The COVID-19 outbreak has placed extraordinary demands upon healthcare systems worldwide. Italy's hospitals have been among the most severely overwhelmed, and as a result, Italian healthcare workers' well-being has been at risk. The aim of this study is to explore the relationships between dimensions of burnout and various psychological features among Italian healthcare workers during the COVID-19 emergency. A group of 267 HCWs from a hospital in the Lazio Region completed self-administered questionnaires online through Google Forms, including the Maslach Burnout (...) Inventory, Resilience Scale, and Intolerance of Uncertainty Scale Short Form. Cluster analysis highlighted two opposite burnout risk profiles: low burnout and high-risk burnout. The high-risk group had lower resilience and greater difficulties in tolerating the uncertainty than the low-burnout group. A set of general linear models confirmed that both IU subscales, prospective and inhibition, moderated the relationship between resilience and burnout. In conclusion, the results showed that individual levels of resilience and one's ability to tolerate uncertainty have been significant factors in determining the impact of the COVID-19 emergency on HCWs. The use of emotional strategies that allow individuals to stay in a critical situation without the need to control it appears to protect against burnout in these circumstances. (shrink)

Background: Ethical and legal issues are increasingly being reported by health caregivers; however, little is known about the nature of these issues in geriatric care. These issues can improve work and care conditions in healthcare, and consequently, the health and welfare of older people. Aim: This literature review aims to identify research focusing on ethical and legal issues in geriatric care, in order to give nurses and other health care workers an overview of existing grievances and possible solutions to take (...) care of old patients in a both ethical and legally correct way. Methods: Using a systematic approach based on Aveyard, a search of the PubMed, CINAHL, and Ethicshare databases was conducted to find out the articles published on ethical and legal issues in geriatric care. Ethical considerations: The approval for the study was obtained from UMIT-The Health and Life Sciences University, Austria. Results: Only 50 articles were included for systematic analysis reporting ethical and legal issues in the geriatric care. The results presented in this article showed that the main ethical issues were related to the older people’s autonomy, respect for their needs, wishes and values, and respect for their decision-making. The main legal issues were related to patients’ rights, advance directives, elderly rights, treatment nutrition dilemma, and autonomy. Conclusion: Further education for professional caregivers, elderly people, and their families is needed on following topics: care planning, directive and living wills, and caregiver-family member relationships to guide and support the elderly people within their decision-making processes and during the end-of-life care. Se propone revisar la elaboración del concepto de piñen examinando la relación entre lenguaje y colonialidad, para, de ese modo, subrayar el potencial político y reivindicativo que comporta en la novela del mismo nombre, Piñen (2019). Dicha hipótesis articulará el ejercicio de lectura aquí propuesto, atendiendo principalmente las estrategias narrativas empleadas –servirse de un polilingüismo que rompe estructuras gramaticales– para descentrar el lenguaje de su determinismo colonial. For families all over the world, going through a pandemic has presented a number of challenges. In particular, social distancing measures involving the closure of schools and day care centers, as well as increasing work hours at home, made parents face very demanding situations. However, we know little about whether parents’ burnout levels are influenced by the age of their children. This study sought to determine whether levels of parental burnout are higher in families with at least one child under (...) the age of four than in families with older children. The second goal was to explore whether having children under 4 years of age moderates the relationship between parental cooperation and PB. A cross-sectional study was conducted with a sample of 651 participants since May 18th until August 27th, 2020. The main results showed that child age is a predictor of PB. Besides, having a child aged 0–4 years old moderates the relationship between parental cooperation and PB. Finally, it was found that in cases where there was at least one child under 4 years of age in the family, with one of the partners who worked remotely, the respondent’s PB rose by 7.9 points. The implications of these results with respect to the consideration of children’s ages in the different parental scenarios were discussed. (shrink)

