Background Informed consent is an integral component of good medical practice. Many researchers have investigated measures to improve the quality of informed consent, but it is not clear which techniques work best and why. To address this problem, we propose developing a core outcome set to evaluate interventions designed to improve the consent process for surgery in adult patients with capacity. Part of this process involves reviewing existing research that has reported what is important to patients and doctors in the (...) informed consent process. Methods This qualitative synthesis comprises four phases: identification of published papers and determining their relevance; appraisal of the quality of the papers; identification and summary of the key findings from each paper while determining the definitiveness of each finding against the primary data; comparison of key themes between papers such that findings are linked across studies. Results Searches of bibliographic databases returned 11,073 titles. Of these, 16 studies met the inclusion criteria. Studies were published between 1996 and 2016 and included a total of 367 patients and 74 health care providers. Thirteen studies collected data using in-depth interviews and constant comparison was the most common means of qualitative analysis. A total of 94 findings were extracted from the primary papers and divided into 17 categories and ultimately 6 synthesised findings related to: patient characteristics, knowledge, communication, the model patient, trust and decision making. Conclusions This qualitative meta-aggregation is the first to examine the issue of informed consent for surgery. It has revealed several outcomes deemed important to capture by patients and clinicians when evaluating the quality of a consent process. Some of these outcomes have not been examined previously in research comparing methods for informed consent. This review is an important step in the development of a COS to evaluate interventions designed to improve the consent process for surgery. To redefine work as a concept, the author develops the theoretical contours of a multidimensional continuum model of women's work that moves away from older dual spheres models, using oral histories of older rural white women from Iowa and Missouri. Based on a grounded theory analysis, the author discusses three important dimensions of a continuum model of work: economic benefits, location, and time control characteristics. These dimensions tend to funnel women into multiple work strategies where they combine several labor options (...) to maximize economic benefit. A multidimensional continuum model allows us to observe some of the mechanisms operating in women's labor decision-making process. It suggests new ways of thinking about women's life course patterns. By providing us with a definition of work that more accurately reflects observed experience, it casts doubt on some of our social policies, especially those regarding the care of children and the elderly. Settler-colonialism is founded in environmental racism, and environmental justice is foundational to all forms of decolonialization. Native American groups located in the Gulf Coast Region of the United States are particularly vulnerable to environmental justice issues such as climate change and oil spills due to their geographic location and reliance on the coastal region for economic and social resources. This study used the framework of historical oppression, resilience, and transcendence to explore the historic and contemporary forms of environmental injustice experienced (...) by a Native American tribe in the Gulf Coast region of the United States. This critical ethnography analyzeda series of individual, family, and focus group semi-structured qualitative interviews with a total of 208 participants. Following the critical ethnographic method, data were interpreted through reconstructive analysis using NVivo. Findings of this study reveal the continuing impact of the BP oil spill and difficulty accessing resources following the spill, complicated by the tribe’s lack of federal recognition. Additional themes include the continuing impact of coastal erosion, historical and contemporary land loss, geographic marginalization, and concerns about a loss of tribal identity when tribal members are forced to relocate. Lack of federal tribal recognition has exacerbated all of these issues for this tribe. This study supports national findings that Native American groups experience extensive historic and contemporary environmental injustices and contextualizes these findings for a Native American tribe in the Gulf Coast region of the United States. Recognizing Native American sovereignty is key to addressing the environmental justice issues described. (shrink)

