Initial responses to questionnaires used to assess participants' understanding of informed consent for malaria vaccine trials conducted in the United States and Mali were tallied. Total scores were analyzed by age, sex, literacy (if known), and location. Ninety-two percent (92%) of answers by United States participants and 85% of answers by Malian participants were correct. Questions more likely to be answered incorrectly in Mali related to risk, and to the type of vaccine. For adult participants, independent predictors of higher scores (...) were younger age and female sex in the United States, and male sex in Mali. Scores in the United States were higher than in Mali (P = 0.005). Despite this difference participants at both sites were well informed overall. Although interpretation must be qualified because questionnaires were not intended as research tools and were not standardized among sites, these results do not support concerns about systematic low understanding among research participants in developing versus developed countries. QuigleyIn 1878, Charles Sanders Peirce closed the first section of "How to Make our Ideas Clear"—an article that William James later declared a "birth certificate of Pragmatism"—on a strangely anecdotal note.1 Using what would become known as the pragmatic method to demolish the notion of Grand Ideas ("Our idea of anything is our idea of its sensible effects"), Peirce (...) also included a lesson from an "old German story":Many a man has cherished for years some vague shadow of an idea, too meaningless to be positively false; he has nevertheless, passionately loved it, has made it his companion by day and by night, and has given to it his strength and his life, leaving all other occupations for its sake, and in short has lived with it and for it, until it has become, as it were, flesh of his flesh and bone of his bone; and then he has waked up some morning to find it gone, cleaned vanished away like the beautiful Melusina of the fable, and the essence of his life gone with it. But Peirce's anecdote did more than further his argument; it was an autobiographical aside—so personal, in fact, that he ordered the passage to be deleted in all future printings of his essay.Melusina was not just an allusion to a mythological figure; it was also a reference to the middle name of his wife, Harriet Melusina Fay. Since his schooldays, Peirce's friend and, later, wife, whom he called [End Page 362] "Zina," had been his constant companion and even the scribe for his fervent aspirations. Under the title "Theories of C. S. Peirce, 1854" she had transcribed his ambitious declaration: "My life is built upon a theory: and if this theory turns out false, my life will turn out a failure" (Brent, p. 51). By the mid 1870s, Peirce's theory, "passionately loved... his companion by day and by night," was finally being elucidated in his articles; however, Melusina herself had "cleaned vanished away," after two decades of intimacy, leaving Peirce in Paris to what his friend Henry James described as "a very lonely and dreary existence." Peirce's inclusion of an anecdote about his own loneliness—"I have myself known such a man"—is poignant even as it reveals his anger. Melusina is both a monster and "beautiful," and with her disappearance, Peirce has lost, "the essence of his life." The personal allusion also highlights the anxiety in the young Peirce's writing. What if his theories came to nothing, remained merely vague, and he lost his whole life and his wife to a misguided dedication? And how ironic would that be given that the idea itself aimed to explode the fallacy of grand ideas? The purpose of this essay is to investigate and critically analyse some of the formative factors which led to the spiritual maturation of a leading Vīraśaiva saint, Basava. This inquiry focuses on a single event in the life of this great reformer of medieval times, i.e. his spiritual conflict leading to his rejection of the upanayana ceremony. The study will proceed through an investigation of the earliest and subsequent sources which veil the personality of Basava. The traditional view will be (...) challenged as one-sided. Using covariant derivatives and the operator definitions of quantum mechanics, gauge invariant Proca and Lehnert equations are derived and the Lorenz condition is eliminated in U(1) invariant electrodynamics. It is shown that the structure of the gauge invariant Lehnert equation is the same in an O(3) invariant theory of electrodynamics.

The phenomenon of Petro Mohyla in the modern humanitarian university is most often viewed precisely from the point of view of understanding his figure not only as a building Church, its defender, in a sense as a Christian conservative, but also as a guardian and building national statehood, creator of religious-national transformation and state-renewal Khmelnytsky era and - later - Mazepa. As the History of the Orthodox Church in Ukraine rightly points out, "after 1632, when the Commonwealth entered into the (...) reign of Wladyslaw IV and was headed by Petro Mohyla of Kiev, the Orthodox Church also entered a new era of its existence. The government's new course in resolving the religious issue, increasing political activity of the Orthodox community gave the Kyiv Metropolitanate the opportunity to restore legal status, to regulate relations with the state and society… This authority … was conditioned by the ability of the church institution to be full-blooded… the inner life of the church became perhaps the overriding task of Petro Mohyla... Background: Debates over legalisation of physician-assisted suicide or euthanasia often warn of a “slippery slope”, predicting abuse of people in vulnerable groups. To assess this concern, the authors examined data from Oregon and the Netherlands, the two principal jurisdictions in which physician-assisted dying is legal and data have been collected over a substantial period.Methods: The data from Oregon comprised all annual and cumulative Department of Human Services reports 1998–2006 and three independent studies; the data from the Netherlands comprised all four (...) government-commissioned nationwide studies of end-of-life decision making and specialised studies. Evidence of any disproportionate impact on 10 groups of potentially vulnerable patients was sought.Results: Rates of assisted dying in Oregon and in the Netherlands showed no evidence of heightened risk for the elderly, women, the uninsured , people with low educational status, the poor, the physically disabled or chronically ill, minors, people with psychiatric illnesses including depression, or racial or ethnic minorities, compared with background populations. The only group with a heightened risk was people with AIDS. While extralegal cases were not the focus of this study, none have been uncovered in Oregon; among extralegal cases in the Netherlands, there was no evidence of higher rates in vulnerable groups.Conclusions: Where assisted dying is already legal, there is no current evidence for the claim that legalised PAS or euthanasia will have disproportionate impact on patients in vulnerable groups. Those who received physician-assisted dying in the jurisdictions studied appeared to enjoy comparative social, economic, educational, professional and other privileges. (shrink)

Given the complexity of modern health care, there exists an urgent need to discover how best to resolve complex bioethical issues. Traditionally, principle based ethics provided the benchmark for guiding ethical decision-making. More recently, however, it has become apparent that this traditional approach is often inadequate in dealing with cur rent health care dilemmas. The notion of caring was advanced initially as an alternative to, then as a complement to, principle based ethics. In this article, caring is conceptual ized as (...) an attitude and is viewed as integral to the advancement of a coherent and inte grated moral approach to ethical decision-making. First, a brief historical description of bioethics is presented. Next, an evolutionary account of caring within bioethics is described. Four fundamental problems associated with the use of caring within bioethics are then outlined. Finally, caring as an attitude is delineated and a case study is used to illustrate the proposed conceptualization of caring. The case study demonstrates that a caring attitude provides for relationship and context, which are elements often neglected by traditional approaches. (shrink)

Cultural pluralism in the Balkans has often been considered as the source of conflict in the region. Against this perspective it is suggested that Enlightenment political thought in southeastern Europe, as represented by the radical republicanism of Rhigas Velestinlis , incorporated the idea of cultural pluralism in a project for a unitary democratic state, modelled on the 'Republic of Virtue', that was expected to replace despotism and to transform its subjects into free citizens. The political culture of this state would (...) be provided by the principles of republican Hellenism while its ethnic pluralism would be located in its civil society. It is stressed that republican Hellenism was a political and moral, not an ethnic, project. This political project however, was preempted by the emergence of cultural nationalism that cancelled the Enlightenment vision of democratic citizenship, equality and recognition for individuals as well as ethnic groups in a unitary republic.
