In patient-centred care, shared decision-making is advocated as the preferred form of medical decision-making. Shared decision-making is supported with reference to patient autonomy without abandoning the patient or giving up the possibility of influencing how the patient is benefited. It is, however, not transparent how shared decision-making is related to autonomy and, in effect, what support autonomy can give shared decision-making. In the article, different forms of shared decision-making are analysed in relation to five different aspects of autonomy: (1) self-realisation; (...) (2) preference satisfaction; (3) self-direction; (4) binary autonomy of the person; (5) gradual autonomy of the person. It is argued that both individually and jointly these aspects will support the models called shared rational deliberative patient choice and joint decision as the preferred versions from an autonomy perspective. Acknowledging that both of these models may fail, the professionally driven best interest compromise model is held out as a satisfactory second-best choice. (shrink)

This article analyses and presents a survey of ethical conflicts in prehospital emergency care. The results are based on six focus group interviews with 29 registered nurses and paramedics working in prehospital emergency care at three different locations: a small town, a part of a major city and a sparsely populated area. Ethical conflict was found to arise in 10 different nodes of conflict: the patient/carer relationship, the patient’s self-determination, the patient’s best interest, the carer’s professional ideals, the carer’s professional (...) role and self-identity, significant others and bystanders, other care professionals, organizational structure and resource management, societal ideals, and other professionals. It is often argued that prehospital care is unique in comparison with other forms of care. However, in this article we do not find support for the idea that ethical conflicts occurring in prehospital care are unique, even if some may be more common in this context. (shrink)

This book deals with the impact of the Reformation debate in Germany on the most prominent intellectual movement of the time: humanism Although it is true that humanism influenced the course of the Reformation, says Erika Rummel, the dynamics of the relationship are better described by saying that humanism was co-opted, perhaps even exploited, in the religious debate.

In his book Philosophy of Right Hegel has criticized Kant´s moral philosophy negatively, labeling it as an “empty formalism”. As a matter of fact, Kantian concepts such as good will, categorical imperative and duty are rejected by Hegel because he considers them formal, empty and unilateral. In spite of this, by means of a scrutiny of Hegel’s argument it can be thought that such a criticism is only based upon Kant’s Groundwork of the Metaphysics of Moral. Therefore, Hegel has a (...) partial interpretation of Kant’s moral philosophy. To show this, I will first present the three concepts at issue, then I will expose Hegel’s critic to these concepts, and finally I will show some other elements in Kantian moral philosophy that allow us to reconsider Hegel’s critic. This is done in order to show that Kant’s moral philosophy is neither empty nor unilateral, despite being formal, like Hegel’s thought. (shrink)

La domanda fondamentale della ricerca si muove attorno alla relazione tra arte, verità e essere, segnalando la natura metafisica del discorso. Questo fatto è sottolineato in quanto l’arte, in ambito filosofico, è di solito trattata in un contesto estetico, ma i due autori scelti vedono nell’arte un legame intrinseco al problema dell’essere, riconoscendo il suo valore anche nell’orizzonte metafisico. Heidegger e Merleau-Ponty avvertono la necessità di superare l’estetica e di svelare le possibilità ontologiche dell’arte, compiendo così una sua «riabilitazione ontologica», (...) mostrando la sua intima correlatività alla questione dell’Essere e con quella della verità. Il vero senso dell’arte non nasce al di fuori di questo contesto ontologico, ed è proprio questo il motivo per cui la riabilitazione ontologica è diventata necessaria: al punto che si potrebbe affermare che l’estetica conduce l’arte fuori della sua vera essenza. L’arte è, di natura sua, ontologica. Si tratta quindi di un tema assai speciale, che già a prima vista dà luogo a diverse domande. Accettando la sfida della crisi della metafisica nell’età contemporanea, è possibile salvaguardare la questione dell’Essere a partire dall’ambito non-filosofico dell’arte? Cosa offre l’arte alla filosofia a proposito della questione dell’Essere? Quale verità si esprime in essa? Cosa giustifica il mettere insieme i tre concetti fondamentali della dissertazione e due autori così diversi, anche se di eredità fenomenologica comune? L’obiettivo della ricerca è di vedere se la riabilitazione ontologica dell’arte potesse in qualche modo salvaguardare la metafisica dell’Essere; e se a partire da ciò che la filosofia può imparare dall’arte, sia possibile arrivare ad una formulazione filosofica della verità vivente, in dialogo con la concezione dei due autori scelti. Previous research has established that the success of strikes, and social movements more broadly, depends on their ability to garner support from the public. However, there is scant published research investigating the response of the public to strike action by healthcare workers. In this study, we address this gap through a study of public responses to UK nursing strikes in 2022–2023, using a data set drawn from Twitter of more than 2300 publicly available tweets. We focus on negative tweets, investigating (...) which societal discourses social media users draw on to oppose strike action by nurses. Using a combination of corpus‐based approaches and discourse analysis, we identified five categories of opposition: (i) discourse discrediting nurses; (ii) discourse discrediting strikes by nurses; (iii) discourse on the National Health System; (iv) discourse about the fairness of strikers' demands and (v) discourse about potential harmful impact. Our findings show how social media users operationalise wider societal discourses about the nursing profession (e.g., associations with care, gender, vocation and sacrifice) as well as recent crises such as the Covid‐19 pandemic to justify their opposition. The results also provide valuable insights into misconceptions about nursing, strike action and patient harm, which can inform strategies for public communication. (shrink)

