Background There is broad international agreement from clinicians and academics that healthcare rationing should be undertaken as explicitly as possible, and the BMA have publicly supported the call for more accountable priority setting for some time. However, studies in the UK and elsewhere suggest that clinicians experience a number of barriers to rationing openly, and the information needs of patients at the point of provision are largely unknown. Methodology In-depth interviews were undertaken with NHS professionals working at the community level (...) of provision, and with patients and professionals receiving or providing treatment for morbid obesity and breast cancer (n=52). Results Nearly all patients wanted to know about healthcare rationing and had high expectations of their clinical professionals to provide all relevant information about treatment options. However, professionals did not always understand these information requirements, and cases of implicit rationing were common. The existence of relevant national guidance was not always known about, meaning that patients were often reliant on other sources of information about treatment options, which included the popular media, the internet, patient advocacy groups and informal networks of support. Discussion Clinical professionals need to understand patients' need for detailed information when it comes to rationing, and to understand that they are the main gateway for this to be provided. However, disclosure could be distressing for both patients and professionals, and thus the most sensitive and acceptable ways to make this information available requires further investigation. This study is an investigation of both comprehension and production of Wh- questions in Malay-speaking children with a developmental language disorder. A total of 15 Malay children with DLD were tested on a set of Wh- questions, comparing their performance with two control groups [15 age-matched typically developing children and 15 younger TD language-matched children]. Malay children with DLD showed a clear asymmetry in comprehension of Wh- questions, with a selective impairment for which NP questions compared with who questions. Age-matched (...) controls performed at ceiling in all Wh- questions, while the language-matched group reported a subject/object asymmetry selective for the which NP, as reported in other languages. In production, both children with DLD and younger children showed a preference for questions with in situ Wh- elements, a structure that is allowed in colloquial Malay, but which is not produced by the age-matched TD group. Several non-adult-like strategies were adopted particularly by the children with DLD to avoid complex sentences, including substitution with yes/no echo questions, production of the wrong Wh- question, and use of a generic Wh- element. The study provides an insight on the mastery of Wh- questions in both typical Malay children and children with DLD. Implications for the definition of a clinical marker for DLD in a free word order language with Wh- in situ option will be discussed. (shrink)

From anxiety about Muslim immigrants in Western Europe to concerns about undocumented workers and cross-border security threats in the United States, disputes over immigration have proliferated and intensified in recent years. These debates are among the most contentious facing constitutional democracies, and they show little sign of fading away. Edited and with an introduction by political scientist Rogers M. Smith, Citizenship, Borders, and Human Needs brings together essays by leading international scholars from a wide range of disciplines to explore (...) the economic, cultural, political, and normative aspects of comparative immigration policies. In the first section, contributors go beyond familiar explanations of immigration's economic effects to explore whose needs are truly helped and harmed by current migration patterns. It has been 140 years since a full biography of William of Auvergne (1180?-1249), which may come as a surprise, given that William was an important gateway of Greek and Arabic thought and philosophy to western Europe in the thirteenth century, and one of the earliest writers in the medieval Latin west on demonology. Lesley Smith's aims in this book are two-fold: first, to take a closer look at William, the human being, how he saw the world and his (...) place in it; and to uncover William's interactions with his Parisian congregation through the nearly 600 sermons he left after his death. Smith has mined these writings, unremarked in previous scholarship, to give us a different perspective on the schoolmaster, bishop of Paris, and strict theologian we have come to know: a preacher who spoke and ministered not just to the powerful and elite, but also to commoners, to the poor, and to the less fortunate. Through a study of the sermons, Smith creates a broader landscape of William's thought and life, highlighting his attention to the importance--and limits--of language, and his attempts to find a way to address the concerns of the larger populace. In his preaching, we get a sense of the balance William achieved, in the way he communicated religious teachings, in his understanding of the concerns of ordinary Parisians, and in his awareness of the ebb and flow of daily life in a medieval city. The book will interest scholars of intellectual history and philosophy, religion, and literary studies more broadly for Smith's innovative method of excavating the sermons in pursuit of William the person, and his humanity. In The Artist-Philosopher and New Philosophy, Smith argues that Western Metaphysics has indeed come to what Heidegger describes as ¿an end.¿ That is hardly to say philosophy as such is over or soon to disappear; rather, its purpose as a medium of cultural change and as a generator of history has run its course. He thus calls for a New Philosophy, conceptualized by the artist-philosopher who ¿makes¿ or ¿poeticizes¿ New Philosophy, spanning literary and theoretical discourses and operating across art (...) in all its forms and across culture in all its locations. Nevertheless, Médecins Sans Frontières may have reasons to revise their objectives and adopt different interventions or methods.

When people encounter potential hazards, their expectations and behaviours can be shaped by a variety of factors including other people's expressions of verbal likelihood. What is the impact of such expressions when a person also has numeric likelihood estimates from the same source? Two studies used a new task involving an abstract virtual environment in which people learned about and reacted to novel hazards. Verbal expressions attributed to peers influenced participants’ behaviour toward hazards even when numeric estimates were also available. (...) Namely, verbal expressions suggesting that the likelihood of harm from a hazard is low yielded more risk taking with respect to said hazard. There were also inverse collateral effects, whereby participants’ behaviour and estimates regarding another hazard in the same context were affected in the opposite direction. These effects may be based on directionality and relativity cues inferred from verbal likelihood expressions. (shrink)
