Women have consistently higher levels of disgust than men. This sex difference is substantial in magnitude, highly replicable, emerges with diverse assessment methods, and affects a wide array of outcomes—including job selection, mate choice, food aversions, and psychological disorders. Despite the importance of this far-reaching sex difference, sound theoretical explanations have lagged behind the empirical discoveries. In this article, we focus on the evolutionary-functional level of analysis, outlining hypotheses capable of explaining why women have higher levels of disgust than men. (...) We present four hypotheses for sexual disgust and six for pathogen disgust, along with testable predictions. Evolutionary approaches to the emotions have traditionally focused on a subset of emotions that are shared with other species, characterized by distinct signals, and designed to solve a few key adaptive problems. By contrast, an evolutionary psychological approach (a) broadens the range of adaptive problems emotions have evolved to solve, (b) includes emotions that lack distinctive signals and are unique to humans, and (c) synthesizes an evolutionary approach with an information-processing perspective. On this view, emotions are superordinate mechanisms that evolved (...) to coordinate the activity of other programs in the solution of adaptive problems. We illustrate the heuristic value of this approach by furnishing novel hypotheses for disgust and sexual arousal and highlighting unexplored areas of research. (shrink)

The target article provides important theoretical contributions to psychology and Bayesian modeling. Despite the article's excellent points, we suggest that it succumbs to a few misconceptions about evolutionary psychology (EP). These include a mischaracterization of evolutionary psychology's approach to optimality; failure to appreciate the centrality of mechanism in EP; and an incorrect depiction of hypothesis testing. An accurate characterization of EP offers more promise for successful integration with Bayesian modeling.

In this study, we replicated what is known about the relative importance of dealbreakers and dealmakers in romantic and sexual relationships and extended it to an examination of self-reports of mate value, self-esteem, and loneliness. In two experiments we manipulated the information people were told about potential partners and asked them about their intentions to have sex again with or go on a second date with opposite sex targets. People were less interested in partners after learning dealbreakers, effects which operated (...) more strongly in the long-term than short-term context, but similarly in men and women. People who reported less self-esteem or more loneliness were more receptive to people with dealbreakers. People who thought they had more mate value, more self-esteem, or less loneliness were more receptive to dealmakers. Results are discussed using sociometer, prospect, and sexual strategies theories. This paper examines the Bahāʾī philosophy with all its religious values and humanitarian practices to assess their relevance to the modern times. The rationale behind the choice of this subject was to find out whether the Bahāʾī philosophy could be yet another force and potential ideology to offer a solution to modern time challenges that are faced globally. Using a qualitative research design, the data was collected through a content analysis and close reading of Bahāʾī scriptures, tablets, letters and other (...) library sources. A close study of the Bahāʾī doctrines, values and faith revealed that the Bahāʾī philosophy extended the message of uniting the whole humanity by uniting all religions and sects, despite their apparent differences. All religions, according to the Bahāʾī faith, are manifestations of God and work towards an identical and universal divine plan to educate the human race. The social ethics of the Bahāʾīs emphasized upon the “oneness of humanity”, abolition of racial discrimination, class distinction, and religious prejudices, without any need of priesthood or rigid rituals of worshipping. If implemented and complied universally in principle, the Bahāʾī philosophy could be one of the potential ideologies to resolve the racial and religious conflicts. (shrink)

