Aim: Patients with advanced cancer need information about end-of-life treatment options in order to make informed decisions. Clinicians vary in the frequency with which they initiate these discussions.Patients and methods: As part of a long-term longitudinal study, patients with an expected 2-year survival of less than 50% who had advanced gastrointestinal or lung cancer or amyotrophic lateral sclerosis were interviewed. Each patient’s medical record was reviewed at enrollment and at 3 months for evidence of the discussion of patient wishes concerning (...) ventilator support, artificial nutrition and hydration , resuscitation and hospice care. A Kaplan–Meier analysis was also performed and 2-year survival calculated.Results: 60 cancer and 32 ALS patients were enrolled. ALS patients were more likely than cancer patients to have evidence of discussion about their wishes for ventilator support , ANH , DNR and hospice care . At 6 months, 91% of ALS patients were alive compared with 62% of cancer patients; at 2 years, 63% of ALS patients were alive compared with 23% of cancer patients .Conclusions: Cancer patients were less likely than ALS patients to have had documented advanced care planning discussions despite worse survival. This may reflect perceptions that ALS has a more predictable course, that advanced cancer has a greater number of treatment options, or differing views about hope. Nevertheless, cancer patients may be less adequately prepared for end-of-life decision-making. (shrink)

Problems concerning future populations help us better understand what it is for one possible future, or world, to be morally better than another. The repugnant conclusion challenges traditional forms of consequentialism, including utilitarianism. A key issue is whether, per the traditional view, we make things morally better by creating additional people whose lives are worth living and by creating nonidentical better-off people in place of less well-off but distinct people. Or can we, instead, make things morally better only by making (...) things better for some particular person or another? Is the structure of morality aggregative in nature or instead person-affecting in nature? While the latter, non-traditional view may tempt us, it also seems to give rise to the nonidentity problem and various inconsistency objections. Can those problems be resolved? Questions raised by Parfit and others decades ago remain unsettled today but as critical as ever. (shrink)

This chapter examines Soren Kierkegaard's view about the concept of love. It suggests that Kierkegaard's ideas about love can be found in Works of Love, which contains a series of deliberations on the Judeo-Christian commandment to love one's neighbour as oneself. The chapter also discusses episodes of the story of human love in Kierkegaard's earlier works, his Either/Or, Fear and Trembling, and Philosophical Fragments. It also argues that Kierkegaard's philosophical, literary, and theological explorations reveal that love is filled with paradox.

Jeffrey Bishop’s The Anticipatory Corpse demonstrates how death is present in and cloaked by contemporary practices of end-of-life care. A key to Bishop’s argument is that for modern medicine the cadaver has become epistemologically normative and that a metaphysics shorn of formal and final causes now shapes contemporary healthcare practices. The essays of this symposium laud and interrogate Bishop’s argument in three ways. First, they raise critical methodological challenges from the perspectives of human rights, Charles Taylor’s concept of social imaginaries, (...) and economics. Second, they demonstrate the analytical power of his argument by detailing how it might be extended to additional issues beyond simply end-of-life care and how it might be brought into conversation with sociology. Third, they engage the constructive turn Bishop takes at the end of the book. Bishop himself also updates readers on the reception of The Anticipatory Corpse, as well as the way his thinking has evolved over the past 5 years since its publication. Tarekat teaches its followers not to glorify conglomerates, to keep their distance from leader, and to live in a way of zuhud, including in the matters of politics. But the Tarekat Qadiriyah wa Naqsyabandiyah in Cukir Jombang was actually involved in practical politics as a supporter of Partai Persatuan Pembangunan. This paper, which was written in the form of ethnographic design according to the concept of the power relations of Foucault to find new perspectives about the actions of Tarekat communities (...) in politics, in this case explaining the pattern of relations that occurred between the Tarekat Cukir and PPP and discovering the factors causing Tarekat Cukir to support PPP. This paper concludes that between Tarekat Cukir and PPP, there is a mutually beneficial pattern of relations in terms of proximity, but in terms of economic politics, the Tarekat Cukir feels disadvantaged. The dominant reason the Tarekat Cukir supports PPP is because of the dependence of Santri on their Mursyid who is a PPP politician. This is reinforced by the history between the two parties and the spiritual ties that connect the teachers who are both PPP politicians and Santris. (shrink)

This article explores yet another paradox – aside from the privacy paradox – related to the datafication of media: citizens trust least the media they use most It investigates the role that daily life plays in shaping the trust that citizens place in datafied media. The study reveals five sets of heuristics guiding the trust assessments of citizens: characteristics of media organisations, old media standards, context of use and purpose, experiences of datafication and understandings of datafication. The article discusses the (...) use of these heuristics and the value that everyday life holds in assessing trust in datafied media. It concludes that, guided by a partial ‘structure of perception’ and enticed into trusting datafied media in the context of their daily lives, citizens may be highly concerned by the datafication of media but use them nevertheless. Cézanne's approach was later adopted by his admirer Maurice Merleau-Ponty, who contributed to it mainly by his invisible dimension of the thought. The close relation between Cézanne's painting and Merleau-Ponty's philosophical work, which in a way transforms the paintings into the words of an elocative language, is the (...) evidence of the bonds existing between philosophy and the works of art. The concepts of "spirit" [dukh], "spiritual," and "spirituality" have had a strange fate in philosophy. It seems that they are constantly and more widely utilized in the literature—as well as when the solution of the basic question of philosophy is formulated as the "relation of matter and spirit," when the "spiritual life of society" or "spiritual culture" is investigated, and when the issue concerns the "spirituality" of the individual as the manifestation of a high level of development of the socialist (...) personality. At the same time, the categorial status of these concepts not only is not recognized in our philosophy, but in encyclopedic dictionaries and monographs it is asserted that "spirit" is an obsolete concept, a favorite of idealistic philosophy, and that in Marxist science it is replaced by the concept of "consciousness.". J. Solomon [Journal de Physique 4, 34 (1933)] produced an argument of great generality claiming to demonstrate the impossibility of hidden variables in quantum theory, an argument which M. Jammer [The Philosophy of Quantum Mechanics(Wiley, New York, 1974)] said raised a number of questions. For the first time, this argument is discussed, a simple hidden variable model violating the argument is analysed in detail, and the error in the proof is located.
