Survival after solid-organ transplantation has improved significantly, and many contemporary transplant recipients are of childbearing potential. There are limited data to guide decision-making surrounding pregnancy after transplantation, variations in clinical practice, and significant knowledge gaps, all of which raise significant ethical issues. Post-transplant pregnancy is associated with an increased risk of maternal and fetal complications. Shared decision-making is a central aspect of patient counselling but is complicated by significant knowledge gaps. Stakeholder interests can be in conflict; exploring these tensions can (...) help patients to evaluate their options and inform their deliberations. We argue that uniform, evidence-based recommendations for pregnancy after solid organ transplantation are needed. Conducting research, including patient-engaged studies, in this area should be priority for the transplant community. (shrink)

In recent years, there has been significant interest in injustices that do not consist in inflicting physical or material harm on others, but operate in more subtle ways, e.g. by targeting our status as epistemic agents. In a similar fashion, this paper aims to bring to the forefront a currently overlooked kind of injustice that occurs in relation to our concepts: conceptual injustice, which is characterised by wrongful in- or exclusion from the application of a concept. The first part of (...) the paper is concerned with spelling out the notion, discussing its characteristic wrongs, and with tracing it in a number of examples. In the second part, I discuss conceptual injustice in relation to connected but different forms of injustice, such as epistemic injustice. This highlights the advantages of having the notion on hand: it enhances our understanding of other forms of injustice like hermeneutical injustice, and allows us to capture a ‘remainder of injustice’ for which we could not account otherwise. Such an absence seems ultimately fatal. Yet as this volume amply demonstrates, the richness of his social theory can be opened up by contemporary feminism.

As has been widely observed, histories of feminism have often been conceived via notions of generation where feminism is positioned as a kind of familial property, a form of inheritance and legacy which is transmitted through generations. Thus feminism and its history have been imagined as following a familial mode of social reproduction. Despite the dominance of this model, it has nonetheless been subject to critique, not least because of its reliance on teleological and progressive notions of history. Judith Roof, (...) for example, has called for new conceptualizations of temporality in an attempt to decouple histories of feminism from a generational model. This need to move away from a generational model is also underscored by what commentators such as Angela McRobbie have identified as a dispersal of feminism – a broad-scale circulation of feminist values in popular culture – as well as by the emergence of a certain kind of reflexivity whereby past and current feminist practices are increasingly subject to critique. For McRobbie such developments are understood as part of the articulation of a post-feminism in which feminism can no longer be passed on as it is positioned as having already passed away. However, in this article via an exploration of recent accounts of the passing of feminism it is suggested that what allows for a declaration of this passing is an attachment to feminism as a particular way of knowing as well as to a particular arrangement of the social. Indeed, it will be suggested that recent histories of feminism and commentaries on the state of contemporary feminism are driven not only by generational logics but also by these attachments – attachments which lose and fix both feminism and women in time. In this article the increasing significance of Bourdieu’s social theory is mapped in recent sociological accounts of gender in late-modern societies. What is highlighted in particular is the influence of Bourdieu’s social theory, and especially his arguments regarding critical reflexivity and social transformation, on a specific thesis which is common to a number of contemporary feminist accounts of gender transformations in late modernity. Here it is suggested that in late modernity there is a lack of fit between habitus and field (...) in certain public spheres of action via an increasing transposition or movement of the feminine habitus from private to public spheres, which is linked to a heightened critical reflexivity vis-à-vis gender and to detraditionalization. In this article, however, a number of limits regarding this line of argument are highlighted, especially those flowing from the unproblematic coupling of reflexivity with detraditionalization. This exploration in turn leads to a critical discussion of Bourdieu’s ideas regarding social transformation. This article focuses on the new economy. While a number of recent analyses have considered how new economic arrangements rework a range of material relations, this article suggests that such considerations have tended to stop short of considering how material relations may be reconstituting vis-à-vis the people who are working in the new economy. This is so, it will be argued, because there is a pervasive assumption of what is termed a social contract model of personhood, where people are assumed (...) to own or at least to strive to accumulate skills, capacities and abilities (that is, labour power) as forms of property in the person. However, what this article underscores is that the relations between people and their labour in the new economy are being reworked. In particular, it highlights how qualities previously associated with people are being disentangled and are becoming the object of processes of qualification and re-qualification. However, such qualities do not take the form of property in the person as claims towards the ‘ownership’ of this labour lie in the domain of audience effects - that is, in relations external to the person. This shift, it is argued further, locates labour in the new economy firmly in the domain of the production and circulation of cultural value. Indeed, more broadly this article suggests that the now commonplace idea that economy and culture are de-differentiating involves a previously unrecognized restructuring of the relations between people and property and even the end of the modern notion of personhood. Felicitas Goodman (1990) observed that naive participants experienced unique trance states, characterized by specific visionary content, when they assumed particular postures and listened to monotonous rattling. Students (n = 284), enrolled in various sections of the course Introduction to Psychology, experienced one of four conditions with their eyes closed: Sitting Quietly with and without Drumming, Standing (Feather Serpent) Posture plus Drumming with and without Suggested Experiences. Participants completed the Phenomenology of Consciousness Inventory (Pekala 1982, 1991c) and wrote narratives following their (...) respective stimulus conditions. Participants reported less imagery, joy, and absorption, but more internal dialogue, self‐awareness, and muscular tension in the posture, compared to the sitting quietly, conditions. Goodman's results of specific posture (content‐related) effects with naive participants were not replicated. (shrink)