Saudi Arabia has had remarkable progress in reducing maternal and neonatal mortality over the past decades. While various challenges persist, such as unequal access to healthcare, a history of pre-existing maternal conditions, and neonatal complications, this review will focus on critical determinants of maternal and neonatal outcomes in Saudi Arabia, including government-driven efforts through Vision 2030 towards enhancing health care infrastructure and policies. The focus has been on antenatal care, neonatal intensive care units, and maternal mortality audit issues, enhancing outcomes. (...) Recommendations are the adoption of standardized risk assessment tools and health disparities abatement between the Saudi population and non-Saudi populations. Hospital administrators play an essential role in the implementation of Antibiotic Stewardship Programs, which is instrumental in the fight against antimicrobial resistance. The foundation and sustenance of such programs has been made possible through leadership; this is through interdisciplinary collaboration that is done with policy formulation and stewardship inclusion into the institutional systems. Indeed, it is immense strength where ID physicians and clinical pharmacists with special training proactively involve themselves in the program. Beyond that, the accreditation standards that incorporate ASPs (...) really enhance compliance and institutional commitment. Obstacles to movement include an absence of resources, unwilling prescriber, and other competing health conditions for priority. To achieve such success, the hospital administrators must ensure adequate staffing, proper utilization of advanced technologies, and instillation of accountability through focused education and participation. Besides the reduction of resistance, ASPs contribute to such improvements in patient outcomes, efficient consumption of resources, and significant reductions in costs. As this narrative review indicates, leadership at the hospital level plays an important role in augmenting antibiotic stewardship. Addressing major barriers, along with strategic implementation, means that administrators may play an important role in further maximizing effectiveness while truly adding to the world-wide effort against antimicrobial resistance. The Sufi saints, in their texts, successfully ingrained Love as the destination of spiritual path, to accomplish the Divine. The objective of this study was to focus on the cult of Sufism, in general, and on the Sufi poetry of Fakhr al-Din ʿIrāqī, in particular as reflected in his anthology, Kitāb al-lamaʿāt. Adopting a historical approach, the study analyzed the evolution of Sufi poetry since the beginning, and examined the poetry of ʿIrāqī in the context of Islamic mysticism. This qualitative (...) study adopted an exploratory content analysis method and documentation research to examine the Sufi philosophy as reflected in the sampled poetry of ʿIrāqī. For the realization of this goal, a research was conducted on the available published resources. The documentation survey helped discover the truth of ʿIrāqī ’s poetry in the context of Spiritual love. It was discovered that ʿIrāqī ’s philosophy of Love was based on metaphysical transcendentalism, where spiritual attainment was achieved through formlessness mirrored in the form of Beauty as the manifestation of Love. Automatic synonym extraction plays an important role in many natural language processing systems, such as those involving information retrieval and question answering. Recently, research has focused on extracting semantic relations from word embeddings since they capture relatedness and similarity between words. However, using word embeddings alone poses problems for synonym extraction because it cannot determine whether the relation between words is synonymy or some other semantic relation. In this paper, we present a novel solution for this problem by proposing the (...) SynoExtractor pipeline, which can be used to filter similar word embeddings to retain synonyms based on specified linguistic rules. Our experiments were conducted using KSUCCA and Gigaword embeddings and trained with CBOW and SG models. We evaluated automatically extracted synonyms by comparing them with Alma’any Arabic synonym thesauri. We also arranged for a manual evaluation by two Arabic linguists. The results of experiments we conducted show that using the SynoExtractor pipeline enhances the precision of synonym extraction compared to using the cosine similarity measure alone. SynoExtractor obtained a 0.605 mean average precision for the King Saud University Corpus of Classical Arabic with 21% improvement over the baseline and a 0.748 MAP for the Gigaword corpus with 25% improvement. SynoExtractor outperformed the Sketch Engine thesaurus for synonym extraction by 32% in terms of MAP. Our work shows promising results for synonym extraction suggesting that our method can also be used with other languages. (shrink)

