ABSTRACT There has been much discussion regarding the acceptable use of sedation for palliation. A particularly contentious practice concerns deep, continuous sedation given to patients who are not imminently dying and given without provision of hydration or nutrition, with the end result that death is hastened. This has been called ‘early terminal sedation’. Early terminal sedation is a practice composed of two legally and ethically accepted treatment options. Under certain conditions, patients have the right to reject hydration and nutrition, even (...) if these are life‐sustaining. Patients are also entitled to sedation as palliation for intolerable, intractable suffering. Though early terminal sedation is thought to be rare at present, the changing nature of palliative medicine suggests its use will increase. Arguments regarding early terminal sedation have failed to recognize early terminal sedation as a distinct legal and ethical entity. It can be seen as both the simple sum of treatment refusal and sedation for palliation, analogous to terminal sedation. It can also be seen as an indivisible palliative treatment, more analogous to assisted suicide or euthanasia. But ultimately, it is wholly analogous neither to terminal sedation given when death is imminent, nor to assisted suicide or euthanasia. This paper contends that early terminal sedation should be considered as a distinct entity. Such a reconception promises to provide a way forward in the debate, practice and policy regarding this contentious area of palliative medicine. (shrink)

Das zentrale »Spätwerk« von Wilfred Bion, die fiktionale Trilogie »A Memoir of the Future« (1975–1979), wurde im deutschsprachigen psychoanalytischen Diskurs bisher wenig beachtet. Die Rezeption wird zweifellos durch die fehlende Übersetzung, aber auch durch die Sperrigkeit und überbordende Komplexität des Werks erschwert. In seiner Trilogie geht es Bion nicht um die Präsentation neuer psychoanalytischer Theorien oder klinischer Fakten, vielmehr ist er darum bemüht, eine Sprache und Form zu finden, die unmittelbare emotionale Erfahrung evoziert. In diesem Erfahrungsbericht sollen in diesem Sinne (...) weniger Inhalte referiert als vielmehr der Versuch einer Untersuchung der Wirkungen, die das Werk im Leser hervorruft, unternommen werden. Das Buch erweist sich als harte Prüfung für die Fähigkeit, Nichtwissen und Nichtverstehen auszuhalten und somit der Verführung zu widerstehen, sich in Altbekanntes zu flüchten und damit das Aufkommen einer tatsächlich neuen Erfahrung zu verhindern. Anhand der detaillierten Analyse eines Kapitels aus dem ersten Band der Trilogie werden die Möglichkeiten der Auseinandersetzung mit dem Werk exemplarisch dargestellt und diskutiert. (shrink)

One of three basic types of desire, claims Aristotle, is thumos (‘spirit,’ ‘passion,’ ‘heart,’ ‘anger,’ ‘impulse’). The other two are epithumia (‘appetite’) and boulêsis (‘wish,’ ‘rational desire’). Yet, he never gives us an account of thumos; it has also received relatively little scholarly attention. I argue that thumos has two key features. First, it is able to cognize what I call ‘social value,’ the agent’s own perceived standing relative to others in a certain domain. In human animals, shame and honor (...) are especially important manifestations of social value. Second, thumos provides non-rational motivation to pursue what affirms the agent’s social value and avoid what denies it. Interpretations that hold thumos just is anger, or that its object is the fine (kalon), I argue, are mistaken. My account also explains the role of thumos in moral education. In a virtuous agent thumos will be affectively attuned to the correct social rankings; it will take the practically wise, the lovers of the fine, or moral exemplars, as authorities. (shrink)

The aim of this paper is to define a partial Propositional Type Theory. Our system is partial in a double sense: the hierarchy of (propositional) types contains partial functions and some expressions of the language, including formulas, may be undefined. The specific interpretation we give to the undefined value is that of Kleene’s strong logic of indeterminacy. We present a semantics for the new system and prove that every element of any domain of the hierarchy has a name in the (...) object language. Finally, we provide a proof system and a (constructive) proof of completeness. (shrink)

The expression of attachment to the divine in certain places among different groups has been documented by anthropologists and sociologists for decades. However, the psychological processes by which this happens are not yet fully understood. This article focuses on the concept of ‘place spirituality’ as a psychological mechanism, which allows the religious believer or non-believer to achieve an organised attachment strategy, involving the interplay of place and spiritual attachment. First, place spirituality is considered as an experience that satisfies the attachment (...) relationship criteria in that geographic places and divine entities can be perceived as ‘objects’ of attachment. Second, it is proposed that the maturational aspects of the attachment repertoire in adults make the place spirituality experience possible since adults’ cognitive abilities are much wider than those of children and can include relationships to geographical spaces and invisible divine entities. Finally, the theme of place spirituality is explored to further position the concept as a relational paradigm for understanding the relationship between place experiences and spiritual attachment. In March 1912 A. N. Whitehead wrote a letter which sheds new and important light on his own view of his mathematical goals. In this article I publish the letter for the first time and relate its contents not only to his mathematical career but also to his scientific, philosophical and educational interests.

