Background: Informed consent is regarded as a contract between autonomous and equal parties and requires the elements of information disclosure, understanding, voluntariness and consent. The validity of informed consent for critically ill patients has been questioned. Little is known about how these patients experience the process of consent.Objective: The aim of this study was to explore critically ill patients’ experience with the principle of informed consent in a clinical trial and their ability to give valid informed consent.Design: 11 stroke patients (...) who had been informed about thrombolytic treatment and had been through the process of deciding whether or not to participate in a thrombolysis trial went through repeated qualitative semistructured interviews.Results: None of the patients had any clear understanding of the purpose of the trial. Neither did they understand the principles of randomisation and voluntariness. Reasons for giving or not giving consent were trust, conceptions of benefits and risks and altruism. Several patients found it immoral to involve patients in the consent procedure and argued that this was the doctors’ responsibility. Others argued that it is a duty to question patients and perceived it as a sign of being treated with respect and dignity. A majority of the patients found the consent process vague and ambiguous.Conclusions: The results indicate that the principle of informed consent from critically ill patients cannot be seen as a contract between equal and autonomous parties. Further studies are needed to explore critically ill patients’ experiences with the process of informed consent. (shrink)

Meta-theories, base-theories and theories of practice in Pratical Theology The problem of meta-theories and base-theories, their interaction and the influence of each on the scientific process, have widely been debated in practical theology. In South Africa two meta-theories namely the system- theory together with the theory that social science is busy with the study of actions, are having the greater deal of attention. The purpose of this article is to point out the fact that an early and absolute choice of (...) particular meta-theories, restricts the work of practical theology because it will influence your base-theories, and can weaken the theological content of practical theology. An attempt is made to offer a model by which the interaction between, meta-theories, base-theories and theories of practice can he understood and which can also help the theologian, busy with practical theology, to steer clear of problems and understand the meta- influences in his own scientific work. An increasing number of European research projects return, or plan to return, individual genomic research results (IRR) to participants. While data access is a data subject’s right under the General Data Protection Regulation (GDPR), and many legal and ethical guidelines allow or require participants to receive personal data generated in research, the practice of returning results is not straightforward and raises several practical and ethical issues. Existing guidelines focusing on return of IRR are mostly project-specific, only discuss which results to (...) return, or were developed outside Europe. To address this gap, we analysed existing normative documents identified online using inductive content analysis. We used this analysis to develop a checklist of steps to assist European researchers considering whether to return IRR to participants. We then sought feedback on the checklist from an interdisciplinary panel of European experts (clinicians, clinical researchers, population-based researchers, biobank managers, ethicists, lawyers and policy makers) to refine the checklist. The checklist outlines seven major components researchers should consider when determining whether, and how, to return results to adult research participants: 1) Decide which results to return; 2) Develop a plan for return of results; 3) Obtain participant informed consent; 4) Collect and analyse data; 5) Confirm results; 6) Disclose research results; 7) Follow-up and monitor. Our checklist provides a clear outline of the steps European researchers can follow to develop ethical and sustainable result return pathways within their own research projects. Further legal analysis is required to ensure this checklist complies with relevant domestic laws. (shrink)