In this event-related potential study a masking technique that prevents conscious perception of words and non-words through attentional distraction was used to reveal the temporal dynamics of word processing under non-conscious and conscious conditions. In the non-conscious condition, ERP responses differed between masked words and non-words from 112 to 160 ms after stimulus-onset over posterior brain areas. The early onset of the word–non-word differences was compatible with previous studies that reported non-conscious access to orthographic information within this time period. Moreover, (...) source localisations provided evidence for automatic activation of prelexical phonological information, whereas no evidence for non-conscious semantic processing was found. When subjects were informed about the masking technique, lexical differences occurred at later time intervals, suggesting conscious access to additional word related information. These results indicate that early visual word processing does not depend entirely on attentional resources, but that non-conscious processing probably is restricted to rather lower-level linguistic information. Three types of problems are raised in this commentary: On the linguistic side, we emphasize the importance of an appropriate definition of the different domains of linguistics. This is needed to define the domains (lexicon-syntax-semantics) to which transformational relations apply. We then question the concept of Broca's aphasia as a “functional” syndrome, associated with a specific lesion. Finally, we discuss evidence from functional brain imaging. The breadth and potential impact of such evidence has grown considerably in the last few years, (...) expanding our knowledge of the multiple contributions of the “Broca's region” to phonological, lexical-semantic, and syntactic processing. “Lumping” under diagnostic labels, such as Broca's aphasia, should be replaced by more detailed linguistic and neurological descriptions of the clinical cases. IntroductionThe assessments of the motor symptoms in Parkinson’s disease are usually limited to clinical rating scales, and it depends on the clinician’s experience. This study aims to propose a machine learning technique algorithm using the variables from upper and lower limbs, to classify people with PD from healthy people, using data from a portable low-cost device. And can be used to support the diagnosis and follow-up of patients in developing countries and remote areas.MethodsWe used Kinect®eMotion system to capture the spatiotemporal (...) gait data from 30 patients with PD and 30 healthy age-matched controls in three walking trials. First, a correlation matrix was made using the variables of upper and lower limbs. After this, we applied a backward feature selection model using R and Python to determine the most relevant variables. Three further analyses were done using variables selected from backward feature selection model, movement disorders specialist, and all the variables from the dataset. We ran seven machine learning models for each model. Dataset was divided 80% for algorithm training and 20% for evaluation. Finally, a causal inference model using the DoWhy library was performed on Dataset B due to its accuracy and simplicity.ResultsThe Random Forest model is the most accurate for all three variable Datasets followed by the support vector machine. The CIM shows a relation between leg variables and the arms swing asymmetry and a proportional relationship between ASA and the diagnosis of PD with a robust estimator.ConclusionsMachine learning techniques based on objective measures using portable low-cost devices are useful and accurate to classify patients with Parkinson’s disease. This method can be used to evaluate patients remotely and help clinicians make decisions regarding follow-up and treatment. (shrink)

This plethora of conceptually distant views have important common concerns in different fields of science and shape, in turn, several societal aspects including laws related, for instance, to inheritance eligibility or abortion, end-of-life care and euthanasia, and reproductive technology. Also, they are fundamental to evaluate opportunity for resuscitation vs. palliative care in extremely preterm infants. In (...) this article, we address one of the most common tenets in medicine: the acceptance that human life starts with first breath, even though several events are well-documented to take place before its occurrence.Main textSeveral studies show how pivotal physiological events take place before first breath. Evidence of a number of neurological events occurring before first breath opens the way to the primacy of the Central Nervous System, given its immediate extra-uterine activation at birth. This activation eventually sets specific physiological conditions that allow the complex sequence of events determining the muscle activity associated with the influx of air in the lung and the settling of a continuous and successful extra-uterine respiration. We would like to invite the scientific community to endorse a clear-cut position against the paradigm of ‘first breath’ as the beginning of life. Herein, we also assume how, a still undefined, yet possibly specific quid in the external environment triggers further physiological response in newborns. Better understanding of the critical events that occur at the beginning of human life is likely to cause great concern and expectations in