Many new genetic tests are used in clinical practice, and the number of available tests is growing. Two important health policy questions arise as these genetic tests become available. The first question, whether a new test should be made available, has been the focus of much recent discussion. The second question concerns defining the appropriate standards surrounding the use of these tests, including patient selection, education, informed consent, test interpretation and counseling.Genetic tests currently move from the research arena, where strategies (...) are put in place to minimize risks,into the clinical context where the practice of medicine is more variable and the risks to patients less understood. In the research setting, risks can be minimized through safeguards including education, consent, and counseling. The frequency and magnitude of psychosocial and clinical risks canbe assessed. In the clinical setting, however, these safeguards may not be used consistently, creating unknown risksfor patients. (shrink)

The social facilitation of eating plays a significant role in influencing individuals’ eating decisions. However, how social eating cues are processed in health promotion messages is unclear. This study examined individuals’ food craving in response to social cues in images and emotional experiences, perceived threat, perceived efficacy, behavioral intentions, and motivational coactivation elicited by social eating cues in obesity prevention fear appeals. Results suggested that the presence of a group of people eating in an image facilitated food craving for the (...) presented foods. Moreover, fear appeals that presented obesity and its consequences with more social eating cues, versus individual eating cues, generated greater positive emotional responses, perceived threat severity, response and self-efficacy, and motivational coactivation indicating more attention and threat vigilance. However, these cues also generated fewer self-reported intentions to change unhealthy eating behaviors. Implications and suggestions for future research are discussed. (shrink)

These essays detail the state of the research, look inside two hospitals that put some of these innovations into practice, and consider how design fits into the moral mission ofhealth care.

This chapter discusses the psychological process of selectively attending to one source of information to the exclusion of others. In a meta-analysis, it reviews different tasks used to study selective attention, under the construct of conflict resolution, as assessed with functional imaging techniques. Both the anterior cingulate cortex and the dorsolateral prefrontal cortex are involved in some aspect of conflict resolution, which is likely related to different mechanisms. The theme of heterogeneity of function is reflected in these results.

Background: The factors influencing parents’ willingness to enroll their children in biobanks are poorly understood. This study sought to assess parents’ willingness to enroll their children, and their perceived benefits, concerns, and information needs under different consent and data-sharing scenarios, and to identify factors associated with willingness. Methods: This large, experimental survey of patients at the 11 eMERGE Network sites used a disproportionate stratified sampling scheme to enrich the sample with historically underrepresented groups. Participants were randomized to receive one of (...) three consent and data-sharing scenarios. Results: In total, 90,000 surveys were mailed and 13,000 individuals responded (15.8% response rate). 5737 respondents were parents of minor children. Overall, 55% (95% confidence interval 50–59%) of parents were willing to enroll their youngest minor child in a hypothetical biobank; willingness did not differ between consent and data-sharing scenarios. Lower educational attainment, higher religiosity, lower trust, worries about privacy, and attitudes about benefits, concerns, and information needs were independently associated with less willingness to allow their child to participate. Of parents who were willing to participate themselves, 25% were not willing to allow their child to participate. Being willing to participate but not willing to allow one’s child to participate was independently associated with multiple factors, including race, lower educational attainment, lower annual household income, public health care insurance, and higher religiosity. Conclusions: Fifty-five percent of parents were willing to allow their youngest minor child to participate in a hypothetical biobank. Building trust, protecting privacy, and addressing attitudes may increase enrollment and diversity in pediatric biobanks. (shrink)

In Political Theology on Edge, the discourse of political theology is seen as situated on an edge—that is, on the edge of a world that is grappling with global warming, a brutal form of neoliberal capitalism, protests against racism and police brutality, and the COVID-19 pandemic. This edge is also a form of eschatology that forces us to imagine new ways of being religious and political in our cohabitation of a fragile and shared planet. Engaging theories of comedy and of critical and literary theory in general, I consider the function of the buffo within opera, why this trope was an historical necessity for the generic development of opera: the buffo as a specific mechanism in the operatic machine, and what this character made possible in its wake. I take as paradigms the buffi of Mozart and Rossini, citing Don Alfonso and Don Bartolo respectively. It is my belief these operas have suffered from the general (...) misprizing in philosophy of comedy. I claim Il Barbiere di Siviglia as a drama of the letter, as positing a subversive economics. I then offer a reading of Così fan tutte as a staging of Foucauldian relations: combinatorial, topological, tactical, strategic. This brings me to what these operas understand by freedom, the subject, and the Enlightenment. What is it these operas demand of us? (shrink)

