Outpatient services are increasingly recognised as an important component of health care provision and may be improved through the application of modern management techniques. We have performed a time and role audit of consultation and waiting times in two medical clinics using different queuing systems: namely, a serial processing clinic where patients wait in a single queue and a quasi-parallel processing clinic where patients are directed to the shortest queue to maintain clinic flow. Data collected were used to construct a (...) computer simulation of patient flows in clinic. Assessment of patient satisfaction in the clinic process was determined using a self-administered questionnaire. Mean waiting time was shorter in the quasi-parallel processing clinic: 26 (SD 17) minutes compared with 36(24) minutes in the serial processing clinic. In the serial processing clinic 61% of patients waited more than 30 minutes compared with 41% in the quasi-parallel processing clinic. In the serial processing clinic 8% of 142 patients surveyed complained of the time spent waiting. The computer simulation we produced was able to determine waiting times with different clinic structures. The simulation showed that reductions in waiting time up to 30% might be achieved by changing our serial processing clinic to a quasi-parallel processing one. Performance of medical outpatient clinics can be improved by examining and changing clinic management. Computer simulation of outpatient clinics offers a means of assessing the impact of such changes on waiting time in clinic and on waiting lists. (shrink)

The Defining Issues Test 2 (DIT-2) and Engineering Ethical Reasoning Instrument (EERI) are designed to measure ethical reasoning of general (DIT-2) and engineering-student (EERI) populations. These tools—and the DIT-2 especially—have gained wide usage for assessing the ethical reasoning of undergraduate students. This paper reports on a research study in which the ethical reasoning of first-year undergraduate engineering students at multiple universities was assessed with both of these tools. In addition to these two instruments, students were also asked to create personal (...) concept maps of the phrase “ethical decision-making.” It was hypothesized that students whose instrument scores reflected more postconventional levels of moral development and more sophisticated ethical reasoning skills would likewise have richer, more detailed concept maps of ethical decision-making, reflecting their deeper levels of understanding of this topic and the complex of related concepts. In fact, there was no significant correlation between the instrument scores and concept map scoring, suggesting that the way first-year students _conceptualize_ ethical decision making does not predict the way they behave when _performing_ scenario-based ethical reasoning (perhaps more situated). This disparity indicates a need to more precisely quantify engineering ethical reasoning and decision making, if we wish to inform assessment outcomes using the results of such quantitative analyses. This work emerged from funded research examining collaboration among stake-holder organizations present at three U.S. nuclear weapons complex sites. The authors examine issues such as how and why stakeholder groups form collaborative alliances when dealing with the target organization, what leaders of stakeholder organizations actually think about when collaborating to deal with the target organization, and what outcomes result from the collaboration process. Drawing on stakeholder theory and research in interorganizational collaboration, the authors used an inductive, interview-based methodology to build (...) a model of collaboration among nonprofit stakeholder groups. The model contributes to the descriptive stream of stakeholder theory and, in turn, has implications for the instrumental stream. Donald Davidson is probably the most eminent living analytic philosopher, and his writings in philosophy of language and philosophy of action have shaped much of the recent work in both these fields. However, despite the obvious shared concerns of literary theory and these aspects of philosophy, up to this point literary theorists have not paid much attention to Davidson's ideas or have only known about them through the interpretations of other philosophers like Richard Rorty. Literary theorists have seen more (...) relevance to their concerns in Continental philosophy and, among analytic philosophers, in the essentially anti-analytic work of J. L. Austin and Ludwig Wittgenstein than in the harder tradition of analytic philosophy—more concerned with logic and philosophy of science—represented by the work of Donald Davidson. _Literary Theory after Davidson _challenges both views, stressing the variety of ways in which Davidson's thought can contribute to the development of literary theory. Davidson himself has contributed a new essay to the collection that explores the interrelations between his theories of language and literature. (shrink)

This paper looks at a dispute decision theory about how best to characterize expected utility maximization and express the logic of rational choice. Where A1, … , An are actions open to some particular agent, and S1, … , Sn are mutually exclusive states of the world such that the agent knows at least one of which obtains, does the logic of rational choice require an agent to consider the conditional probability of choice Ai given that some state Si obtains, (...) Prob(Ai/Si). Or, is the logic of choice better represented by considering the probability of the counterfactual if Ai then Si, Prob(Ai ⟥-> Si). Causal decision theory, developed by Allan Gibbard, William Harper, and David Lewis defend the counterfactual analysis; whereas, Richard Jeffrey and others defend the conditional probability analysis, evidential decision theory. I argue that the problems posed by cases of decision instability favor evidential decision theory. (shrink)