In “Bourdieusian reflections on language: Unavoidable conditions of the real speech situation”, Simon Susen proposes that Bourdieu’s account of language is based on a number of ontological presuppositions. While the extensive commentary on Bourdieu’s analysis of language tends to bracket these assumptions—not least because of an enduring attachment to the “sociological Bourdieu”—Susen insists that a recognition of the ontological features of language is consistent with Bourdieu’s own writings. While Susen’s ontological retrieval may be controversial, especially to those attached to the (...) “sociological Bourdieu”, in this reply I suggest that in “Bourdieusian reflections on language” we find something more interesting than any such controversy. Specifically, Susen’s ontological retrieval opens out Bourdieu’s oeuvre to currents in the social sciences which question the limits of the sociology of the social in the context of an increasingly ontological life. In this reply, I ask how Susen’s “ontological Bourdieu” fares in a dialogue with this body of work. (shrink)

This article engages the crisis of measure currently being articulated within social and cultural theory and the associated claim that this crisis should compel an embrace of methods which seek to know the heterogeneous, the multiple, the complex and the vague. Taking the rise of immaterial forms of labour and value as paradigmatic of the crisis of measure, it questions the use of the figure of a domestically labouring woman who lacks ownership of her labour to illuminate this crisis, as (...) well as the structural equivalence currently being forced between ‘women’s work’ (especially the work of social reproduction) and productive activities. It does so with reference to the changing relationship between women and social reproduction, a changing relationship which suggests that at the heart of the crisis of measure is a restructuring of time. Cognitive science can help literary scholars formulate specific questions to be answered by archival research. This essay takes, as its starting point, embedded mental states (that is, mental states about mental states) and their role in generating literary subjectivity. It then follows the transformation of embedded mental states throughout several manuscripts of Christa Wolf’s autobiographical novel, Patterns of Childhood (Kindheitsmuster, 1976), available at the Berlin Academy of Arts. The author shows that later versions of Patterns of Childhood have more complex (...) embedments in the chapter describing the adolescent protagonist’s relationship with her schoolteacher. This textual development is integral to the process whereby the presumably authentic memories of the past are constructed to fit the present needs of the person who is doing the remembering. Accompanying the three case studies of the manuscript revision is a discussion of theoretical and practical implications of this “cognitive-archival” approach to literature. (shrink)

Recent social and cultural theory has emphasized that in risk culture the achievement of a reflexive self-identity is a key resource, for example, in terms of employment, citizenship and intimacy. Commentators on shifts in the organization of health have also stressed the significance of achieving a self-reflexive identity. So, for example, knowing, self-monitoring subjects have emerged as optimal citizens in relation to health. While there is certainly some critical commentary on these kinds of moves, nevertheless reflexive sexual subjects in relation (...) to health have received less critical attention. In this article, through an analysis of HIV antibody testing and, in particular, by looking at the making of reflexivity through the practice of testing, I consider the emergence of a heterosexuality defined in terms of reflexivity. In so doing I suggest that reflexivity should not be understood - as many commentators suggest - as an effect but rather as constitutive of risk culture. (shrink)