Algorithmic probability and friends: Proceedings of the Ray Solomonoff 85th memorial conference is a collection of original work and surveys. The Solomonoff 85th memorial conference was held at Monash University's Clayton campus in Melbourne, Australia as a tribute to pioneer, Ray Solomonoff, honouring his various pioneering works - most particularly, his revolutionary insight in the early 1960s that the universality of Universal Turing Machines could be used for universal Bayesian prediction and artificial intelligence. This work continues to increasingly influence (...) and under-pin statistics, econometrics, machine learning, data mining, inductive inference, search algorithms, data compression, theories of intelligence and philosophy of science - and applications of these areas. Ray not only envisioned this as the path to genuine artificial intelligence, but also, still in the 1960s, anticipated stages of progress in machine intelligence which would ultimately lead to machines surpassing human intelligence. Ray warned of the need to anticipate and discuss the potential consequences - and dangers - sooner rather than later. Possibly foremostly, Ray Solomonoff was a fine, happy, frugal and adventurous human being of gentle resolve who managed to fund himself while electing to conduct so much of his paradigm-changing research outside of the university system. Williams waco, tx: baylor university press, 2014. 196 pp. $39.95.In a year when nine people were killed in a historic black church and a litany of African American lives have been extinguished by police brutality, to say this book (...) about Dietrich Bonhoeffer and racialized interpretations of Jesus is timely is an understatement.Bonhoeffer was in search of a meaningful Christian theology through an authentic encounter with Jesus. Reggie Williams argues that Bonhoeffer, while [End Page 205] communing with people of color in 1930 Harlem at the height of its literary movement, found that meaning in the “black Christ” tradition otherwise unfamiliar to the young scholar. Bonhoeffer carried the identity of the black Christ to Germany, where he actively resisted Nazi atrocities and was executed in 1945. How is it that his ethic of resistance could be so singular, given that he was formed in much the same way as his “fundamentally oblivious” German peers (2)? Williams credits the difference to Harlem, where Bonhoeffer was schooled in hermeneutics and empathy; he further contends that Bonhoeffer’s practical experience in New York as well as the South and Cuba informed and motivated his attempts to actively eradicate the “lethal assumptions” of his peers in the German academy about God and humanity (3).On the undeniable influence of a hermeneutic, Williams states that a community of believers must have one; not only that, its hermeneutic must be connected to justice that extends concern beyond the confines of “self and kind” (3). Here Williams introduces the concept of empathy, which he describes as an ability to enter into the context of another’s suffering. Quite unintentionally, he informs the Rachel Dolezal controversy in noting that one does so without “losing grasp of one’s own separate identity” (3). Williams posits that Bonhoeffer must have experienced a “healthy and appropriate” empathy evidenced by his transformation from a theology that sanctioned racism prior to Harlem, to a willingness to share vicariously in the daily toll of racism in the black community, to resistance of racism upon returning to Germany (4). Bonhoeffer was not hamstrung by his social identity; he did not let being a white German scholar impede his ability to enter into the black American community and learn from it, “as though he had never been an outsider at all” (78).Williams argues that Bonhoeffer’s German peers did not possess such empathy because they had not shared in his Harlem experience. But there were in fact others in Germany who did not allow German nationalism to supersede Christ-centered resistance, even without the benefit of such an encounter. Although Williams does not address this fact in his argument, the historical documentation assembled in this book makes it plain that Harlem, the black literary movement, and Adam Clayton Powell Sr.’s ecclesiology did indeed have a profound effect on Bonhoeffer and his radical ethic of resistance.In addition to the exploration of Bonhoeffer’s time in Harlem, this book raises important considerations about scholarship and pedagogy that would benefit a broad audience. Williams, using Bonhoeffer’s own words, emphasizes that his early theology had been a demonstration of intellect rather than an expression of faith. Although he had written two dissertations, he was “not yet a Christian” when he set off for New York (109). When he returned, he lamented the German academy’s “guardians of purity” in Bonn who policed the borders of tradition—what was and was not taught in the classroom or preached on [End Page 206] Sunday (111). Words of Life is the sequel and companion to Phenomenology and the "Theological Turn," edited by Dominique Janicaud, Jean-Francois Courtine, Jean-Louis Chrétien, Michel Henry, Jean-Luc Marion, and Paul Ricoeur. In that volume, Janicaud accuses Levinas, Henry, Marion, and Chrétien of "veering" from phenomenological neutrality to a theologically inflected phenomenology. By contrast, the contributors to this collection interrogate whether phenomenology's proper starting point is agnostic or atheistic. Many hold the view that phenomenology after the theological turn may very well be true (...) both to itself and to the phenomenological "things themselves." In one way or another, all of these essays contend with the limits and expectations of phenomenology. As such, they are all concerned with what counts as "proper" phenomenology and even the very structure of phenomenology. None of them, however, is limited to such questions. Indeed, the rich tapestry that they weave tells us much about human experience. Themes such as faith, hope, love, grace, the gift, the sacraments, the words of Christ, suffering, joy, life, the call, touch, listening, wounding, and humility are woven throughout the various meditations in this volume. The contributors use striking examples to illuminate the structure and limits of phenomenology and, in turn, phenomenology serves to clarify those very examples. Legal decision-support systems have the potential to improve access to justice, administrative efficiency, and judicial consistency, but broad adoption of such systems is contingent on development of technologies with low knowledge-engineering, validation, and maintenance costs. This paper describes two approaches to an important form of legal decision support—explainable outcome prediction—that obviate both annotation of an entire decision corpus and manual processing of new cases. The first approach, which uses an attention network for prediction and attention weights to highlight salient case (...) text, was shown to be capable of predicting decisions, but attention-weight-based text highlighting did not demonstrably improve human decision speed or accuracy in an evaluation with 61 human subjects. The second approach, termed semi-supervised case annotation for legal explanations, exploits structural and semantic regularities in case corpora to identify textual patterns that have both predictable relationships to case decisions and explanatory value. Two years ago, the Journal of Law, Medicine & Ethics published volume 28, number 4, devoted to a symposium entitled Human Subjects Research and the Role of Institutional Review Boards - Conflicts and Challenges. I had the good fortune to be asked to serve as editor of that issue. In her introduction to the symposium, the then editor-in-chief of the journal, Ellen Wright Clayton, observed that the country is currently undergoing a major reexamination of how biomedical research is conducted. (...) While that reexamination has continued in the interim, some very recent events raise questions about the extent to which this will continue, at least in the short run, with equal vigor. The intervening years have witnessed a variety of new directions and events. The federal Office of Human Research Protections, directed by Dr. Greg Koski, who wrote a brief commentary for the last symposium,L has taken a new direction, strongly stressing the need for institutions and their institutional review boards ORBS) to engage in extensive educational and quality improvement efforts with both researchers and their own member. Biography of Habermas -- Critical theory -- Habermas and his works -- An assessment of the impact of Habermas -- Conclusion.

