Despite enthusiasm for patient-centered care, the practice of patient-centered care is proving challenging. Further, it is curious that the literature about this subject does not explicitly address patient autonomy, since (1) patients guide care in patient-centered care, and (2) respect for patient autonomy is a prominent health-care value. We argue that by explicitly adopting a relational conception of autonomy as an essential component, patient-centered care becomes more coherent, is strengthened, and could help practitioners to make better use of a principle (...) of respect for autonomy. Alternative food networks are emerging in opposition to industrial food systems, but are criticized as being exclusive, since customers’ ability to patronize these market spaces is premised upon their ability to pay higher prices for what are considered the healthiest, freshest foods. In response, there is growing interest in widening the demographic profile given access to these alternative foods. This research asks: what barriers do alternative food businesses face in providing access and inclusion for low income consumers? Surveys and interviews (...) with 45 alternative food businesses in British Columbia, Canada uncovered five key barriers. The findings indicate that the barriers are symptomatic of structural issues in the Canadian food and social welfare systems. Although opportunities exist for business operators to widen access for low income shoppers, these alone cannot meaningfully ameliorate food-access inequality. Rather, these barriers underscore issues of income-disparity, poverty, and food-access inequality more broadly, and require structural and societal change to rectify. (shrink)

Examined the role of philosophical beliefs in psychotherapy approach preference. It was hypothesized that trainees would prefer approaches that most closely correspond to their personal philosophical beliefs. 59 students were given audiotaped presentations. Three dimensions of the Ss' philosophical commitments were examined in relation to their relative preferences for 3 therapy approaches: rationalist, constructivist and behavioral. Results show that Ss tended to prefer a specific approach that most corresponded to their own ontological, epistemological and causal commitments. Eating locally continues to be promoted as an alternative to growing concerns related to industrialized, global, corporate agriculture. Buying from local famers and producers is seen as a way to promote a healthier diet, reduce environmental impacts, and sustain communities. The promotion of the local food movement presents the question: is it possible to feed a community primarily from the foods produced locally? We conducted a systematic analysis comparing the United States Department of Agriculture’s (USDA) recommended dietary requirements for the (...) estimated 2008 population with annual local agricultural production for the years 2004–2008 within the counties of the Willamette Valley growing region. Our results indicate that current agricultural production in this highly fertile region does not meet the dietary needs of the local inhabitants for any of the USDA’s six food groups: grains, vegetables, fruits, dairy, meat and beans, and oils. In the most recent year of our analysis, 2008, Willamette Valley agriculture production met 67% of annual required grains, 10% of vegetable needs, 24% of fruits, 59% of dairy, 58% of meat and beans, and 0% of dietary oil requirements. Over the past 5 years there have been significant fluctuations in crop production, particularly in 2006 when grain yields dropped to 29% of needs met. Additionally, many of these commodities are exported as cash crops, thus not contributing to meeting local food needs. We discuss these results as well as areas of potential for increasing production of edible crops for local consumption in the region. (shrink)