The use of fictional models is extensive and rewarding in modern science. This fact captured the attention of philosophers of science, who are focusing on questions such as the following: is it possible for a fictional model to be explanatory? And, if so, in virtue of what is such a fictional model explanatory? In this paper, I discuss these questions in relation to the realism vs. anti-realism debate in philosophy of science. I focus on work developed by Alisa Bokulich who (...) has argued for the explanatory role played by fictional models, claiming that it can be accommodated by realism in ‘a broad sense’. To this end, Bokulich introduced what she called ‘an eikonic conception’ of explanation, as opposed to the so-called ‘ontic’ conception. This move, Bokulich claims, allows non-causal explanations to be acceptable to a modified version, as opposed to a ‘naïve’ brand of realism. Roughly, a fictional model can capture aspects of reality if it manages to act as a proxy for a theory targeting a physical system. As Bokulich explains with the example of James Clerk Maxwell’s use of mechanical models, this is due to structural correspondences between a model and a theory at a certain level of abstraction. However, Bokulich does not explain how the model grasps reality. In this paper, I fill this missing piece of the puzzle. While I agree with Bokulich on the explanatory function of fictional models, I shall invoke her reading of Maxwell’s work to draw a different conclusion. Specifically, I argue that structural correspondences at various levels form what I call ‘a ladder of abstractions’ which connects aspects of a target system to the corresponding depths of descriptions. Modifying in these terms the scope of ‘representing a system’, I suggest that Bokulich’s proposed modification of realism is obviated. (shrink)

Excellent patient care within the emergency department requires interdisciplinary training, teamwork, and communication to manage the chaos of the environment. Specifically, invasive procedures required to manage airway, breathing, and circulation via intubation, chest compressions, and establishing intravenous access can provide a direct benefit to save lives but also have the potential to harm both patients and health care clinicians alike; emergency health care clinicians can be exposed to significant amounts of blood and body fluids as well as other threats of (...) physical and psychological harm. The ethical components of care in this environment are often under‐recognized due to the need for rapid patient assessment and immediate action. Moreover, challenges to practice that can include lack of qualified personnel, equipment, and other resources to provide safe care to a large volume of patients can lead to moral distress in ED staff. Because the ED is a high‐uncertainty, high‐acuity environment, continuing interprofessional communication, collaboration, and planning is critical. Opportunities for multidisciplinary policy dialogue and the development of professional guidelines can make the ED a safer environment for both patients and providers. (shrink)

Background: Cone Beam Computed Tomography (CBCT), the focus of a number of radiotherapy fundraising campaigns in the mid-2000s, was introduced accompanied by a fanfare of newness and discourses of ‘hope’, ‘inspiring clinical confidence’ and ‘accuracy’. The CBCT system, used in the delivery of Radiotherapy treatment, was incorporated into strategic planning priorities across the United Kingdom based on a rationale of self-evidence. During this time, the way in which the new system was discussed with patients was variable. Research objectives: The purpose (...) of this study was to uncover how experimental practices were embedded and enacted during the use of a new technological system, specifically relating to how patients were enrolled during introductory phases of technology adoption. Research design and context: Drawing on ethnographic work and interviews with staff members in one hospital, the study examines staff discussions prior to the introduction of the Cone-Beam CT imaging system in radiotherapy. It considers how staff views were at odds with practices that occurred during the ‘experimental’ stages of use and how these were shared with patients. Ethical considerations: Approval was obtained from the Local National Health Service Research Ethics Committee and National Health Service Main Research Ethics Committee (REC 07/Q1308/16) for the interview and ethnographic stages, respectively. All names have been changed and participants signed a consent form. Findings: Staff reported a lack of evidence, absence of proof and perturbing doubts with the X-ray volumetric imaging. Both patients’ and practitioners’ partial understanding about the risks and benefits of the system created incommensurable ideas regarding its use and what the patients’ role was during these introductory stages. Conclusion: Maintaining partial truth telling renders patients’ experiences of new treatment at odds with ‘experimental’ practice. This has wide-reaching implications for practice. (shrink)

Despite many obvious differences, the ruins of the atomic attack on Hiroshima and at the post-9.11 World Trade Center generate a number of shared characteristics concerning struggles over the reconstruction, commodification of memory, and the contestations over historical knowledge with which to explain what led to such violence. The paper discusses the politics of memory in these two sites of victimization, but also contrast it with emerging discourse on yet another lesser known instance of violence against women of Asia and (...) Pacific who fell victim to the wartime Japanese military sexual enslavement. In so doing, the paper begins to explore some of the elements that generate boundaries of compassion and amnesia toward violence against the racialized, colonized, gendered bodies. (shrink)

