According to anti-reductionism in the epistemology of testimony, testimonial entitlement is easy to come by: all you need to do is listen to what you are being told. Say you like anti-reductionism; one question that you will need to answer is how come testimonial entitlement comes so cheap; after all, people are free to lie. This paper has two aims: first, it looks at the main anti-reductionist answers to this question and argues that they fail. Second, it goes on a (...) rescue mission on behalf of anti-reductionism. I put forth a novel anti-reductionist account, which I dub ‘Testimonial Contractarianism’. According to the view defended here, in virtue of the social contract in play, compliance with the norms governing speech acts is the default position for speakers. Talk of epistemic dilemmas is old talk in epistemology. But are there such things? In this paper I argue for modest skepticism about epistemic dilemmas. In order to do that, I first point out that not all normative conflicts constitute dilemmas: more needs to be the case. Second, I look into the moral dilemmas literature for inspiration and identify a set of conditions that need to be at work for a mere normative conflict to be a genuine normative dilemma. Last, (...) Proponents of evidence-based psychiatry (EBP) point to its ability to demonstrate the efficacy of various psychiatric treatments, promising improved mental health outcomes and more efficient use of healthcare resources as a result. Policymakers and insurers have embraced EBP in hopes that these goals will be realized. This paper defends a novel view of hermeneutical epistemic injustice. To this effect, it starts by arguing that Miranda Fricker’s account is too restrictive: hermeneutical epistemic injustice is more ubiquitous than her account allows. That is because, contra Fricker, conceptual ignorance is not necessary for HEI: hermeneutical epistemic injustice essentially involves a failure in concept application rather than in concept possession. Further on, I unpack hermeneutical epistemic injustice as unjustly brought about basing failure. ObjectiveTo examine the effect of family and perceived organizational support on the relationship between nurse adaptability and their experience with COVID-related PTSD (post-traumatic stress disorder) symptoms in frontline nurses working on COVID-19 units.BackgroundProximity to and survival of life-threatening events contribute to a diagnosis of PTSD, which is characterized by avoidance of reminders of trauma, intrusive thoughts, flashbacks of events, sleep disturbances, and hypervigilance. Using the job-demands and resource model, we examined the effect of adaptability, family support, and perceived organizational support (...) on PTSD symptoms for nurses during the COVID-19 pandemic. Specifically, we tested whether perceptions of environmental supports—i.e., family and organizational support—moderated the relationship between nurse adaptability and COVID-related PTSD symptoms.MethodsA sample of frontline nurses working on COVID-19 units during the COVID-19 pandemic in Texas (N= 277) participated in this cross-sectional, observational study. Frontline Nurses reported demographic information and completed surveys designed to measure adaptability, perceived organizational support, family support, and COVID-related PTSD symptoms.ResultsAdaptability was significantly positively correlated (medium effects) to perceived organizational and family support (r= 0.51 and 0.56, respectively,p< 0.01). Adaptability and perceived organizational support were also negatively correlated with COVID-related PTSD symptoms (medium effects). Adaptability was negatively correlated with COVID-related PTSD symptoms, supporting Hypothesis 1 (r= −0.43,p< 0.01). Perceived organizational support was also significantly negatively correlated with COVID-19-related PTSD symptoms (r= −0.30,p< 0.01). Family support was not significantly correlated with COVID-related PTSD but was positively related to experiencing COVID-related PTSD after other variables were accounted for.ConclusionFindings suggest that individual adaptability and organizational support may reduce PTSD severity in frontline nurses working during a crisis; however, family support may increase PTSD symptoms. We provide suggestions for strengthening individual adaptability and healthcare leadership including remaining highly engaged to show support by providing rapid communication, remaining calm during difficult circumstances, and maintaining a consistent, physical presence during difficult times. Moreover, our results suggest additional support for nurses with families to adapt to crisis. (shrink)