School choice has lately risen to the top of the list of potential solutions to America's educational problems, particularly for the poor and the most disadvantaged members of society. Indeed, in the last few years several states have held referendums on the use of vouchers in private and parochial schools, and more recently, the Supreme Court reviewed the constitutionality of a scholarship program that uses vouchers issued to parents. While there has been much debate over the empirical and methodological aspects (...) of school choice policies, discussions related to the effects such policies may have on the nation's moral economy and civil society have been few and far between. Assassins are often regarded as ahistorical figures of evil. In this article, I contest this view by analysing the assassination of President William McKinley by Leon Czolgosz in 1901. There are two purposes to this article. The first is to situate McKinley’s assassination within the history and development of the social sciences, principally sociology, rather than assume that the assassin is a trans-historical representation of willful irresponsibility. This volume contains the English translation of Felix Kaufmann's (1895-1945) main work Methodenlehre der Sozialwissenschaften (1936). In this book, Kaufmann develops a general theory of knowledge of the social sciences in his role as a cross-border commuter between Husserl's phenomenology, Kelsen's pure theory of law and the logical positivism of the Vienna Circle. This multilayered inquiry connects the value-oriented reflections of a general philosophy of science with the specificity of the methods and theories of the social sciences, as opposed to (...) abstract natural science and psychology. The core focus of the study is the attempt to elucidate how and under what conditions scientific knowledge about social facts, empirically justified and theoretically embedded, can be obtained. The empirical basis of knowledge within the social sciences forms a phenomenological concept of experience. According to Kaufmann, this concept of experience exhibits a complex structure. Within the meaning-interpretation of human action as the core of knowledge in the social sciences, this structure reaches out across the isolated act of verification toward the synthesis of external and internal experiences. The book opens with a detailed and useful introduction by Ingeborg K. Helling, which introduces the historical and theoretical background of Kaufmann's study and specifically illuminates his relation to Alfred Schütz and John Dewey. Problem: A dualistic position faces considerable problems as Mitterer, inter alia, clearly pointed out. Mitterer not only wants to name these problems, but to provide a genuine alternative with his non-dualism. However, this non-dualistic alternative also contains severe problems. Thus this text suggests preferring Wittgenstein's concept of a pragmatic investigation of language-games to Mitterer's non-dualism in order to tackle the problems of dualism. Solution: With recourse to Wittgenstein's pragmatic investigation of language-games, a fundamental problem of dualism can be solved. With (...) the concept of certainty, Wittgenstein succeeds in avoiding an ontological grounding in an independent world -- or, as Mitterer would put it, the assumption of a "beyond of discourse." At the same time, the assumption of an independent world as a concept that provides a basis for our language-games is maintained on an epistemological level. This assumption, however, is not maintained as a phenomenon that requires to be substantiated but as a certainty that is constitutive for language-games and does not need to be substantiated. Such a concept is suitable for preventing epistemological operations such as knowledge, doubt, giving reasons, etc., from being made void, without having to provide an ontological basis for them. Implications: Wittgenstein's point of view therefore provides an attractive alternative to Mitterer's non-dualism. By getting rid of the "beyond of discourse," Mitterer's non-dualism faces the problem of not being able to explain how we can manage to understand epistemological operations within our language-games without referring to a "beyond of discourse." From this point of view arises the consequence that it would make sense to analyze language-games from a pragmatic standpoint rather than to keep on honing non-dualistic vocabulary. (shrink)

Einstein, in his “Zur Elektrodynamik bewegter Körper”, gave a physical (operational) meaning to “time” of a remote event in describing “motion” by introducing the concept of “synchronous stationary clocks located at different places”. But with regard to “place” in describing motion, he assumed without analysis the concept of a system of co-ordinates.In the present paper, we propose a way of giving physical (operational) meaning to the concepts of “place” and “co-ordinate system”, and show how the observer can define both the (...) place and time of a remote event. Following Einstein, we consider another system “in uniform motion of translation relatively to the former”. Without assuming “the properties of homogeneity which we attribute to space and time”, we show that the definitions of space and time in the two systems are linearly related. We deduce some novel consequences of our approach regarding faster-than-light observers and particles, “one-way” and “two-way” velocities of light, symmetry, the “group property” of inertial reference frames, length contraction and time dilatation, and the “twin paradox”. Finally, we point out a flaw in Einstein’s argument in the “Electrodynamical Part” of his paper and show that the Lorentz force formula and Einstein’s formula for transformation of field quantities are mutually consistent. We show that for faster-than-light bodies, a simple modification of Planck’s formula for mass suffices. (Except for the reference to Planck’s formula, we restrict ourselves to Physics of 1905.). Several recent papers dealing with Plato's position on the imperfection of the phenomenal world draw heavily on the differences between two kinds of predicates in order to show the following: In the middle dialogues, Plato posits Forms only as referents of what the writers call incomplete predicates. He does not posit Forms as referents for complete predicates. When interpreters ignore the differences between these kinds of predicates, they ascribe too radical a view regarding the imperfection of the phenomenal world to (...) the middle dialogues. The ontology of these dialogues includes sensible particulars which retain their identity over time even though they are continually changing in some respect or other. (shrink)

On semantics and the process of word-generated knowledge in Hindu philosophy.

This paper brings together the work of Nicholas Wolterstorff and William Alston in speech-act theory with the aim of providing a deeper understanding of the nature of divine speaking through the medium of Scripture. Despite the fecundity of Wolterstorff's seminal work on the philosophical theology of Scripture, aspects of his speech-act centric account are underdeveloped and would benefit from the contributions of William Alston. In particular, his account of divine speech-acts could be fruitfully expanded by incorporating the concept of ‘taking (...) responsibility’ that is central to Alston's analysis of the nature of illocutionary acts. This would elucidate both the way in which readers of Scripture encounter divine practical authority in the reading process and the nature of the response of faith that is demanded of them. (shrink)