scientists, researchers and physicians working in the domain of brain, and its physiology, and mental health.ConclusionsThe comparison between beliefs and evidence-based observations generates confusion, misperceptions and false expectations in society, hence, in the scientific and medical community. Physicians working in the world of competitive sports face unique ethical challenges, many of which center around conflicts of interest. Team-employed physicians have obligations to act in the club's best interest while caring for the individual athlete. As such, they must balance issues like protecting versus sharing health information, as well as issues regarding autonomous informed consent versus paternalistic decision making in determining whether an athlete may compete safely. Moreover, the physician has to deal with an athlete's decisions about performance (...) enhancement and return to play, pursuit of which may not be in the athlete's long-term best interests but may benefit the athlete and team in the short term. These difficult tasks are complicated by the lack of evidence-based standards in a field influenced by the lure of financial gains for multiple parties involved. Living with a chronic condition represents a strenuous experience that often could be lived as a sequence of waiting and crisis times. Therapeutic path incidents could represent however a catalysts and revelatory time, useful to patients to discover their own resources. A qualitative study according to the phenomenological hermeneutic perspective was conducted to understand the kind of skills expressed by the patients during a difficult episode, and the characteristics that identify patients who can overcome them better. From September 2019 to (...) August 2020 twenty-three patients followed-up in four Hospital Units of Northeastern of Italy were enrolled. All the patients described an episode linked whit their pathological condition; they mentioned mainly supportive, reactive, or behavioral strategies and an acceptance attitude. The sample characteristics may explain the recurring acceptance attitude; however, the younger individuals, those with a higher level of education and those who faced precedent negative life experiences seem to better overcome challenging events. It can’t be taken for granted that patients possess all the skills necessary to overcome the frequent challenging episodes they have to face; for this reason, healthcare professionals should assess their learning and reactive attitude in order to tailor for them therapeutic education paths. Since its emergence, the novel coronavirus disease of 2019 has had enormous physical, social, and psychological impacts worldwide. The aim of this article was to identify elements of our knowledge on asbestos exposure and malignant mesothelioma that can provide insight into the psychological impact of the COVID-19 pandemic and be used to develop adequate interventions. Although the etiology of Covid-19 and MM differs, their psychological impacts have common characteristics: in both diseases, there is a feeling of being exposed through aerial (...) contagion to an “invisible killer” without boundaries that can strike even the strongest individuals. In both cases, affected persons can experience personality dysfunction, anxiety, depression, and posttraumatic symptoms; helplessness, hopelessness, and projection of destructive thoughts onto external forces often emerge, while defense mechanisms such as denial, splitting, repression, and reduced emotional expression are used by individuals to contain their overwhelming anxieties. We believe that in both diseases, an integrated multidimensional intervention offered by hospitals and other public health services is the most effective approach to alleviating patients’ and caregivers’ psychological distress. In particular, we emphasize that in the context of both MM and COVID-19, Brief Psychoanalytic Group therapy can help patients and caregivers attribute meaning to the significant changes in their lives related to the experience of the disease and identify adaptive strategies and more realistic relational modalities to deal with what has happened to them. We also highlight the importance of developing a surveillance system that includes individual anamnestic evaluation of occupational risk factors for COVID-19 disease. (shrink)

One of the most unpredictable things the pandemic brought to our societies was the closure of hospitals and other health services to visitors. Preventing the spread of infection was the main reason for these decisions in the early days of the pandemic when there was no clarity about the means of transmission and the origin of the virus. However, in view of the persistence of the restrictions to date and the numerous negative consequences they have had on the professional and (...) personal quality of life of doctors, nurses, patients and carers, the aim of this article is to analyse in depth the reasons why these decisions have been and, above all, are still being applied, also considering the hypothesis that the choices made so far in the training of health professionals should be questioned. The persistence of restrictions, the