As part of the preparations to establish a population-based biobank in a large Israeli health organization, we aimed to investigate through focus groups the knowledge, perceptions and attitudes of insured Israelis, toward biobanking, and then, after input from focus groups’ participants, to empirically assess the impact of a revised recruitment process on recruitment rates. 1) Six Focus group discussions were conducted with individuals who had routine blood laboratory tests taken in the last 2 years. 2) After addressing the issues raised (...) in the focus groups and revising the recruitment process, individuals undergoing routine blood tests in phlebotomy clinics were invited to participate in the future biobank. At the outset of the focus groups there was an overall positive response to the prospect of a population-based biobank. Concerns revolved around infringement on privacy, fears of the “big brother”, and anxiety about inequality. Reaction to the language of the informed consent document revolved around concerns over ability to maintain anonymity, to withdraw consent, involvement of commercial entities, and the general tenor of the informed consent, which was perceived as legalistic and unilateral. In general, the longer participants were exposed to discussion about the biobank, the less likely they were to consent to sign in. Overall, only 20% of the 60 participants stated they would agree to sign in by the end of the 2 hour group session. The feedback obtained from the focus groups was used in the second stage of the study. A team of recruiters received extensive training to enable fruitful discussion and a detailed explanation to questions and concerns raised during the recruitment process. During the second stage of the study, after revising the consent form and training recruiters, a 53% consent rate was observed among 10,262 participants, more than 4 fold higher than estimated at the focus group stage. The qualitative focus group research helped identify important perceptions and concerns, which were subsequently addressed in the revised consent form and in the discussion the recruiters had with potential biobank donors. We consider bargaining situations where two players evaluate outcomes with reference-dependent utility functions, analyzing the effect of differing levels of loss aversion on bargaining outcomes. We find that as with risk aversion, increasing loss aversion for a player leads to worse outcomes for that player in bargaining situations. An extension of Nash's axioms is used to define a solution for bargaining problems with exogenous reference points. Using this solution concept we endogenize the reference points into the model and find a (...) unique solution giving reference points and outcomes that satisfy two reasonable properties, which we predict would be observed in a steady state. The resulting solution also emerges in two other approaches, a strategic (non-cooperative) approach using Rubinstein's (1982) alternating offers model and a dynamic approach in which we find that even under weak assumptions, outcomes and reference points converge to the steady state solution from any non-equilibrium state. How can adverse experiences in early life, such as maltreatment, exert such powerful negative effects on health decades later? The answer may lie in changes to DNA. New research suggests that exposure to stress can accelerate the erosion of DNA segments called telomeres. Shorter telomere length correlates with chronological age and also disease morbidity and mortality. Thus, telomere erosion is a potential mechanism linking childhood stress to health problems later in life. However, an array of mechanistic, methodological, and basic biological (...) questions must be addressed in order to translate telomere discoveries into clinical applications for monitoring health and predicting disease risk. On the basis of a case study of the integration of Haredi Jewish women into the Israeli high-tech industry, we explore how gender–religiosity intersectionality affects ultra-conservative women’s participation in the labor market and their ability to negotiate with employers for corporate work–family practices that address their idiosyncratic requirements. We highlight the importance of pious women’s affiliation to their highly organized religious communities while taking a process-centered approach to intersectionality and focusing on the matrix of domination formed by the Israeli state, (...) employers, and the organized ultra-orthodox community. We dub this set of actors “the unholy-trinity” and argue that it constructs a specific, religion-centric inequality regime that restrains women’s job and earning opportunities. At the same time, the “unholy trinity” also empowers women in their struggle to create a working environment that is receptive to their religiosity and what that commitment demands of them. (shrink)