This paper argues that this debate is afflicted by a category mistake: strictly speaking, there is no such thing as an epistemic norm for action. To this effect, I introduce a distinction between epistemic norms and norms with epistemic content; I argue that while it is plausible that norms of the latter type will govern action in general, epistemic norms will only govern actions characteristically associated (...) with delivering epistemic goods. DNR decisions are frequently made in oncology and hematology care and physicians and nurses may face related ethical dilemmas. Ethics is considered a basic competence in health care and can be understood as a capacity to handle a task that involves an ethical dilemma in an adequate, ethically responsible manner. One model of ethical competence for healthcare staff includes three main aspects: being, doing and knowing, suggesting that ethical competence requires abilities of character, action and knowledge. Ethical competence can be (...) developed through experience, communication and education, and a supportive environment is necessary for maintaining a high ethical competence. The aim of the present study was to investigate how nurses and physicians in oncology and hematology care understand the concept of ethical competence in order to make, or be involved in, DNR decisions and how such skills can be learned and developed. A further aim was to investigate the role of guidelines in relation to the development of ethical competence in DNR decisions. Individual interviews were conducted with fifteen nurses and sixteen physicians. The interviews were analyzed using thematic content analysis. Physicians and nurses in the study reflected on their ethical competence in relation to DNR decisions, on what it should comprise and how it could be developed. The ethical competence described by the respondents related to the concepts being, doing and knowing. In order to make ethically sound DNR decisions in oncology and hematology care, physicians and nurses need to develop appropriate virtues, improve their knowledge of ethical theories and relevant clinical guidelines. Ethical competence also includes the ability to act upon ethical judgements. Continued ethical education and discussions for further development of a common ethical language and a good ethical working climate can improve ethical competence and help nurses and physicians cooperate better with regard to patients in relation to DNR decisions, in their efforts to act in the best interest of the patient. BackgroundIn cancer care, do not resuscitate (DNR) orders are common in the terminal phase of the illness, which implies that the responsible physician in advance decides that in case of a cardiac arrest neither basic nor advanced Coronary Pulmonary Rescue should be performed. Swedish regulations prescribe that DNR decisions should be made by the responsible physician, preferably in co-operation with members of the team. If possible, the patient should consent, and significant others should be informed of the decision. Previous studies (...) have shown that physicians and nurses can experience ethical dilemmas in relation to DNR decisions, but knowledge about what ethical reasoning they perform is lacking. Therefore, the aim was to describe and explore what ethical reasoning physicians and nurses apply in relation to DNR-decisions in oncology and hematology care.MethodsA qualitative, descriptive and explorative design was used, based on 287 free-text comments in a study-specific questionnaire, answered by 216 physicians and nurses working in 16 oncology and hematology wards in Sweden. Comments were given by 89 participants.ResultsThe participants applied a situation-based ethical reasoning in relation to DNR-decisions. The reasons given for this were both deontological and utilitarian in kind. Also, expressions of care ethics were found in the material. Universal rules or guidelines were seen as problematic. Concerning the importance of the subject, nurses to a higher extent underlined the importance of discussing DNR-situations, while physicians described DNR-decisions as over-investigated and not such a big issue in their daily work.ConclusionThe study revealed that DNR-decisions in oncology and hematology care gave rise to ethical considerations. Important ethical values described by the participants were to avoid doing harm and to secure a peaceful and “natural” death with dignity for their dying patients. A preference for the expression “allow for natural death” instead of the traditional term “do not resuscitate” was found in the material. (shrink)

According to the achievement account of the value of knowledge, knowledge is finally valuable because it is a species of a finally valuable genus, achievement. The achievement account is said to solve Pritchard's tertiary value problem, the problem of showing that knowledge enjoys a different kind of value than mere true belief. This paper argues, first, that AA fails to solve TVP, and, second, that Pritchard's motivations for TVP are inadequate. They do, however, motivate a weaker value problem, one that (...) can be solved by showing that knowledge is weakly superior to mere true belief. Finally, third, we argue that knowledge does enjoy this form of weak superiority. Since this argument rests on AA's key species-genus claim, AA can retain the upper hand in the debate. (shrink)