Wittgenstein famously regarded philosophy as an activity and not as a body of doctrine. And yet within the secondary literature there is little agreement as to what Wittgenstein took the purpose of that activity to be. In this paper, I claim that the purpose of philosophical activity, at least according to the Later Wittgenstein, was to solve philosophical problems. As support for this claim, I argue that our everyday talk about the mind presents us with a philosophical problem about the (...) mind. Focusing then on what Wittgenstein says about understanding and using his distinction between internal and external relations, I show how we can solve this problem. If my reading is accepted, then the purpose of philosophical activity, according to the later Wittgenstein, was not therapy. As such, later Wittgenstein should not be read as a therapist. (shrink)

This article explores the fate in Wittgenstein's mature work, of the issue of the unsayable, heart of the Tractatus project. According to the traditional interpretation of his work, he left behind that concern as a product of his early mistakes, later refuted in the Investigations. This article attempts to show that, in a certain sense, it is obvious that the problem of ineffability is still behind all his efforts. That it is constitutive not only of Wittgenstein's conception of language, but (...) is also the point of what he calls the vision of aspects, on which his latest writings focus. (shrink)

Evolutionary functional theories about morality assume that morality has been evolutionarily selected because it produces a cooperative and pro-social effect. This assumption wrongly implies that the trait which produces the selected effect is fully known. Furthermore, a SE theory does not seem to explain the peculiarities of moral thought, which are mostly synthesized in the features of moral judgment: its authoritative and inescapable character. This paper provides an alternative SE explanation and argues that a causal role functional explanation is also (...) necessary because this kind of explanation establishes the way the selected effect interacts with basic human evaluative propensities, which not only makes it possible to give an account of the practical clout of moral judgment, but of the phenomenon of moral ambivalence as well. However, the connection between reason and emotionand their influence on morality remains unexplained. (shrink)

Precise measurements of well-being would be of profound societal importance. Yet, the sceptical worry that we cannot use social science instruments and tests to measure well-being is widely discussed by philosophers and scientists. A recent and interesting philosophical argument has pointed to the psychometric procedures of construct validation to address this sceptical worry. The argument has proposed that these procedures could warrant confidence in our ability to measure well-being. The present paper evaluates whether this type of argument succeeds. The answer (...) is that it depends on which methodological background assumptions are motivating the sceptical worry to begin with. We show this by doing two things. First, we clarify (a) the different types of well-being theories involved in the science of well-being, and (b) the general methodological dimensions of well-being theorising. Second, we apply these distinctions and argue that construct validation is an unsuccessful response to measurement scepticism if this scepticism is motivated by a form of methodological non-naturalism. In the light of this, the overall point of the paper is that philosophers and scientists, when discussing measurement of well-being, should explicate their deeper methodological commitments. We further suggest that making such explicit commitments might present philosophers with a dilemma. (shrink)

I defend an alternative theory of conversational implicatures that does without Grice's notion of making‐as‐if‐to‐say. This theory characterises conversationally implicating that p as a way to mean that p by saying that q or by saying nothing. Cases that Grice's theory cannot capture are captured, and cases that Grice's theory misdescribes are correctly described. A distinction between conversational implicatures and pragmatic inferences from what speakers express is required, as well as a non‐implicature treatment of figurative speech.

Vigorous Fodorian criticism may make it seem impossible for Inferential Role Semantics (IRS) to accommodate compositionality. In this paper, first, I introduce a neo-Fregean version of IRS that appeals centrally to the notion of rationality. Second, I show how such a theory can respect compositionality by means of semantic rules. Third, I argue that, even if we consider top-down compositional derivability: a) the Fodorian is not justified in claiming that it involves so-called reverse compositionality; and b) a defender of IRS (...) can still offer a satisfactory account in terms of the inferential capacities of rational thinkers. Data from three separate studies conducted in Maputo, Mozambique, in 1993 are used to analyse the relationship between the type of social environment in which women work and their fertility and contraceptive use. The analysis finds that women who work in more collectivized environments have fewer children and are more likely to use modern contraception than women who work in more individualized milieus and those who do not work outside the home. Most of these differences persist in multivariate tests. It (...) is argued that collectivized work environments are most conducive to diffusion and legitimation of reproductive innovations. In contrast, individualized environments tend to isolate women and therefore may retard their acceptance of innovative fertility-related behaviour. (shrink)

Resumen: En este trabajo se parte de las tesis de Foucault sobre la integración de ciertos dispositivos de control y de un régimen médico-sexual en el Derecho, que definieron las identidades de los sujetos, para explicar el modo en que estos mecanismos han influido en la regulación jurídica de los cambios de identidad de género. Asimismo, se detalla cómo la medicalización de las identidades por parte del Derecho contradice la evolución del concepto de género. A partir de este análisis, el (...) artículo propone una reflexión sobre algunos cambios legislativos recientes y sobre los puntos de encuentro y desencuentro entre las diversas corrientes al objeto de defender un nuevo discurso justificatorio capaz de reinterpretar los conceptos de autonomía y autodeterminación sexual.Abstract: Taking into account Foucault’s thesis about security dispositives and the medical-sexual regime within law, which have defined the identity of subjects, this paper focus on how these have influenced the legal regulation of gender identity changes. Furthermore, it also focus on how the medicalization of gender identities contrasts with the meaning of concept provided by gender studies. Thus, an examination of recent regulatory changes and the crossing points among the former theories is proposed with a view to promote new meanings and justify discourses in order to reinterpret the concepts of autonomy and sexual self-determination. This essay addresses dominant critiques of Francisco Suárez’s metaphysical project raised by many contemporary philosophers of religion. Those critiques often center upon two main claims. (1) God and creature are both comprehended under the concept of being such that God amounts to just one more being among others. As such, a univocal community of being results wherein God’s divine transcendence and irreducibility to creation are destroyed. (2) Since Suárez employs a univocal concept of being when conducting his metaphysical speculations about (...) God, he has (unwittingly) abandoned the God of Christian revelation in favor of a conceptual idol. The author argues that both critiques harbor a severe misunderstanding of Suárezian metaphysics, and that it is precisely in turning to the concept of being that Suárez defends both God’s irreducible transcendence and his incomprehensibility. Paradoxically, to think of God in terms of being is just to leave God unthought. (shrink)
