Reports of phantom limbs amongst aplasics have often been presented as evidence that body image is ‘hard-wired’ in the brain and that neither sensory input nor proprioceptive feedback are essential to its formation. Although attempts have been made to account for these phantoms by other means, these have been on a case by case basis and no satisfactory alternative framework has been proposed. This paper collates the accounts of aplasic phantoms and presents them as compatible with a four-part hypothesis, in (...) which body image is learnt from experience during both pre- and post-natal development, and in which cross-cortical connections and mirror neurons play prominent roles. This model unites several previously disparate theories to offer a viable solution to several longstanding phantom limb mysteries and serves to highlight avenues worthy of further inquiry. (shrink)

Positive spillover occurs when changes in one behaviour influence changes in subsequent behaviours. Evidence for such spillover and an understanding of when and how it may occur is still limited. This paper presents findings of a one year longitudinal behaviour change project led by a commercial retailer in the UK & Ireland to examine behaviour change and potential spillover of pro-environmental behaviour, and how this may be associated with changes in environmental identity and perceptions of ease and affordability as well (...) as perceptions of how participation in the project has helped support behaviour change. We draw on both quantitative and qualitative. Study 1 examines quantitative data from the experimental and a matched control group. Study 2 reports qualitative findings from a follow up interview study with participants of the experimental group. As expected, we found significant changes in reported pro-environmental behaviour and identity in the experimental group as well as some indications of behavioural spillover. These changes were not significantly associated with changes in environmental identity. The interviews suggested that group dynamics played an important role in facilitating a sense of efficacy and promote sustained behaviour change and spillover. Moreover, the support by a trusted entity was deemed to be of crucial importance. (shrink)

The nature of the relationship between mind and body is one of the greatest remaining mysteries. As such, the historical origin of the current dominant belief that mind is a function of the brain takes on especial significance. In this article I aim to explore and explain how and why this belief emerged in early 19th-century Britain. Between 1815 and 1819 two brain-based physiologies of mind were the subject of controversy and debate in Britain: the system of phrenology devised by (...) Franz Joseph Gall, and William Lawrence’s lectures at the Royal College of Surgeons. Both owed a profound intellectual debt to continental comparative anatomy. In the final quarter of the 18th-century, Johann Friedrich Blumenbach, Petrus Camper and Johann Gottfried von Herder had broken away from the traditional doctrine of the Great Chain of Being by allowing for a clear anatomical distinction between ‘man’ and beast based on the morphology of the skull. This reconceptualization of man as an anatomically distinct being gave Gall and Lawrence grounds to propose that the peculiarities of the human mind were dependent on mankind’s unique cerebral size and structure. The 1988 Education Reform Act legislated for a statutory curriculum in state-funded schools in England and Wales. This study explores how, out of a common curriculum framework for both countries, there emerged a school curriculum that was adapted to the distinctiveness of the linguistic and cultural context in Wales. The roles of those most closely involved in policy development in Wales are examined as is the relationship between the 'national' and 'territorial' arenas of policy-making in the months leading up to (...) publication of the Education Reform Bill in November 1987. It is argued that a pragmatic approach to policy development in Wales during that period laid the foundations both for a reworking of the 1988 curriculum framework during the 1990s and for potentially more radical change following the 2002 Education Act. A growing number of small fashion entrepreneurs seek to offer an alternative to the mainstream fashion industry, which, in its obsession with novelty and growth, often ignores the costs to society and the environment. There is a need to develop a deeper understanding of how these fashion entrepreneurs may be agents for change in their industry. Using rich data from an in-depth study of 27 UK-based entrepreneurs, we offer such analysis, drawing on a novel framework that combines MacIntyre’s virtue ethics (...) scheme and Jonas’s responsibility imperative. We identify key virtues—passionate commitment, authenticity, humility, sharing and empowering, and temperance—which shape the entrepreneurs’ practices in line with their accepted and perceived responsibility for sustainability. These virtues shape the entrepreneurial processes that seek an active negotiation and harmonisation of sustainability and the demands of business. In particular, we pay attention to the different ways in which fashion entrepreneurs take responsibility for sustainability which ranges from a ‘growth for impact’ strategy to staying small and seeking impact through influence and replication. Conclusions are drawn on conceptualising the varieties of ‘postgrowth entrepreneurship’ that aligns the virtues of a deep-rooted sense of responsibility with business goals. This shows what can be possible and prefigures an alternative fashion industry. (shrink)