While the Internet has emerged as a significant resource for women negotiating the questions and circumstances that arise during conception, pregnancy and childbirth, it remains unclear what role the Internet plays in challenging the current biomedical paradigm and empowering women to make meaningful choices. This article explores how women use the Internet to manage their pregnancies and mediate their doctor–patient relationships, particularly examining the role of social class and personal health history in shaping such Internet use. Drawing from in-depth interviews (...) with white middle-class mothers, the findings show that rather than using technology to resist the dominant biomedical paradigm, most women turned to online resources that affirmed mainstream medical authority and continued to rely on their doctors. By providing the means to confirm normalcy and take control in their reproductive experiences, the Internet enables socially privileged women to more fully perform the informed patient role in order to demonstrate their competence as mothers. Because pluralism at its heart is an epistemic problem in philosophy, what is at issue in discussions of philosophical pluralism are the definitions of who counts as a knower and what counts as knowledge. In this philopoetic article, in which philosophic claims are interwoven with poetic and narrative recountings of my own experiences with racist patriarchal violence in the discipline, I argue for an epistemic approach to creating pluralism in philosophy through the satisfaction of seven conditions. These conditions require, at (...) minimum, that we consider the importance of hiring multiply-positioned persons; that we critique patriarchy and white hegemonic discourses; that we rethink assumptions about epistemic credibility; and that we dispense with “perverse dialogues.” Finally, we must encourage “true dialogue” and create a “new dialogic” in philosophy. En este artículo analizamos las narrativas de los grandes medios de comunicación en la actualidad, en particular, de los diarios de mayor circulación nacional: Clarín y La Nación, en sus versiones digitales. Nos centramos, específicamente, en la relación neoliberalismo – reforma laboral – representaciones mediáticas, y proponemos un análisis de las lecturas preferenciales (Hall, 1981) que destacan estos medios sobre una posible reforma laboral en Argentina, a partir de 2017. El objetivo de este estudio es indagar los mecanismos discursivos y (...) las estrategias de representación que utilizan estos medios para producir y reproducir ciertas miradas sobre la flexibilización y las reformas laborales. Los interrogantes que se plantean son: ¿Qué se visibiliza, qué se enfatiza y qué se oculta en estas narrativas mediáticas digitales cuando se informa sobre una posible reforma laboral? ¿De qué modo se buscan sentar las bases ideológicas para crear un consenso social sobre la cuestión? ¿En qué marco de significación se destaca al trabajo, la flexibilización laboral, la precariedad? ¿Cómo son representados empresarios y sindicatos? Metodológicamente, se utiliza un enfoque sociosemiótico que focaliza en el análisis de los sentidos sociales que recorren a las nociones de flexibilización laboral, reformas y precariedad, a partir de los elementos formales, de contenido y de fuentes, presentes en las noticias digitales sobre la temática. Consideramos que las representaciones privilegiadas y los sentidos sociales que los medios, sobre todo los más concentrados y con un lugar jerarquizado en la estructura mediática nacional, atribuyen a reformas laborales, conflictos, formas de flexibilización, condiciones de trabajo, resultan una variable interesante para explorar los procesos culturales de construcción del sentido común y de reproducción de estereotipos sociales, en contextos neoliberales. Dispositional Essentialism, as commonly conceived, consists in the claims that at least some of the fundamental properties essentially confer certain causal-nomological roles on their bearers, and that these properties give rise to the natural modalities. As such, the view is generally taken to be committed to a realist conception of properties as either universals or tropes, and to be thus incompatible with nominalism as understood in the strict sense. Pace this common assumption of the ontological import of Dispositional Essentialism, the (...) aim of this paper is to explore a nominalist version of the view, Austere Nominalist Dispositional Essentialism. The core features of the proposed account are that it eschews all kinds of properties, takes certain predicative truths as fundamental, and employs the so-called generic notion of essence. As I will argue, the account is significantly closer to the core idea behind Dispositional Essentialism than the only nominalist account in the vicinity of Dispositional Essentialism that has been offered so far—Ann Whittle’s Causal Nominalism—and is immune to crucial problems that affect this view. In this interesting and engaging book, Shabel offers an interpretation of Kant's philosophy of mathematics as expressed in his critical writings. Shabel's analysis is based on the insight that Kant's philosophical standpoint on mathematics cannot be understood without an investigation into his perception of mathematical practice in the seventeenth and eighteenth centuries. She aims to illuminate Kant's theory of the construction of concepts in pure intuition—the basis for his conclusion that mathematical knowledge is synthetic a priori. She does this through (...) a contextualized interpretation of his notion of mathematical construction, which she argues can be approached by looking at Euclid's Elements and Christian Wolff's mathematical textbooks. The importance of the former for her interpretation is justified by the fact that nearly all of Kant's