People suffering from Obsessive-Compulsive Disorder (OCD) do things they do not want to do, and/or they think things they do not want to think. In about 10 percent of OCD patients, none of the available treatment options is effective. A small group of these patients is currently being treated with deep brain stimulation (DBS). Deep brain stimulation involves the implantation of electrodes in the brain. These electrodes give a continuous electrical pulse to the brain area in which they are implanted. (...) It turns out that patients may experience profound changes as a result of DBS treatment. It is not just the symptoms that change; patients rather seem to experience a different way of being in the world. These global effects are insufficiently captured by traditional psychiatric scales, which mainly consist of behavioural measures of the severity of the symptoms. In this article we aim to capture the changes in the patients’ phenomenology and make sense of the broad range of changes they report. For that we introduce an enactive, affordance-based model that fleshes out the dynamic interactions between person and world in four aspects. The first aspect is the patients’ experience of the world. We propose to specify the patients’ world in terms of a field of affordances, with the three dimensions of broadness of scope (‘width’ of the field), temporal horizon (‘depth’), and relevance of the perceived affordances (‘height’). The second aspect is the person-side of the interaction, that is, the patients’ self-experience, notably their moods and feelings. Thirdly, we point to the different characteristics of the way in which patients relate to the world. And lastly, the existential stance refers to the stance that patients take towards the changes they experience: the second-order evaluative relation to their interactions and themselves. With our model we intend to specify the notion of being in the world in order to do justice to the phenomenological effects of DBS treatment. (shrink)

We show that many large cardinal notions up to measurability can be characterized through the existence of certain filters for small models of set theory. This correspondence will allow us to obtain a canonical way in which to assign ideals to many large cardinal notions. This assignment coincides with classical large cardinal ideals whenever such ideals had been defined before. Moreover, in many important cases, relations between these ideals reflect the ordering of the corresponding large cardinal properties both under direct (...) implication and consistency strength. Medical professionals often deny patients who inject opioids a second or third heart valve replacement, even if such a surgery is medically indicated. However, such a position is not well defended. As this paper demonstrates, the ethical literature on the topic too often fails to develop and apply an ethical lens to analyse the issue of multiple valve replacements. This paper addresses this lacuna by analysing the case of Mr Walsh, a composite case which protects the identity of any one (...) patient, through the principlist approach of Beauchamp and Childress. It argues that the hospital should offer Mr Walsh, a second valve replacement because the procedure is: medically indicated, autonomously requested, non-maleficent, beneficent and does not violate a formal account of justice. The paper concludes with clinical ethical guidelines for valve surgery for patients with opioid use disorder. (shrink)

BackgroundIt has been well investigated that nature exposure intervention can restore directed attention and improve subsequent cognitive performance. The impairment of decision-making skills in mentally fatigued soccer players was attributed to the inability of attention allocation. However, nature exposure as the potential intervention to counter mental fatigue and improve the subsequent decision-making skill in soccer players has never been investigated.ObjectsThis study aimed to evaluate the effects of nature exposure intervention on decision-making skills among mentally fatigued university soccer players. Moreover, different (...) durations of nature exposure were also evaluated.MethodsA random control between-subject design was adopted. Players were randomly assigned into six groups with three different durations of the experimental group compared with the corresponding control group. All players were first mentally fatigued by performing a 45-min Stroop task; then, they viewed virtual photos of natural or urban scenes; and finally, they performed a soccer decision-making task.ResultsThe subjective ratings of mental fatigue were significantly higher following the Stroop task. Only Exp 3 significantly improved decision-making reaction time compared with Con 3. Moreover, the accuracy slightly increased in Exp 3 after the intervention.ConclusionIn line with attention restoration theory, nature exposure significantly improved decision-making skills in mentally fatigue university players. However, the duration must be 12.50 min for each stimulus to stay longer to attract involuntary attention. (shrink)

In this paper, we argue that boredom can be an important experience that contributes to personal autonomous agency by providing authentic motivation, and that strategies of social media providers to bind users’ attention to their platforms undermine this authenticity. As discussed in social epistemology and media ethics for a while now, such strategies can lead to so-called epistemic or filter bubbles. Our analysis of the relation between boredom and social media use focuses on a similarly impairing effect of social media (...) on users’ autonomy, which we call authenticity bubbles. (shrink)