In recent years, the issue of accepting a higher cost per health improvement for orphan drugs has been the subject of discussion in health care policy agencies and the academic literature. This article aims to provide an analysis of broadly egalitarian arguments for and against accepting higher costs per health improvement. More specifically, we aim to investigate which arguments one should agree upon putting aside and where further explorations are needed. We identify three kinds of arguments in the (...) literature: considerations of substantial equality, formal equality, and opportunity cost. We argue that considerations of substantial equality do not support higher costs per health improvement orphan drugs, even if such considerations are considered valid. On the contrary, arguments of formal equality may support accepting a higher cost per health improvement for orphan drugs. However, in order to do so, a number of both normative and empirical issues must be resolved; these issues are identified in the article. For instance, it must be settled to what extent the opportunity cost in terms of foregone health for other patients is acceptable in order to uphold formal equality. We conclude that certain arguments can be set aside, and future focus should be put on the unresolved normative and empirical issues related to formal equality and opportunity cost. (shrink)

This study investigated the status quo of article retractions by Chinese researchers. The bibliometric information of 834 retractions from the Web of Science SCI-expanded database were downloaded and analysed. The results showed that the number of retractions increased in the past two decades, and misconduct such as plagiarism, fraud, and faked peer review explained approximately three quarters of the retractions. Meanwhile, a large proportion of the retractions seemed typical of deliberate fraud, which might be evidenced by retractions authored by repeat (...) offenders of data fraud and those due to faked peer review. In addition, a majority of Chinese fraudulent authors seemed to aim their articles which contained a possible misconduct at low-impact journals, regardless of the types of misconduct. The system of scientific evaluation, the “publish or perish” pressure Chinese researchers are facing, and the relatively low costs of scientific integrity may be responsible for the scientific integrity. We suggested more integrity education and severe sanctions for the policy-makers, as well as change in the peer review system and transparent retraction notices for journal administrators. (shrink)

We show that Smullyan's analytic tableaux cannot p-simulate the truth-tables. We identify the cause of this computational breakdown and relate it to an underlying semantic difficulty which is common to the whole tradition originating in Gentzen's sequent calculus, namely the dissonance between cut-free proofs and the Principle of Bivalence. Finally we discuss some ways in which this principle can be built into a tableau-like method without affecting its analytic nature.