I left to be finished for my journey thither, nor any notice of it [...]. I agreed with mr. Allen upon price and the time that he should finish it, if he hath failed me he hath done me the greatest injury that can be. 1A great injury indeed, because Greaves's journey to Italy and the Levant was all about measuring—luckily the instrument did reach him at some later stage. The thirty-six-year-old Professor of Geometry at Gresham College was taking the measurements of countless monuments and objects in the locations he visited. In Rome he measured, among many other ancient structures, Cestius' Pyramid and St. Peter's basilica. In Lucca, deeply impressed, he counted his paces around the beautiful city walls. In Siena he observed together with a "Mathematical Professor" one of the Sidera Medicea using "a glass." In Egypt he even hurt his eyes gazing at the sun, looking for sunspots and measuring its diameter. 2 His measuring mission, however, culminated in the fixing of the [End Page 555] latitudes of Istanbul, Rhodes, and Alexandria. That he miscalculated the latitude of Rhodes is of less consequence for his present-day readers. 4 However, one of the most obscure and therefore telling of his measurement activities was the survey of the Pyramids of Giza, which resulted in the Pyramidographia (1646). 5 This remarkable learned treatise and travel account hybrid, which is at the focus of the present study, gives us a glimpse into the rich and complex world of scientific antiquarians.Greaves is most conveniently remembered today as an Orientalist. Trust in science post-Covid appears to be a complex matter. On the one hand, the COVID-19 pandemic added value to the epistemic trustworthiness of scientific opinion and its potential to drive evidence-based policies, while it also spurred scientific distrust and societal polarization (e.g., vaccines), especially on social media. In this work we sought to understand the ways in which trust in science might be bolstered by adopting a multistakeholder perspective. This objective was achieved by considering stakeholders’ views on (a) _how_ (...) perceived key actors affect trust in science, and (b) _what_ proposed actions can be taken by each actor identified. Data were collected using 16 focus groups and 10 individual interviews across different European contexts with general public (_n_ = 66), journalists (_n_ = 23) and scientists (_n_ = 35), and were analysed using thematic analysis. Regarding _how_ perceived key actors affect trust in science, participants viewed policymakers, media, scientific and social media actors as occupying a dual function (facilitators and hinderers of trust in science), and pointed to the value of multi-actor collaboration. Regarding _what_ actions should be taken for enhancing trust in science, participants indicated the value of enhancing understanding of scientific integrity and practices, through science literacy and science communication, and also pointed to social media platform regulation. Implications stemming from the data are discussed, considering how multiple identified stewards of trust can contribute to an ecosystem of trust. (shrink)

By exploring how gender norms and material interests vary between women in different classes, this article highlights interactions between class and gender that mitigate against the mobilization of political support for activist family policies in the United States. Ironically, while educated women in professional and managerial jobs are ideologically most favorable toward the dual earner/dual carer model, it is not in their economic interest for the state to make it happen. Scandinavian-style interventions would impose costs on relatively privileged women in (...) their role as child care consumers. This is a retrospective of surrogacy in Israel in the context of medically assisted reproduction. The practice of surrogacy emerged in the 1980s, suggesting a radical view of women as autonomous reproductive agents free from double standards of patriarchy. In 1995 Israel enacted a law that regulated domestic surrogacy for the benefit of infertile married women in accord with orthodox views of halakha. In recent years we see the growth of inter-country practices catering to otherwise ineligible intended parents, and a (...) demand for posthumous male reproduction. Meanwhile, women become objects of intrusive medical procedures and commodities of a global market, more often than not for the benefit of others rather than themselves. Yet the essence of human reproduction remains relational, and calls for care and responsibility. (shrink)

During much of his prolific career, the late historian Jacob Talmon was preoccupied with revolutionary movements, and was especially unsettled by, and attracted to, the force displayed by the French and Russian Revolutions. The young United States’ long and bloody war against the British Empire, followed by the creation of a republican novus ordo seclorum, supposedly fitted Talmon's revolutionary model and narrative. Hence, it is hard to account for the complete absence of the American Revolution from Talmon's extensive and celebrated (...) trilogy. This paper examines how Talmon understood revolutions and how the major historiographical schools interpreting the American Revolution could not accommodate, for different reasons, Talmon's paradigm of the nature and essence of revolutions. The paper further demonstrates how not only the failings of different historical interpretive schemes convinced Talmon to ignore the American Revolution. Rather, since the American Revolution could be conceived either as Lockean or Machiavellian, but in any event not as Rousseauian, Talmon overlooked its Atlantic nature; he chose to focus solely on messianic Europe. The paper will thus analyze the meaning and consequence of the fact that Talmon left the examination of the pursuit of happiness to Americanists, and chose to leave 1776 out of his corpus. In the latter half of the 20th century, Western medicine moved death from the home to the hospital. As a result, the process of dying seems to have lost its spiritual dimension, and become a matter of prolonging material life by means of medical technology. The novel quandaries that arose led in turn to medico-legal regulation. This paper describes the recent regulation of dying in Israel under its Dying Patient Law, 2005. The Law recognizes advance directives in principle, but limits (...) their effect and form through complex medico-legal artifices. It reflects a culture that places high value on both scientific medicine and the sanctity of life as such, and illustrates a medical culture that pitches battle against death. At the same time, the Law constructs the will of the individual in a medico-legal language that is alien to the lay person. The paper suggests an alternative approach to advance care planning that is patient-centred and addresses the psycho-social needs of the individual in terms of her relational autonomy. From this perspective, advance care planning becomes an opportunity to extract the patient from the medical context and allow her to speak about her approaching death with close ones in her own terms of reference. To this end, there is a need for facilitation of an intimate encounter where patients can speak about their concerns with their loved ones. The paper also presents a methodological approach of attentive listening, which can be applied across diverse cultures and circumstances. (shrink)

