While the value of early detection of dementia is largely agreed upon, population-based screening as a means of early detection is controversial. This controversial status means that such screening is not recommended in most national dementia plans. Some current practices, however, resemble screening but are labelled “case-finding” or “detection of cognitive impairment”. Labelled as such, they may avoid the ethical scrutiny that population-based screening may be subject to. This article examines conceptualizations of screening and case-finding. It shows how the definitions (...) and delimitations of the concepts (the _what_ of screening) are drawn into the ethical, political, and practical dimensions that screening assessment criteria or principles are intended to clarify and control (the _how_ of screening, how it is and how it should be performed). As a result, different conceptualizations of screening provide the opportunity to rethink what ethical assessments should take place: the conceptualizations have different ethico-political implications. The article argues that population-based systematic screening, population-based opportunistic screening, and case-finding should be clearly distinguished. (shrink)

This thesis examines the practice of haemodialysis and kidney transplantation, the two medical therapies available for persons with kidney failure, from a phenomenological perspective. A basic assumption made in the thesis is that contemporary biomedicine is deeply embedded in the cultural, historical, economic, and political circumstances provided by the particular local, national, and transnational contexts in which it is practiced. The aim of the thesis is twofold. On the one hand, the aim is to examine the forms of person- and (...) patienthood enacted and negotiated in haemodialysis and kidney transplantation care and in the daily lives of persons with kidney failure. On the other hand, the aim is to investigate the ways in which the enacted and negotiated forms of person- and patienthood are culturally embedded and normatively charged. In order to examine and investigate this twofold aim, an empirical material has been gathered that comprises observations and in-depth interviews with patients and caregivers at four haemodialysis units, one in Riga, Latvia, and three in Stockholm, Sweden. The theoretical approach and methodology of the study is cultural and phenomenological in character, drawing on an ethnological and anthropological understanding of culture as processual and relational, and on a phenomenological understanding of personhood as embodied and intertwined with the surrounding world. The thesis shows that patients’ encounters and attempts to deal with the diagnoses and treatments associated with kidney failure are complex and often misalign them with the normatively charged orientations enacted and recommended by medicine. The complex situation that emerges when they undergo transplantation, for example, stands in stark contrast to the widespread official view of organ transplantation as a self-evidently health-bringing and normalising therapy. Through their repeated and extensive experiences of undergoing haemodialysis and living with the disease, patients eventually become able to create a synthesis between their lived experiences of their own body and their body as a medical object – what in the thesis is called a ‘sick body’ – a synthesis that allows them to reorient themselves in life and experience a sense of direction. This process relies, to a large extent, on the temporal structure that haemodialysis affords life; it is by repeatedly undergoing the treatment that patients become able to create a sick body. Even so, many of them find this temporal structure problematic; they experience it as disruptive of their control and future-orientedness and as causing an existentially difficult-to-handle boredom. The thesis also shows that the political developments and the norms prevalent in the two national contexts studied greatly affect the orientations of the treatment practices and the participants’ lives. In both Riga and Stockholm, ideals of freedom, activity, control, and self-actualisation influence what forms of patienthood and personhood are enacted. The study indicates that persons who fall ill with a serious and chronic disease only gradually become able to understand and actively cope with their differently embodied circumstances of life. This suggests that medical professionals should not too hastily enlist their patients as experts on their own bodies, but rather provide them with the time and support necessary for making repeated attempts at creating and maintaining a life with a sick body. This article examines a population-based opportunistic screening practice for cognitive impairment that takes place at a hospital in Sweden. At the hospital, there is a routine in place that stipulates that all patients over the age of 65 who are admitted to the ward will be offered testing for cognitive impairment, unless they have been tested within the last six months or have been diagnosed with any form of cognitive impairment. However, our analysis shows that this routine is not universally (...) and mechanically applied. Rather, the health care professionals have developed and use an ethico-political judgment, concerning, for example,whomto test,whento offer the tests, andhowto explain and perform them. This article explores the role and practice of this form of judgment, emphasising its political and ethical nature. The analysis is based on qualitative interviews with professionals and patients, and draws on the theories of Aristotle and Hannah Arendt. (shrink)