In many eighteenth‐century agricultural texts, peasants were depicted as an impediment to agrarian improvement, superstitious and resistant to novelty. That was the stereotype one often encountered, in any case, in more programmatic writing about agriculture from this period. A closer look at the era's technical literature tells a more complicated story, however. Much as traveling European naturalists relied on local intermediaries in far corners of the globe, elite agricultural improvers back home relied on local rural knowledge as they drafted (...) a new technical literature on farming. This article looks at Jacob Guyer (also known as Kleinjogg), a Swiss farmer who became an enlightened celebrity after the Zurich doctor Hans Caspar Hirzel published a biography of him in 1761. Hirzel called Kleinjogg a “philosophical farmer”; the article explores the implications of this title, and examines the various ways that elite eighteenth‐century authors used contact with rural people and direct experience of farming as a source of authority. In this paper, we argue that there are important ethical questions about healthcare improvement which are underexplored. We start by drawing on two existing literatures: first, the prevailing, primarily governance-oriented, application of ethics to healthcare ‘quality improvement’ (QI), and second, the application of QI to healthcare ethics. We show that these are insufficient for ethical analysis of healthcare improvement. In pursuit of a broader agenda for an ethics of healthcare improvement, we note that QI and ethics (...) can, in some respects, be treated as closely related concerns and not simply as externally related agendas. To support our argument, we explore the gap between ‘quality’ and ‘ethics’ discourses and ask about the possible differences between ‘good quality healthcare’ and ‘good healthcare’. We suggest that the word ‘quality’ both adds to and subtracts from the idea of ‘good healthcare’, and in particular that the technicist inflection of quality discourses needs to be set in the context of broader conceptualisations of healthcare improvement. We introduce the distinction between quality as a measurable property and quality as an evaluative judgement, suggesting that a core, but neglected, question for an ethics of healthcare improvement is striking the balance between these two conceptions of quality. The moral vision of the "Analects" notably includes among our moral responsibilities the need to style behavior such that the propriety of one's dispositions is evident in one's manner and demeanor. While the sage effortlessly fulfills this responsibility, the moral learner must actively strive to shape her demeanor and manner. This essay considers her resources for doing so where becoming effortlessly sagely is a distant, if not unreachable, possibility. While the "Analects" clearly proffers the li as the principal mechanism for (...) developing an appropriate style, the models provided by Zigong and Zilu, two of the text's most vividly depicted moral learners, demonstrate what an improvement in the domain of style requires and significantly indicate an account of moral style in which formal propriety must be vouchsafed by the personally revelatory. There is something very appealing about the idea that we are epistemic agents. One reason—if not the main reason—is that, while we are undoubtedly fallible creatures, us being epistemic agents that do things means that it might just be within our power to improve and thereby do better. One important way in which we would want to improve is in relation to our well-established tendency for cognitive bias. Still, the proper role of epistemic agency in us avoiding or correcting for (...) cognitive bias is highly limited. In fact, what we know from empirical psychology—particularly with respect to our tendencies for overconfidence—suggests that we cannot rely on ourselves for epistemic improvement, and have good reason to impose significant constraints on our ability to exercise such agency in ameliorative contexts. Does Seneca entirely reject the utility of dialectical study for moral improvement? No, I argue here. Focusing on Letter 87, I propose that Seneca raises and disarms objections to formal Stoic arguments to help moral progressors avoid backsliding and advance towards ethical knowledge. I trace this method back to Chrysippus and show that reading Letter 87 in this Chrysippean framework yields a satisfying explanation of its otherwise puzzling features.

The small-improvement argument is usually considered the most powerful argument against comparability, viz the view that for any two alternatives an agent is rationally required either to prefer one of the alternatives to the other or to be indifferent between them. We argue that while there might be reasons to believe each of the premises in the small-improvement argument, there is a conflict between these reasons. As a result, the reasons do not provide support for believing the conjunction (...) of the premises. Without support for the conjunction of the premises, the small-improvement argument for incomparability fails. (shrink)

Mounting evidence suggests that participation in clinical trials confers neither advantage nor disadvantage on those enrolled. Narrow focus on the question of a “trial effect,” however, distracts from a broader mechanism by which patients may benefit from ongoing clinical research. We hypothesize that the existence of clinical trials infrastructure—the organizational culture, systems, and expertise that develop as a product of sustained participation in cooperative clinical trials research—may function as a quality improvement lever, improving the quality of care and outcomes (...) of all patients within an institution or region independent of their individual participation in trials. We further contend that this “infrastructure effect” can yield particular benefits for patients in low- and middle-income countries. The hypothesis of an infrastructure effect as a quality improvement intervention, if correct, justifies enhanced research capacity in LMIC as a pillar of health system development. (shrink)