Some of the human needs are more important than others in order to be inevitable. One of these needs which cannot be avoided is the need for belonging to any authority. Whatever the name, religion, nation, homeland, flag etc. all these concepts are the reflections of the sense of belonging that comes with human existence. This article will discuss how Kuwaiti novelist Saud Alsanousi reflects the crisis of a child who is born from a secret relationship with a Filipino (...) woman's son who came to Kuwait as a housekeeper in Kuwait. When he returned to Kuwait, where he carried his passport and felt himself unfamiliar in the Philippines because of his physical structure and the Arabs, he was again excluded by his relatives because of his Filipino mother. The novel tries to draw attention to the socio-economic problems of workers coming from South East Asia to the Gulf countries, especially from the character of Jose, who has a nauseous mood due to the uncertainty between Philippine-Kuwait and Christian-Muslim affiliations. The introduction of foreign workers who constitute more than half of the population in the Gulf countries will be reviewed. Afterwards, a short summary of the novel will be given and the main theme of the lack of disability will be analyzed. Summary: In 1973, Syria and Egypt started a air strike against Israel in order to recover their lost territory in the previous wars and their reputation in the international area. Some Western countries -especially the United States- have shown all kinds of military aid to Israel in the face of this attack. Thanks to these aids the Syrian and Egyptian armies have not achieved their intended goals. This attitude of Western countries to Israel has attracted the reaction of other Gulf countries -especially the Saudi administration- which has a large amount of oil reserves. Gulf countries -which have not gotten a strong military option- tried to use the oil in their hands for the first time as a political trump card. These oil-exporting countries have reduced oil production in response. As a result, the economies of Western countries which were industrialized and dependent on oil have been negatively affected by this decision. In contrast, Gulf States have gained a sudden and massive fortune due to oil prices rising three or four times. The Gulf States, which have led this wealth to development in many areas like education, health, urbanism, etc. The need for qualified staff in these fields was supplied by other Arab countries such as Egypt, Syria, and Sudan. However, as the years passed oil prices decreased and so oil revenues decreased. This reduction led the Gulf States -which have only oil-based economies- to look for cheaper labor. While the foreign employment provided from the Arab countries previously later this need has begun to supply from far East Asian countries which are economically very weak. Vast number of employees emigrated to the Gulf countries to work in the labor service sector with very low wages especially from the Philippines, Indonesia, Bangladesh, and Malaysia. This segment -which has employed with very low wages- has been subjected to injustice and discrimination. The injustices faced by the Far East Asian foreign labor force deprived of social rights have been the subject of many painful stories over time. The modern novel -which aims to reflect the life and the human being with all its details- has reflected the stories of the victims of that employers. Saq al-Bamboo (Bamboo Bar) is one of these novels which has published by Kuwaiti writer Saud Alsanousi in 2012. Within a short time after the publication of the work the novel was interested in the Arab world and in 2013 he was awarded the International Arabic Novelist Prize. The novel is consisting of five chapters and each chapter is divided into three sections 1,2,3... The author conveys the crisis of unetnicity from Jose’s chracter who was born from a Filipino mother and a Kuwaiti father. A translator named Ibrahim Selâm -which is a translation of the souvenirs and diaries belonging to Jose from the Philippine language to Arabic is intended to increase the realism of the work. Josephine -who came to Kuwait to work from the Philippines- was settled in 1985 in one of the country's most reputable families. The old woman who owns the house has a son named Rasheed. Rasheed was interested in Josephine who worked as a servant in their house and he married her with a secret marriage. Because the marriage of a Kuwaiti man and a foreign woman who was working as a housekeeper in Kuwait is accepted as a great shame that will cast a shadow on the dignity and honor of the family the marriage which was concealed by the couple inside and outside of the house, which could only come together with privacy has appeared after months. Rasheed -who was expelled from the house with Josephine- has temporarily found another house. Rasheed and his wife, who thought that his mother's attitude would change as their children were born have disappointed. Rasheed after realizing that his mother would never be satisfied, finally lost his ability to resist this crisis and sent his wife back to. With this development the problem of-identity crisis of Jose’s chracter is dealt with in a multi-faceted way. Jose, who left his age with his mother from Kuwait, had suffered from familial negativities that