Advances in technology are bringing greater insight into the mind, raising a host of privacy concerns. However, the basic psychological mechanisms underlying the perception of privacy violations are poorly understood. Here, we explore the relation between the perception of privacy violations and access to information related to one’s “self.” In two studies using demographically diverse samples, we find that privacy violations resulting from various monitoring technologies are mediated by the extent to which the monitoring is thought to provide access to (...) self-relevant information, and generally neuromonitoring did not rate among the more invasive monitoring types. However, brain monitoring was judged to be more of a privacy violation when described as providing access to self-relevant information than when no such access was possible, and control participants did not judge the invasiveness of neuromonitoring any differently than those told it provided no access to self-relevant information. The Alternative Model of Personality Disorders integrates several theoretical models of personality functioning, including interpersonal theory. The interpersonal circumplex dimensions of warmth and dominance can be conceptualized as traits similar to those in AMPD Criterion B, but interpersonal theory also offers dynamic hypotheses about how these variables that change from moment to moment, which help to operationalize some of the processes alluded to in AMPD Criterion A. In the psychotherapy literature, dynamic interpersonal behaviors are thought to be critical for identifying (...) therapeutic alliance ruptures, yet few studies have examined moment-to-moment interpersonal behaviors that are associated with alliance ruptures at an idiographic level. The current study examined the concurrent and cross-lagged relationships between interpersonal behaviors and alliance ruptures within each session in the famous Gloria films. Interpersonal behaviors as well as alliance ruptures were calculated at half minute intervals for each dyad. We identified distinct interpersonal patterns associated with alliance ruptures for each session: Gloria ’s warmth was positively related with withdrawal ruptures concurrently in the session with Carl Rogers; Gloria’s dominance and coldness were related with increased confrontation ruptures in the session with Fritz Perls concurrently, while her coldness was also predicted by confrontation ruptures at previous moments; lastly, both Gloria’s dominance and Albert Ellis’s submissiveness were positively related with withdrawal ruptures. These interpersonal patterns demonstrated the promise of using AMPD dimensions to conceptualize momentary interpersonal processes related to therapy ruptures, as well as the clinical importance of attuning to repetitive, dyad-specific interpersonal cues of ruptures within each session. ObjectiveTo further delineate risk and resilience factors contributing to trajectories of mental health symptoms experienced by college students through the pandemic.Participantsn = 183 college students.MethodsLinear mixed models examined time effects on depression and anxiety. Propensity-matched subgroups exhibiting “increased” versus “low and stable” depression symptoms from before to after the pandemic-onset were compared on pre-pandemic demographic and psychological factors and COVID-related experiences and coping strategies.ResultsStudents experienced worsening of mental health symptoms throughout the pandemic, particularly during Fall 2020 compared with Fall 2019. The propensity-matched subgroup exhibiting relative resilience reported less alcohol use prior to the pandemic, greater use of active coping strategies, and less of an impact on their college progress.ConclusionsResults point to several potential targets of screening and intervention to decrease residual impacts of the pandemic. Throughout the history of philosophy there has been a sustained interest in the concepts of knowledge, truth and meaning; interest in the concepts of error, falsity and nonsense, on the other hand, has been intermittent and spasmodic. Error, for example, has suffered at the expense of knowledge to such an extent that sometimes its very existence has been denied, or it has been explained away as being merely the absence of or privation of knowledge; many theories of truth are so (...) constructed that no place can be found for falsity, and theories about what constitutes making sense pay, on the whole, little heed to what constitutes nonsense. In this paper I hope to do something to redress the balance so far as error is concerned. My remarks are prompted by the hope that, just as we may best understand health through the study of disease, so a consideration of error or failure may throw light on knowledge or success. It is clearly not very informative to say of error, falsity and nonsense that they are merely the absence of knowledge, truth and sense; indeed it is just as laconic as a proposed medical definition of disease as the absence of health. U.S. regulations do not afford consistent protections to human research subjects. One complaint is that they focus on federally sponsored research, with private research covered only if it falls under the jurisdiction of the Food and Drug Administration. This paper examines a deeper problem: Even when the regulations do apply, they still do not afford consistent standards of protection. The U.S. Common Rule and related FDA regulations lack a workable regulatory control mechanism.

This paper presents the behavioral interview model that we developed to formalize our hiring practices when we, most recently, needed to hire a new clinical ethicist to join our staff at the Center for Ethics at Washington Hospital Center.

In debates over the regulation of communication related to dual-use research, the risks that such communication creates must be weighed against against the value of scientific autonomy. The censorship of such communication seems justifiable in certain cases, given the potentially catastrophic applications of some dual-use research. This conclusion however, gives rise to another kind of danger: that regulators will use overly simplistic cost-benefit analysis to rationalize excessive regulation of scientific research. In response to this, we show how institutional design principles (...) and normative frameworks from free speech theory can be used to help extend the argument for regulating dangerous dual-use research beyond overly simplistic cost-benefit reasoning, but without reverting to an implausibly absolutist view of scientific autonomy. In international law, the term "jus cogens" refers to norms that are considered peremptory in the sense that they are mandatory and do not admit derogation. Although the jus cogens concept has achieved widespread acceptance, international legal theory has yet to furnish a satisfying account of jus cogens's legal basis. We argue that peremptory norms are inextricably linked to the sovereign powers assumed by all states. The key to understanding international jus cogens lies in Immanuel Kant 's discussion of the (...) innate right of children to their parents' care. Drawing on Kant 's account, our theory of jus cogens posits that states exercise sovereign authority as fiduciaries of the people subject to their power. An immanent feature of this state-subject fiduciary relationship is that the state must comply with jus cogens. The fiduciary theory clarifies jus cogens's content by generating discrete criteria for identifying peremptory norms. (shrink)

The notion of a critical successor [5] in relational semantics has been central to most classic modal completeness proofs in interpretability logics. In this paper we shall work with a more general notion, that of an assuring successor. This will enable more concisely formulated completeness proofs, both with respect to ordinary and generalised Veltman semantics. Due to their interesting theoretical properties, we will devote some space to the study of a particular kind of assuring labels, the so‐called full labels and (...) maximal labels. After a general treatment of assuringness, we shall apply it to obtain a completeness result for the modal logic w.r.t. generalised semantics for a restricted class of frames. (shrink)

Delivering high quality genomics-informed care to patients requires accurate test results whose clinical implications are understood. While other actors, including state agencies, professional organizations, and clinicians, are involved, this article focuses on the extent to which the federal agencies that play the most prominent roles — the Centers for Medicare and Medicaid Services enforcing CLIA and the FDA — effectively ensure that these elements are met and concludes by suggesting possible ways to improve their oversight of genomic testing.

In this article, we raise ethical concerns about the potential misuse of open-source biology : biological research and development that progresses through an organisational model of radical openness, deskilling, and innovation. We compare this organisational structure to that of the open-source software model, and detail salient ethical implications of this model. We demonstrate that OSB, in virtue of its commitment to openness, may be resistant to governance attempts.
