Community-based interventions (CBIs) show promise as effective and cost-effective obesity prevention initiatives. CBIs are typically complex interventions, including multiple settings, strategies and stakeholders. Cost-effectiveness evidence, however, generally only considers a narrow range of costs and benefits associated with anthropometric outcomes. While it is recognised that the complexity of CBIs may result in broader non-health societal and community benefits, the identification, measurement, and quantification of these outcomes is limited. This study aimed to understand the perspectives of stakeholders on the broader benefits (...) of CBIs and their measurement, as well as perceptions of CBI cost-effectiveness. Purposive sampling was used to recruit participants from three stakeholder groups (lead researchers, funders, and community stakeholders of CBIs). Online semi-structured interviews were conducted, taking a constructivist approach. Coding, theme development and analysis were based on published guidance for thematic analysis. Twenty-six stakeholders participated in the interviews (12 lead researchers; 7 funders; 6 community stakeholders). Six key themes emerged; (1) Impacts of CBIs (health impacts and broader impacts); (2) Broader benefits were important to stakeholders; (3) Measurement of benefits are challenging; (4) CBIs were considered cost-effective; (5) Framing CBIs for community engagement (6) Making equitable impacts and sustaining changes—successes and challenges. Across all stakeholders, broader benefits, particularly the establishment of networks and partnerships within communities, were seen as important outcomes of CBIs. Participants viewed the CBI approach to obesity prevention as cost-effective, however, there were challenges in measuring, quantifying and valuing broader benefits. Development of tools to measure and quantify broader benefits would allow for more comprehensive evaluation of the cost-effectiveness of CBIs for obesity prevention. (shrink)

In August 2001, the Israeli Ministry of Health issued its Limitation of Smoking in Public Places Order, categorically forbidding smoking in hospitals. This forced the mental health system to cope with the issue of smoking inside psychiatric hospitals. The main problem was smoking by compulsorily hospitalized psychiatric patients in closed wards. An attempt by a psychiatric hospital to implement the tobacco smoking restraint instruction by banning the sale of cigarettes inside the hospital led to the development of a black market (...) and cases of patient exploitation in return for cigarettes. This article surveys the literature dealing with smoking among psychiatric patients, the role of smoking in patients and the moral dilemmas of taking steps to prevent smoking in psychiatric hospitals. It addresses the need for public discussion on professional caregivers’ dilemmas between their commitment to uphold the law and their duty to act as advocates for their patients’ rights and welfare. In 1820 two French scientists – Pierre Joseph Pelletier and Jean Bienaimé Caventou – discovered and named the active alkaloid substance extracted from cinchona bark: quinine. The bark from the ‘wondrous’ fever tree, and its antimalarial properties, however, had long been known to both colonial scientists and indigenous Peruvians. From the mid-seventeenth century, cinchona bark, taken from trees that grow on the eastern slopes of the Andes, was part of a global circulation of botanical knowledge, practice and profit. By the (...) 1850s, Europeans eager to bypass South American trade routes to access cinchona plants established plantations across the global South in French Algeria, Dutch Java and British India. Wardian cases – plant terrariums named after British physician Nathaniel Bagshaw Ward – would fuel new imperial efforts to curb malaria, contemporaries argued. And yet cinchona trees proved difficult to transport over land and sea, and did not easily or universally thrive in new tropical climates. As a result of the growing demand and uncertainty around cinchona, as Pratik Chakrabarti has argued, from the late eighteenth century there was ‘a global scientific obsession’ with finding a ‘substitute’ for cinchona, particularly local alternatives in India and China.1. The great urbanist Jane Jacobs details how urban planning impacts the social interactions and social networks responsible for the economic death or life of a city. How might urban planning impinge on the moral values that underlie that development? I draw on Jacobs’s work on the moral foundations of commercial society to identify two “urban values” (tolerance and innovation). I then examine how these values support the social networks and processes that facilitate urban-based innovation and how urban planning can strengthen (...) or undermine those values. I use the examples of urban planning in the 15th Ward of Syracuse, New York and of city building in the private development of Cayalá in Guatemala City to illustrate these points. (shrink)

The ‘expropriation of the expropriators’ is a delicious turn of phrase, one that Marx even compares to Hegel’s infamous ‘negation of the negation’. But what does it mean, and is it still relevant today? Before I analyse the content of Marx’s expression, I briefly consider contemporary legal understandings of expropriation, as well as some examples of it. In the remainder of the essay, I spell out different kinds of expropriation in Marx and focus on an ambiguity at the core of (...) the notion of ‘expropriating the expropriators’, namely, whether it describes an immanent and objective tendency within the development of the capitalist mode of production or else actively prescribes a form of revolutionary political praxis for the working class. My answer is that it does both, though not without tension. Finally, I develop some implications of these reflections by showing how the concept of expropriation can be put to use today, in struggles around housing, climate and work. (shrink)