had passed through Kuwait years later in his twenties, and “familial negativities through his return to Kuwait and depressions caused by the sense of belonging. In an interesting coincidence when he met Gassân, one of the closest friends of Jose’s father in the Philippines, he returned to his dream Paradise ’Kuwait”. The journey from Jesus who has known as “Arabo to “Jesus” started with great hope because he was born from a Kuwaiti father in his own near the Philippines and likened to Arabs alike. But perhaps more of the crisis in the Philippines is waiting for him in Kuwait. Because his father was killed during the years when the Iraqi army occupied Kuwait. Although his father's mother welcomed Jose at first, exclusionary attitude towards foreigners in Kuwait caused her to take a negative attitude towards here grandson. The work is a continuation of the tradition of novels of criticism of the Arab society that has been going on for almost a century. The fact that novel was able to make deep psychological analyzes with his comprehensible language and simple style without being ambiguous and to keep the tension alive by leaving the problems at the end of the episodes and thus to provide immersion in this way reinforced the fictional soundness of the work. The novel also presents fictional material within the Arab society for the researchers in the field of religion psychology and sociology of religion, in order to show how a non-Muslim feels amoung the Muslim society. O Fasl al-maqal, umas das obras originais de Averróis, se destaca por seu caráter polêmico. Sendo redigida como um pronunciamento jurídico, tem como um de seus objetivos principais analisar o estatuto da filosofia em face da Lei islâmica, ao verificar qual é a atitude que deve ser adotada por todo aquele que travar contato com as obras da antiguidade grega. Em um cenário em que a legitimidade da filosofia se coloca em questão, o diagnóstico e a solução propostos por Averróis (...) se destacam ao apresentar uma interpretação singular da Lei ante a filosofia, concebendo a segunda como um elemento necessário para a saúde da comunidade. Este artigo pretende analisar essa interpretação, na qual três pontos se colocam na consideração da relação entre filosofia e revelação: o (1) asseguramento da possibilidade do assentimento religioso individual e (2) da manutenção do bem-estar geral junto à (3) possibilidade de se afirmar a legitimidade de um intérprete que garanta o assentimento correto aos crentes não filósofos. Las naciones y pueblos indígenas presentan preocupantes índices respecto a la salud, así como otras carencias como la pérdida de sus tierras, la alimentación, la educación y, en general, el ejercicio de sus derechos, consagrados por organizaciones internacionales como la ONU y la OMS-OPS. En Ecuador, de acuerdo con el orden jurídico constitucional, se han instrumentado políticas de salud que incluyen el lineamiento general de un enfoque intercultural de la atenciónprimaria que incorpora los saberes y prácticas ancestrales. The development of the Minimalist Program, Noam Chomsky's most recent generative model of linguistics, has been highly influential over the last twenty years. It has had significant implications not only for the conduct of linguistic analysis itself, but also for our understanding of the status of linguistics as a science. The reflections and analyses in this book contain insights into the strengths and the weaknesses of the MP. Among these are, a clarification of the content of the Strong Minimalist Thesis (...) ; a synthesis of Chomsky's linguistic and interdisciplinary discourses; and an analysis of the notion of optimal computation from conceptual, empirical and philosophical perspectives. This book will encourage graduate students and researchers in linguistics to reflect on the foundations of their discipline, and the interdisciplinary nature of the topics explored will appeal to those studying biolinguistics, neurolinguistics, the philosophy of language and other related disciplines. BackgroundA different ethos with respect to the perception of medical ethics prevails in societies in transition such as those in the Arabian Peninsula, which makes it difficult to apply international principles of bioethics in medical practice. This study aimed to develop and psychometrically test an instrument that measures physicians’ awareness of bioethics and medical law and their attitudes towards the practice of medical ethics. Additionally, it examined physician correlates influencing the awareness of bioethics.MethodsFollowing a rigorous review of relevant literature by (...) a panel of experts, a 13-item instrument, the Omani physicians’ bioethics and medical law awareness (OBMLA) questionnaire was developed with the aim of assessing physicians’ awareness of bioethics and medical law. The study tool’s construct validity and