This case recounts the founding of HealthSouth Rehabilitation, its rapid growth, financial mishandlings and the struggle former CFO Aaron Beam had in dealing with a conscience that kept him awake at night. Beam had met HealthSouth founder Richard Scrushy when applying for a job with Lifemark Hospital Corporation in Texas in 1980. After Lifemark was bought by AMI in 1983, Scrushy invited Beam to join him in the launching of his new company in Birmingham, Alabama. The uniqueness of the (...) hospital was that it would provide inpatient surgery for those who had suffered an injury, but it would also provide outpatient services such as rehabilitation, nutrition guidance and psychological counseling. This type of structure was appealing to insurance companies because it would move the patient out of the hospital much quicker and save the cost of extended hospitalization. The company experienced rapid growth and went public within three years of launch. By 1996, the top executives of the company realized that they would not be able to meet Wall Street analysts’ forecasts which would affect the price of their stock and reduce their holdings. They decided to “cook the books” temporarily to meet quarterly expectations. However, once the financial mishandling had begun, the executives found it difficult to reverse their actions and report accurate numbers to the public. Beam decided to retire from the company in 1996; and after this action, he continued to find it difficult to sleep at night because of his involvement in the fraud. The issue in the case is whether Beam should consider coming forward and revealing to the Securities and Exchange Commission how the books had been doctored so that he could salvage his damaged conscience. Against the background of the Second World War and post-war cultural change in Australia, this review article discusses the establishment of the profession of art history and art curatorial scholarship in Australia in the 1940s and 1950s. The key figures in this transformation were Franz Philipp and Ursula Hoff (European ‘savant’ refugees from Nazism and anti-Semitism), and the British scholar Joseph Burke (appointed as Herald Professor of Fine Arts at Melbourne University). These figures played pivotal roles in Sir Keith Murdoch's (...) ‘civilizing mission’ to reform Australian culture – a mission centred on Melbourne University and the National Gallery of Victoria under its new director, Daryl Lindsay. Within the context of the ‘culture wars’ dividing the Australian art world at this time, these reformers and pioneering scholars found themselves opposed by a radical art avant-garde under the leadership of the art patron John Reed. The work of key modernist Australian artists, above all Sidney Nolan and Russell Drysdale, became touchstones for radical and conservative positions in this conflict. (shrink)