Cultured meat is a lab grown product that aims to tackle the cravings of omnivores who struggle to switch to a plant-based diet, while still being friendly to animals and the environment. Possibly, in time, the curiosity to apply this technology towards human meat production will emerge. However, when presented with the thought of eating cultured human meat potential consumers’ reaction greatly varies from pure disgust to indifference to excitement. This instinctive response indicates a lack of preformed judgements towards the (...) topic. Without a clear vision on the possibility of cultured human meat, scattered and uncertain regulations will fail to uphold paramount moral values. The risk is that we would either dig into this option out of excitement, or ban it without convincing motivations. The ethical theories of deontology and consequentialism can be followed to investigate this divisive issue. With an evaluation based on disgust I argue that the deontological perspective is mostly concerned with values of identity and humanness, while with a chain-reaction reasoning I argue that consequentialism would be concerned with health safety, privacy and equality. I conclude that cultured human meat is not acceptable. (shrink)

Background The Constitutional Court of South Korea declared that an abortion ban was unconstitutional on April 11, 2019. The National Health Care System will provide abortion care across the country as a formal medical service. Conscientious objection is an issue raised during the construction of legal reforms. Methods One hundred sixty-seven perioperative nurses responded to the survey questionnaire. Nurses’ perception about conscientious objection, support of legislation regarding conscientious objection, and intention to object were measured. Logistic regression was used to explore (...) the factors associated with support of the legislation and the intention to conscientiously object. Results Only 28.8% of the responding nurses were aware of health care professionals’ conscientious objection. The majority felt that patients’ rights should be prioritized over health care professionals’ conscientious objection. On the other hand, 45.8% supported the legislation on conscientious objection to abortion, and 42.5% indicated a willingness to refuse to participate in an abortion case if conscientious objection was permitted. Religion, awareness of conscientious objection, and prioritizing of nurses’ right to conscientious objection were significantly associated with supporting the legislation. Moreover, religion and prioritizing nurses' rights were significantly associated with the intention to conscientiously object. Conclusions This study provides information necessary for further discussion of nurses’ conscientious objection. Nursing leaders, researchers, and educators should appeal to nurses and involve them in making policies that balance a women's right to non-discrimination and to receiving appropriate care with nurses' rights to maintain their moral integrity without compromising their professional obligation. This article explores how both corporate governance and corporate social responsibility can be improved by using insights from complexity theory. Complexity theory reveals that decentralized governance architecture is required for firms to absorb competently the increased intricacies, variety of variables, and objectives introduced by CSR. The current predominant form of centralized governance based on command-and-control hierarchies copes with complexities by reducing data inputs. This approach results in firms reducing their objectives, concerns, and insights about CSR. Firms with a decentralized “network” (...) form of governance architecture are used to illustrate how the data inputs of each manager can be reduced through the decomposition of decision-making labor to improve the capability of the firm to intelligently absorb and manage complexity. Network governance also introduces a division of powers with stakeholders to facilitate information flow and strengthen incentives to manage the enterprise to enhance both shareholder value and CSR. Participants scoring high and low on a measure of repressive coping style (Mendolia, 2002) first learned a series of related word pairs (cue–target). Half of the cues were homographs. In the subsequent think/no-think phase (Anderson & Green, 2001), they responded with targets on some trials and suppressed thoughts of targets on others. Suppressed targets were always emotionally negative, as were targets associated with baseline cues reserved for the final test. Some participants were provided with emotionally benign or positive substitutes to (...) help them suppress, and these substitutes were related to different meanings of the homographic cues, compared to those established by the targets. On the final test, all cues were presented for target recall. Only the repressors significantly benefited from the provision of positive substitutes to aid forgetting of the negative targets, regardless of the nature of the cues. (shrink)

The emergence of new digital technologies in modern work organizations is also changing the way employees and employers communicate, design work processes and responsibilities, and delegate. This paper takes an interdisciplinary—namely sociological and philosophical—perspective on the use of AI in healthcare work organizations. Using this example, structural power relations in modern work organizations are first examined from a sociological perspective, and it is shown how these structural power relations, decision-making processes, and areas of responsibility shift when AI is used. In (...) the subsequent ethical part, opportunities for a fairer organization of work, but also dangers due to possibly changed power relations are elaborated and evaluated by presenting a realistic scenario from everyday clinical practice. After combining a proceduralist account of organizational ethics with a virtue-ethical approach, it is argued that certain organizational and character dispositions are necessary for employers and employees to meet the challenge of changing structural power relations in the future. With the same goal, a summative sociological perspective discusses challenges to workplace co-determination. (shrink)