internal consistency reliability were examined by exploratory factor analysis (EFA) and Cronbach’s alpha. In a cross-sectional study, the questionnaire was distributed among a random sample of 200 physicians at a tertiary hospital in Muscat, Oman. Participant characteristics that may influence awareness of bioethics and medical law were explored.ResultsThe EFA of the OBMLA questionnaire resulted in three well-loading factors: (1) Physicians’ bioethics practice subscale (2) incentive related bioethics subscale and (3) medical law awareness subscale. Internal consistency reliability ranged between Cronbach’s α: 0.73–0.8. Of the total 200 participants, 52% reported that teaching medical ethics during medical school was inadequate. The overall mean (standard deviation, SD) of the bioethics awareness score and Omani medical law awareness were 27.6 (3.5) and 10.1 (2.1) respectively. The majority of physicians (73%) reported that they frequently encountered ethical dilemmas in their practice and 24.5% endorsed the view that unethical decisions tended to occur in their practice.ConclusionThe study provides an insight into the practice of bioethics, and the awareness of bioethics and medical law among physicians in a teaching hospital in Oman. The OBMLA questionnaire appears to be a valid and reliable tool to assess a physician’s awareness of bioethics and medical law. In this preliminary study, it appears that participants have suboptimal scores on the indices which measure practice and awareness of bioethics and medical law. (shrink)

There are few examples in the literature of objective measures for the assessment of donor willingness. The author describes the scoring system in use at his own renal transplant unit which has brought objectivity to the process of determining the willingness of living related donors. In this system, a total score to determine the degree of willingness or unwillingness is calculated based on responses to a series of questions. The author believes that with minor modifications this system could be implemented (...) by transplant units in different countries and cultures to screen out donors who are acting under duress. There has been increased interest in the “dark side” of organizational behavior in recent decades. Workplace bullying, in particular, has received growing attention in the social sciences literature. However, this literature has lacked an integrated approach. More specifically, few studies have investigated causes at levels beyond the individual, such as the group or organization. Extending victim precipitation theory, we present a conceptual model of workplace bullying incorporating factors at the individual-, dyadic-, group-, and organizational-levels. Aims To examine the understanding of the ethical dilemmas associated with Big Data and artificial intelligence (AI) among Jordanian medical students, physicians in training, and senior practitioners. Methods We implemented a literature-validated questionnaire to examine the knowledge, attitudes, and practices of the target population during the period between April and August 2023. Themes of ethical debate included privacy breaches, consent, ownership, augmented biases, epistemology, and accountability. Participants’ responses were showcased using descriptive statistics and compared between groups using t-test or ANOVA. (...) Results We included 466 participants. The greater majority of respondents were interns and residents (50.2%), followed by medical students (38.0%). Most participants were affiliated with university institutions (62.4%). In terms of privacy, participants acknowledged that Big Data and AI were susceptible to privacy breaches (39.3%); however, 59.0% found such breaches justifiable under certain conditions. For ethical debacles involving informed consent, 41.6% and 44.6% were aware that obtaining informed consent posed an ethical limitation in Big Data and AI applications and denounced the concept of “broad consent”, respectively. In terms of ownership, 49.6% acknowledged that data cannot be owned yet accepted that institutions could hold a quasi-control of such data (59.0%). Less than 50% of participants were aware of Big Data and AI’s abilities to augment or create new biases in healthcare. Furthermore, participants agreed that researchers, institutions, and legislative bodies were responsible for ensuring the ethical implementation of Big Data and AI. Finally, while demonstrating limited experience with using such technology, participants generally had positive views of the role of Big Data and AI in complementing healthcare. Conclusion Jordanian medical students, physicians in training and senior practitioners have limited awareness of the ethical risks associated with Big Data and AI. Institutions are responsible for raising awareness, especially with the upsurge of such technology. (shrink)

