BackgroundOperation room (OR) planning is a complex process, especially in large hospitals with high rates of unplanned emergency procedures. Postponing elective surgery in order to provide capacity for emergency operations is inevitable at times. Elderly patients, residents of nursing homes, women, patients with low socioeconomic status and ethnic minorities are at risk for undertreatment in other contexts, as suggested by reports in the medical literature. We hypothesized that specific patient groups could be at higher risk for having their elective surgery (...) rescheduled for non-medical reasons.MethodsIn this single center, prospective observational trial, we analysed 2519 patients undergoing elective surgery from October 2018 to May 2019. A 14-item questionnaire was handed out to illicit patient details. Additional characteristics were collected using electronic patient records. Information on the timely performance of the scheduled surgery was obtained using the OR’s patient data management system. 6.45% of all planned procedures analysed were postponed. Association of specific variables with postponement rates were analysed using the Mann–Whitney U test and Fisher's exact test/χ2-test.ResultsSignificantly higher rates of postponing elective surgery were found in elderly patients. No significant differences in postponing rates were found for the variables gender, nationality (Germany, EU, non-EU), native language, professional medical background and level of education. Significantly lower rescheduling rates were found in patients with ties to hospital staff and in patients with a private health insurer.ConclusionsElderly patients, retirees and nursing home residents seem to be at higher risk for having their elective surgery rescheduled. However, owing to the study design, causality could not be proven. Our findings raise concern about possible undertreatment of these patient groups and provide data on short-term postponement of elective surgery.Trial registration DRKS00015836. Collective emotions are at the heart of any society and become evident in gatherings, crowds, or responses to widely salient events. However, they remain poorly understood and conceptualized in scientific terms. Here, we provide first steps towards a theory of collective emotions. We first review accounts of the social and cultural embeddedness of emotion that contribute to understanding collective emotions from three broad perspectives: face-to-face encounters, culture and shared knowledge, and identification with a social collective. In discussing their strengths and (...) shortcomings and highlighting areas of conceptual overlap, we translate these views into a number of bottom–up mechanisms that explain collective emotion elicitation on the levels of social cognition, expressive behavior, and social practices. A central question in online human sentence comprehension is, “How are linguistic relations established between different parts of a sentence?” Previous work has shown that this dependency resolution process can be computationally expensive, but the underlying reasons for this are still unclear. This article argues that dependency resolution is mediated by cue‐based retrieval, constrained by independently motivated working memory principles defined in a cognitive architecture. To demonstrate this, this article investigates an unusual instance of dependency resolution, the processing of negative (...) and positive polarity items, and confirms a surprising prediction of the cue‐based retrieval model: Partial‐cue matches—which constitute a kind of similarity‐based interference—can give rise to the intrusion of ungrammatical retrieval candidates, leading to both processing slow‐downs and even errors of judgment that take the form of illusions of grammaticality in patently ungrammatical structures. A notable achievement is that good quantitative fits are achieved without adjusting the key model parameters. (shrink)

In their recently published book Nudge (2008) Richard H. Thaler and Cass R. Sunstein (T&S) defend a position labelled as ‘libertarian paternalism’. Their thinking appeals to both the right and the left of the political spectrum, as evidenced by the bedfellows they keep on either side of the Atlantic. In the US, they have advised Barack Obama, while, in the UK, they were welcomed with open arms by the David Cameron's camp (Chakrabortty 2008). I will consider the following questions. What (...) is Nudge? How is it different from social advertisement? Does Nudge induce genuine preference change? Does Nudge build moral character? Is there a moral difference between the use of Nudge as opposed to subliminal images to reach policy objectives? And what are the moral constraints on Nudge? (shrink)

A new possible world semantics for deontic logic is proposed. Its intuitive basis is that prohibitive predicates (such as "wrong" and "prohibited") have the property of negativity, i.e. that what is worse than something wrong is itself wrong. The logic of prohibitive predicates is built on this property and on preference logic. Prescriptive predicates are defined in terms of prohibitive predicates, according to the wellknown formula "ought" = "wrong that not". In this preference-based deontic logic (PDL), those theorems that give (...) rise to the paradoxes of standard deontic logic (SDL) are not obtained. (E.g., O(p & q) → Op & Oq and Op → O(p v q)) are theorems of SDL but not of PDL.) The more plausible theorems of SDL, however, can be derived in PDL. (shrink)

Different similarity relations on sets are introduced, and their logical properties are investigated. Close relationships are shown to hold between similarity relations that are based on symmetrical difference and operators of belief contraction that are based on relational selection functions. Two new rationality criteria for minimal belief contraction, the maximizing property and the reducing property, are proposed.