This paper focuses on Confucian formulations of personhood and the implications they may have for bioethics and medical practice. We discuss how an appreciation of the Confucian concept of personhood can provide insights into the practice of informed consent and, in particular, the role of family members and physicians in medical decision-making in societies influenced by Confucian culture. We suggest that Western notions of informed consent appear ethically misguided when viewed from a Confucian perspective.

Within the field of philosophy, animals have traditionally been studied from two perspectives: that of self-knowledge and that of ethics. The analysis of the differences between humans and animals has served our desire to understand our own specificity, whereas ethical discussions have ultimately aimed at finding the right way to treat animals. This dissertation proposes a different way of looking at non-human animals: it investigates the question of how non-human animals appear to us humans in our perceptual experience. The analysis (...) focuses on the empathetic, embodied understanding of animals diverse movements and other expressions. The theoretical point of departure for the research is phenomenological philosophy, in particular Maurice Merleau-Ponty s phenomenology of the body. Edmund Husserl s and Edith Stein s analyses of empathy and embodiment are also crucial to the work. In this tradition, empathy means understanding the other s experience through her bodily expressions and seeing the other body as living, as well as motivated and directed towards the surrounding world. The dissertation both explicates and criticizes the earlier phenomenological notions of empathy and human specificity. In order to elucidate the fundamental structures of our experience of non-human animals, it also applies the phenomenological method, which consists of a phenomenological reduction and a free variation of the different aspects of experience. It is shown that our experiences of non-human animals involve a recognition of both similarities and differences. This recognition, however, is not primarily based on intellectual comparisons but is lived as an embodied relationship to another body, and its manifestations vary from one instant to the next. The analysis also reveals that the object of empathy is not the other s experience as such, not even as it is manifested by the other s movements, but rather the other s embodied situation, enriched by elements that remain outside the scope of the other s experience. The dissertation shows that human existence is intertwined with the existence of non-human animals on four levels: those of empathetic sensations, reciprocal communication, experience of the surrounding world and self-definitions. The animals different modes of perception prove to expand our understanding of what is perceivable and how things can be perceived. The presence of non-human animals in our perceptual world is revealed as something that both shows us the limits of our own embodiment and enables us to overcome these limits in empathetic acts. Finally, it is demonstrated that the life of non-human animals is intertwined with ours in a far more complex way than has been presupposed in traditional descriptions of human-animal differences. The fourth volume of the Amsterdam edition of the apologias contains the critical edition of the Latin text of the apologia against the English scholar Edward Lee.

The paper investigates various definitions of the concept of opinion as opposed to factual or evidence-based statements and proposes a taxonomy of opinions expressed in English as identified in selected social media. A discussion situates opinions in the realm of pragmatics and reaches to philosophy of language and cognitive science. The research methodology combines a thorough linguistic analysis of opinions, proposing their multifaceted taxonomy with the automatically generated lexical embeddings of positive and negative lexicon acquired from the analysed opinionated texts. As proposed, the definition of the concept of opinion is best apprehended when looked upon in terms of an opinion event, with a number of necessary conditions on the one hand, and those that are characteristic of an explicit opinion prototype on the other. Lists of opinion discourse markers show their preferential uses either in positive or negative opinionated texts; however, no sets of necessary and/or sufficient opinion markers properties have been acquired from the analysed texts. The conclusions indicate a polysemous understanding of naturally occurring social media opinionated texts and a definitional flexibility of the boundaries around lexical positive and negative types of opinion markers. The (...) paper will deal with religion from another perspective, namely, it will try to see whether religion has truly disappeared from the public life as it is sometimes assumed. How were the ideas about God used or misused by some politicians and leaders, such as Osama bin Laden, Saddam Hussein, and George W. Bush? What to do if the media enable us to perceive how religion is made political and how politics is made religionist? (shrink)