The variety and complexity of the eight papers in this Symposium issue are evidence that immigration is a tough nut to crack both as a matter of policy and application. There is no way that any short summary can do justice to these papers, which take a variety of moral, economic, historical, and empirical approaches to some of the recurrent issues in the field, so it is best in this short issue to try to situate the problem in a general (...) classical liberal context, to show where the pressure points are most likely to appear.The initial way to approach this problem is to start with the simple proposition that the world today is divided into national territories where the practical norm is well-nigh universal: That each sovereign state has exclusive control over its own territory, which, among other things, allows it to decide which individuals are members of that state. These citizens have privileges that may be exercised, no questions asked, on such key matters as entering labor markets, owning property, voting, and otherwise participating in political affairs. All these activities may be denied or granted, at will, to any and all outsiders by the territorial sovereign.No one doubts the need for having some form of territorial control. The need to have some monopoly of force could not be achieved if the entire world, with its infinite diversity of locations and populations, were under a single sovereign. Indeed, in many cases, the divisions within a given country may be so pronounced that separation rather than aggregation is the norm. Thus, at the end of the British Raj, India split into India and Pakistan, and later Pakistan split into Pakistan and Bangladesh. Less dramatically, Czechoslovakia split into two countries, the Czech Republic and Slovakia, while the former Yugoslavia split into five nations, starting from the northwest: Slovenia, Croatia, Bosnia and Herzegovina, Serbia, and Macedonia. Clearly, these ethic differences, be they large or small, are a major obstacle to the expansion of the size of the state. And today, the territorial instinct reasserts itself as the member states of the European Union impose more restrictions on immigration.Yet at the same time, the normative foundations of this system seem elusive. There is the following deep skepticism about the importance of these borders, expressed in these essays by Jeremy Waldron and Ilya Somin, that these national boundary lines should have such a pronounced role in political economy. Michael Blake pursues this theme further by asking the extent to which classical liberals can justify exclusion through the use of violence—or is it force? The inquiry into the dominant role of territoriality can then be expanded, as by Peter Margulies, to examine all sorts of negative externalities that could be created both by easy and strict rules of admission.Taken as a whole, there is no doubt that these national barriers are a serious obstacle to the free trade in goods and services between two individuals, one inside the boundary and one outside. The general view is that these barriers block the gains from trade, which is what drives economic progress, so that ideally all tariffs and quantitative restrictions should give way to a system of free trade in which petty objections by domestic competitors of foreign firms should not dictate local policies.It should, of course, be evident that powerful conflicting forces on free trade and protectionism ebb and flow over time. It should therefore come as no surprise that the stakes are even higher where what is at stake is the movement of people across national boundaries. The loss to any given country from keeping out potential citizens can be enormous, and we see powerful political forces clamp down on open immigration, so many of these gains from trade are lost. The situation can be even more heartbreaking when the ability to leave a country exposes innocent people to imprisonment or death.So what is the explanation for so many nations closing their borders? Much of the explanation comes from old-fashioned protectionism. Your computer programmers will take the jobs of our specialists. There is all sorts of occupational licensing and other barriers to entry within a domestic economy, so there is no reason to expect that these forces will not exert even greater influence against outsiders who do not, after all, have a voice in the local economy and who, therefore, as Alex Nowrasteh, Michael Howard, and Andrew Forrester observe, have little ability to influence domestic policy even after they enter a country. In addition, as Gabriel Chin discusses, strong racial prejudices also lead to exclusions from the domestic economy. Not only are there objections in, say, the United States and much of Western Europe, of exclusion on straight racial grounds, as exemplified by Chinese exclusion acts. The force of these exclusions are also subject to political change. Thus, the Immigration Act of 1924 (the Johnson-Reed Act) was part of a white supremacist program, which gave way two decades later to a very different legal regime with the Immigration Act of 1965, which put an end to the venerable system of national-origin quotas.Yet two other reasons for exclusion are constantly at issue. Immigrants not only bring in their labor but also their families, cultures, and traditions that, in public spaces, may clash with local norms and cultures on such elementary and critical matters as to whether women should dress modestly and wear head coverings. Will freewheeling Danes and Swedes feel comfortable if large numbers of Muslims continue their traditional practices with strong barriers between men and women? There are no obvious market mechanisms that will mitigate these issues, and the situation becomes even more fraught if there are large welfare or educational costs that fall, at least in the short run, on local citizens. These are compounded by the fact that immigrants will typically gain political rights that could easily skew the balance of political power between established and newly arrived ethnic groups by altering the outcome of national and local elections. And letting people in is not just done on a simple open-borders policy. Thus, in many countries, as Lance Pritchett discusses, there is the controversial option of letting foreigners in on short-term labor visas that require them to leave after a certain period. And there are further questions about whether people should be deported if they lied on their entry applications or committed some crime while in the new country, as has occurred in the United States. The numbers of cases can be high and their complexity great, so it is fair to ask, along with Jill Family, whether some self-conscious exceptions to the Administrative Procedure Act of 1946, analogous statutes elsewhere, should be regarded as part of the new normal.It is therefore necessary to deal with the thrust and counterthrust on all these issues. It is also fair to ask whether classical liberalism has a distinctive view on these issues, or whether the combination of private gains and multidirectional externalities gives rise to empirical questions that, as always, require further reflection, even when certain situations—the major influx of illegal immigrants on the US southern border—will generate such powerful emotions that detached intellectual inquiry is ever more difficult. We cannot answer these questions, and thus offer to you a set of essays that take a stab at these difficult topics. (shrink)

ALL classical palaeographers are familiar with Persius' roster of writing materials: Now we take the book to hand, and the two-coloured parchment purged of hair, and papyrus, and a knotty reed. Of the items mentioned, only ‘two-coloured parchment’ presents any difficulty.I hope in this article to present at least a possible solution to this problem. An obvious supposition would be that Persius is merely referring to the natural differences in colour between the two sides of a piece of parchment.