Det er muligens ikke lenger et spørsmål om, men når mennesker med Downs syndrom ikke lenger finnes blant oss. Moderne medisinsk teknologi brukt i masseundersøkelser av gravide, tilsier at det nesten ikke behøver å bli født mennesker med Downs syndrom i fremtiden. I den politiske debatten i flere av de nordiske landene er dette kontroversielt. Det handler om et samfunn der det er plass til alle, hevdes det. På samme tid tas moderne genteknologi i bruk for bl.a. å forhindre at (...) mennesker disponert for Huntingtons sykdom eller arvelig brystkreft blir født. Om noen generasjoner kan slike sykdommer være utryddet. Dette påkaller imidlertid ikke henvisninger til «sortering» og «seleksjon». I denne artikkelen undersøkes de viktigste forutsetningene for hvorfor fosterdiagnostikk på noen indikasjoner vekker etisk debatt, mens det ikke skjer i andre tilfeller. Det argumenteres for at den medisinske forståelsen av alvorlighetsgrad ved en sykdom/funksjonshemming ikke gir oss nøkkelen til å forstå den etiske bekymringen rundt seleksjon. Identitets- og anerkjennelsesproblematikk, hevdes det, er kjernen i kontroversene rundt fosterdiagnostikk i Norden. Derfor står det noe vesentlig på spill i fosterdiagnostikkdebatten. Background Genuine uncertainty on superiority of one intervention over the other is called equipoise. Physician-investigators in randomized controlled trials need equipoise at least in studies with more than minimal risks. Ideally, this equipoise is also present in patient-participants. In pediatrics, data on equipoise are lacking. We hypothesize that 1) lack of equipoise at enrolment among parents may reduce recruitment; 2) lack of equipoise during participation may reduce retention in patients assigned to a less favoured treatment-strategy. Methods We compared preferences of (...) parents/patients at enrolment, documented by a questionnaire, with preferences developed during follow-up by an interview-study to investigate equipoise of child-participants and parents in the BeSt-for-Kids-study. This trial in new-onset Juvenile Idiopathic Arthritis-patients consists of three strategies. One strategy comprises initial treatment with a biological disease-modifying-antirheumatic-drug, currently not standard-of-care. Semi-structured interviews were conducted with 23 parents and 7 patients, median 11 months after enrolment. Results Initially most parents and children were not in equipoise. Parents/patients who refused participation, regularly declined due to specific preferences. Many participating families preferred the biological-first-strategy. They participated to have a chance for this initial treatment, and would even consider stopping trial-participation when not randomized for it. Their conviction of superiority of the biological-first strategy was based on knowledge from internet and close relations. According to four parents, the physician-investigator preferred the biological-first-strategy, but the majority stated that she had no preferred strategy. In phase 2, preferences tended to change to the treatment actually received. Conclusions Lack of equipoise during enrolment did not reduce study recruitment, mainly due to the fact that preferred treatment was only available within the study. Still, when developing a trial it is important to evaluate whether the physicians’ research question is in line with preferences of the patient-group. By exploring so-called ‘informed patient-group’-equipoise, successful recruitment may be enhanced and bias avoided. In our study, lack of equipoise during trial-participation did not reduce retention in those assigned to a less favoured option. We observed a change for preference towards treatment actually received, possibly explained by comparable outcomes in all three arms. (shrink)

Summary This paper investigates the nature of creativity in language and linguistics. Following Sampson (2016), it distinguishes between F-creativity (which roughly equals linguistic productivity) and E-creativity (which leads to new and unexpected innovations). These two notions of creativity are discussed on the basis of examples from three different domains: snow cloning, mismatch/coercion, and aberration. It is shown that pure E-creativity may only be found in the case of aberration. Both snow cloning and mismatch/coercion are examples for F-creativity, but to varying (...) degrees. As a consequence, it is suggested that in practice, F- and E-creativity actually form a cline, rather than a dichotomy. (shrink)