In contrast to standardized guidelines, personalized medicine and person centered care are two notions that have recently developed and are aspiring for more individualized health care for each single patient. While having a similar drive toward individualized care, their sources are markedly different. While personalized medicine stems from a biomedical framework, person centered care originates from a caring perspective, and a wish for a more holistic view of patients. It is unclear to what extent these two concepts can be combined (...) or if they conflict at fundamental or pragmatic levels. This paper reviews existing literature in both medicine and related philosophy to analyze closer the meaning of the two notions, and to explore the extent to which they overlap or oppose each other, in theory or in practice, in particular regarding ethical assumptions and their respective practical implications. Dominik Tatarka, Slovak (Czechoslovak) writer, philosopher, social and political thinker. His literary and philosophical works are firmly linked with his personal experience, combined with socio-political events in Czechoslovakia. They are based on several principles of which the most important is respect for democratic principles and the rule of Christian universalism, which returned in the end of the 70th last century. The article is focused on the modification of the key problems of Slovak author: the freedom of the individual and the (...) freedom of the community. Tatarka in his texts and also in his life always tried to understand an individual, not judging him. That was his moral strength and reason behind the superiority over many opponents. When comparing his views with the ideas of other authors (M. Bátorová, M. Hamada, Vaclav Havel, C. Miłosz, S. Marais...), this article will highlights the uniqueness of texts, as well as the importance of the interpretation of the concept of freedom in the context of Central Europe marked by two totalitarian ideologies. Analysoin Aaltolan tapaa jakaa empatia erilaisiin muotoihin ja kysyn, tuleeko nämä muodot ymmärtää toisensa poissulkevina vai osittain päällekkäisinä kategorioina. Tämän lisäksi pohdin, missä määrin empatia voidaan jäsentää yhtäältä ”hyväksi ja hyödylliseksi” tai vaihtoehtoisesti ”pahaksi ja haitalliseksi”, ja voidaanko empatiaa ”käyttää” tahdonalaisesti, kuten kirjassa esitetään, vai onko se väistämätön ja jossakin määrin hallitsematon osa ruumiillista olemassaoloa. Kysyn myös, onko eläinten hyväksikäytössä kysymys oikeanlaisen empatian puutteesta vai sen ohella (...) tai sen sijaan kulttuuriin ja elettyyn ruumiiseen monin eri tavoin laskostuneista tottumuksista, arvoista ja hierarkioista. Lopuksi kuvaan husserlilaista erottelua alkuperäisen ja ei-alkuperäisen kokemuksen välillä sekä erittelen laajemminkin fenomenologisia empatiakäsityksiä, joita käsitellään kirjan luvussa “Embodied Empathy”. Argumentoin, että fenomenologinen empatiaperinne on ymmärrettävä viime kädessä heterogeenisenä, vaikka ruumiin ilmaisevuuden korostus yhdistääkin eri ajattelijoita sen piirissä. Tässä yhteydessä esitän myös oman tulkintani Edith Steinin fenomenologisesta empatiakäsityksestä. (shrink)

In this paper we explore the relation between health-care needs and patients’ desires within shared decision-making in a context of priority setting in health care. We begin by outlining some general characteristics of the concept of health-care need as well as the notions of SDM and desire. Secondly we will discuss how to distinguish between needs and desires for health care. Thirdly we present three cases which all aim to bring out and discuss a number of queries which seem to (...) arise due to the double focus on a patient’s need and what that patient desires. These queries regard the following themes: the objectivity and moral force of needs, the prediction about what kind of patients which will appear on a micro level, implications for ranking in priority setting, difficulties regarding assessing and comparing benefits, and implications for evidence-based medicine. In patient centred care, shared decision making is a central feature and widely referred to as a norm for patient centred medical consultation. However, it is far from clear how to distinguish SDM from standard models and ideals for medical decision making, such as paternalism and patient choice, and e.g., whether paternalism and patient choice can involve a greater degree of the sort of sharing involved in SDM and still retain their essential features. In the article, different versions of SDM (...) are explored, versions compatible with paternalism and patient choice as well as versions that go beyond these traditional decision making models. Whenever SDM is discussed or introduced it is of importance to be clear over which of these different versions are being pursued, since they connect to basic values and ideals of health care in different ways. It is further argued that we have reason to pursue versions of SDM involving, what is called, a high level dynamics in medical decision-making. This leaves four alternative models to choose between depending on how we balance between the values of patient best interest, patient autonomy, and an effective decision in terms of patient compliance or adherence: Shared Rational Deliberative Patient Choice, Shared Rational Deliberative Paternalism, Shared Rational Deliberative Joint Decision, and Professionally Driven Best Interest Compromise. In relation to these models it is argued that we ideally should use the Shared Rational Deliberative Joint Decision model. However, when the patient and professional fail to reach consensus we will have reason to pursue the Professionally Driven Best Interest Compromise model since this will best harmonise between the different values at stake: patient best interest, patient autonomy, patient adherence and a continued care relationship. This essay is based on a talk the author gave at the By the Book conference in Florence in June 2019. It examines the power dynamics in the Italian publishing world from the perspective of a fiction writer. Writing and publishing are two completely different worlds that can be differently approached. What’s the point of writing? Should writers write about what they know, or about what they do not know? Can publishing be put off? What’s the role of literary agents (...) in the publishing process?