This article explores an example of person-centred care: the work of so-called renal care coordinators. The empirical basis of the article consists of qualitative interviews with renal care coordinators, alongside participant observations of their patient interactions. During the analyses of the empirical material, I found that that one of the coordinators’ most fundamental ambitions is to get to know who the patient is. This is also a central tenet of person-centred care. The aim of the article is not only to (...) argue for the plausibility of this tenet, but also, and more importantly, to highlight and explore its implications in the context of healthcare, through the example of renal care coordination. By drawing on the philosophy of Hannah Arendt, the article shows that the disclosure of who the patient is that takes place in person-centred care requires speech and action, which are modes of human activity that initiate processes characterized by unpredictability, uncertainty, and irreversibility. This unpredictability, uncertainty, and irreversibility, found to be inherent in person-centred care, is then discussed in relation to the pursuit of certainty characterizing contemporary evidence-based medicine. At the end of the article the conclusion is drawn that, if healthcare is to be person-centred, it must find ways of accommodating the contradictory pursuits of certainty and uncertainty found in evidence-based medicine and person-centred care respectively. Phenomenological analyses of ageing and old age have examined themes such as alterity, finitude, and time, not seldom from the perspective of “healthy” aging. Phenomenologists have also offered detailed analyses of lived experiences of illness including lived experiences of dementia. This article offers a phenomenological account of what we label as entering the grey zone of aging between “healthy” aging and aging with a disease. This account is developed through a qualitative phenomenological philosophy analysis of elderly persons’ lived experiences of (...) being tested for dementia through primary care in Sweden, i.e., within a cultural context where dementia commonly is understood as a frightening a loss of self even though this understanding also is questioned. To enter this grey zone of aging, we argue, does not dissolve dynamic self-becoming but can involve an experience of oneself as being old. Further, in the grey zone, the self experiences itself as neither fully healthy nor as having a disease, and as needing to negotiate and live this ambiguity. To enter this grey zone is to enter an affectively charged, sociocultural and medicalized zone, and while the self can still act in different ways within it, staying in the grey zone can result in a re-orientation in the self’s mode of being, in ways that are thoroughly beyond its control. To stay in the grey zone can have detrimental effects on the self, even though the self does not have a disease: the self can become “stuck” in a reflective mode of attending to embodiment, aging, health, and disease. This report presents a comprehensive set of recommendations for protection of human beings who are trafficked for the purpose of organ removal or are targeted for such trafficking. Developed by an interdisciplinary group of international experts under the auspices of the project Trafficking in Human Beings for the Purpose of Organ Removal, these recommendations are grounded in the view that an individual who parts with an organ for money within an illegal scheme is ipso facto a victim and that the (...) crime of trafficking in human beings for the purpose of organ removal intersects with the crime of trafficking in organs. Consequently, the protection of victims should be a priority for all actors involved in antitrafficking activities: those combating organ-related crimes, such as health organizations and survivor support services, and those combating trafficking in human beings, such as the criminal justice sectors. Taking into account the special characteristics of THBOR, the authors identify 5 key stakeholders in the protection of human beings trafficked for organ removal or targeted for such trafficking: states, law enforcement agencies and judiciary, nongovernmental organizations working in the areas of human rights and antitrafficking, transplant centers and health professionals involved in transplant medicine, and oversight bodies. For each stakeholder, the authors identify key areas of concern and concrete measures to identify and protect the victims of THBOR. The aim of the recommendations is to contribute to the development of a nonlegislative response to THBOR, to promote the exchange of knowledge and best practices in the area of victim protection, and to facilitate the development of a policy-driven action plan for the protection of THBOR victims in the European Union and worldwide. Wie wollen wir leben und wie können wir unserem Leben Sinn geben angesichts der drängenden Krisen der Gegenwart? Wie können solche Fragen überhaupt beantwortet werden, wenn eine säkulare Kultur scheinbar nur Antworten auf Fragen nach dem „wie?“, nicht aber nach dem „wozu?“ erlaubt. Martin Kolmar deutet im vorliegenden Buch die gegenwärtigen und bevorstehenden Krisen, allen voran die Klimakrise, als Krise der westlichen Denk- und Wahrnehmungsweisen und versucht einen Ausweg daraus aufzuzeigen. The project of this paper is to synthesize enactivist cognitive science and practice theory in order to develop a new account of pretend play. Pretend play is usually conceived of as a representationalist phenomenon where a pretender projects a fictional mental representation onto reality. It thus seems that pretense can only be explained in representationalist terms. In this paper, we oppose this usual approach. We instead propose not only new explanatory tools for pretend play, but also a fundamental reconceptualization of (...) the phenomena of pretend play, that is, of the very explanandum of theories of pretense. To do so, we suggest combining the turn to action and embodiment in the cognitive sciences with the practice turn in the humanities. From our point of view, pretend play has to be seen in its role in human life as a whole, which is to help children to learn to master the complex sociocultural contingencies of the manifold social practices that make up social reality. Pretend play should therefore be conceived as alternative sense-making that is always related, in varying ways, to ordinary social practices. Pretenders do not need to project mental representations onto reality, but make sense of their surroundings in different ways than encultured adults in ordinary practices. In the paper, we spell out this view and show how it enables an enactivist reconceptualization of imagination, intentions and knowledge, which are usually thought of as being available only to representationalist accounts of pretense. Drosophila flies began to be used in the study of species evolution during the late 1930s. The geneticists Natasha Sivertzeva-Dobzhansky and Elizabeth Reed pioneered this work in the United States, and María Monclús conducted similar studies in Spain. The research they carried out with their husbands enabled Drosophila population genetics to take off and reveals a genealogy of women geneticists grounded in mutual inspiration. Their work also shows that women were present in population genetics from the beginning, although their contributions (...) have previously remained unacknowledged. To assess the merits and demerits of the content of Culp’s educational programme, the paper does three things: First, it discusses whether Culp’s defence against conceivable objections manages to effectively dispel the charge of cosmopolitan arrogance. Second, it spells out one implication of epistemic modesty, which Culp considers a core competence to be imparted by citizenship education. Third, it reflects upon the tricky task of motivating individuals to comply with the demands of justice. Taken together, the paper argues that Culp’s (...) case is impressively strong but nevertheless tends to suffer from a rationalist constriction. It does not leave sufficient room for tradition in private life and public reasoning. This rationalist constriction is problematic from a normative philosophical, and especially a practice-oriented viewpoint. (shrink)