constant reduction of human and economic resources in the face of an increase in the demand for care have exacerbated the distance between health professionals and users, to the point of escalating into actual acts of violence and aggression. According to the Critical Pedagogy approach, educators can play a central role in activating formative and collaborative strategies, such as maieutic groups, to rebuild an alliance between those who today find themselves on opposite sides: health professionals on the one hand, and patients and families on the other. The illness immersion condition prevents patients from enjoying everything worth living life for. In any case, according to Frank, this condition could represent one of the most insightful experiences towards understanding the meaning of life. Using the metaphor of phoenix taken from May, Frank identified four kinds of embodiments through which the phoenix can reveal itself in a patient after an illness immersion experience: the phoenix that could ever be and the phoenix that might have been; the recurrent and cumulative (...) phoenix and the reluctant phoenix. Based on these considerations, a qualitative study was conducted in order to give voice to the different phoenix embodiments focusing on several autobiographies of patients afflicted with severe or terminal illness. The analysis highlights several similarities that the different phoenixes shared ; however, a deep difference emerged in their worldview which proves able to lead them towards different epiphanies. These evidences reveal that every sick person should be understood, stimulated and accompanied throughout his lived experience of illness by an expressly designed and dedicated care. (shrink)

An extensive literature has focused on the impact of new public management oriented structural changes on academics’ practice and identity. These critical studies have been resolute in concluding that NPM inevitably leads to a degeneration of academics’ ethos and values. Drawing from the moral philosophy of Alasdair MacIntyre, we argue that these previous analyses have overlooked the moral agency of the academics and their role in ‘moralizing’ and consequently shaping the ethical nature of their practices. The paper provides a new (...) theoretical understanding of NPM-oriented reforms in light of the virtue ethics approach, thereby directing the attention to the moral character and moral agency of academics. Our analysis of interviews collected in the business department of a Danish university provides an example of how individuals have divergent ethical understandings of these structural changes and enact/resist pre-defined social roles in different ways. While in some cases the NPM agenda of the institutions has triggered internal moral conflict and a crisis of moral character, in other cases the new logic resonates with academics’ values and evaluative standards. Partially departing from the theoretical ground of MacIntyre, we conclude that academics can play a crucial role in shaping the morality of NPM-oriented institutions and in transforming these settings into suitable contexts for the cultivation of virtues. It is generally believed that one argument advanced by Aristotle in favor of the political authority of the multitude is that large groups can make better decisions by pooling their knowledge than individuals or small groups can make alone. This is supported by two analogies, one apparently involving a “potluck dinner” and the other aesthetic judgment. This article suggests that that interpretation of Aristotle’s argument is implausible given the historical context and several features of the text. It argues that Aristotle’s (...) support for the rule of the multitude rested not on its superior knowledge but rather on his belief that the virtue of individuals can be aggregated and even amplified when they act collectively. This significantly alters our understanding of Aristotle’s political thought and presents a powerful alternative to the epistemic defenses of mass political activity popular today. (shrink)

Justified belief is a core concept in epistemology and there has been an increasing interest in its logic over the last years. While many logical investigations consider justified belief as an operator, in this paper, we propose a logic for justified belief in which the relevant notion is treated as a predicate instead. Although this gives rise to the possibility of liar-like paradoxes, a predicate treatment allows for a rich and highly expressive framework, which lives up to the universal ambitions (...) of investigating epistemological concepts. We start with a base theory for justified belief, and then systematically present putative additional axioms for justified belief. We provide an overview of consistency results when the additional principles are added to the base theory, and discuss their philosophical plausibility. (shrink)