Ernst Mayr argued that the emergence of biology as a special science in the early nineteenth century was possible due to the demise of the mathematical model of science and its insistence on demonstrative knowledge. More recently, John Zammito has claimed that the rise of biology as a special science was due to a distinctive experimental, anti-metaphysical, anti-mathematical, and anti-rationalist strand of thought coming from outside of Germany. In this paper we argue that this narrative neglects the important role played (...) by the mathematical and axiomatic model of science in the emergence of biology as a special science. We show that several major actors involved in the emergence of biology as a science in Germany were working with an axiomatic conception of science that goes back at least to Aristotle and was popular in mid-eighteenth-century German academic circles due to its endorsement by Christian Wolff. More specifically, we show that at least two major contributors to the emergence of biology in Germany—Caspar Friedrich Wolff and Gottfried Reinhold Treviranus—sought to provide a conception of the new science of life that satisfies the criteria of a traditional axiomatic ideal of science. Both C.F. Wolff and Treviranus took over strong commitments to the axiomatic model of science from major philosophers of their time, Christian Wolff and Friedrich Wilhelm Joseph Schelling, respectively. The ideal of biology as an axiomatic science with specific biological fundamental concepts and principles thus played a role in the emergence of biology as a special science. (shrink)

This problem cannot yet be considered to have been resolved. As has been emphasized, among others by Academician A. I. Berg , even one of the basic concepts of cybernetics — the concept of information — does not yet have a satisfactory definition. This situation is primarily due to the fact that study of information problems is, (...) in general, of recent origin. Mat er så mangt, som David Kaplan påpeker i innledningen til The Philosophy of Food, blant annet næring, natur, kultur, et sosialt gode, åndelighet, begjærsgjenstand og estetisk objekt. Ikke minst er det et politisk tema på så mange måter. Derfor inviterte vi til et temanummer om matens etikk, for det er lite som berører våre liv så sterkt og fundamentalt som mat. Det er et område som til alle tider har hatt tydelige etiske regler, både hva man kan og ikke (...) kan spise, men også hvordan man skal behandle mat. I Første Mosebok står det: «Med svette i ansiktet skal du spise ditt brød». Vi gjør oss fortjent til å spise gjennom vårt arbeid. Til det er det også knyttet sterke tradisjoner rundt det å dele mat, takke for den, behandle den med respekt og ikke la noe gå til spille. Dette er tradisjoner som står stadig svakere ettersom vår kontakt med opphavet til mat i jordbruk, jakt og fiske blir svekket. Vi står ikke lenger i et direkte og intimt forhold til maten i vår del av verden, og det får betydning for matens etikk. (shrink)

Dette nummeret av Etikk i praksis fokuserer på et av de største, om ikke det største, satsningsområdet vi har sett innenfor forskningsverdenen. Det handler om nanoteknologi, et felt som forventes å få betydelige positive samfunnsmessige konsekvenser i form av industriutvikling og forbedrede produkter for befolkningen. Nanoteknologien er et omfattende og uoversiktlig fagfelt, der det viktigste felles kjennetegnet er at en opererer med strukturer som er 100 nanometer eller mindre. Materialer i denne størrelsesorden har endrete egenskaper som innebærer en rekke nye (...) anvendelsesmuligheter innen medisin, elektronikk, fysikk og energiproduksjon. Artiklene i temadelen omhandler utfordringer og muligheter for utvikling av en etikk-komponent innenfor dette satsningsområdet. Når vi ser på hvor tidlig vi er i utvikling av nanoetikk som fagfelt, kan man spørre om det er for tidlig å ha et temanummer i et nordisk tidsskrift. Tross alt trenger en tid for å kunne si noe forskningsmessig interessant om et fagområde. Samtidig er det første internasjonale tidsskriftet for nanoetikk over ett år gammelt, og de nordiske land er generelt langt framme i teknologiutvikling, så det skulle være god grunn til å presentere feltet. I tillegg er de senere års studier av teknologiutvikling som en etisk og samfunnsmessig størrelse i økende grad opptatt av å være i forkant av utviklingen, og da er det rimelig å formidle tidlige resultat av denne oppstrømsforskningen. This book synthesizes the game-theoretic modeling of decision-making processes and an ancient moral requirement, called the Golden Rule of ethics (GR). This rule states, "Behave to others as you would like them to behave to you." The GR is one of the oldest, most widespread and specific moral requirements that appear in Christianity, Islam, Judaism, Buddhism, and Confucianism. The book constructs and justifies mathematical models of dynamic socio-economic processes and phenomena that reveal the mechanism of the GR and are based (...) on the concept of Berge equilibrium. The GR can be naturally used for resolving or balancing conflicts, and its "altruistic character" obviously excludes wars, blood-letting, and armed clashes. The previous book of the authors, The Berge Equilibrium: A Game-Theoretic Framework for the Golden Rule of Ethics, covers the static case of the GR. In this book, the dynamic case of the GR is investigated using the altruistic concept of Berge equilibrium and three factors as follows: a) a modification of N.N. Krasovskii's