PurposeThe purpose of this paper is to study the ethical behaviour of Muslim IT professionals in an attempt to stop many unethical practices such as software piracy, software intellectual property violations and general software development.Design/methodology/approachThis study examines the computer ethical principles presented in the Association for Computing Machinery code of conduct from an Islamic point of view through studying some relevant verses of The Holy Quran and Hadiths of Prophet Mohammed. An evaluation of the benefits of this newly proposed Islamic (...) computer code of ethics is presented by surveying how IT students, IT professionals and software house owners would adhere to it. The evaluation was carried out by teaching different groups of participants aspects of the ACM code of conduct and the proposed Islamic computer ethics code and then surveying their opinions by a questionnaire.FindingsThe study demonstrates that teaching computer ethics in general, and computer ethics from an Islamic point of view in particular, clearly contributes to ethical behaviour of Muslim IT professionals with regards to software development issues.Research limitations/implicationsIt is not possible to put participants through real life scenarios and practically observe how they would react to different ethical situations. The empirical research thus relies on what participants said they would do.Practical implicationsThe newly developed Islamic code of computer ethics and its proposed implementation method can be utilised by organisations for their benefit.Originality/valueThe study is the first to address the issue of computer ethics and Islam for IT professionals. Authors endure considerable hardship carrying out biomedical research, from generating ideas to completing their manuscripts and submitting their findings and data to a journal. When researchers submit to journals, they entrust their findings and ideas to editors and peer reviewers who are expected to respect the confidentiality of peer review. Inherent trust in peer review is built on the ethical conduct of authors, editors and reviewers, and on the respect of this confidentiality. If such confidentiality is breached by unethical reviewers (...) who might steal or plagiarize the authors’ ideas, researchers will lose trust in peer review and may resist submitting their findings to that journal. Science loses as a result, scientific and medical advances slow down, knowledge may become scarce, and it is unlikely that increasing bias in the literature will be detected or eliminated. In such a climate, society will ultimately be deprived from scientific and medical advances. Despite a rise in documented cases of abused peer review, there is still a relative lack of qualitative and quantitative studies on reviewer-related misconduct, most likely because evidence is difficult to come by. Our paper presents an assessment of editors’ and reviewers’ responsibilities in preserving the confidentiality of manuscripts during the peer review process, in response to a 2016 case of intellectual property theft by a reviewer. Our main objectives are to propose additional measures that would offer protection of authors’ intellectual ideas from predatory reviewers, and increase researchers’ awareness of the responsible reviewing of journal articles and reporting of biomedical research. Fake news detection involves predicting the likelihood that a particular news article is intentionally deceptive. Arabic FND started to receive more attention in the last decade, and many detection approaches demonstrated some ability to detect fake news on multiple datasets. However, most existing approaches do not consider recent advances in natural language processing, i.e., the use of neural networks and transformers. This paper presents a comprehensive comparative study of neural network and transformer-based language models used for Arabic FND. We examine (...) the use of neural networks and transformer-based language models for Arabic FND and show their performance compared to each other. We also conduct an extensive analysis of the possible reasons for the difference in performance results obtained by different approaches. The results demonstrate that transformer-based models outperform the neural network-based solutions, which led to an increase in the F1 score from 0.83 to 0.95, and it boosted the accuracy by 16% compared to the best in neural network-based solutions. Finally, we highlight the main gaps in Arabic FND research and suggest future research directions. (shrink)

