about itself. It is no wonder that there has been a stirring—not so much a resistance to deconstruction (there has been that too, of course)—that expresses a desire to go beyond it. There has even been the idea of returning to Kant and trying again." Critical Theory Since 1965 is a big, useful book. Representational redescription (Karmiloff-Smith 1994a; 1994) translates implicit, procedural knowledge into explicit, declarative knowledge. Explicit knowledge is an enabling condition of cognitive flexibility. The articulation and inferential integration of knowledge are important in explaining flexibility. There is an interesting connection to the availability of knowledge for verbal report, but no clear explanatory work is done by the idea of knowledge that is available to consciousness.

Our intention is to contribute to the development of Canadian Nursing and Medical Education (NursMed) and efforts to redress deepening, intersecting health and social inequities. This paper addresses the following two research questions: (1) What are the ways in which Decolonial, Intersectional Pedagogies can inform Canadian NursMed Education with a focus on critically examining settler‐colonialism, health equity, and social justice? (2) What are the potential struggles and adaptations required to integrate Decolonial, Intersectional Pedagogies within Canadian NursMed Education in service of (...) redressing intersecting health and social inequities? Briefly, Decolonial, Intersectional Pedagogies are philosophies of learning that encourage teachers and students to reflect on health through the lenses of settler‐colonialism, health equity, and social justice. Drawing on critical ethnographic research methods, we conducted in‐depth interviews with 25 faculty members and engaged in participant observation of classrooms in university‐based Canadian NursMed Education. The research findings are organized into three major themes, beginning with common institutional features influencing pedagogical approaches. The next set of findings addresses the complex strategies participants apply to integrate Decolonial, Intersectional Pedagogies. Lastly, the findings illustrate the emotional and spiritual toll some faculty members face when attempting to deliver Decolonial, Intersectional Pedagogies. We conclude that through the application of Decolonial, Intersectional Pedagogies teachers and students can support movements towards health equity, social justice, and unlearning/undoing settler‐colonialism. This study contributes new knowledge to stimulate dialog and action regarding the role of health professions education, specifically Nursing and Medicine as an upstream determinant of health in settler‐colonial nations such as Canada, United States, Australia, and New Zealand. The COVID-19 pandemic has negatively impacted the psychosocial functioning of children and families. It is important to consider adversity in relation to processes of positive adaptation. To date, there are no empirically validated multi-item scales measuring COVID-related positive adaptation within families. The aim of the current study was to develop and validate a new measure: the Family Positive Adaptation during COVID-19 Scale. The sample included 372 female and 158 male caregivers of children ages 5–18 years old from the United Kingdom, (...) the United States, Canada, and Australia, who completed measures in May 2020. Participants responded to a 14-item survey indexing a range of perceived coping and adaptation behaviors at the beginning of the pandemic. An exploratory factor analysis yielded an optimal one-factor solution comprised of seven items related to family cohesion, flexibility, routines, and meaning-making. Multigroup confirmatory factor analysis demonstrated measurement invariance across female and male caregivers, demonstrating that the factor structure, loadings, and thresholds did not vary by caregiver sex. There was evidence for concurrent validity with significant bivariate correlations between the Family PACS scores and measures of caregiver positive coping, parenting practices, couple satisfaction, and family functioning, but not negatively-valenced constructs. Findings inform our conceptualization of how families have adapted to adverse pandemic-related conditions. Further, we provide preliminary support for the Family PACS as a practical tool for evaluating positive family adaptation during this global crisis, with implications for future widespread crises. This article takes the rupturing of normative, linear, reproductive time that occurs in the event of miscarriage as a potentially generative philosophical moment—a catalyst to rethink pregnancy aside from the expectation of child-production. Pregnant time is usually imagined as a linear passage toward birth. Accordingly, the one who “miscarries” appears as suspended within an arrested journey that never arrived at its destination, or indeed, as ejected from pregnant time altogether. But here I propose to rethink both pregnancy and miscarriage through (...) the lens of “suspended time”—a theoretical move that shifts the accent from the future as the dominating frame of reference to the lived present. Drawing on work by Kathryn Bond Stockton, Lauren Berlant, Lisa Baraitser, and others, the article explores overlooked temporalities of pregnancy and miscarriage that operate not in the mode of futural projection or futural loss, but rather through present-oriented forms of adjustment and sensing, attachment and intimacy, maintenance and care. By “suspending the future,” then, we can resist the oppositional framing of pregnancy and miscarriage, because if pregnant time is not represented in exclusively future-oriented terms as being-toward-birth, then miscarriage need not be understood as pregnancy's undoing. This overview of historiography suggests that the richness of the letters generates fresh scholarly questions and that Darwin, paradoxically, is becoming progressively deconstructed as a key figure in the history of science.

