BackgroundParent-mediated intervention has been studied in promoting skill acquisition or behavior change in the children with autism spectrum disorder. Most studies emphasize on the improvement of child’s core symptoms or maladaptive behaviors, making parental perceived competence and self-efficacy secondary. Yet, the evaluations of intervention implementation are under-reported, especially when translating such interventions into a new population or context. This research investigated the intervention implementation of a 12-week parent coaching intervention which was delivered through telehealth and tailored to Chinese population. The (...) intervention was based on the Parent-mediated Early Start Denver Model with culturally adapted lectures, manuals, and demonstration and commentary videos. This study aimed to evaluate the intervention implementation by assessing parents’ satisfaction, acceptability, appropriateness, and feasibility.MethodA randomized controlled trial was conducted with two telehealth conditions: self-directed and web+group therapy. Parents in the self-directed condition received intervention individually through the online learning platform. The web+group therapy condition navigated the same program with weekly 1.5-h group coaching sessions via videoconferencing. This mixed-methods study used a concurrent convergent design to evaluate the intervention implementation at post-intervention. The quantitative data was collected from the Program Evaluation Survey and the qualitative data was collected from five focus groups.ResultsParents in self-directed group reported significantly lower scores in total perceived competence than parents in web+group therapy condition, while there was no group difference on the total self-efficacy. Tailored feedback, demonstration and commentary videos, peer commenting, live coaching, and guided reflection were the top-five acceptable telehealth strategies that were strongly endorsed by parents. Family centered care, home-based intervention, strategies relative to daily activities, the remote learning platform, and the program-based community were elements that parents considered when evaluating the program’s appropriateness. Parent modeling, step-by-step instructions, and tailored feedback were key components in making intervention strategies feasible for parents to implement at home.ConclusionFindings indicate the application of telehealth was acceptable, appropriate, and feasible for Chinese parents. Group-based parent coaching intervention via videoconferencing could be a promising home-based service model to increase parental perceived competence. A large-scale RCT is needed to investigate the effectiveness of group-based PMI via telehealth. (shrink)

Let me start with my conclusions: like most other philosophers of his era, Spinoza did not have well-developed views on consciousness and its place in the mind. Somewhat paradoxically, however, a basic tenet of his metaphysics generated a problem which might have been solved if he had thought more about those issues. So in the end, then, Spinoza did not have much to say about consciousness even though the coherency or at least the plausibility of his system demanded it. With (...) such being my assessment of Spinoza’s views on consciousness, it will come as no surprise that I regard the prospects for a robust and coherent Spinozistic theory of consciousness as dim. As explained later, I differ in this respect from some prominent Spinoza scholars. At the same time, even if we won’t find much guidance from Spinoza for thinking about consciousness, I believe that he has much to teach us about the mind. In my view, Spinoza’s philosophy of mind is instructive precisely because it attempts to understand the mind without reference to consciousness. This can and should be a healthy corrective to contemporary philosophy of mind, which is prone to inflate the place of consciousness in the mind. To make and defend all of these points, I divide my chapter into four sections. In the first, I offer a sketch of consciousness in seventeenth-century philosophy generally and Spinoza’s work specifically, a sketch which is intended to show that consciousness did not feature prominently in their accounts of the mind. Then, I note how a major problem plagues Spinoza’s account of the relationship between the physical and the mental, and I show how two attempted solutions of this problem both fail. By way of conclusion, I suggest why Spinoza might be interesting to philosophers of mind today, his silence on consciousness notwithstanding. (shrink)

One argument for the moving spotlight theory is that it better explains certain aspects of our temporal phenomenology than does any static theory of time. Call this the argument from passage phenomenology. In this paper it is argued that insofar as moving spotlight theorists take this to be a sound argument they ought embrace a new version of the moving spotlight theory according to which the moving spotlight is a cresting wave of causal efficacy. On this view it is more (...) than just presentness that is temporary: a range of other fundamental properties are also temporary because presentness synchronically changes the fundamental properties that are instantiated in the present moment, from those instantiated when that moment is future, or past. On this view having experiences as of presentness co-varies with the presence of presentness, allowing the moving spotlight theorist to provide a compelling explanation for why we have the temporal phenomenology we do, and to explain how we can know that we are present by reflecting on the nature of said phenomenology. It begins by characterizing what is involved in having a virtuous character trait. It then reviews some of the claims made in two of the leading research traditions on traits in psychology: situationism and the Big Five model. Philip Goff has recently argued that due to the ‘subject-summing problem’, panpsychism cannot explain consciousness. The subject-summing problem is a problem which is analogous to the physicalist's explanatory gap; it is a gap between the micro-experiential facts and the macro-experiential facts. Goff also suggests that there could be a solution by way of a ‘phenomenal bonding relation’, but believes that this solution is not up to scratch because we cannot form a positive not-merely-role-playing concept of this relation. In this paper, (...) I argue that the phenomenal bonding solution is up to scratch. I argue that the panpsychist, by carefully inspecting their phenomenology and scrutinising their concepts, can form a positive concept of the phenomenal bonding relation. By doing this they can start to get around their explanatory gap. (shrink)