In his treatment of a Kantian concept of moral freedom, Schiller argues for two kinds of freedom: freedom in the spirit of autonomous practical reason and freedom in which man is considered a mixed (sensual and rational) being. It is apparent that Schiller is on a Reinholdian path. He follows Reinhold’s theory of free will in conceiving of moral freedom primarily as the capacity to decide between the material drive as a sensible, self-interested drive and the formal drive as a (...) rational, unselfish drive. But it is also obvious that Schiller modifies Reinhold’s results in order to obtain a concept of aesthetic freedom. This project is important in view of a deeper understanding of the concept of aesthetic consciousness but is of little use in achieving a better understanding of the concept of moral freedom. Besieged by ongoing economic crises, global health emergencies, geopolitical instabilities, ecological devastation, and growing political resentments, the intractable nature of the problems that configure the present has never loomed larger or more darkly. But what, indeed, is a problem? Problematising the modern image that treats problems as obstacles to be overcome by the progress of technoscientific knowledge and policy, this introductory article lays the groundwork for a generative conceptualisation of problems. Reweaving intercontinental connections between traditions of French philosophy and American (...) pragmatism, it proffers a conception of the problematic as a mode of existence that is irreducible to the subjective, the methodological, or the epistemological. Problems go all the way down and up, requiring nothing less than an art of metamorphosis capable of engendering processes of creation, invention, and transformation in whose hold bodies and practices, knowledges and lives, thoughts and worlds, are done and undone, made and remade. A source of much difficulty and confusion in the interpretation of quantum mechanics is a naive realism about operators. By this we refer to various ways of taking too seriously the notion of operator-as-observable, and in particular to the all too casual talk about measuring operators that occurs when the subject is quantum mechanics. Without a specification of what should be meant by measuring a quantum observable, such an expression can have no clear meaning. A definite specification is provided by (...) Bohmian mechanics, a theory that emerges from Schrödinger's equation for a system of particles when we merely insist that particles means particles. Bohmian mechanics clarifies the status and the role of operators as observables in quantum mechanics by providing the operational details absent from standard quantum mechanics. It thereby allows us to readily dismiss all the radical claims traditionally enveloping the transition from the classical to the quantum realm — for example, that we must abandon classical logic or classical probability. The moral is rather simple: Beware naive realism, especially about operators! (shrink)