The rapid development of assisted reproduction technologies for the treatment of infertility appears to empower women through expanding their individual choice, but it is also creating new forms of suffering for them and their collaborators, especially in the context of transnational third-party reproduction. This paper explores the possibility of framing the ethical discourse around third-party reproduction by bringing attention to concerns of altruistic empathy for women who collaborate in the reproductive process, in addition to those of individualistic choice. This would (...) entail moving beyond an ethic of liberty that is based on self-interest and the language of rights, to an alternative ethic of care that is based on self-restraint and the language of responsibilities. An ethic of care and responsibility would cultivate the empathetic self-reflection of the autonomous actor in relation to those others who are part of the enterprise of bringing a child into the world. One defining feature of the Hindu religious worldviews is a belief in the impermanence of the body and its perception as a source of suffering due to a misguided attachment of the self to its corporeal manifestation. This view is expressed in several important traditions, including classical yoga, which perceives the physical body as an impediment to attaining liberation and irrelevant in the state of liberation.However, the perception of the physical body in liberation is going through ontological changes in early (...) medieval Haṭhayoga texts. The dichotomy between embodied and disembodied states of the body when one is liberated is the focus of this article. In several medieval yoga texts dating between the eleventh and the fifteenth centuries, the ontological immortal state of living liberation is more similar to physical death. Often the exact text holds both views of embodied and disembodied states simultaneously with no overt inconsistency. I argue that these tensions and contradictions can be explained in two ways: by looking at the assessed timeline of the texts and by reflecting on their sectarian and social origin. (shrink)

Haṭhayoga texts, influential in the development of modern posture-based yoga, offer a conceptual framework intertwining physical cultivation, health, and soteriological aims. This article explores two interconnected inquiries regarding early Haṭhayoga texts: How do they conceptualize health, and how do they perceive the relationship between health and soteriology? These investigations illuminate the place of health in early yoga texts within Hindu traditions prior to its global recognition. This article reveals that early _haṭha_ texts present a holistic approach to health, highlighting the (...) interconnectedness of curing the mind and healing the body. This process entails not only eradicating physical diseases but also the transcendence of all forms of suffering. The transformative process of eliminating the misleading self-identity (_ahaṁkāra_) is pivotal, leading directly to health. This relationship extends beyond metaphorical comparisons of healing ignorance and suffering. This ontological framework proposes a harmonious integration between the material and soteriological realms, viewing the healthy body as a physical manifestation of the divine essence. Thus, the emergence of the biomedical discourse surrounding yoga in modern times did not occur autonomously. Haṭhayoga, with its roots dating back to the beginning of the second millennium CE, played a crucial role in shaping this historical transformation. (shrink)

This article examines the current state of end-of-life care in internal medicine wards in Israel, through an analysis of medical practice and the existing legal framework. The authors demonstrate the processes that lead chronically ill, elderly patients to perceive death as an unexpected phenomenon that is to be avoided at all costs. This perception stems, among other things, from the lack of public debate on questions relating to the end of life and the dominant cultural expectation that physicians provide curative (...) interventions. This results in a dearth of palliative care for the elderly along with a growing number of medical interventions that are of questionable value. The authors propose an alternative approach that highlights individual well-being and that demonstrates the potential areas of intervention by which death can be transformed into an expected and acceptable occurrence for the old and infirm. This approach allows patients to avoid unnecessary interventions and to reduce the burden of responsibility on family members and physicians, who are currently being called on to make end-of-life choices under exigent circumstances. We present these dilemmas by focusing on typical cases of incompetent elderly patients when there is no clear documentation of their wishes regarding treatment, and when their families do not have a coherent perception of what they may have wanted or of the care that would be most appropriate for them. We conclude with a call to action that highlights the need for greater awareness—through public discourse and private discussions—of end-of-life medical choices before the onset of ill-health and incompetence. The COVID-19 pandemic poses a major challenge to policy makers on how to encourage compliance to social distancing and personal protection rules. This paper compares the effectiveness of two policies that aim to increase the frequency of responsible health behavior using smartphone-tracking applications. The first involves enhanced alert capabilities, which remove social externalities and protect the users from others’ reckless behavior. The second adds a rule enforcement mechanism that reduces the users’ benefit from reckless behavior. Both strategies should be effective (...) if agents are expected-value maximizers, risk averse, and behave in accordance with cumulative prospect theory or in accordance with the Cognitive Hierarchy model. A multi-player trust-game experiment was designed to compare the effectiveness of the two policies. The results reveal a substantial advantage to the enforcement application, even one with occasional misses. The enhanced-alert strategy was completely ineffective. The findings align with the small samples hypothesis, suggesting that decision makers tend to select the options that lead to the best payoff in a small sample of similar past experiences. In the current context, the tendency to rely on a small sample appears to be more consequential than other deviations from rational choice. (shrink)