Moral distress is a major problem for nurses, other clinicians, and the health system itself. But if properly understood and responded to, it is also a promising guide for healthcare improvement. When individuals experience moral distress or burnout, their reports must be seen as crucial data requiring careful attention to the individuals and to the organization. Distress and burnout will often point to important opportunities for system improvements, which may in turn reduce the experience of distress. For this potential (...) virtuous cycle to happen, individuals must be able to articulate their concerns without fear of retribution, and organizational leaders must be able to listen in an undefensive, improvementoriented manner. Buddhist philosophical traditions share the Hellenistic orientation toward therapy, particularly with regard to therapeutic interventions in our emotional life. As Pierre Hadot and Martha Nussbaum have ably argued, for the Hellenistic philosophers, philosophy itself is a therapy of the emotions. In this paper, I shift the focus of the contemporary philosophical literature on therapies of the emotions, which investigates almost exclusively the Hellenistic philosophers, and instead draw on the therapies developed by Tibetan Buddhist philosophers and yogis, in particular Gampopa (1079–1153), (...) Tsongkhapa (1357–1419), and Patrul Rinpoche (1808–87). The texts I use are in the genre of Tibetan literature called lo jong (blo sbyong), or mind (blo)- training (sbyong). I argue that the philosophical analysis of the Tibetan Buddhist therapies gives us a model of moral improve- ment that is both novel and promising. This model focuses on the morally improving agent’s capacity for self-awareness and relaxation, her playful approach to emotional-ethical obstacles, and the cultivation of sophisticated, context-sensitive and appropriately flexible judgments about her emotional-ethical life. I also offer a rough taxonomy of Tibetan therapies of the emotions, which will serve as a vehicle to understanding the view of moral improvement described above. (shrink)

The Inherent inseparability of psychopathology and phenomenology is generally a known fact, established and popularised by Karl Jaspers in Allgemeine Psychopathologie. In the following paper, I will show the development of interdisciplinary methodology initiated by Jaspers, and discuss it by combining M. Merleau Ponty`s theory of embodiment, R. D. Laing`s existential-phenomenological approach, and T. Fuchs` concept of brain resonance and integral causality with the hermeneutical thoughts of Paul Ricœur regarding the notion of selfhood. The main thesis proposes that fusion of (...) hermeneutics and phenomenology with psychiatry can play a significant methodological role in the approach to diagnosis and treatment of the psychotic disorders spectre, especially regarding the most complex yet inaccurately defined and evaluated mental disorder – schizophrenia. Due to the fundamental interpretative capacity of both disciplines regarding the understanding of the displacement (translocation) of subject, disembodiment, delusions and consciousness, I am considering them as a prerequisite for the effective expansion of psychiatric practice. The Mode of Relativity in Agrippa’s Five Modes does not fit with the other four modes, and disrupts an otherwise elegant system. We argue that it is not the familiar argument from epistemic relativism, but a formal condition on the structure of justifications: the principle that epistemic grounding relations cannot be reflexive. This understanding of Agrippan Relativity leads to a better understanding of the Modes of Hypothesis and Reciprocity, a clearer outline of the structure of Agrippa’s system as a whole, (...) and a new insight into the Two Modes that follow the Five. One of the great achievements of Pierre Hadot has been to chart how philosophy's self‐conception has shifted over time, first as the culture of the classical world gave way to that of medieval Christianity, and then again through the long and gradual emergence of the modern age. Hadot himself suggests that the crucial shift came in the middle ages, as a result of the growing dominance of Christianity. The chapter argues that philosophy in both its classical and medieval incarnations has (...) consistently involved systematic logical inquiry of an abstract. Descartes’ philosophy is Janus‐faced, affording us frequent glances back to the tradition he grew out of, but also allowing glimpses forward to the modern age that his ideas helped to launch. Individual differences have been neglected in decision-making research on heuristics and cognitive biases. Addressing that issue requires having reliable measures. The author first reviewed the research on the measurement of individual differences in cognitive biases. While reliable measures of a dozen biases are currently available, our review revealed that some measures require improvement and measures of other key biases are still lacking (e.g., confirmation bias). We then conducted empirical work showing that adjustments produced a significant improvement of some (...) measures and that confirmation bias can be reliably measured. Overall, our review and findings highlight that the measurement of individual differences in cognitive biases is still in its infancy. In particular, we suggest that contextualized (in addition to generic) measures need to be improved or developed. (shrink)