In this chapter, I contend that conversational philosophy has benefited immensely from the particularist and universalist projects in African philosophy tradition. As a result, it has been able to overcome the lapses inherent in particularism, universalism and eclecticism/pluralism. I argue that conversational philosophy is a more robust approach than the above-mentioned approaches, which have the problems of uniqueness, aping of Western philosophy and vagueness, respectively. I claim that conversational philosophy conceives African philosophy not as a tradition peculiar to a given (...) place, nor as one that apes Western tradition, nor as an eclectic discipline enshrouded in vagueness; but as a philosophical tradition that is both selectively particular and universal in nature in which ideas are conceptualized using linguistic resources from African worldviews but with universal applicability. I conclude by demonstrating that the mechanism of conversational philosophy is one that carefully approaches discourse on African philosophy by borrowing a leaf from both the particularist vision and the universalist vision in order to establish the discipline of African philosophy as a tradition in its own right. This paper continues a discussion concerning the distinction between disability and handicap initiated in this volume by Steven D. Edwards. Edwards argues that the reasons advanced by the WHO for this distinction in its International Classification of Impairments, Disabilities and Handicaps (ICIDH) are not valid. Edwards also criticizes my own quite different grounds for distinguishing between the two concepts. His general conclusion is that the distinction is superfluous. In this paper I claim that Edwards's reasoning is invalid. I present five (...) arguments for my case, viz., the arguments from: a) practical necessity, b) cost-effectiveness, c) clinical practice, d) the subject's vital goals, and e) the universal presence of some basic action. My own conclusion thus is that there is a need within health care for a distinction between disabilities and handicaps. I also indicate that there may be a need for further distinctions along the dimension of action-generation which is presented in the paper. (shrink)

We critically revisit the definition of thermal equilibrium, in its operational formulation, provided by standard thermodynamics. We show that it refers to experimental conditions which break the covariance of the theory at a fundamental level and that, therefore, it cannot be applied to the case of moving bodies. We propose an extension of this definition which is manifestly covariant and can be applied to the study of isolated systems in special relativity. The zeroth law of thermodynamics is, then, proven to (...) establish an equivalence relation among bodies which have not only the same temperature, but also the same center of mass four-velocity.