Background: Using the power of Internet, crowdfunding platforms are currently changing the traditional landscape of fundraising. Social media-based IT platforms in particular are bringing the creators of crowdfunding projects closer than ever to potential investors. A large variety of factors function as determinants of individuals' intention to participate in crowdfunding and have an intertwined impact on funding as the ultimate project goal.Objectives: For a better understanding of investor behavior in social media-based crowdfunding projects, this paper covers identifying, analyzing, and classifying (...) general and specific factors of investor motivation, based on the literature in the field.The main focus is the relationship between the affordances provided by social media-based crowdfunding platforms and the psychological determinants of investor motivation in innovative start-up projects.Methods: Using IEEE Explore, Clarivate Web of Science, ScienceDirect, and Scopus, we conducted a systematic review of the existing research on the emerging role of crowdfunding as a disruptive technology in financing the start-up innovative projects. The paper explores the main determinants of investor motivation and aims to streamline the success factors in crowdfunding campaigns.Results: A total of 1,216 publications were identified after searching the aforementioned databases and, upon refining the results, 515 articles were considered for the final sample. After reading the titles and abstracts, the sample was reduced to 78 articles that were read in-depth and synthesized in accordance with the defined research questions. The selected articles were clustered into three main categories: general studies, determinants of investor behavior, and success factors.Conclusions: In the new global economy, crowdfunding platforms have become the nexus between the emerging creators of innovative products and services and the necessary funding sources. This connection is possible via a cumulative collection of contributions from multiple investors recruited from the audience of the selected platform, without time or space constraints. However, the determinants of the investment decision are very different in the case of social media-based crowdfunding platforms compared to determinants in the mainstream environment. This paper surveys these motivators and reveals how platform features can be used to persuade individuals to make a financial contribution toward the success of a project. (shrink)

Aborda a ética no tratamento temático da informação dentro da dinâmica do acesso e uso informacional. Objetiva discutir teoricamente questões éticas em tratamento temático da informação e o impacto dessas sobre a acessibilidade informacional. Metodologicamente, tem-se pesquisa de ordem teórica, exploratório-descritiva e de abordagem qualitativa. Emprega revisão de literatura sobre artigos de periódicos, trabalhos publicados em eventos e capítulos de livro do campo biblioteconômico-informacional. A reflexão teórica evidencia pontos de necessário aperfeiçoamento, a fim de que se garanta um processo representativo (...) imbuído de imparcialidade, limitado à atribuição de assunto que seja concernente ao documento, mas que ao mesmo tempo, não traga perdas para o usuário da informação. Delimita-se, por fim, a necessária integração entre o trato temático, a ética e a acessibilidade informacional, em vias de fomentar a cadeia documental em torno da produção, da organização e do uso da informação. Architecture has always been a magnificent and much debated platform to express the spirit of the times, world views, everyday life, and aesthetics. It is a daring materialisation of private and public visions, of applied art and the avant-garde alike.

At the end of 1907 within a couple of months Lunačarskij met both Gor’kij and Brzozowski in Italy and found many important points of contact with each. To compare Lunačarskij’s thought at that time with Brzozowski’s “philosophical program” of 1907 casts some new light on the great variety of interpretations that enlivened Easter European Marxism at the beginning of the twentieth century. On the one hand, it explains Lunačarskij’s “economism” as distinct both from Brzozowski’s extreme anthropologism and Gor’kij’s “cosmism”; on (...) the other, it shows that Lunačarskij’s “philosophy of labour” promoted a violent attitude of conquest and humankind’s domination of nature. Although he criticized Brzozowski’s sympathies with German Idealism, Lunačarskij shared with him a deep appreciation of human creative power, which is evident in his peculiar form of collectivism as well. Vicarious responsibility is a notoriously puzzling notion in normative reasoning. In this article we will explore two fundamental issues, which we will call the “explication problem” and the “justification problem”. The former issue concerns how vicarious responsibility can plausibly be defined in terms of other normative concepts. The latter issue concerns how ascriptions of vicarious responsibility can be justified. We will address these two problems by combining ideas taken from legal theory and moral philosophy. Our analysis will emphasise the importance (...) of the voluntary involvement of the normative parties considered liable in a relation with other normative parties who causally contributed to a prohibited state-of-affairs. This article is articulated in three voices of scholars who have worked on