What I intend to show is that the Leibnizian language studies—the formal ones as well as those on natural languages—from his early plans for academies and language societies on up to his studies of etymology and to his interest in foreign languages and in logical, geometrical, arithmetical, and other formal calculi, has to be seen as an important contribution to the idea of enlightenment. Their importance was such that Christian Wolff was able to transform the Leibnizian ideas into the mighty (...) movement of Leibniz-Wolffian metaphysics, a movement paralleled to the tradition of Thomasius. Mental health-related data generated by app users during the routine use of Consumer Mental Health Apps (CMHAs) are being increasingly leveraged for research and product improvement studies. However, it remains unclear which ethical safeguards and practices should be implemented by researchers and app developers to protect users during these studies, and concerns have been raised over their current implementation in CMHAs. To better understand which ethical safeguards and practices are implemented, why and how, 17 app developers and researchers were (...) interviewed who had been involved in using CMHA data for studies. Interviewees discussed the impact on stakeholder interests, sufficiency thresholds and procedural alterations of informed consent, data protection, gathering app user perspectives and representing users in app design and study conduct, and ensuring adequate support. Although the reasoning behind how and why these ethical safeguards and practices should be implemented showed considerable variability and several gaps, interviewees converged on various general lines of reasoning. This allowed for the development of a coherent and nuanced account that could prove useful for future CMHA studies and which could stimulate further normative investigation of the role of ethical safeguards and practices in these studies. Ongoing evaluation of a clinical ethics consultation service (ECS) allows for continuous quality improvement, a process-based, data-driven approach for improving the quality of a service. Evaluations by stakeholders involved in a consultation can provide realtime feedback about what is working well and what might need to be improved. Although numerous authors have previously presented data from research studies on the effectiveness of clinical ethics consultation, few ECSs routinely send evaluations as an ongoing component of their everyday clinical activities. The (...) primary purpose of this article is to equip and encourage others to engage in ongoing evaluation of their own ECS. Toward that end, the following resources are shared: (1) the survey tool used to gather the evaluation data, (2) the procedure used to elicit and collate responses, and (3) how the resulting data are used to support continuous quality improvement and justify the continued financial support of the ECS to hospital administration. The idea of social improvement, including the concept of ‘reciprocity’, had substantially been developed in the Anglo-Irish trade disputes since the late seventeenth century. Towards the end of the eighteenth century, however, commentators became more sceptical of ‘reciprocity’. The Irish reception of Adam Smith's Wealth of Nations must be situated in this historical context, and the article explores the implications of the relevant discourses for John Robertson's concept of Enlightenment. Like in Scotland, ‘improvement’ was considered significant in eighteenth-century (...) Ireland. Nevertheless, political economy played different roles in the two nations because of their different political systems and circumstances. Many, including Adam Smith, believed that political and constitutional, rather than economic, reforms would be more crucial to improving Irish society. (shrink)

School effectiveness research together with what is now described as the 'school improvement movement' has captured both the Conservative and New Labour imaginations as a basis for educational planning and policy making in the UK. Internationally school effectiveness enjoys and expanding and enthusiastic audience. This book provides a critique of this research genre, particularly in the light of the recent calls for teaching to go 'back to the basics'. The editors argue that this school effectiveness research is simplistic in (...) its analysis of educational problems. Far from getting to the bottom of the problem of failing students and schools, they argue, these 'movements' are merely scratching at the surface of the problems and coming up with notions for superficial improvements. Policy work is often cited as one of the primary functions of Hospital Ethics Committees (HECs), along with consultation and education. Hospital policies can have far reaching effects on a wide array of stakeholders including, care providers, patients, families, the culture of the organisation and the community at large. In comparison with the wealth of information available about the emerging practice of ethics consultation, relatively little attention has been paid to the policy work of HECs. In this paper, we hope (...) to advance the development of best practices in HEC policy work by describing the quality improvement process that we undertook at Hamilton Health Sciences, Hamilton, Ontario, Canada. In the first section of the paper we describe the context of our HEC policy work, and the shortcomings of our historical review process. In subsequent sections, we detail the quality improvement project we undertook in 2010, the results of the project and the specific tools we developed to enhance the quality of HEC policy work. Our goal in sharing this organisational case study is to prompt other HECs to publish qualitative descriptions of their policy work, in order to generate a body of knowledge that can inform the development of best practices for ethics policy review.