The academic journey to a widely acknowledged Machine Consciousness is anticipated to be an emotional one. Both in terms of the active debate provoked by the subject and a hypothesized need to encapsulate an analogue of emotions in an artificial system in order to progress towards machine consciousness. This paper considers the inspiration that the concepts related to emotion may contribute to cognitive systems when approaching conscious-like behavior. Specifically, emotions can set goals including balancing explore versus exploit, facilitate action in (...) unknown domains and modify existing behaviors, which are explored in cognitive robotics experiments. The influence of liberal political ideology on nursing sciencePrevious notions of science as impartial and value-neutral have been refuted by contemporary views of science as influenced by social, political and ideological values. By locating nursing science in the dominant political ideology of liberalism, the author examines how nursing knowledge is influenced by liberal philosophical assumptions. The central tenets of liberal political philosophy — individualism, egalitarianism, freedom, tolerance, neutrality, and a free-market economy — are primarily manifested in relation to: (i) the (...) individualistic focus of our science; (ii) our view of society as essentially egalitarian and equitable; (iii) our preference for politically neutral knowledge development, and (iv) an economy of knowledge development that supports rather than challenges the status quo. I argue that exposing, rather than ignoring, the liberal ideological values inherent in nursing science will render these assumptions open to debate, stimulate ongoing development of critically oriented knowledge, and increase our capacity to influence the social, political and economic determinants of health. Cornelius Castoriadis made a significant and distinctive contribution to the development of the notion of the dialectic of control. In the first instance, Castoriadis formulated an important reconceptualization and restatement of the Marxist conception of the central contradiction of capitalism. He argued that capitalism depended on the creativity of workers while excluding them from effective control. Similarly, Castoriadis sought to extend the Marxist analysis of those tendencies present within the structuration of the labour process that may prefigure a socialist reorganization (...) of production. Castoriadis’s analyses of capitalism during the phase of his involvement with Socialism or Barbarism are likewise informed by his assessment of state socialist regimes. In particular, this assessment provided important insights into the modalities of control in modern society and the complications of transcending forms of institutional domination in modernity. It will be argued that some of the distinctive intentions of Castoriadis’s later elucidation of the social imaginary can be traced to his interpretation of bureaucratic capitalism and that this is evident in his subsequent accounts of the capitalist imaginary. In his later theory, Castoriadis interprets the problem of the dialectic of control in terms of the relationship between instituting and instituted society. Castoriadis’s analysis of capitalism during the period of Socialism or Barbarism will be situated in the wider debates over capitalism at that time. Similarly, Castoriadis’s departure from some of the philosophical sources that influenced the development of the notion of the dialectic of control will be explored. (shrink)

Working women in the Caribbean and Latin America are more active in the labor market than their counterparts in most other regions of the world. Yet, they remain much less economically mobile than working men. Using research from a long-term study in Martinique, this article offers a new view of the cross-class construction of women's economic immobility. Research results suggest that irrespective of a woman's socioeconomic status, household structure, education, skills, or freedom from domestic chores, the organization of her work (...) is patterned in ways that preclude economic growth. When women try to “get ahead,” they invest more of their own time; men, by contrast, put others to work. I argue that these and other gender-based patterns of work organization and network management express a hidden but enduring legacy of a patriarchal value system. (shrink)

The current main source of transplantable organs is from heart-beating donors. These are patients who have suffered a catastrophic brain injury, been ventilated, declared dead by neurological criteria, and had their vital functions maintained mechanically until the point of transplantation. But the demand for organs far outstrips the supply, and these patients are not the only potential donors. The idea behind non-heart-beating transplantation is to expand the donor pool by including in it patients who are in hopeless conditions but who (...) are not dying because of brain injury and hence will not suffer the neurological death necessary to become heart-beating donors. As long as we continue to hold the so-called dead donor rule, according to which dying donors cannot have their organs taken before they are dead, this requires that death be able to be declared by alternative criteria, specifically by cardiopulmonary criteria. The challenge is to find such criteria that will identify a state that the public will readily recognize as death and that will facilitate non-heart-beating transplantation. a. (shrink)

Nursing Inquiry 2010; 17: 336–345 ‘Now we call it research’: participatory health research involving marginalized women who use drugsIn this paper, we discuss and analyse the strategies employed and challenges encountered when conducting a recent feminist participatory action research study with highly marginalized women who were illicit drug users in an inner city area of Vancouver, Canada. Through an analysis of the political economy of participatory praxis within current neoliberal contexts, we focus (...) on three main areas: (i) reconceptualizing the pragmatics of participation; (ii) the microeconomic implications of participatory research, including ethical issues in payment for research participation; and (iii) the value and limits of using research as a tool for activism and empowerment. We conclude with a brief discussion of what we see to be some of the most salient social justice implications arising from feminist and participatory approaches to health research within neoliberal political spaces. Non-medical sex selection is premised on the notion that the sexes are not interchangeable. Studies of individuals who undergo sex selection for non-medical reasons, or who have a preference for a son or daughter, show that they assume their child will conform to the stereotypical roles and norms associated with their sex. However, the evidence currently available has not succeeded in showing that the gender traits and inclinations sought are caused by a “male brain” or a “female brain”. Therefore, as (...) far as we know, there is no biological reason why parents cannot have the kind of parenting experience they seek with a child of any sex. Yet gender essentialism, a set of unfounded assumptions about the sexes which pervade society and underpin sexism, prevents parents from realising this freedom. In other words, unfounded assumptions about gender constrain not only a child’s autonomy, but also the parent’s. To date, reproductive autonomy in relation to sex selection has predominantly been regarded merely as the freedom to choose the sex of one’s child. This paper points to at least two interpretations of reproductive autonomy and argues that sex selection, by being premised on gender essentialism and/or the social pressure on parents to ensure their children conform to gender norms, undermines reproductive autonomy on both accounts. Quality improvement mechanisms increasingly use outcome measures to evaluate health care providers. This move toward outcome measures is a radical departure from the traditional focus on process measures. More radical still is the proposal to shift from relatively simple and proximal measures of outcome, such as mortality, to complex outcomes, such as quality of life. While the practical, scientific, and ethical issues associated with the use of outcomes such as mortality and morbidity to compare health care providers have been well (...) rehearsed, the specific concerns associated with the use of quality of life measures in quality of care research have received little attention. As with much research on quality of life there is a tendency to assume that the disadvantages are outweighed by the general virtue of “listening” to patients. In this paper we disagree with this assumption and argue that quality of life is a process, not an outcome. (shrink)

Such a perspective characterizes parental decisions to vaccinate their children as a collective action problem and reveals the presence of an incentive to free ride—to enjoy the benefits of others’ efforts to vaccinate their children without vaccinating one’s own.