mathematical examples in the Critique are Euclidean propositions. The importance of the latter is revealed through the fact that Wolff's textbooks were not only widely read and representative of the state of elementary mathematics during Kant's time; Kant was also intimately familiar with them. During the thirty years prior to the publication of the Critique, he used the textbooks in the college-level introductory courses in mathematics and physics that he taught.In the introduction to her book, Shabel helpfully distinguishes her approach to Kant's philosophy of mathematics from that of previous commentators. (wife of patient) and Jeanne KerwinMy husband suffered from sudden onset of heart failure with a very low ejection fraction and was on IV Milrinone at the age of 47. One of the most powerful things he told me was that he was not afraid to die and therefore did not want to move forward with Milrinone. He eventually “did it for the kids.” After (...) the Milrinone drip was no longer working, he was offered an LVAD (left ventricular assist device) to keep him alive. He refused, but the doctor again convinced him to “do it for your kids” and said, “we put these devices in 80-year-olds.” He did not have time to explore any other options and reluctantly accepted the LVAD. It was implanted in May of 2014 in a very long and complicated surgery with a long and difficult recovery. He was in the hospital for a long time, and then he refused to go to rehab upon discharge, making it difficult for our family to manage, as he was extremely weak and needed care of his wound dressing. He worried about the financial [End Page 7] impact of a heart transplant and the medications required, as we were already struggling with the medical bills. He refused to have more surgery with the potential complications and never agreed to get on the transplant waiting list.After the implanted LVAD, my husband was not able to do most of what he loved in life. He couldn’t work (he was a truck driver), he couldn’t go fishing or swimming in the ocean, he couldn’t go hunting and fishing, and his overall physical condition was so poor that he couldn’t cook, garden and, most aggravating to him, he could not take a regular hot shower. The LVAD had to be covered with plastic in order to avoid getting the drive-line area wet, so bathing was a complicated process. He had trouble with stairs, had neuropathic pain in his legs from diabetes, and because of the pain medications, he felt cloudy and dizzy and slept most of the day in his recliner. I took care of him, even after I had to go back to work, and at the same time, I managed our two children (ages 9 and 12 at the time of LVAD).After almost two years of living with the LVAD, multiple hospitalizations for GI bleeds then seizures, more medications, and weekly blood draws for INR1, my husband requested that the LVAD be de-activated and that he be allowed to die of his heart failure. He described his life as “miserable.” He watched as his misery impacted his children and me. He suffered daily and had none of the joys that made life worth living for him. When we approached the cardiac team that implanted and monitored his LVAD with his request, they said “no.” They would not de-activate the LVAD because it was working. It was keeping him alive and he had no other terminal illness. His palliative doctor, who was treating his pain, also heard his request and told us to call for an “ethics consult” to determine if he would be “allowed” to have the LVAD de-activated. We got the number and called on March 31, 2016.At the time of the call to Ethics, we had no idea what “Ethics” was, other than a general definition of the word ethics as in professional behaviors and such. We did not know what to expect but we called. We would never have known to call “Ethics” if the palliative doctor had not advised us to do so, and my husband might have suffered a longer, more painful life both mentally and physically prolonged by the LVAD.We explained the request to the ethics consultant. She listened to my husband describe his daily living and his dissatisfaction with the quality of his life, his physical and emotional suffering, and his wish to rid himself of the LVAD and allow nature to take its course. Alienation and Connection addresses social constructs that perpetuate alienation through suffering. The contributors discuss how alienation through suffering in a variety of contexts can be transformed into connection and reconnection: human relationship with the environment, economic and social systems that disconnect and reconnect, cultural constructs that divide or can heal, encountered difference that brings opportunity, and various manifestations of personal pain that can be survived and even overcome.

This article focuses on a subset of refugees who engage in entrepreneurship shortly after relocating to a new host community; it explores identity-related antecedents and integration consequences of different entrepreneurship strategies in the new location. It draws from acculturation psychology and founder identity theory to argue that, post-arrival, new refugees consider (a) how to prioritize the identity associated with their former life and (b) the degree of connection they desire in the host community. For some, these preferences drive heterogeneous entrepreneurial (...) behaviors associated with different acculturation outcomes—including marginalization, separation, integration, and assimilation. Specifically, we describe two refugee entrepreneur identity management strategies: reinvention (distance from home country identity) and reinforcement (close connection to home country identity) and propose their different acculturation outcomes. The theory-based arguments emphasize how attending to identity preferences of refugees can improve stakeholder responses.