Veganic agriculture, often described as farming that is free of synthetic and animal-based inputs, represents an alternative to chemical-based industrial agriculture and the prevailing alternative, organic agriculture, respectively. Despite the promise of veganic methods in diverse realms such as food safety, environmental sustainability, and animal liberation, it has a small literature base. This article draws primarily on interviews conducted in 2018 with 25 veganic farmers from 19 farms in the United States to establish some baseline empirical research on this farming (...) community. Its qualitative perspectives illuminate farmer perceptions of and experiences with veganic growing, including definitions, knowledge acquisition, values, and challenges. Results highlight a lack of agreement about the meaning of veganic agriculture in terms of allowable inputs and scope. Participants have drawn on a wide array of veganic and non-veganic resources to ascend their veganic production learning curves, also relying on experimentation and trial-and-error. Their farming is motivated by a diversity of real and perceived benefits, most notably consistency with veganism, food safety advantages, and plant and soil health benefits. Veganic product sourcing and the dearth of veganic agriculture-specific resources present considerable challenges to farmers. The article briefly discusses possibilities for developing veganic agriculture in the United States, such as through a US-based certification system and farmers’ associations, based on considerations of the trajectory of the US organic farming movement and veganic developments in Europe. Finally, the article suggests the importance of expanded research into soil health and fertility in plant-based systems to support practicing and potential veganic farmers. (shrink)

In recent literature, several authors attempt to naturalize epistemic normativity by employing an etiological account of functions. The thought is that epistemic entitlement consists in the normal functioning of our belief-acquisition systems, where the latter acquire the function to reliably deliver true beliefs through a history of biological benefit.

Artificial intelligence (AI) technologies, like machine learning and natural language processing, present previously unheard-of chances to quickly and accurately evaluate enormous volumes of healthcare data, assisting with clinical decision-making and enhancing patient outcomes. Aim thorough examination and analysis of artificial intelligence's impact on healthcare leadership, with a particular emphasis on present nursing trends and their implications for the future. The study (...) tries to uncover the advantages, difficulties, and consequences of AI integration by looking at how AI technologies including clinical decision support systems, predictive analytics, robots, natural language processing, and telehealth are being used in nursing practice. Method: A comprehensive analysis including research articles published between 2015 and 2024 was carried out. To give a thorough overview of AI's present and future uses in healthcare, major themes and trends were found and summarized. Results: By stressing AI's role in improving diagnostic accuracy and patient outcomes, the study highlights the technology's major contributions to drug discovery, virtual patient care, and medical imaging. Human-centered design concerns, the necessity of educational changes, and ethical challenges surrounding the application of AI surfaced as crucial topics needing attention. Conclusion: AI has enormous potential to transform healthcare by enhancing operational effectiveness, optimizing the delivery of care, and increasing diagnostic precision. Still, ethical issues must be resolved, interdisciplinary cooperation must be promoted, and educational frameworks must be improved in order to provide healthcare workers with the necessary AI skills. Background: Within oncology and hematology care, patients are sometimes considered to have such a poor prognosis that they can receive a do not resuscitate order from the physician responsible, stipulating that neither basic nor advanced coronary pulmonary rescue be performed in the event of a cardiac arrest. Studies on do not resuscitate decisions within oncology and hematology units, focusing on the specific role of the nurse in relation to these decisions, are scarce. Objective: The aim of this study was to (...) investigate hematology and oncology nurses’ experiences and perceptions of do not resuscitate orders, in order to achieve a deeper understanding of the nurses’ specific role in these decisions. Research design: A qualitative, descriptive methodology with individual semi-structured interviews was used. Participants and research context: A total of 15 nurses from eight hematology/oncology wards in four hospitals in Sweden were interviewed individually. Ethical considerations: In accordance with national regulations, an ethical review was not required for this study. The research followed international guidelines for empirical research, as outlined in the Helsinki Declaration. Findings: The nurses strived for good nursing care through balancing harms and goods and observing integrity and quality of life as important values. Experienced hindrances for good care were unclear and poorly documented decisions, uninformed patients and relatives, and disagreements among the caregivers and family. The nurses expressed a need for an ongoing discussion on do not resuscitate decisions, including all concerned parties. Conclusion: In order to provide good nursing care, nurses need clear and well-documented do not resuscitate orders, and patients and relatives need to be well informed and included in the decisions. This chapter develops a novel Neo-Moorean view. The view falls squarely within the Radical Neo-Moorean camp, in that it holds that closure holds unrestrictedly, warrant transmits through Moore’s inference, and that there is nothing wrong – epistemically or dialectically – with Moore’s argument. Nevertheless, the account is superior to extant Radical Neo-Mooreanisms in explanatory power: it explains both the precise variety of epistemic failure exhibited by the sceptic, and the intuition of reasonableness when it comes to the sceptic’s resistance to (...)