I explain the notion of contributory injustice, a kind of epistemic injustice, and argue that it occurs within psychiatric services, affecting those who hear voices. I argue that individual effort on the part of clinicians to avoid perpetrating this injustice is an insufficient response to the problem; mitigating the injustice will require open and meaningful dialogue between clinicians and service user organisations, as well as individuals. I suggest that clinicians must become familiar with and take seriously concepts and frameworks for (...) understanding mental distress developed in service user communities, such as Hearing Voices Network, and by individual service users. This is especially necessary when these concepts and frameworks explicitly conflict with medical or technical understandings of users’ experiences. I defend this proposal against three objections. In this paper, I explore one of the most important but least discussed components of an evidentialist case for or against theism: its intrinsic plausibility and simplicity as a theory aside from the evidence. This is a crucial consideration in any inductive framework, whether Inference to the Best Explanation, probabilism, or another. In the context of Bayesian reasoning, this corresponds to an assessment of theism's intrinsic probability. I offer a survey of how philosophers of science have attempted to evaluate the (...) intrinsic plausibility and simplicity of scientific theories more generally, before applying these considerations to the question of God's existence. Objective: Caloric vestibular stimulation has traditionally been used as a tool for neurological diagnosis. More recently, however, it has been applied to a range of phenomena within the cognitive neurosciences. Here, we provide an overview of such studies and review our work using CVS to investigate the neural mechanisms of a visual phenomenon - binocular rivalry. We outline the interhemispheric switch model of rivalry supported by this work and its extension to a metarivalry model of interocular-grouping phenomena. In addition, studies (...) showing a slow rate of binocular rivalry in bipolar disorder are discussed, and the relationship between this finding and the interhemispheric switch model is described. We also review the effects of CVS in various clinical contexts, explain how the technique is performed and discuss methodological issues in its application. (shrink)

The development of photodynamic therapy and anti-vascular endothelial growth factor agents have revolutionized the treatment of retinal diseases, transforming the retina subspecialty by ushering in an age of pharmacological treatments for a wide range of diseases, including age-related macular degeneration.

Mood disorders, including major depressive disorder and bipolar disorder, are highly prevalent, frequently disabling, and sometimes deadly. Additional research and more effective medications are desperately needed, but clinical trials research in mood disorders is fraught with ethical issues. Although many authors have discussed these issues, most do so from a theoretical viewpoint. This manuscript uses available empirical data to inform a discussion of the primary ethical issues raised in mood disorders research. These include issues of consent and decision-making capacity, including (...) patients’ motivations for participating in research. We also address drug withdrawals, placebo controls, and the overall safety of research. Finally, we examine the extant literature for studies discussing potential indirect benefits of clinical trials research to participants. Taken together, the evidence suggests that clinical trials research incorporating drug withdrawals and placebo controls can be conducted safely and ethically, even in patients with severe or treatment-resistant mood disorders. In fact, given the dearth of effective treatment options for this population, it is our opinion that a moral imperative exists to extend the offer of research participation to severely ill or treatment-resistant groups. Miller aims to stimulate new work in religious ethics through discussions of ethnography, ethnocentrism, relativism, and moral criticism; the ethics of empathy; the meaning of moral responsibility in relation to children and friends; civic virtue, loyalty, war, and alterity; the normative and psychological dimensions of memory; and religion and democratic life.

For a minority of children managed in the NICU, there is a need for more complex technologic assistance in order to sustain life, mitigate a more chronic debilitation from a pervasive life-limiting condition, or provide a bridge from life-sustaining therapy to a more semi-permanent treatment such as organ transplantation. This chapter will address two major types of technology assistance for infants and children—tracheostomy and assisted home ventilation, and dialysis—and the myriad complications and considerations that they raise. Some attention to why (...) clinicians may be so inclined to impose technology as a solution to life-limiting conditions will be noted, as well as why some parents may seem to insist on pursuing technology. Seeing a red hat can (i) increase my credence in the hat is red, and (ii) introduce a negative dependence between that proposition and po- tential undermining defeaters such as the light is red. The rigidity of Jeffrey Conditionalization makes this awkward, as rigidity preserves inde- pendence. The picture is less awkward given ‘Holistic Conditionalization’, or so it is claimed. I defend Jeffrey Conditionalization’s consistency with underminable perceptual learning and its superiority to Holistic Conditionalization, arguing that the latter is merely (...) a special case of the former, is itself rigid, and is committed to implausible accounts of perceptual con- firmation and of undermining defeat. The movement to assess teacher competency is becoming a central concern for professional educators, state departments of education, and the public. The major underlying assumption of this concern is that primary and secondary school-aged children are falling far behind in basic skills as compared with their counterparts in other countries. A further concern is that, within this country, variability in teacher competency may exacerbate differences among children of various socioeconomic, racial and ethnic backgrounds and thus perpetuate long-term educational and economic (...) inequalities. To support these assumptions, critics of teacher preparation institutions cite declining SAT and ACT test scores of those entering teacher training, and also this constitutes sufficient grounds for assuming that becoming a teacher is less intellectually demanding than preparing for other fields.
