Background This paper examines the degree of agreement among simple dichotomous self-report, validated screening results, and biochemical screening results of prenatal alcohol and other drug use among pregnant women. Method Secondary analysis was conducted on a cohort of pregnant women 16 years or older, presenting for prenatal care in the greater Cape Town, South Africa. Dichotomous verbal screening is a standard of care, and pregnant patients reporting alcohol and other drug use in dichotomous verbal screenings were asked to engage in (...) screening using the Alcohol Smoking and Substance Involvement Screening Test (ASSIST) and urinalysis. Results Significant agreements between dichotomous and ASSIST scores were observed (K = 0.73–0.76). A higher rate of self-reported (36.9%) alcohol use was detected, relative to urine screening (19.6%) with a predictive value of 34.9; while underreporting of illicit substance use was observed (3.6% self-report vs. 8.8% urine screening) with an overall predictive value of 50.0. Conclusion Dichotomous verbal screening was considered valid after comparison with the ASSIST; however, combined use with urine screenings can be recommended especially for identifying illicit substance use in order to accurately detect alcohol and other drug use in pregnancy, so that women can be identified and referred for appropriate interventions where needed. Some fine-tunings resulted from conversations with Timothy Smiley, Charles Kahn, Josiah Gould, John Kearns,John Glanvillle, and William Parry.The criticism of Aristotle's theory of propositions found at the end of this 1974 presentation was retracted in Corcoran's 2009 HPL article "Aristotle's demonstrative logic".

Today many adult children find themselves in the position of caring for elderly parents and attending to the other demands of life. Because of the unique balance of power in the adult child/elderly parent relationship as well as other negative influences, many adult children find caring for parents a frustrating task. This article argues a solution to this dilemma can be found in a renewed appreciation of filial piety as it specifically relates to caring for elderly parents. Using the moral (...) insights of Aristotle and Aquinas, this paper develops a contemporary theoretical framework for geriatric filial piety that incorporates the traditional virtues of gratitude, respect, honor and obedience. The revelation that data obtained for the US-based National Surgical Adjuvant Breast and Bowel Project (NSABP) from subjects enrolled at Hôpital Saint-Luc in Montreal was falsified has eroded public trust in research. Institutions can educate researchers and help prevent unethical research practices by establishing procedures to monitor research involving human subjects. Research monitoring encompasses four categories of activity: annual reviews of continuing research, monitoring of informed consent, monitoring of adherence to approved protocols and monitoring of the integrity of data. The (...) authors describe characteristics of research projects that may call for monitoring procedures in each category. The form taken by such monitoring depends on the nature of the protocol. Although appropriate research monitoring requires substantial investment of personnel and financial resources, it is required under guidelines regulating research involving human subjects in Canada. Research monitoring is a step forward in re-establishing public confidence in medical research. (shrink)

This article reconsiders the presence and value of danger in outdoor and adventurous activities and sports in safety-conscious societies, especially in relation to the education of children and youth. Based on an original analysis of the relation between the concepts of ‘risk’ and ‘danger’, we offer an account of the relation between challenge, adventure, risk and danger, and emphasise the importance of teaching risk recognition, risk assessment, risk management and risk avoidance to children and youth, without the necessity of exposing (...) them to jeopardy in dangerous situations. Our conclusion is that ‘Safe Danger’ describes what educators should seek, namely: the educational benefits of the challenges set by risk-taking and the demands of risk-facing, including those in adventurous situations, which are obtainable in the absence of significant danger, and which contribute to risk education. The educational value of adventurous and outdoor pursuits lies elsewhere than in the opportunities that they present for danger-facing, for example in their promotion of self-reliance, confidence, ability to team-work, and especially in their promotion of risk education, as an integral aspect of everyday life planning, as preparation for the day’s adventurous challenges, and as an instrument of task completion. A founder in 1830 of the National Colonization Society, Charles Tennant advocated government support for emigration to Britain's colonies as a means of alleviating poverty at home and boosting the workforce overseas. Briefly representing St Albans in Parliament, he later wrote treatises on contemporary political and financial questions, notably arguing for the abolition of income tax in The People's Blue Book. Also published anonymously, the present work, which appeared in 1864, offers a critique of John Stuart Mill's Utilitarianism. Tennant (...) argues that happiness does not consist in utility, but rather in conformity to divine will as described by the Christian faith. Nevertheless, Tennant says, we ought to promote utility, as this is likely to be conducive to happiness. He then applies this view in detail to contemporary problems of government, domestic policy, taxation, colonies, dependencies, and foreign policy. (shrink)

The gradual appearance and relative stabilisation of the names of different kinds of martial activities in different cultures and contexts has led to confusion and to an unhelpful and unjustifiable elision of meanings, which merges different modes of combat and other martial activities. To gain a clearer perspective on this area, we must enquire into the criteria according to which the various kinds of martial activities are classified. Our assessment of the literature suggests that there is no satisfactory and well-justified (...) overall cross-cultural account of the classification of martial activities. This paper provides a revisionary classification and offers an explanation and a justification of the five main categories identified: close combat, warrior arts, martial paths, martial arts and martial sports; as well as some minor ones, such as martial training, martial therapy, martial display, martial games and martial dance.