To know the scale of an aulos it is necessary to have a complete instrument. None of the surviving auloi are complete. It is the purpose of this article to attempt to reconstruct the missing parts of several auloi and thus to determine their scales. All musical sounds consist of regular vibrations. The interval between two notes may be expressed as the rate of vibration of the higher note divided by the rate of vibration of the lower note. This ratio (...) is very roughly the same as the length of the air column for the lower note divided by the length of the air column for the higher note. This gross over-simplification has been used for all calculations in this article. The width of the air column, the size of the holes, and the reed itself affect the pitch considerably. The player also has a large control over the precise note played. And so the notes obtained by the ancients might have been very different from the notes obtained by ignoring these factors; although the player's control over pitch might have been used to lessen the effects of the other factors. This book provides a systematic analysis of many common argumentation schemes and a compendium of 96 schemes. The study of these schemes, or forms of argument that capture stereotypical patterns of human reasoning, is at the core of argumentation research. Surveying all aspects of argumentation schemes from the ground up, the book takes the reader from the elementary exposition in the first chapter to the latest state of the art in the research efforts to formalize and classify the schemes, outlined (...) in the last chapter. Are there things we should value because they are, quite simply, good? Richard Kraut argues that there are not. Goodness, he holds, is not a reason-giving property - in fact, there may be no such thing. It is an illusory and insidious category of practical thought.

Virtue perspectivism is a bi-level epistemology according to which there are two grades of knowledge: animal and reflective. The exercise of reliable competences suffices to give us animal knowledge; but we can then use these same competences to gain a second-order assuring perspective, one through which we may appreciate those faculties as reliable and in doing so place our first-order knowledge in a competent second-order perspective. Virtue perspectivism has considerable theoretical power, especially when it comes to vindicating our external world (...) knowledge against threats of scepticism and regress. Prominent critics, however, doubt whether the view ultimately hangs together without succumbing to vicious circularity. In this paper, I am going to focus on circularity-based criticisms of virtue perspectivism raised in various places by Barry Stroud, Baron Reed and Richard Fumerton, and I will argue that virtue perspectivism can ultimately withstand each of them. This paper argues that a popular account of intentions can be extended to beliefs. Beliefs are stable all-out states that allow for planning and coordination in a way that is tractable for cognitively limited creatures like human beings. Scepticism is expressed that there is really anything like credences as standardly understood.

The notion of a “query” occurred in legal, medical, theological and scientific writings during the early modern period. Whereas the “questionary” (from c. 1400s) sought replies from within a doctrine (such as Galenic medicine), in the 1600s the query posed open-ended inquiries, seeking empirical information from travellers, explorers and others. During the 1660s in Britain, three versions of the query (and lists of queries) emerged. Distinctions need to be made between queries seeking information via observation and those asking for experimentation, (...) and between those aiming to keep theory to one side and those that framed theoretical conjectures. My examples are drawn from the work of the Royal Society of London (founded 1660) and from some of its leading members, especially Robert Boyle, Robert Hooke and Isaac Newton. (shrink)

That India once had a sustained ‘dialogue’ with Scottish Philosophy is not gener- ally known, or that the exchange occurred in the medium of Sanskrit, not English. The essay explores an important cross-cultural encounter in the colonial context of mid 19th-century Benares where two Scots, John Muir and James Ballantyne, served as principals of a Sanskrit college established by the East India Company. Educated toward the end of the Scottish Enlightenment, they endeavoured to translate such distinctive concepts of ‘Scotch Metaphysics’ (...) as Externalism into Indian philosophical categories. The ensuing ‘dialogue’ with Brahmin interlocutors shows that the prob- lems they faced were less terminological than conceptual, having to do with contras- tive ways of understanding ‘mind’ and ‘man’. Between the two Scots, there were also signifi cant differences, although both had gone to India as Scottish Calvinists. While Muir remained largely impervious to Indian infl uence, Ballantyne was profoundly changed, becoming, in effect, a ‘Vedantic Calvinist’. (shrink)

Sainsbury and Tye present a new theory, 'originalism', which provides natural, simple solutions to puzzles about thought that have troubled philosophers for centuries. They argue that concepts are to be individuated by their origin, rather than epistemically or semantically. Although thought is special, no special mystery attaches to its nature.

The book begins with an overview that introduces the Theorem and the issues surrounding it, and explores how the essays that follow contribute to our understanding of those issues.

