Moral distress has received much attention in the international nursing literature in recent years. In this article, we describe the evolution of the concept of moral distress among nursing theorists from its initial delineation by the philosopher Jameton to its subsequent deployment as an umbrella concept describing the impact of moral constraints on health professionals and the patients for whom they care. The article raises worries about the way in which the concept of moral distress has been portrayed in some (...) nursing research and expresses concern about the fact that research, so far, has been largely confined to determining the prevalence of experiences of moral distress among nurses. We conclude by proposing a reconsideration, possible reconstruction and multidisciplinary approach to understanding the experiences of all health professionals who have to make difficult moral judgements and decisions in complex situations. Tronto explores the “care crisis” that now pervades advanced industrial societies, in which women are doing more paid work and, consequently, less of the care work of civil society. Tronto urges advanced industrial societies to rethink who is responsible for care and recognize the role that government should play in ensuring that care is provided for those who need it. Unfortunately, citizenship has traditionally been defined in ways that make no provision for responsibilities to care for others. Tronto observes that (...) “privatizing” care by relegating it to the marketplace does not provide a solution to the care crisis, since paid care work is subject to exploitation, partly because it is often done by illegal immigrants from Third World countries. Despite expecting privatization to be the likely solution to the problem, Tronto nevertheless recommends that care work be regarded as a governmental responsibility in order to make it more valued publicly. Recent events around the globe reflect the tensions and ethical dilemmas of the postmodern, postcolonial and neocolonial world that have far reaching implications for health, well-being, and human suffering. As we consider what is at stake, and what this means for local lives and human relationships, we need to examine whether the theories we draw on are adequate to further our understanding of health, and the social and material conditions of human suffering. In this paper I begin to explore the (...) question, ‘Can postcolonial feminist theories provide an inclusive scholarship that would further our understanding of human suffering and open up a path to healing?’ At issue here is whether this scholarship adds another dimension to a praxis-oriented nursing science. Artificial persons are those who speak and act for others. Nurses speak and act for patients as well as for physicians and institutions, or, more aptly, institutionalized medicine. Yet, acting for institutionalized medicine can be harmful to nurses, due to the psychological experience of moral distress and the loss of integrity of their practice. This paper illustrates the harm to nurses as expressed in narratives of their practice, and suggests some initial steps we might take in resisting the artificial personhood (...) imposed by institutionalized medicine. Good work in nursing is work that is scientifically effective as well as morally and socially responsible. The purpose of this study was to examine variables that sustain good work among entering nurses (with one to five years of experience) and experienced professional nurses despite the obstacles they encounter. In addition to role models and mentors, entering and experienced nurses identified team work, cohesiveness and shared values as levers for good work. These nurses used prioritization, team building and contemplative practices (...) to overcome obstacles. Entering professional nurses tended to avoid conflict in the work setting. Experienced nurses reported forming teams of decision makers who share similar values to resolve conflict. These findings have implications for nurse education and nursing practice. Reflection on the importance of values and virtue in sustaining good work is important. Entering professional nurses should be encouraged to seek positive role models and reflect on the lessons that can be learned from experienced exemplary nurses. (shrink)

This study was designed to address the issue of whether there is a general processor for the perception of emotion or whether there are separate processors. We examined the relationships among three channels of emotional communication in 100 healthy right-handed adult males and females. The channels were facial, prosodic/intonational, and lexical/verbal; both identification and discrimination tasks of emotional perception were utilised. Statistical analyses controlled for nonemotional perceptual factors and subject characteristics (i.e. demographic and general cognitive). For identification, multiple significant correlations (...) were found among the channels. For discrimination, fewer correlations were significant. Overall, these results provide support for the notion of a general processor for emotional perceptual identification in normal adult subjects. (shrink)

In Clinical Ethics, Robert Timko argues that the moral dilemmas of clinical medical practice can best be resolved within a framework of prima facie duties, and that the most stringent duty is that of nonmaleficence. Timko shows that respect for individual autonomy and the principle of beneficence are inadequate for the moral practice of medicine since simple adherence to either principle may be insufficient for the provision of "due care.".