BackgroundDefensive medicine (DM) practice refers to the ordering or prescription of unnecessary treatments or tests while avoiding risky procedures for critically ill patients with the aim to alleviate the physician’s legal responsibility and preserve reputation. Although DM practice is recognized, its dimensions are still uncertain. The subject has been highly investigated in developed countries, but unfortunately, many developing countries are unable to investigate it properly. DM has many serious ramifications, exemplified by the increase in treatment costs for patients and health (...) systems, patients’ exposure to risks, and negative effects on the psychological health of both health providers and recipients. Ultimately, the most serious consequence is the ethical consequences.MethodsThis work is based on a review of the literature related to DM worldwide and a comparison with the available knowledge found in Jordan. It is qualitative with a descriptive nature, aiming to diagnose the current DM practice in Jordan.ResultsThis is the first published article that discusses DM in Jordan by diagnosing its ethical and economic consequences for the health system as well as for patients. Despite the knowledge of the reasons that support its practice, little is being done to solve this issue. The absence of agreeable medical malpractice law, the dearth of unified medical protocols, the overwhelming pressure imposed by patients on medical staff, and the deteriorating patient-physician relationship are some of the causes of DM practice. Surely, the solution to these issues is to focus on fortifying the ethical and humanitarian aspects on the side of both the physician and the patient to ensure positive collaboration. The ethical aim of the physician to treat the patient faithfully and do what is possible to help combined with the appreciation of the physician’s efforts and the choice to not take advantage of the physician through litigation could be the most reasonable solution in the near future.ConclusionJordan is suffering from DM due to the limited financial expenditure on the health sector and the impracticality of medical malpractice law. The authors highlight that the cardinal step in solving this dilemma is restoring the ethical dimension of the patient-physician relationship. (shrink)

This paper examines the phenomenon of Skolombo in Calabar in relation to the challenge of urban subalternism in that context. This is against the backdrop of the evolution of the Skolombo into a rising urban subaltern category involved in the underworld and ant-social activities. By means of exploratory and conversational discourse that relies on extant literature as well as insights from personal communications, the paper posits that Skolombo phenomenon represents an existential struggle by abandoned and rejected street children who are (...) surviving against structural societal victimization. Away from home, these children have found the streets, not only an inevitable abode but also a space for opportunistic survival. Over the years, they have evolved a pattern of street living characterized, among other things, by restiveness, touting, gangsterism, and criminality. Associated with this pattern of existence is an emerging subaltern identity that highlights a crisis of urbanity in Calabar metropolis of Nigeria. A equipe da Enfermagem é privilegiada, pois, é o cuidador que permanece vinte e quatro horas na assistência direta do paciente, podendo ter uma visão ampliada da pessoa no processo do adoecimento, tendo a oportunidade de incorporar e desenvolver os princípios dos Cuidados Paliativos (CP). Na perspectiva holística da saúde a religião/religiosidade/espiritualidade é uma das possibilidades de cuidado e manejo, porém ainda muito negligenciada pelos profissionais da saúde, seja por desconhecimento ou preconceitos e tabus, mas que se torna essencial no (...) cuidado de pacientes em CP. Por meio deste relato de experiência, objetiva-se ressaltar a importância da religião/religiosidade/espiritualidade, no cuidado e manejo de pacientes em CP, a partir da experiência de uma profissional da enfermagem, sua prática em cuidados e docência, supervisionando alunos com pacientes em contexto dos CP. Anos acompanhando alunos na prática, demonstraram que os profissionais da enfermagem têm dificuldade de identificar o Sofrimento Espiritual (SE) dos pacientes, e quando identificavam, não sabiam o que fazer com esta demanda, delegando a outros profissionais da equipe, que também tinham dificuldades, causando um (des)cuidado na Dimensão Espiritual (DE) do paciente em CP, trazendo-o mais sofrimento e desconforto com sua finitude. Como resultado dessas reflexões, está sendo desenvolvida uma pesquisa de pós-doutorado, destinada a identificar a DE, construção de roteiro de educação permanente. Em síntese, esses profissionais tendo a capacitação para identificar o SE, explorar e cuidar da DE, poderão promover a proteção da saúde mental, tanto do paciente em CP quanto sua, e tornarem-se profissionais mais sensíveis na finitude. In this study, we discuss the colonial project as an eliminatory structure of indigenous ways of knowing and doing that is built