Jim Joyce has presented an argument for Probabilism based on considerations of epistemic utility [Joyce, 1998]. In a recent paper, I adapted this argument to give an argument for Probablism and the Principal Principle based on similar considerations [Pettigrew, 2012]. Joyce’s argument assumes that a credence in a true proposition is better the closer it is to maximal credence, whilst a credence in a false proposition is better the closer it is to minimal credence. By contrast, my argument in that (...) paper assumed (roughly) that a credence in a proposition is better the closer it is to the objective chance of that proposition. In this paper, I present an epistemic utility argument for Probabilism and the Principal Principle that retains Joyce’s assumption rather than the alternative I endorsed in the earlier paper. I argue that this results in a superior argument for these norms. (shrink)

It is sometimes proposed that killing or harming abortion providers is the only logically consistent position available to opponents of abortion. Since lethal violence against morally responsible attackers is normally viewed as justified in order to defend innocent parties, pro-lifers should also think so in the case of the abortion doctor and so they should act to defend the unborn. In our paper, we defend the mainstream pro-life view against killing abortion doctors. We argue that the pro-life view can, in (...) various ways, reject the assumption that defensive violence to save innocent individuals is always permissible. Now even if that assumption is accepted, we contend that defensive violence against abortion doctors still is not justified. Drawing on Frances Kamm’s work, we contend that there are structural similarities between abortion and letting someone die who needs your help to stay alive; and we argue that it is impermissible to kill those who kill in order to avoid giving life-saving aid. The argument of this paper is that we should think of the extension of cognitive abilities and cognitive character in integrationist terms. Cognitive abilities are extended by acquired practices of creating and manipulating information that is stored in a publicly accessible environment. I call these cognitive practices (2007). In contrast to Pritchard (2010) I argue that such processes are integrated into our cognitive characters rather than artefacts; such as notebooks. There are two routes to cognitive extension that I contrast in (...) the paper, the first I call artefact extension which is the now classic position of the causal coupling of an agent with an artefact. This approach needs to overcome the objection from cognitive outsourcing: that we simply get an artefact or tool to do the cognitive processing for us without extending our cognitive abilities. Enculturated cognition, by contrast, does not claim that artefacts themselves extend our cognitive abilities, but rather that the acquired practices for manipulating artefacts and the information stored in them extend our cognitive abilities (by augmenting and transforming them). In the rest of the paper I provide a series of arguments and cases which demonstrate that an enculturated approach works better for both epistemic and cognitive cases of the extension of ability and character. What are the possible varieties of cognition-artifact relations, and which dimensions are relevant for exploring these varieties? This question is answered in two steps. First, three levels of functional and informational integration between human agent and cognitive artifact are distinguished. These levels are based on the degree of interactivity and direction of information flow, and range from monocausal and bicausal relations to continuous reciprocal causation. In these levels there is a hierarchy of integrative processes in which there is an increasing (...) degree of hybridization. Therefore, from a functional and informational level of abstraction, it becomes increasingly difficult to distinguish between agent and artifact in this hierarchy. Second, a multidimensional matrix for exploring cognition-artifact relations is sketched. The dimensions in the matrix include reliability, durability, trust, procedural and representational transparency, individualization, bandwidth, speed of information flow, distribution of computation, and cognitive and artifactual transformation. Together, these dimensions constitute a multidimensional space in which particular cognition-artifact relations can be located. The higher a particular cognition-artifact relation scores on these dimensions, the more integration and hybridization occurs, and the more tightly coupled the overall system is. It is then better, for explanatory reasons, to see agent and artifact as one cognitive system with a distributed informational architecture. (shrink)

Comparative case studies of lawyer deviance and discipline offer a unique perspective on how and why lawyers misbehave, how regulatory bodies respond, and the efficacy of those responses. Such studies also provide valuable pedagogic tools, opening the eyes of law students to the ways in which they, too, could transgress ethical rules. This special issue builds on my two books on misbehaving lawyers in New York and California by presenting vivid accounts of such lawyers in the UK, Canada, Australia, New (...) In the world according to Hume, people are complicated creatures, with convoluted, often contradictory characters. Consider, for example, Hume's controversial assessment of Charles I: "The character of this prince, as that of most men, if not of all men, was mixed .... To consider him in the most favourable light, it may be affirmed, that his dignity was free from pride, his humanity from weakness, his bravery from rashness, his temperance from austerity, his frugality from avarice .... To speak the (...) most harshly of him, we may affirm, that many of his good qualities were attended with some latent frailty, which, though seemingly inconsiderable, was able, when seconded by the extreme malevolence of his fortune, to disappoint them of all their influence: His beneficent disposition was clouded by a manner not very gracious; his virtue was tinctured with superstition; his good sense was disfigured by a deference to persons of a capacity inferior to his own; and his moderate temper exempted him not from hasty and precipitate resolutions." This sketch shows Charles in all his complexities, with his virtues, near virtues, and contradicting virtues. I have quoted it at length because it is hard to summarize without losing the subtleties that lie within it.
