The reliability of clinical practice guidelines has been disputed because guideline panel members are often burdened with financial conflicts of interest. Current recommendations for COI regulation advise not only detailed declaration but also active management of conflicts. To continuously assess COI declaration and management in German guidelines we established the public database LeitlinienWatch. We analyzed all German guidelines at the highest methodological level that included recommendations for pharmacological therapy according to five criteria: declaration and assessment of COI, composition of the (...) guideline development group, independence of the coordinators and lead authors, imposed abstentions because of COI and public external review. Each criterion was assessed using predefined outcome categories. Most guidelines contained a detailed declaration of COI. However, none of the guidelines provided full transparency of COI assessment results. The guideline group was composed of a majority of participants with COI in 55% of the guidelines, no guideline was free of participants with COI. Only 9% of guidelines had coordinators and lead authors without any financial COI. Most guidelines did not provide a rule for abstentions for participants with COI. In 21% of guidelines there was a rule, but abstentions were either not practiced or not documented, whereas in 7% partial abstentions and in 2% complete abstentions were documented. Two thirds of the guideline drafts were not externally reviewed via a public website. COI are usually documented in detail in German guidelines of the highest methodological level. However, considerable improvement is needed regarding active management of COI, including recruitment of independent experts for guideline projects, abstention from voting for participants with COI and external review of the guideline draft. We assume that the publicly available ratings on GuidelineWatch will improve the handling of conflicts of interest in guideline development. (shrink)

Why and how have supply chain codes of conduct diffused among lead firms around the globe? Prior research has drawn on both institutional and stakeholder theories to explain the adoption of codes, but no study has modeled adoption as a temporally dynamic process of diffusion. We propose that the drivers of adoption shift over time, from exclusively nonmarket to eventually market-based mechanisms as well. In an analysis of an original data set of more than 1,800 firms between the years 2006 (...) and 2015, we find that strong nonmarket labor institutions in a firm’s home country are critical to initiating and sustaining the diffusion process. Market mechanisms, such as investor scrutiny and brand risk, emerge as important later. Contrary to prior research, we did not find a significant effect from nongovernmental organization (NGO) pressure. We conclude that markets for corporate social responsibility can and do arise, but only after they are effectuated by nonmarket institutions. (shrink)

Huxley also made his own significant scientific contributions, and he was influential in the development of science education despite having had only two years of formal schooling. He established his scientific reputation through experiments on aquatic life carried out during a voyage to Australia while working as an assistant surgeon in the Royal Navy; ultimately he became President (...) of the Royal Society. Throughout his life Huxley struggled with issues of faith, and he coined the term 'agnostic' to describe his beliefs. Pavel INCOMPLETE WORLDS, RITUAL EMOTIONS' IN recent years, the notion of "fictional world" has enjoyed a considerable rise in fortune. The expression, however, is not entirely new. To refer to the world of a literary work, of a novel or of a play, has always been a favorite way of speaking for literary critics and aestheticians. In most cases, these were informal worlds. A discussion of (...) the world of Balzac, Dostoyevsky, HenryJames, or Proust often amounts to a description of each author's preferences in die selection and rearrangement of his material. Such discussion primarily fits authors who purposefully attempt to build comprehensive representational artifacts. One speaks quite naturally about l'univers balzacien, or about the universe of Musil. But is one entitled to refer with equal ease to the world ofBenjamin Constant, meagerly perceptible in his Adolphe, a short novel built in the spirit of economy, displaying just a few characters and a minimum of worldly information? Perhaps density and comprehensiveness are not indispensable characteristics of literary worlds. Aestheticians and narratologists using the "fictional worlds" terminology seem to believe mat any kind of artistic text projects a world, be it rich or poor. Such worlds are imagined in the fashion of modern philosophy of logic as states of affairs, more or less comprehensive. Some writers believe that the states of affairs comprised in fictional worlds must be fairly sizable, since diey should include "not only what the author indicates, but also whatever is required (entailed) by that which he indicates." 1 The laws of nature which are not specifically contradicted by the text would belong to its world. Thus, a few notorious cases aside, there is no reason to doubt that every child born in fiction has been engendered by a human fadier as long as the text does not report an exception. And if it does? Then, assuming that the laws of logic still govern if not fiction itself at least our understanding of it, we may appeal to what "possible world" philosophers have called "small miracles." * Editor's note: This article, as well as the contributions by Gerald Prince, Peter van Inwagen, and Kendall Walton which follow it, grew out ofa symposium on "Styles ofFictionality," organized by Professor Pavel and held at Harvard University in 1981. While everything else would preserve the properties it possesses in the actual world, with the laws of nature and the caused chains maintained, a small local change, somewhat miraculous, would modify only the color of the venerable building. Similarly, the world of Balzac may be understood as the world of early and mid-nineteenth-century France, slightly modified by the addition of a few hundred imagined characters. That diese conform surprisingly well to the general laws of French society of the time is so much the goal of Balzac's successful project, that for the sociologically-minded reader die' actual nonexistence of Rastignac, Rubempré, Vautrin, or Birotteau may appear a mere accident. These characters, one may surmise, display every trait that actual counterparts of theirs would possess, except existence. Hence, why would Rastignac's or Vautrin's emergence in a world omerwise identical to nineteenth-century France amount to anydiing more than a "small miracle"? On the other hand, Balzac's world being blessed wim hundreds of such miracles, the very magnitude of the project vindicates the attribute of "visionary," ascribed to Balzac by Béguin, or, if metaphorically undercut, mat of"God the Father" given him by Thibaudet. For, would not such magnitude entail more than just local changes in an otherwise faithful representation of the "actual" nineteenthcentury France? Similarly, would we interpret a fictional world in which a child has been engendered not by a human father, but by a divinity, as identical in all respects to the actual world, except for the limited modification attributed to a small miracle? 2 If we grant for a moment the improbable assumption that nineteenth-century France was a given and that Balzac... A physician and medical reformer enthused by the scientific and cultural progress of the Enlightenment as it took hold in Britain, Thomas Percival wrote on many topics, including public health and demography. His volume on medical ethics is considered the first modern formulation, and it and several of his other works are reissued in this series. This short book of improving tales, first published in 1777, and revised and enlarged in 1779, was originally written for his own children, and, (...) as he says, the articles 'are placed in the order in which they were written … as leisure allowed, or as the subjects of them were suggested'. The little stories contain lessons on obedience to parents, family affection, and kindness to animals, among many other examples of moral instruction. Percival refers to the book as 'Part the First', but a further collection seems never to have been published. (shrink)

This paper provides a general method for defining a generalized quantum observable that supplies properly normalized conditional probabilities for the time of occurrence. This method treats the time of occurrence as a probabilistic variable whose value is to be determined by experiment and predicted by the Born rule. This avoids the problematic assumption that a question about the time at which an event occurs must be answered through instantaneous measurements of a projector by an observer, common to both Rovelli and (...) Oppenheim et al.. I also address the interpretation of experiments purporting to demonstrate the quantum Zeno effect, used by Oppenheim et al. to justify an inherent uncertainty for measurements of times. (shrink)

Various human rights are widely recognized in codified and customary international law. These human rights promise all human beings protection against specific severe harms that might be inflicted on them domestically or by foreigners. Yet, international law also establishes and maintains institutional structures that greatly contribute to violations of these human rights: Fundamental components of international law systematically obstruct the aspirations of poor populations fordemocratic self-government, civil rights, and minimal economic sufficiency.