We give a sharper version of a theorem of Rosický, Trnková and Adámek [13], and a new proof of a theorem of Rosický [12], both about colimits in categories of structures. Unlike the original proofs, which use category-theoretic methods, we use set-theoretic arguments involving elementary embeddings given by large cardinals such as $\alpha$-strongly compact and $C^{(n)}$-extendible cardinals.

In describing and explaining the sexes, medicine and science participated in the delineation of what was "feminine" and what was "masculine" in the Middle Ages. Hildegard of Bingen and Albertus Magnus, among others, writing about gynecology, the human constitution, fetal development, or the naturalistic dimensions of divine Creation, became increasingly interested in issues surrounding reproduction and sexuality. Did women as well as men produce procreative seed? How did the physiology of the sexes influence their healthy states and their susceptibility to (...) disease? Who derived more pleasure from sexual intercourse, men or women? The answers to such questions created a network of flexible concepts which did not endorse a single model of male-female relations, but did affect views on the health consequences of sexual abstinence for women and men and on the allocation of responsibility for infertility - problems with much social and religious significance in the Middle Ages. Sometimes at odds with, and sometimes in accord with other forces in medieval society, medicine and natural philosophy helped to construct a set of notions that divided significant portions of the world - from the behavior of animals to the operations of astrological signs - into "masculine" and "feminine." Even cases that seemed to exist outside the definitions of this duality, for example, hermaphrodite features or homosexual behavior, were brought under control by the application of gendered labels, such as "masculine women.". Pr. 46al7ff.; An. Post. 74b25, 76a26-3 o, 84b14-18, 88a31; Cael. 3o6a6ff.; Gen. An. 748a7ff.). In an early introduction to a course of lectures on the natural sciences, Aristotle also indicates the order in which those sciences ought to be presented (Mete. 338a2o-~9, 339a5-9; cf. The plan is to start with a general discussion of change and motion, then progress to studies of specific natural phenomena, ending with the many species of plants and animals. Nussbaum's conclusion is probably not intended to be as Platonic as it may appear at first blush, for she seems actually to be speaking only of connections between sciences of different substances. There is no suggestion that the study of beauty or health or geometry, say, is on a par with the sciences of substances. Nor does she suggest that Aristotle wavers in his conviction that there are fundamental cleavages between the practical and theoretical sciences, which will make a full-blown science of ethics-politics look very different from the demonstrative science of meteorology, for example. Nevertheless, even if limited to the natural sciences of substances, her claim remains an important and interesting one. In April 2004 the Irish Government commissioned Judge Maureen Harding Clark to compile a report to ascertain the rate of caesarean hysterectomies at Our Lady of Lourdes Hospital in Drogheda, Republic of Ireland. The report came about as a result of complaints by midwives into questionable practices that were mainly (but not solely) attributed to one particular obstetrician. In this article we examine the findings of this Report through a feminist lens in order to explore what a feminist reading of (...) the Report and the events that led to the inquiry will bring to light. We consider how sex and gender feature in the Lourdes case, draw attention to the deeply gendered asymmetries of power and privilege that existed between the men and women at the centre of this inquiry, and explore the impact such asymmetries had on this particular situation. Natalie Davis is a quintessential storyteller in the way theorized by Walter Benjamin, Hannah Arendt, and Michel de Certeau. Her work decenters history not simply because it grants agency and so historical visibility to those who have been hidden from history or left on its margins, but also because her stories reveal the complexities of human experience and so challenge the received categories with which we are accustomed to thinking about the world.

Historically, academic freedom is a concept aimed at resolving conflicts about the relationship between power and knowledge, politics and truth, action and thought by positing a sharp distinction between them, a distinction that has been difficult to maintain. This paper analyzes those tensions by looking at early statements of the founders of the American Association of University Professors , by exploring the paradoxes of disciplinary authority which at once guarantees and limits professorial autonomy, and by examining several cases in which (...) the question of the meaning of academic responsibility was in dispute. It argues that because the tensions are not susceptible to final resolution, the principle of academic freedom must be preserved in order to mediate them. (shrink)