The idea that the changing entropy of a system is relevant to explaining why we know more about the system's past than about its future has been criticized on several fronts. This paper assesses the criticisms and clarifies the epistemology of the inference problem. It deploys a Markov process model to investigate the relationship between entropy and temporally asymmetric inference.

As a distinctive voice in the current philosophy of technology, postphenomenology elucidates various ways of how technologies “shape” both the world and humans in it. Distancing itself from more speculative approaches, postphenomenology advocates the so-called empirical turn in philosophy of technology: It focuses on diverse effects of particular technologies instead of speculating on the essence of technology and its general impact. Critics of postphenomenology argue that by turning to particularities and emphasizing that technologies are always open to different uses and (...) interpretations, postphenomenology becomes unable to realize how profoundly technology determines our being in the world. Seeking to evaluate the postphenomenological ability to radically reflect on the human being conditioned by technology, I discuss the two most pertinent criticisms of postphenomenology: an “existential” one by Robert C. Scharff and an “ontological” one by Jochem Zwier, Vincent Blok, and Pieter Lemmens. Assessing the ontological alternative, I point to incapacity of Heidegger’s concept of Enframing to do justice to material technologies. Simultaneously, I acknowledge the necessity of speculating on technology as transcending concrete technologies. Such speculating would be instrumental in reviving Ihde’s idea of non-neutrality of technology in its full philosophical potency. (shrink)

Additional theorizing about mathematical practice is needed in order to ground appeals to truly useful notions of the virtues in mathematics. This paper aims to contribute to this theorizing, first, by characterizing mathematical practice as being epistemic and “objectual” in the sense of Knorr Cetina The practice turn in contemporary theory, Routledge, London, 2001). Then, it elaborates a MacIntyrean framework for extracting conceptions of the virtues related to mathematical practice so understood. Finally, it makes the case that Wittgenstein’s methodology for (...) examining mathematics and its practice is the most appropriate one to use for the actual investigation of mathematical practice within this MacIntyrean framework. At each stage of thinking through mathematical practice by these means, places where new virtue-theoretic questions are opened up for investigation are noted and briefly explored. (shrink)

We study generalizations of Dodd parameters and establish their fine structural properties in Jensen extender models with λ-indexing. These properties are one of the key tools in various combinatorial constructions, such as constructions of square sequences and morasses.