The question of what works in psychotherapy has been a subject of debate in the recent years, occupying both clinicians and researchers. In this study, we aimed to assess the current perspectives held by clinicians regarding the processes which produce changes in psychotherapy, as well as the predictors of specific views. Licensed therapists, consisting mainly of psychodynamically and integratively oriented psychologists, were asked to write in their own words what they think works in psychotherapy. Thematic analysis was employed to assess (...) the main mechanisms of change as perceived by the therapists. Differences in the prevalence of specific themes were assessed using Friedman and Wilcoxon signed-rank tests. Univariate logistic regressions were employed to assess the factors that predict the probability of reporting a specific mechanism of change. The results indicated that the therapeutic bond was the most highly reported mechanism of change, followed by theory-driven mechanisms of change, therapist characteristics, therapist professionalism, and client motivation. Male therapists were more likely to indicate the professionalism as a mechanism of change compared to female therapists. Higher education was associated with lower reports of therapists’ characteristics as the mechanisms of change. These results suggest that therapists acknowledge the importance of the working alliance, and that the perception of the mechanism of change is associated with various factors which comprise therapist orientation. Moreover, the class $\mathcal D_{\aleph _n}$ of directed sets of cardinality $\leq \aleph _n$ contains an isomorphic copy of the poset of Dyck $(n+2)$ -paths. Furthermore, we give a complete description whether two successive elements in the copy contain another directed set in between or not.

The military service of combat soldiers may pose many threats to their well being and often take a toll on body and mind, influencing the physical and emotional make-up of combatants and veterans. The current study aims to enhance our knowledge about the combat experiences and the challenges that female soldiers face both during and after their service. The study is based on qualitative methods and narrative analysis of in-depth semi-structured personal interviews with twenty military veterans. It aims to analyze (...) the narratives of American and Israeli female combat soldiers regarding their military service, with emphasis on the soldiers’ descriptions, in their own words, about their difficulties, challenges, coping and successes during their service and transition to civilian life. A recurring theme in the interviews with the veterans of both militaries was the need to be heard and the fact that societies, therapists, and military institutions do not always truly listen to female veterans’ experiences and are not really interested in what actually ails them. Our research suggests that conventional methods used in research relating to veterans might at times be inadequate, because the inherent categorization might abstract, pathologize, and fragment a wide array of soldiers’ modes of post-combat being. Moreover, female veterans’ voices will not be fully heard unless we allow them to be active participants in generating knowledge about themselves. (shrink)

However, where to locate the floor is a matter of values and thus largely in the realm of politics.Resorting to the courts in matters of public policy can be seen as the residual of the inability of the remaining two branches of government, the legislature and the executive, to succeed in allocating resources and regulating the system. In this paper I investigate responses to sentence (“yes-no”) questions in Greek dialogue which are neither a clear-cut ‘yes’ nor a ‘no’. I describe, classify, and discuss a range of patterns for expressing this strategy of indirectness, beginning with an example termed μέση ἀπόκρισις in the commentary of Olympiodorus to Plato Gorgias. Ancient rhetorical sources also discuss strategies for evading clear-cut non-polar responses to sentence questions in sections on the notion of ἀπόκρισις (and ἐρώτησις) without a fixed terminology. The fabricated (...) and quoted examples which these ancient sources give, serve as a point of departure for my classification of patterns in occurrences collected from literary dialogues (selections from Plato, and a corpus study of the Greek comedies of Aristophanes and Menander). The analysis of mechanisms for these non-polar responses draws on philological as well as linguistic scholarship from Greek antiquity through contemporary dialogue analysis, and refers also to contexts and conventions of characterization in Ancient Greek sources.