Systems of urban transportation are largely shaped through planning practices. In transport economics, the benefits of infrastructure investments consist mainly of travel time savings calculated using monetary values of time. The economic interpretation of the value of travel time has significantly shaped our urban environment and transportation schemes. However, there is often an underlying assumption of transferability between time and money, which arguably does not sufficiently take into account the specific features of time. In this paper, we analyze the various (...) properties of time as an economic resource using findings in behavioral economics and psychology. Due to limitations in the standard model, it is proposed that an alternative model value should be investigated in which time rather than money is the primary carrier of and the basic features of such a model are outlined. An improved understanding the nature of time as a source of utility puts us in a better position to determine what aspects of time matter. Additionally, the analysis can be applied to further develop modeling where value of time plays a significant role; such as new models for the planning of urban transport. (shrink)

A probabilistic explication is offered of equipoise and uncertainty in clinical trials. In order to be useful in the justification of clinical trials, equipoise has to be interpreted in terms of overlapping probability distributions of possible treatment outcomes, rather than point estimates representing expectation values. Uncertainty about treatment outcomes is shown to be a necessary but insufficient condition for the ethical defensibility of clinical trials. Additional requirements are proposed for the nature of that uncertainty. The indecisiveness of our criteria for (...) cautious decision-making under uncertainty creates the leeway that makes clinical trials defensible. (shrink)

Collective migration is a key process that is critical during development, as well as in physiological and pathophysiological processes including tissue repair, wound healing and cancer. Studies in genetic model organisms have made important contributions to our current understanding of the mechanisms that shape cells into different tissues during morphogenesis. Recent advances in high‐resolution and live‐cell‐imaging techniques provided new insights into the social behavior of cells based on careful visual observations within the context of a living tissue. In this review, (...) we will compare Drosophila testis nascent myotube migration with established in vivo model systems, elucidate similarities, new features and principles in collective cell migration. The article analyses the constitution of subjectivity in Ruben Östlund’s film Force Majeure. At the centre of attention stands the male protagonist who is uncapable of reconciling his inner nature with the external expectations. If the film may be un- derstood as a critique of existing middle-class conventions, it also reproduces a highly conventional ideal of the self-identical subject. The article argues that this confusion or irony is an expression of a Cartesian subject – still prevalent in the film – (...) in crisis. A neglected aspect of Descartes’ theory is that the autonomy of the subjects presupposes the existence of God. The problem for Östlund’s char- acters is that there is no God. Still, they act as if he, or as if any au- thority might, legitimize their subjectivity. Thus, the whole existence becomes a series of performances. The idea of an inner nature cor- responds with the notion of an outer nature. The latter is certainly very present in Force Majeure, but at the same time this nature is constantly problematized. On an allegorical level, the film may thus be read as a comment on the Anthropocene, a state where we no longer know neither what “nature”, nor “culture” is. (shrink)

In spite of their practical importance, the connections between technology and mathematics have not received much scholarly attention. This article begins by outlining how the technology–mathematics relationship has developed, from the use of simple aide-mémoires for counting and arithmetic, via the use of mathematics in weaving, building and other trades, and the introduction of calculus to solve technological problems, to the modern use of computers to solve both technological and mathematical problems. Three important philosophical issues emerge from this historical résumé: (...) how mathematical knowledge depends on technology, the definition of the hybrid concept of a computation, and the usefulness of mathematics in technology. Each of these issues is briefly discussed, and it is shown that in order to analyze them, we need to combine tools and ideas from both the philosophy of technology and the philosophy of mathematics. In conclusion, it is argued that much more of interest can be found in the historically and philosophically unexplored terrains of the technology–mathematics relationship. (shrink)

Real-life decision-making often begins with a disorderly decision problem that has to be clarified and systematized before a decision can be made. This is the process of decision structuring that has largely been ignored both in decision theory and applied decision analysis. In this contribution, ten major components of decision structuring are identified, namely the determination of its scope, subdivision, agency, timing, options, control ascriptions, framing, horizon, criteria and restructuring. The remainder set A⟂B of a set of sentences A modulo a set of sentences B is the set of all maximal subsets of A not implying any element of B. A remainder equation is an expression containing remainder sets, such as {A} = B⟂X, in which at least one set is unknown. Solutions to some classes of remainder equations are reported, and some unsolved problems are listed.

The primary goal of this essay is to demonstrate how considerations from computational complexity theory can inform grammatical theorizing. To this end, generalized phrase structure grammar (GPSG) linguistic theory is revised so that its power more closely matches the limited ability of an ideal speaker-hearer: GPSG Recognition is EXP-POLY time hard, while Revised GPSG Recognition is NP-complete. A second goal is to provide a theoretical framework within which to better understand the wide range of existing GPSG models, embodied in formal (...) definitions as well as in implemented computer programs. Value statements can be divided into three major groups according to how their criteria of evaluation are specified. The first of these groups consists of those value statements that are unspecified with respect to the criteria of evaluation. Here is one example: Her decision was very good. The second group consists of the viewpoint-specified value statements. In these value statements, an explicit point of view is given, from which the evaluation is made.