Based on Moscovici’s classical study on the cultivation of psychoanalytic ideas in France in the 1950’s and our own research on modern biotechnology, we propose a paradigm for researching social representations. Following a consideration of the nature of representations and of the ‘iconoclastic suspicion’ that haunts them, we propose a model of the emergence of meaning relating three elements: subjects, objects, and projects. The basic unit of analysis is the elongated triangle of mediation : subject 1, object, project, and subject (...) 2, captured in the image of a ‘Toblerone’. Such social units cultivate, that is produce, circulate and receive representation which may be embodied in four modes–habitual behaviour, individual cognition, informal communication and formal communication–and in three mediums–words, visual images or non-linguistic sounds. We propose an operational definition of a ‘social representation’ as the comparison of four characteristics of communication systems: the content structures , the typified processes , and their functions , within the context of segmented social milieus. Seven implications for research on social representations are outlined: content and process; segmentation by social milieus rather than taxonomies; cultivation studies within social milieus; multi-method analysis; time structures and longitudinal data; the crossover of cultural projects and trajectories; the disinterested research attitude. This ideal type paradigm leads to an operational clarification to identify new research questions, and to guide the design and evaluation of studies on social representations. (shrink)

Building on recent developments in radical democratic theory, in this article we articulate and explore a fresh perspective for theorists and activists of radical democracy: a ‘living critique of domination’. Characterized by a two-fold analytical effort, a ‘living critique of domination’ calls for a radical critique of contemporary forms of power and control coupled with a reappraisal of emancipatory political experiences created by the political action of the Many. We demonstrate that this project responds to the theoretical and practical challenges (...) faced by a politics of emancipation today. Our article offers a first articulation of this living critique through a discussion of three recent political experiences, namely, the 2016 French uprising, Nuit Debout, as well as the Black Lives Matter and #MeToo movements. From the early 1790s until after the turn of the century, a very productive but also controversial exchange took place between Reinhold and Fichte. Though many key aspects of post-Kantian philosophy were discussed, the philosophical confrontation between Reinhold and Fichte is most instructive for the understanding of post-Kantian philosophy. The exchange started when Fichte published his verdict on Reinhold's Elementarphilosophie and disapproved of its fundamental principle. In 1794 Fichte challenged Reinhold by presenting his Wissenschaftslehre. Reinhold was not convinced of Fichte's (...) foundation of philosophy at first, but announced that he accepted the Wissenschaftslehre in 1797. While Reinhold and Fichte officially collaborated in the following three years, tensions concerning fundamental questions were still present. When Reinhold adopted Rational Realism, his relation to Fichte deteriorated and the exchange between the two finally ended. The contributions in the present collection focus on the central systematic issues at the different stages of the confrontation between Fichte and Reinhold, thereby illuminating questions that are essential to the understanding of the evolution of post-Kantian German philosophy. A close reading of the three Bhavanakramah texts, written by Kamalasila, reveals that their author was aware of two competing concepts of meditation prevalent in Tibet at the time of their composition. The two concepts of meditation,associated with the Sanskrit words bhavana and dhyana, can be related respectively to the Indian and Chinese sides of the well-known debates at bSam yas. The account of the Mahayana path outlined in these texts implies an acceptance of the precedence of bhavana over dhyana. (...) In this paper I argue that Kamalasila advocated bhavana – a conception of meditation which encompasses non-conceptual dhyana, but which also includes a discernment of reality that is conceptual in nature. Such conceptual discernment should not be understood simply as a process of ordinary rational understanding but rather as constituting a special kind of meditative wisdom. A failure to recognize the subtle differences between Kamalasila’s employment of the terms dhyana and bhavana, along with his advocacy of the latter, could easily lead to mistranslation and, with this, a basic misunderstanding of his position. In particular, it could lead to a conception of insight that is overly intellectual in nature. Given the historically important role that these texts played in the formation of Tibetan Buddhism, the implications of such a misconception could be far-reaching. This paper attempts to clarify the key meditation terminology found in the Bhavanakramas as well as demonstrate the rationale for using ‘meditation’ as the default translation for bhavana. (shrink)