Kant's comments on animal minds have provoked radically different readings, with some contending animals have clear and distinct awareness of their world and others contending animals lack consciousness altogether. This paper argues that Kant's comments have received such divergent responses because, according to Kant, we inevitably slide into a deceptive anthropomorphism when talking about non-human minds. While Kant follows contemporaries, such as H.S. Reimarus, in arguing humans can only conceive of animal minds by analogy with their own, Kant's transcendental idealism (...) shifts the meaning of this analogy. The upshot is that animal minds, just like the mind of God, are intrinsically unimaginable for humans. We can talk about them, but it is mere talk; we cannot know what we are talking about. (shrink)

Payers have shaped the healthcare system in the United States as fee-for-service has facilitated a care model that prioritizes volume over the sake of patient care. This worsens health disparities, especially in safety net facilities where ancillary social work is both necessary clinically and completely uncompensated. Using concepts from Iris Marion Young’s Responsibility for Justice, it can be concluded that payers have a moral responsibility for reimbursing social care to address historical injustices. In this article, I describe the ethical hazards (...) in paying for social care and propose a way to finance this through value-based payments. (shrink)

ArgumentRecurring economic crises, like the one of 2007-2008, led to criticism of economic research and a demand to develop new strategies to avoid them. Standard economic theories use conventional approaches to deal with economic challenges, heterodox theories try to develop alternatives with which to face them. It remains unclear whether the 2007-2008 crisis led to a change in economic research as well as to a consideration of alternative approaches. We used co-word analysis to map the structure of economic research in (...) Germany between 2005 and 2014. Core topics within economic research, such as “market” or “production” hardly shift and can be linked to standard economic theories. Peripheral topics such as “inequality” or “unemployment” show greater dynamics. However, only a few of these topics can be linked to heterodox approaches. Certain changes have occurred in reaction to the 2007-2008 crisis. However, the unchanged importance of standard economic topics raises the question whether these changes are sufficient to face coming economic challenges. (shrink)

Une douleur, une expérience olfactive, une perception visuelle, une intention, une croyance et un remords ont-ils une nature mentale commune? Si oui, la science peut-elle la découvrir? Le concept d'intentionnalité a été introduit en philosophie afin de clarifier le contenu de ces questions. Il désigne la capacité de viser mentalement des objets et de se représenter mentalement des états de choses. ABSTRACT In this article, I argue that an essential part of our obligation as teachers and scholars of philosophy is to insist that the ultimate point of criticism is to foster the development of increasingly better explanations of natural and social phenomena. Doing so, moreover, requires that we cultivate in ourselves and our students a sense of gratitude for the very possibility of human flourishing and scientific advance. I illustrate these claims by showing how Dewey's analysis in Human Nature and (...) Conduct is the sine qua non of a good explanation. (shrink)

Because higher education brings members of academic communities in direct contact with students, the reflective higher education student is in an excellent position for developing two important intellectual virtues: confidence and humility. However, academic communities differ as to whether their members reach consensus, and their teaching practices reflect this difference. In this essay, Ward Jones argues that both consensus‐reaching and non‐consensus‐reaching communities can encourage the development of intellectual confidence and humility in their students, although each will do so in (...) very different ways. The title of this dissertation is a question: Is there a future to Marxist humanism? The work itself is an affirmative answer. The motive behind asking this question is the perennial debate surrounding the relevance of Marxism as a school of social and political thought. There are aspects of Marxism that are, arguably, no longer tenable, yet there are others that are more relevant today than ever. It is the argument of the following dissertation that Marxist humanism is of continued (...) relevance as both an analytical tool as well as a fertile ground for a political project of emancipation. However, the notion of a uniquely "Marxist humanism" is problematic. The fact that it is "Marxist" might lead to the confusion that it is thus based solely on the teachings of Karl Marx. Although his works provide the groundwork to the present project, it is the spirit and not necessarily the letter of Marxism that motivates the following discussion. That is, Marx's humanist project can continue without necessarily relying on his more questionable doctrines. The spirit of Marxist humanism lies in its demand for human well-being through mutual recognition achieved within an appropriately amenable social space wherein economic relations form the foundation to other equally as important intersubjective relationships. Marxist humanism is concerned primarily with the promotion of human values, namely, the alleviation of suffering and the promotion of human flourishing through a systemic change in the structure of society, beginning with economics but encompassing all areas of interpersonality...
