In babies born with this defect the left side of the heart is much smaller than the right and is unable to pump sufficient blood to sustain life for more than a few weeks. This original analysis examines the three leading traditional solutions to the dilemma of divine foreknowledge and human free will--those arising from Boethius, from Ockham, and from Molina. Though all three solutions are rejected in their best-known forms, three new solutions are proposed, and Zagzebski concludes that divine foreknowledge is compatible with human freedom. The discussion includes the relation between the foreknowledge dilemma and problems about the nature of time and the causal relation; the logic of counterfactual conditionals; and the differences (...) between divine and human knowing states. An appendix introduces a new foreknowledge dilemma that purports to show that omniscient foreknowledge conflicts with deep intuitions about temporal asymmetry, quite apart from considerations of free will. This article considers why truth-telling is so difficult in institutions, and deceit and paltering are so common. Drawing on recent examples of churches and charities exposed for covering up the truth about abuse, the article explores the institutional barriers to truthfulness and considers how they might be removed.

Although tissue plasminogen activator (tPA) is the only medication approved by the United States Food and Drug Administration (FDA) for acute ischemic stroke, there is no consensus about the need for informed consent for its use. As a result, hospitals throughout the U.S. have varying requirements regarding obtaining informed consent from patients for the use of tPA, ranging from no requirement for informed consent to a requirement for verbal or written informed consent. We conducted a study to (1) determine current (...) beliefs about obtaining patients’ informed consent for tPA among a large group of stroke clinicians and (2) identify the ethical, clinical, and organizational factors that influence tPA consent practices. Semi-structured interviews were conducted by trained and experienced investigators and research staff to identify key barriers to implementing acute stroke services. Part of the interview explored current beliefs and practices around informed consent for tPA.This was a multicenter study that included 38 Veterans Health Administration (VHA) hospital locations. Participants were 68 stroke team clinicians, serving primarily on the neurology (35 percent) or emergency medicine (41 percent) service. We conducted thematic analysis based on principles of grounded theory to identify codes about consent for tPA. We used interpretive convergence to ensure consistency among the individual investigators’ codes and to ensure that all of the investigators agreed on coding and themes.We found that 38 percent of the stroke clinicians did not believe any form of consent was necessary for tPA, 47 percent thought that some form of consent was necessary, and 15 percent were unsure. Clinicians who believed tPA required informed consent were divided on whether consent should be written (40 percent) or verbal (60 percent). We identified three factors describing clinicians’ attitudes about consent: (1) legal and policy factors, (2) ethical factors, and (3) medical factors.The lack of consensus regarding consent for tPA creates the potential for delays in treatment, uneasiness among clinicians, and legal liability. The identified factors provide a potential framework to guide discussions about developing a standard of care for acquiring the informed consent of patients for the administration of tPA. (shrink)

This article describes the connection between feminist theory and the canon of political thought. It explains that feminist approaches to the canon of political theory are characterized by deep ambivalence and the majority of canonical authors have mostly dismissed women as political beings in their own right and casted them instead as mere appendages to citizen man. The article suggests that the question of how to make political judgments about other cultures and practices that deeply affect women is particularly important (...) for feminist theory today. Greco argues that knowledge by transmission involves joint agency whose norms are governed by the quality of the social relations in the transmission, and these norms differ from the norms generating knowledge in the source. This approach to the transmission of knowledge allows Greco to respond to three common arguments against the rationality of religious belief on testimony: the argument against belief in miracles, the argument from luck, and the argument from peer disagreement. I agree with Greco’s position on the (...) transmission of knowledge or reasonable belief in these cases and argue that the distinction between first person and third person reasons for belief can strengthen his position. The experience upon which a belief is based can be transferred intersubjectively and strengthens the ground for the belief based on experience, including the belief in miracles. Trust is another first person reason for belief, not only a background condition for reasonable transmission, and it also can be transmitted and is an important component in the formation and preservation of traditions. This feature permits a reply to the arguments from luck and from disagreement with people outside the tradition. But this feature also shows why we find these arguments threatening since we are rationally committed to a minimal degree of trust in all other reasonable persons. (shrink)

Since 1991, the Patient Self-Determination Act (PSDA) has required all health care institutions that receive Federal funds to inform patients upon admission of their rights to make decisions about medical care and to execute advance directives. Implementation of the PSDA presents a special challenge for state mental hospitals. The relevance and possible negative therapeutic impact of discussing end of life decisions at the time of an acute psychiatric admission has recently been raised in the literature. Other ethical dilemmas arising from (...) the interplay between mental illness and informed consent for medical treatment, particularly for older patients committed to state mental hospitals, have been highlighted by the PSDA.