mathematical formalization of differential positional games (DPGs), in view of the counterexamples given by A.I. Subbotin and A.F. Kononenko; b) the method of guiding control, proposed by N.N. Krasovskii; c) the Germeier convolution of the payoff functions of different players. Additionally, the book features exercises, problems, and solution tips collected together in Appendix 1, as well as new approaches to conflict resolution as presented in Appendices 2-4. The book will be of use to undergraduate and graduate students and experts in the field of decision-making in complex control and management systems, as well as anyone interested in game theory and applications. The publication of Frank Furedi's Paranoid Parenting in 2001 was trend-setting in the sense that it addresses parents directly in a way that is intended to be both critical and supportive, by helping parents to look through a sociological lens at their alleged predicament. Furedi's hope is that this will lead to the restoration of parental self-confidence, which he claims to be sorely lacking in contemporary (Western?) society. I argue that such a project would be more likely to succeed if (...) one were to hold a less dim view of the way both parents and other individuals are connected with their own society. By introducing a cultural-hermeneutical perspective on human agency, based on a specific reading of Heidegger and Taylor, I suggest a more constructive way to reconnect parents with the ongoing conversations in their communities and to conceptualise parenting support. This volume has its already distant or1g1n in an inter national conference on Evolutionary Epistemology the editors organized at the University of Ghent in November 1984. This conference aimed to follow up the endeavor started at the ERISS (Epistemologically Relevant Internalist Sociology of Science) conference organized by Don Campbell and Alex Rosen berg at Cazenovia Lake, New York, in June 1981, whilst in jecting the gist of certain current continental intellectual developments into a debate whose focus, we thought, was in (...) danger of being narrowed too much, considering the still underdeveloped state of affairs in the field. Broadly speaking, evolutionary epistemology today con sists of two interrelated, yet qualitatively distinct inves tigative efforts. Both are drawing on Darwinian concepts, which may explain why many people have failed to discriminate them. One is the study of the evolution of the cognitive apparatus of living organisms, which is first and foremost the province of biologists and psychologists (H. C. Plotkin, Ed. , Learning, Development, and Culture: Essays in Evolu tionary Epistemology, New York, Wiley, 1984), although quite a few philosophers - professional or vocational - have also felt the need to express themselves on this vast subject (F. M. Wuketits, Ed. , Conce ts and Approaches in Evolutionary Epistemology, Dordrecht Boston, Reidel, 1984). The other approach deals with the evolution of science, and has been dominated hitherto by (allegedly) 'naturalized' philosophers; no book-length survey of this literature is available at present. I define serendipity as the art of making an unsought finding. And I propose an overview of my collection of serendipities, the largest yet assembled, chiefly in science and technology, but also in art, by giving a list of ‘serendipity patterns’. Although my list of ‘patterns’ is just a list and not a classification, it serves to introduce a new and possibly stimulating perspective on the old subject of serendipity. In the Preface to his impressive and engaging new commentary on the Critique of Pure Reason, Jay Rosenberg informs us that the book is both a product of his own lectures and a “direct descendent of Wilfrid Sellars’ legendary introduction to Kant” . Its origins in the classroom give Accessing Kant a refreshingly pedagogical tone. Throughout, Rosen-berg—who was a student of Sellars’ at the University of Pittsburgh—makes felicitous use of clear examples, familiar problems and authors, and visual aids to clarify (...) “the Big Picture” operating in the first Critique. Mercifully, however, his style owes little to Sellars’ own brutally opaque prose.Rosenberg also shares his teacher’s enthusiasm for the history of philosophy. The reader will not find Rosenberg sloughing off Kant’s “lesser predecessors” in the manner of P. F. Strawson. However, the history on display in Accessing Kant has more to do with the curricular demands of English-speaking philosophy departments than with Kant’s own intellectual and cultural context. This means that his Kant argues across the ages with giants rather than with his peers in Halle and Berlin. Nor, despite the occasional reference to Kant’s handwritten marginalia, does Rosenberg devote any effort to. (shrink)