questions of war, visual culture, and contemporary political aesthetics that also relates to art and film practices. Media theorist Jussi Parikka, literary scholar Anders Engberg-Pedersen, and visual culture researcher Daniela Agostinho address the relations between images, aesthetics and operations through the lens of two books published concomitantly, Parikka’s Operational Images: From the Visual to the Invisual and Engberg-Pedersen’s Martial Aesthetics: How War Became an Art Form. Both (...) books expand the scope of what Czechoslovakian-born filmmaker Harun Farocki termed “operational images” in his experimental documentaries and theoretical writings from the early 2000s. Through his analyses of the politics of imagery in the military-industrial context, Farocki notably defined “operational images” as images that do not depict or represent but rather perform tasks such as tracking, surveilling, detecting, and targeting. For both Parikka and Engberg-Pedersen, Farocki’s central concept of operational images forms a point of departure for writing media archaeologies of the present. In a three-voiced dialogue, the authors unfold operations as a “machine of articulation,” a conceptual and analytical device that reveals surprising linkages and frictions across different themes, techniques, scales, and historical periods. Among the many institutions of sending, television certainly represents the most successful one over long decades. Nevertheless, given the changes that the medium incurs in terms of broadcasting and broadcasts as a consequence of digitalisation, many proclaim – and not entirely without reason – the post-broadcast era. This paper studies these changes by way of a reading of different television series. The format of the series seems particularly suitable to this end, since the broadcasting of television seems deeply linked to (...) seriality and the serial as a principle. Presenteeism—going to work while ill—is a widespread phenomenon worldwide. Previous research has concentrated mainly on its negative effects. This study investigates the positive consequences of presenteeism derived from a comprehensive content model of presenteeism that was developed on the basis of negative effects. In a quantitative online-survey employees rated the degree of experienced or potential positive effects depending on whether they had worked while ill or not during the previous year. Results revealed that all postulated positive effects described in the (...) content model were relevant. Most positive effects were rated significantly higher by participants who had shown presenteeism in comparison to those who had not. The positive effects significantly predicted presenteeism propensity for participants having shown presenteeism. In addition, an overall rating of positive effects was significantly related to presenteeism, however, to a lesser degree. Overall, the results demonstrate the applicability of the content model to positive effects of presenteeism. They point to the need for further investigation of them and their consideration for the management of presenteeism. (shrink)

This article focuses on the emergence and development of gerontology in communist Bulgaria, looking at the interplay of various circumstances: scientific and political, national and international. We ask if an apparently ideologically neutral field of knowledge such as gerontology may have had some intrinsic qualities imbued by the regimes of knowledge production under a communist regime. More specifically, we ask to what extent and in which ways the production of such specialized, putatively universal knowledge could be ideologically driven and/or politically (...) controlled. To this end, we unpack the ideological, political, institutional, and epistemic circumstances that may have affected the emergence, the institutionalization, and the paradigm of Bulgarian gerontology. We focus in on the social actors, both individuals and organizations, and the roles they played in the process, as well as on international networking and the uses of international contacts and agendas. The roots of the controversial readings of Spinoza during Soviet times date back to the history of Russian Marxism. Spinoza was a most influential figure whom different Marxist currents and thinkers wanted to have on their side. This article examines the most relevant interpretations. First, it sketches some fundamental traits of Plekhanov’s understanding of Spinoza’s ontology and epistemology, from his critique of German revisionism at the end of the 1890s to his polemics against empiriocriticism and its Russian impact. Spinoza was (...) a particularly relevant and authoritative thinker for the so-called “Machists” as well, and some of their different interpretations are analyzed as representing their critical approach to Marxism. Finally, Lunacharskiiʼs discussion is considered as a peculiar alternative to Plekhanov’s orthodox understanding not only of Spinoza but of Marxism itself. While Plekhanov judged Spinoza’s “theological” language a relic of his epoch, Lunacharskii maintained that it was precisely his “religious” feelings that made Spinoza a forerunner of Marxism. (shrink)