into Canadian social and health institutions. We elaborate on the role nursing plays in maintaining systemic racism, marginalization and discrimination of Indigenous Peoples. Based on historical practices and present‐day circumstances, we argue that changing language in research and school curriculums turns decolonization into what Tuck and Yang call a ‘metaphor’. Rather, we propose decolonization as a political project where (...) nurses acknowledge their involvement in colonial harms and disrupt the assumptions that continue to shape how nurses interact with Indigenous people, including knowledge systems that perpetuate colonial interests and privilege. Decolonization requires nurses to understand the colonial practices that led to dispossession of land, erasure of knowledge, culture and identity, while upholding indigenous ways of knowing and doing in health, healing and living. As a political manifesto that liberates indigenous life from oppressive structures of colonialism and capitalism, The Red Deal is presented as a visionary platform for decolonization. The aim of this study is to articulate three dimensions of caretaking within The Red Deal as a framework to decolonize nursing knowledge development and practice. Based on the philosophical dimension embedded in The Red Deal that revoke norms and knowledge assumptions of capitalism that destroy indigenous ways of knowing and doing, we underscore an approach toward decolonizing nursing. Our approach rejects the apolitical nature of nursing as well as the unilateral western scientific knowledge approach to knowledge development and recognition. A critical emancipatory approach that addresses the socio‐political and historical context of health care, recognizes dispossession of land and adopts a ‘multilogical’ vision of knowledge that gives space for representation and voice is needed for true decolonization of nursing. Background: Pembrolizumab, an inhibitory anti-PD-1 antibody, has led to significantly meaningful improvements in advanced melanoma. This meta-analysis discusses and presents the efficacy and safety of pembrolizumab versus control treatments in randomized controlled trials. Methods: Electronic databases were searched systematically for RCTs using pembrolizumab in advanced melanomas. Pooled estimates were calculated under random-effect models for overall response rates, PFS, OS, and TRAEs. Results: Nine RCTs including 5132 patients were analyzed. Pembrolizumab significantly realized better results in ORR, PFS and OS HR 0.62, (...) 95% CI 0.54, 0.71, P < 0.001 and HR 0.70, 95% CI 0.63, 0.78, P < 0.001 for control. PFS and OS were consistently better in first, second, and further lines and in groups stratified for PD-L1 status. Grade 3–5 TRAEs were lower with pembrolizumab than with chemotherapy 13.3% vs. 25.9% and ipilimumab 16.9% vs. 27.3%. Conclusions: Pembrolizumab monotherapy meaningfully improves ORRs, delays progression, and prolongs survival in advanced melanoma compared to current standard regimens, with the latter given its favorable toxicity profile. As such, pembrolizumab should be considered the new standard of care for treatment-naïve and previously treated subjects. (shrink)

Hepatocellular carcinoma (HCC) presents a significant problem that necessitates a greater understanding of its underlying molecular complexity in order to improve diagnostics and therapies. Recent studies have shed light on the critical role that mitochondrial dysfunction plays in the development and course of HCC. Once thought to be primarily involved in the synthesis of cellular energy, mitochondria are now known to be key participants in the regulation of a variety of cellular functions that go beyond bioenergetics. The purpose of this (...) review is to elucidate the diagnostic and therapeutic implications of mitochondrial dysfunction in HCC by synthesising ideas from conventional and contemporary scientific literature. Within diagnostics, the discovery of mitochondrial biomarkers such as mutations in mitochondrial DNA and abnormalities in respiratory chain complex activities provides a state-of-the-art viewpoint for the timely identification and accurate description of HCC. In addition, non-invasive imaging methods that focus on mitochondrial function offer prospective means of improving clinical diagnostic accuracy. Moving on to treatment, a growing area of study investigates how mitochondria-targeted therapies might be used to treat HCC. Novel substances and repurposed medications with the potential to induce selective apoptosis in cancer cells, reduce oxidative stress, and restore mitochondrial homeostasis are highly promising. In the fight against hepatocellular carcinoma, an understanding of the complex interactions between mitochondrial dysfunction and HCC opens new diagnostic opportunities and paves the way for individualised treatment plans.
